SOME COMBINATION LAMPS THAT ARE NOT EQUIPPED WITH AMBER SIDE REFLECTORS FAIL TO CONFORM TO FEDERAL MOTOR VEHICLE SAFETY STANDARD NO. 108, LAMPS, REFLECTIVE DEVICES, AND ASSOCIATED EQUIPMENT.

I WAS SLOWLY TURNING TO THE RIGHT INTO A PARKING SPACE AND HAD MY FOOT ON THE BRAKE WHEN THE CAR SUDDENLY ACCELERATED. FORTUNATELY, THERE WAS A TREE THERE TO STOP THE CAR. IN THE PREVIOUS MONTHS, SEVERAL OF THE PANEL WARNING LIGHTS HAD BEEN COMING ON AND OFF. I HAD TOYOTA CHECK THEM. THEY REPLACED THE EGR VALVE BUT IT KEPT HAPPENING SO I TOOK IT BACK. THEY SAID THEY CHECKED EVERYTHING AND COULDN'T FIND ANYTHING WRONG. I'M NOT SURE IF THIS HAS ANYTHING TO DO WITH THE CAR SUDDENLY ACCELERATING, BUT IT MIGHT HAVE.

ON 4/5/16, I WAS DRIVING ON THE HIGHWAY IN THE RIGHT LANE GOING STRAIGHT, I PUT CRUISE CONTROL ON AND IT WAS WORKING OKAY FOR APPROX 15 MINUTES. CAR SUDDENLY SPED UP TO 80 AND WENT UP IN FLAMES FROM UNDER THE HOOD WITHIN MINUTES. I WAS ABLE TO GET OUT OF THE CAR UNHARMED BUT IT IS A TOTAL LOSS.

I IMMEDIATELY DROVE THE VEHICLE TO MECHANICSVILLE TOYOTA (MECHANICSVILLE, VA) AND HAD THE PROBLEM ASSESSED BY THE SERVICE PERSON, [XXX]. SHE SAID SHE HAD SEEN THIS PROBLEM BEFORE AND THE CHANCES WERE PRETTY HIGH THAT THE BRAKE SWITCH (ABOVE THE BRAKE PEDAL) WAS DEFECTIVE. SHE SAID THAT CAUSES THE GEARSHIFT TO LOCK UP AND FOR THE BRAKE LIGHTS NOT TO WORK. INDEED MY BRAKE LIGHTS WERE NOT WORKING AT THAT TIME. THE CONTACT STATED THAT THE TIRE FAILURE WAS LOCATED ON THE FRONT PASSENGER SIDE OF THE VEHICLE. THE TREAD HAD SEPARATED FROM THE TIRE WHILE DRIVING 55 MPH. THE CONTACT CALLED AN INDEPENDENT MECHANIC TO COME AND INSTALL THE SPARE TIRE. THE CONTACT DROVE THE VEHICLE TO HER RESIDENCE AND HAD THE TIRE REPLACED. I ALSO TOLD THEM IT WAS MAKING A WHOO, WHOO, WHOO SOUND IN THE PASSENGER FRONT END AND THAT THE LAST TIME IT WAS MAKING A SOUND LIKE THAT I NEEDED A NEW AXLE. THEY CALLED ME SHORTLY AFTER I LEFT AND SAID THAT IT NEEDED A NEW AXLE. DIDN'T SAY WHY, JUST THAT IT NEEDED ONE. (INFLATED PRICING) I GAVE THEM APPROVAL TO INSTALL A NEW AXLE. 3 TIMES SINCE PURCHASING MY NEW 1994 CAMRY (V6 SEDAN), THE ENGINE HAS COMPLETELY DIED WHILE DRIVING. ALL POWER LOST TO ENGINE, POWER STEERING, AND POWER BRAKES. HAPPENED WHEN BRAND NEW. I WAS DRIVING DOWN THE 91 FREEWAY IN CA AND ALL OF A SUDDEN MY ACCELERATOR STUCK. I WAS FRANTIC TRYING TO STOP MY CAR. I PRESSED ON MY BRAKES AS HARD AS I COULD. IN THE MEANTIME I GOT OVER TO THE SIDE OF THE FREEWAY AND EXITED THE NEAREST OFF RAMP (CARMENITA). I DROVE THE STREETS FOR AWHILE AND CAME TO A RESIDENTIAL NEIGHBORHOOD. SOMETIME DURING THIS TIME THE PROBLEM SEEMED TO GET BETTER. I PULLED OVER TO A HOUSE WITH TWO WOMEN STANDING IN FRONT AND I WAS ABLE TO STOP AND ASKED THEM FOR HELP. ONCE I TURNED OFF THE ENGINE AND BACK ON AGAIN THE PROBLEM WAS GONE. I WAS ABLE TO CONTINUE TO MY DESTINATION WHICH WAS TO VISIT MY DAUGHTER IN THE HOSPITAL. UNDER THAT IT SAYS "00208" WITH A SPACE, FOLLOWED BY "32921". TO THE RIGHT OF THIS IS A GRID WITH YEARS 94, 95, AND 96 EACH FOLLOWED BY 12 SQUARES. THE YEAR 94 WITH THE FIRST BOX HAS A DOT IN THE MIDDLE, PRESUMABLY MADE IN JANUARY 1994. AT 67,320 MILES, UPON TURNING THE IGNITION KEY, THE ENGINE EXPLODED BELOW THE FILTER HOUSING AND THE CRANKSHAFT WAS VISIBLE. ENGINE REPLACEMENT WAS THE ONLY REMEDY. AT THE TIME OF THE FAILURE,I SUSPECTED THAT THE ENGINE FAILURE WAS DUE TO A DEFECT IN THE ENGINE BLOCK. I DID NOT KNOW THAT THE CAMRY ENGINE HAD SLUDGE-RELATED PROBLEMS. I HAVE ALL MAINTENANCE RECORDS ABOUT MY CAMRY THAT SHOW THAT OIL AND FILTER WAS CHANGED REGULARLY BY A TOYOTA DEALER OR COMPETENT CAR MECHANIC. THUS THE FAILURE WAS NOT CAUSED BY NEGLECT. ON APRIL 24, 2005, I SENT A LETTER TO TOYOTA CUSTOMER ASSISTANCE CENTER, TOYOTA MOTOR SALES, U.S.A., TORRANCE, CA 90501 THAT DESCRIBED THE EVENT AND REQUESTED REPLY. THERE WAS NO REPLY TO THIS DATE. WIFES CAR STARTED SMOKING AT 59,000 MILES. I WENT TO LOCAL TOYOTA AND THEY SAID THAT THE OIL HAD NOT BEEN CHANGED ON A REGULAR BASIS. I TOLD THEM THAT WAS NOT TRUE. I ASKED HOW MANY OTHERS ARE HAVING THE SAME TROUBLE AND THEY SAID THAT TOYOTA ENGINES DO NOT GIVE ANY PROBLEMS WHEN THE OIL IS CHANGED. SERVICE MANAGER TOLD ME IN SECRET THAT SOME OWNERS HAVE PUT TRANSMISSION FLUID IN THE OIL AS A DETERGENT AND BREAK UP THE SLUDGE. WE DROVE THE CAR UNTIL IT HAD 73196 MILES AND FOUND A TOYOTA SERVICE MANAGER WHO HAD A SHOP WHO QUOTED ME THE $2100.00. HE SAID IT WOULD COST $4,800.00 IF TOYOTA DID IT. I WAS FURIOUS WITH TOYOTA AND LET HIM DO IT. OIL CHANGES DONE APPROX EVERY 4000 MILES SINCE OWNERSHIP. PERSON AT FT WAYNE TOYOTA BOUGHT CAR TO PUT NEW ENGINE IN AND TOOK A SCOPE THROUGH IT- SAID ENGINE WAS FULL OF SLUDGE, METAL CHUNKS AND IT LOOKED LIKE THE RINGS HAD JUST SUDDENLY SNAPPED AND ENDED UP IN ENGINE.

I APPLIED TO BE CONSIDERED FOR THE OIL SLUDGE PROGRAM BUT WAS DENIED BECAUSE MY VEHICLE IS OLDER THAN THE MODELS IN THE OIL SLUDGE PROGRAM.
