This let the car wash pull my car throught the car wash on a one track driver side system. My windshield was covered with soap and the car surged forward and hit the SUV in front of me. When the soap rinsed off I wash under the car in front of me. I put the car in reverse from neutral. It caused no damage to the car in front but caused about $1700.00 damage to my car. They inform me the car will not accept after market parts & I'll need to purchase one from them instead for around $600. Yes. $600 for an O2 sensor. At only 129k miles, the transmission failed. The car stopped in the middle of an intersection & would not move forward, nearly causing me & my child to be hit. Mechanic informs me that it can be upwards of $7k to replace with a used transmission, $11k for new, & that's only WHEN they can get the part, which is on back order for MONTHS. This is because the supply can't keep up with the demand, as this model has widespread transmission issues. It was explicitly stated it is a "manufacturer defect." Yet, no recall. My online research absolutely supports that this is a large & very common problem that should warrant some responsibility from Toyota. It was on a rollback 4 times in 1 year. So, I was put in a position where I was forced to buy another car, since obviously I can't go without one for months. My 6 year old car (at time of breakdown) is now basically junk. This eliminates all rear visibility during backing maneuvers, posing a serious risk of striking pedestrians, cyclists, or objects — especially children in blind spots who cannot otherwise be seen. In addition, the infotainment screen has shut off entirely at least once while the vehicle was being driven, creating a dangerous distraction and loss of vehicle controls. I want to be clear that my dealership has acted in complete good faith and has done everything within their power to diagnose and resolve this issue. They have worked diligently on my behalf and are not the source of the problem. The failure to provide a fix lies squarely with Mitsubishi Motors North America, which has been unable or unwilling to supply an adequate repair, software update, or resolution despite being made fully aware of this ongoing safety defect. I have contacted Mitsubishi Motors North America directly on multiple occasions. They have failed to provide a satisfactory explanation, a committed timeline for a fix, or any path toward resolution. This is not a dealer-level issue — this is a manufacturer-level failure to address a known defect. This is not a convenience issue — the failure of a federally mandated backup camera system is a safety defect. Federal law (49 CFR Part 571, FMVSS 111) requires rear visibility systems to function properly at all times. I respectfully request that NHTSA open a formal investigation into this defect and determine whether a recall or technical service bulletin is warranted for the 2025 Mitsubishi Outlander PHEV.Read more

... fted hard since approximately 30,000 miles, but I was led to believe it was normal behavior for the 10-speed transmission. The vehicle has been lightly used and properly maintained through Ford. It is still on its original tires and brakes, which supports that this is not a high-abuse or heavy-use vehicle. A major transmission failure at 79,000 miles under normal use is not reasonable vehicle durability. The failure involves the CDF drum/cylinder, a known failure point in the Ford 10R80 transmission. This can cause harsh shifting, delayed engagement, gear skipping, slipping, abnormal acceleration behavior, and unpredictable shifting. These conditions create a safety concern when accelerating into traffic, merging, turning across lanes, passing, or driving at highway speeds. Ford has issued technical service guidance related to the 10R80 transmission and CDF drum/cylinder failure, including diagnostic procedures for hydraulic leakage and internal repair. This confirms the issue is known and identifiable, yet owners are still facing extremely expensive failures outside the standard warranty period. Another concern is that Ford is proposing remanufactured transmission replacements rather than new replacements, despite reports of repeat CDF-related failures after remanufactured units if updated CDF components are not included. This is not normal wear and tear. This is premature failure of a known defective transmission component that affects drivability, reliability, and safety. Ford should not leave customers responsible for potentially multiple $10,000+ repair costs for a known 10R80/CDF drum failure.Read more

The acceleration was strong enough that the vehicle jumped over the parking bumper with both front tires and struck a fixed post. Even after impact with the post, the vehicle continued to push forward as if still accelerating, until it hit another barrier and finally came to a stop. At no time did I press the accelerator pedal. The vehicle’s behavior felt completely uncontrolled and unsafe. This incident could have resulted in serious injury or death if it had occurred in traffic or with pedestrians in front of the vehicle. Safety Risk: The sudden loss of power at highway speeds created an immediate risk of a rear-end collision. Because the electronic transmission locked and would not allow the vehicle to be shifted into Neutral, it became a fixed object on a high-speed shoulder. THE PETITIONER OWNS A MY 2006 TOYOTA CAMRY WITH A 4-CYLINDER ENGINE THAT WAS PURCHASED NEW IN JANUARY 2006. THE PETITIONER ALSO PREVIOUSLY OWNED A MY 2005 CAMRY. HE ALLEGES THAT BOTH VEHICLES EXHIBITED VEHICLE ENGINE SURGING, WHICH HE DESCRIBED AS A SHORT DURATION (1 TO 2 SECOND) INCREASE IN ENGINE
... SPEED OCCURRING WHILE THE ACCELERATOR PEDAL IS NOT DEPRESSED. IN AN INITIAL INTERVIEW, THE PETITIONER ESTIMATED THAT 6 TO 8 SURGE INCIDENTS, OF VARYING SEVERITY, OCCURRED IN THE MY 2006 VEHICLE OVER THE COURSE OF 10,000 MILES AND 7 MONTHS OF OWNERSHIP. THE PETITIONER REPORTS THAT THE BRAKE SYSTEM IS EFFECTIVE AT OVERCOMING THE SURGE. HOWEVER, HE IS CONCERNED ABOUT REPORTS FILED WITH NHTSA ALLEGING UNCONTROLLEDSURGING IN MY 2002 TO 2006 CAMRY VEHICLES BRINGING THOSE VEHICLES TO A HIGH RATE OF SPEED (IN SOME CASES, PURPORTEDLY, WITH THE BRAKES APPLIED).AFTER REVIEW AND ANALYSIS OF THE AVAILABLE INFORMATION, ODI HAS NOT IDENTIFIED A VEHICLE-BASED DEFECT THAT WOULD HAVE PRODUCED THE ALLEGED ENGINE SURGE IN THE PETITIONER?S VEHICLE, NOR WAS IT ABLE TO WITNESS SUCH AN EVENT WHEN ROAD TESTING THE PETITIONER?S VEHICLE.EVALUATION OF A SUSPECT THROTTLE ACTUATOR REMOVED FROM THE PETITIONER?S VEHICLE DID NOT REVEAL A COMPONENT PROBLEM. WARRANTY AND PARTS SALES OF THE ACTUATOR ARE UNREMARKABLE.THESE DATA DO NOT SUPPORT THE EXISTENCE OF A WIDE-SPREAD DEFECT OR ONGOING CONCERN. THE FAULT DETECTION AND REACTION STRATEGY DESCRIBED IN TOYOTA?S TECHNICAL DOCUMENTS INDICATES THAT A LOSS OF THROTTLE CONTROL DUE TO A COMPONENT OR SYSTEM FAILURE WOULD BE DETECTED WITHIN A ONE SECOND PERIODAFTER WHICH ENGINE POWER WOULD BE LIMITED. THE PETITIONER?S MY 2006 VEHICLE BRAKE SYSTEM OVERCOMES FULL ENGINE POWER AT EASILY ACHIEVABLE BRAKE PEDAL FORCES.
