Incident date: Jul 10, 2026
On July 10, 2026, my vehicle was parked, turned off, and unattended. The front windows were partially rolled down because the outside temperature was approximately 80°F. I had been away from the vehicle for about one hour. Approximately 20 minutes before discovering the incident, I heard fire truck
... s nearby but did not initially realize they were responding to my vehicle. When I returned, I discovered that the vehicle had caught fire and that the fire department had already extinguished it. I do not know what component or system failed or where the fire originated. Before parking the vehicle, I did not observe any warning lights, dashboard messages, smoke, unusual odors, overheating, electrical issues, mechanical problems, or other symptoms that would have indicated a possible fire or malfunction. Because the vehicle caught fire while parked and turned off, the incident created a serious safety risk to nearby people, surrounding vehicles, homes, and other property. Fortunately, I am not aware of anyone being inside or immediately next to the vehicle when the fire began. A fire marshal is currently investigating the incident and the cause remains unknown. The issue has not been reproduced or confirmed by a dealership, manufacturer, or independent repair facility. That I am aware of. The firemarshalle said it appears to be manufactured defects. The vehicle has not yet been inspected by the manufacturer to my knowledge. The damaged vehicle and affected components should be available for inspection upon request, subject to the requirements of the fire investigation.Read more

Component/System: Smart Key System / Automatic Door Locking System I own a 2023 Toyota Camry LE equipped with a Smart Key System. I parked the vehicle, turned off the engine, and left the smart key inside the vehicle. My [XXX] child remained securely strapped into a rear child safety seat and was
... sleeping inside. I briefly stepped outside to wipe dust from the windshield while standing next to the vehicle. After the driver’s door closed, the vehicle automatically locked itself with the smart key still inside. I did not intentionally lock the vehicle. I was unable to unlock the doors because the smart key was locked inside the vehicle. My [XXX] child was trapped inside the locked vehicle, creating an emergency situation. To rescue my child, I had no choice but to break the driver’s side window. There were no warning lights or warning messages before the incident. The cause is unknown. The vehicle has not yet been inspected to determine why it automatically locked with the smart key inside. Based on the circumstances, the Smart Key System either failed to detect the smart key or the vehicle improperly allowed the doors to lock despite the key remaining inside. I believe this presents a serious safety risk because it can trap children inside the vehicle. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

See attached document. Dear Jeep Customer Care and Chrysler Capital,I am writing to formally request the termination of my current lease agreement for my 2023 Jeep Wrangler 4xe, VIN: [XXX], due to the severe safety risk posed by NHTSA Recall 25V-74 l, concerning the high-voltage battery fire hazard.
... Due to the severity of this unaddressed safety hazard, I am unable to use the vehicle for its intended purpose. This significant safety defect renders inoperable as designed and intended for use, but more importantly the vehicle is unsafe and unsuitable for normal use, as it risks fire while driving or parked as stated in your recall notice.Your own notices further indicate not charge it as designed for normal function.Furthermore, I live in a townhome surrounded by other families now put at risk and cannot safely park this vehicle outside due to the extreme fire risk, making it impractical and unsafe for my living situation. This also puts others at risk when parked in parking structures at the airport that I frequent on a weekly basis. When the same recall occurred in late 2024, I waited for four months without resolution as Jeep developed a remedy. With the new recall, I immediately contacted Jeep customer service, who informed me on December 3, 2025, that no solution is currently available and none is anticipated until late Ql 2026 (as documented during my call with Riana, case #[XXX]).I was advised to contact Chrysler Capital regarding a potential lease suspension due to the seriousness of the recall. At 11am EST, I called Chrysler Capital and spoke with Maren (ID774178). To the Office of Defects Investigation, I am submitting this formal complaint regarding NHTSA Recall 22V703000 (Kia Recall SC249) involving my 2020 Kia Sorento (VIN: [XXX] ). This recall identifies a fire risk due to a potential electrical short in the tow hitch harness module. The recall notice s
... pecifically warns that a vehicle fire may occur while parked or driving. After receiving the recall notification, we made multiple attempts to schedule the required repair with Pompano Kia, the authorized dealership in our area. Despite repeated efforts, we were not provided with a confirmed repair appointment, and the recall remained incomplete. On [1-19-2026], before the recall remedy was performed, the vehicle caught fire while being driven on a highway in Florida. The vehicle became fully engulfed and was declared a total loss. Law enforcement and/or fire services responded. The incident appears consistent with the fire risk described in the recall. Following the incident, we visited the dealership to report the fire and the unresolved recall. We were told the matter would be reviewed and that we would receive a follow-up call. No follow-up was received. Attempts to contact the dealership through publicly listed email addresses resulted in delivery failure notices. According to NHTSA records, this recall affects tens of thousands of vehicles nationwide. We are concerned about: • Timeliness and availability of recall repairs • Dealership accessibility and communication • Risk to consumers when recall remedies are not completed before failure occurs We also attempted to consult several attorneys regarding this incident. We were advised that due to the volume of recall-related fire cases involving this manufacturer, representation may not be feasible unless severe bodily injury is involved. This further raises concern about the scale and seriousness of the issue. We respectfully request that NHTSA review: • Whether recall 22V703000 is being remedied INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATIONRead more

The LED headlight on my 2019 Land Rover Velar is no longer working properly. The built-in Daytime Running Light (DRL) has stopped working completely. This is an important safety feature that helps other drivers see my vehicle during the day. I've already been pulled over three times because of this
... issue, so it's something that really needs to be fixed. The dealership in Maryland told me the only fix is to replace the entire headlight unit, which costs over $6,000. That’s an extremely high cost for a safety-related problem. I’ve seen many similar complaints online from other owners of 2018–2022 Velar and other Range Rover models, all experiencing the same DRL failure. This seems to be a common issue. When the DRL doesn't work, it makes the vehicle less visible and more dangerous to drive during the day. Given the high cost, the safety risk, and how often this seems to happen, I believe this issue needs to be investigated and possibly recalled. I'm very disappointed that such a costly repair is required for a basic safety feature.Read more

Incident date: Feb 10, 2023
I am writing because I am beyond frustrated with the treatment I’ve received from Gwinnett Chrysler Dodge Jeep Ram (5054 U.S. 78, Stone Mountain, GA 30087 ). I have been a long‑time Jeep customer, and I cannot believe everything that has been going on. This situation has become untenable, and I
... demand a resolution immediately. Back in May, I brought my Jeep in to have a hybrid electrical vehicle system issue addressed, and I paid for that service in full. In July, the same warning light came back on. When I returned the vehicle to the dealership, they first tried to shift blame—saying perhaps the previous service provider messed something up. Then they told me my engine needed to be replaced. When I reminded them that they were the last ones to work on the car, the tone changed. My vehicle is only a little over two years old—how is an engine replacement justified? They quoted me $14,000 to make this repair. I contacted Jeep Customer Care. Their inspection quoted $18,000, but with warranty coverage I was told I would owe $5,500. Even then, they tried to force me to take out a loan. Reluctantly, I paid the $5,500, as I rely on my vehicle for work and to transport my daughter to and from school. I was required to pay $4,000 upfront before any work would begin, and the engine was ordered. Over a month passed, with several promised completion dates (August 27, 28, 29) that came and went without completion. I flew in on August 29 expecting to pick up my vehicle, only to be told it was not ready because the new engine had been mistakenly sent back instead of the old one. This error is incomprehensible and has severely damaged my trust in the dealership. As of September, I was informed that my Jeep would be completed by September 9. That deadline has passed, and my vehicle remains unfinished. I have incurred additional expenses for flights and Uber/Lyft rides due to the dealership's missed deadlines and miscommunication. Not once was I offered a loaner vehicle, nor didRead more

On [XXX], while driving a 2023 Hyundai TUCSON SEL, the steering wheel suddenly and completely detached from the steering column and ended up in the driver's lap. This sudden and absolute loss of steering control is a catastrophic failure of a primary safety system that placed the driver's life and p
... otentially the lives of others in immediate and extreme peril. It was only due to sheer luck that the vehicle was brought to a stop without an accident or injury. Following the incident, due to fear and anxiety, the driver is now in treatment and has been unable to work. The vehicle was transported to Ron Marhofer Hyundai on the same day. The following day, the driver was met with a profound lack of respect and professionalism from the service advisor, Declan McCready. The driver and her mother were implicitly told they did not know what they were talking about and were treated with a profound lack of respect. The service advisor initially asserted there was no warranty coverage for the issue and that it would cost nearly $500.00 to fix. He also claimed his entire shop of mechanics looked at the car and found nothing was broken. However, the very next day, the dealership abruptly reversed their diagnosis and instructed the driver to get a rental car, stating they would now be fixing the vehicle under warranty. The repair order confirms the technician's findings: "verified concern steering column separated stopped bracket bent and separated replaced column housing operating normal". The vehicle had 14,479 miles at the time of the failure. There were no prior warnings or symptoms before the incident. ... as happened in to motors in same vehicle. Both motor malfunctions verified by service centers. First motor was replaced at no cost to us by Ford Motor Company (FMC). They indicated they knew there was an issue with the motor and replaced it; however it took us 4 calls to FMC to get this acknowledgement and assistance. The first three times we called we were told that there was no known issue and it was our problem. Now that the replacement motor is doing the same thing, approximately 2 years/ 50,000 miles later we are in the same situation. Current calls to FMC indicate it is our problem even though the replacement motor is under warranty. Problem has been inspected by Ford dealership service center in addition to a private certified service center, AAA Auto Group. Check engine light appeared both times at the time of breakdown, not any period of time before. All of our documentation is sitting with the vehicle at a Ford dealership at the moment. If this is an issue we can continue wiht NHTSA we will get the service records uploaded ASAP.Read more

Power Window Master Switch Fire
On October 10, 2012, Toyota initiated a safety recall of approximately 2.5 million U.S. vehicles involving the following models and model years:2007-2009 Camry and Camry Hybrid, 2009 Corolla and Corolla Matrix, 2008 Highlander and Highlander Hybrid, 2007-2009 RAV4, 2008-2009 Scion xB and xD, 2008-20
... 09 Sequoia, 2007-2008 Tundra, 2007-2008 Yaris, and 2009 Pontiac Vibe.See NHTSA recall number 12V-491 for more details, including the affected vehicle manufacture dates for each vehicle model.Note that only early-production 2009 model year vehicles are included in the recall.Toyota and GM dealers will inspect the power window master switch (PWMS) assemblies and apply a specialized grease that inhibits heat build-up.The circuit board in the PWMS may be replaced with a new one if notchy or sticking feeling is observed during the inspection. The failure report summary above shows counts for the vehicles identified at the start of this Engineering Analysis, specifically the approximately 1.4 million vehicles that share an identical PWMS assembly design including, model year 2007-2009 Camry, Camry Hybrid, RAV4, and Yaris built from September 2006 through August 2008, as well as all 2008 Highlander Hybrid.Toyota subsequently identified approximately one million additional vehicles that have a slightly different PWMS design but contain the same defect condition.Note that the totals for complaints and fires shown above may include duplicative reports (between ODI and manufacturer reports). This Engineering Analysis is closed.See attached report for additional information. The ODI reports cited above can be viewed at www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification numbers: 10323848, 10360414, 10392622, 10394453, 10411328, 10429983, 10436663, 10446699, 10447416, 10447417, 10447419, 10447444, 10447515, 10447845, 10447947, 10449270, 10450133, 10450568, 10450606, 10450641, 10451821, 10455540, 10455614, 10456353, 10456438, 10457854, 10457955, 10459730, 10460166, 10460691, 10461220, 10462140, 10462200, 10462247, 10462404, 10462705, 10462761, 10464451, 10464769, 10465421, 10466279, 10466667, 10468126, 10468265, 10475230, 10470313, 10470684, 10471551, 10471824, 10472336, 10472910, 10473753, 10473967, 10473969, 10476921, 10477469, 10478563 and 10478657.Read more

Power Window Master Switch Fire
ODI has identified certain Toyota vehicles experiencing a higher rate of fire and thermal event incidents among all Toyota vehicles that use the same power window master switch design. They include a total of approximately 1.4 million model year (MY) 2007 through 2009 Camry, Camry Hybrid, RAV4 and Ya
... ris vehicles built from approximately September 2006 through August 2008, as well as all MY 2008 Highlander Hybrid vehicles.This Preliminary Evaluation has been upgraded to an Engineering Analysis (EA12-006) to further study the alleged defect and its consequences.The ODI reports cited above can be viewed at www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification numbers: 10461220, 10460691, 10460166, 10459730, 10457955, 10457854, 10456438, 10456353, 10455614, 10455540, 10451821, 10450641, 10450606, 10450568, 10450133, 10449270, 10447947, 10447845, 10447515, 10447444, 10447419, 10447417, 10447416, 10446699, 10436663, 10429983, 10411328, 10407290, 10394453, 10392622, 10360414, 10323848.Read more

ENGINE SURGING
THE PETITIONER OWNS A MY 2006 TOYOTA CAMRY WITH A 4-CYLINDER ENGINE THAT WAS PURCHASED NEW IN JANUARY 2006. THE PETITIONER ALSO PREVIOUSLY OWNED A MY 2005 CAMRY. HE ALLEGES THAT BOTH VEHICLES EXHIBITED VEHICLE ENGINE SURGING, WHICH HE DESCRIBED AS A SHORT DURATION (1 TO 2 SECOND) INCREASE IN ENGINE
... SPEED OCCURRING WHILE THE ACCELERATOR PEDAL IS NOT DEPRESSED. IN AN INITIAL INTERVIEW, THE PETITIONER ESTIMATED THAT 6 TO 8 SURGE INCIDENTS, OF VARYING SEVERITY, OCCURRED IN THE MY 2006 VEHICLE OVER THE COURSE OF 10,000 MILES AND 7 MONTHS OF OWNERSHIP. THE PETITIONER REPORTS THAT THE BRAKE SYSTEM IS EFFECTIVE AT OVERCOMING THE SURGE. HOWEVER, HE IS CONCERNED ABOUT REPORTS FILED WITH NHTSA ALLEGING UNCONTROLLEDSURGING IN MY 2002 TO 2006 CAMRY VEHICLES BRINGING THOSE VEHICLES TO A HIGH RATE OF SPEED (IN SOME CASES, PURPORTEDLY, WITH THE BRAKES APPLIED).AFTER REVIEW AND ANALYSIS OF THE AVAILABLE INFORMATION, ODI HAS NOT IDENTIFIED A VEHICLE-BASED DEFECT THAT WOULD HAVE PRODUCED THE ALLEGED ENGINE SURGE IN THE PETITIONER?S VEHICLE, NOR WAS IT ABLE TO WITNESS SUCH AN EVENT WHEN ROAD TESTING THE PETITIONER?S VEHICLE.EVALUATION OF A SUSPECT THROTTLE ACTUATOR REMOVED FROM THE PETITIONER?S VEHICLE DID NOT REVEAL A COMPONENT PROBLEM. WARRANTY AND PARTS SALES OF THE ACTUATOR ARE UNREMARKABLE.THESE DATA DO NOT SUPPORT THE EXISTENCE OF A WIDE-SPREAD DEFECT OR ONGOING CONCERN. THE FAULT DETECTION AND REACTION STRATEGY DESCRIBED IN TOYOTA?S TECHNICAL DOCUMENTS INDICATES THAT A LOSS OF THROTTLE CONTROL DUE TO A COMPONENT OR SYSTEM FAILURE WOULD BE DETECTED WITHIN A ONE SECOND PERIODAFTER WHICH ENGINE POWER WOULD BE LIMITED. THE PETITIONER?S MY 2006 VEHICLE BRAKE SYSTEM OVERCOMES FULL ENGINE POWER AT EASILY ACHIEVABLE BRAKE PEDAL FORCES. BASED ON THE ANALYSIS CONDUCTED, IT IS UNLIKELY THAT NHTSA WOULD ISSUE AN ORDER FOR THE NOTIFICATION AND REMEDY OF A SAFETY RELATED DEFECT AT THE CONCLUSION OF THE INVESTIGATION REQUESTED IN THE PETITION.CONSEQUENTLY, IN VIEW OF THE NEED TO ALLOCATE AND PRIORITIZE NHTSA?S LIMITED RESOURCES TO BEST ACCOMPLISH THE AGENCY?S SAFETY MISSION, THE PETITION IS DENIED.PLEASE SEE THE MARCH 9, 2007 FEDERAL REGISTER NOTICE FOR FURTHER DETAILS.Read more
