The contact's son-in-law owns a 2016 Ford Transit. The contact stated that while her son-in-law was driving at highway speeds, an abnormal, popping sound began to emit from the vehicle. The contact also stated that white steam began to emit from the engine as her son-in-law pulled over and had the v
... ehicle towed to an independent mechanic. The mechanic performed an inspection and determined that the driveshaft flexible coupling had cracked which lead to transmission damage. Upon investigation, the mechanic discovered NHTSA Campaign Number:19V767000(Power Train) which he linked to the failure. The contact then had the vehicle towed to a dealer where they acknowledged the recall; however, they informed the contact that the transmission was not covered under recall and the contact was given an estimate for the transmission repair. The contact contested that her son-in-law never received a recall notice about the recall; therefore, the manufacturer should be responsible for the repair. The manufacturer was notified of the failure and stated that it was the owner's responsibility to alert them of any address changes and they are not responsible for the contact not being aware of the recall. The contact was given a case number. The vehicle was not repaired. The failure mileage was approximately 105,000.Read more

In the Fall of 2024 we started having trouble with our F150 Ford Truck's transmission. It would drop gears, even from 6th gear to 1st gear, as the recall has stated, so we took it to someone who specifically deals with transmissions, and he stated that the truck was throwing a code that shows that t
... he transmission shift module or sensor was bad. He told us to check with Ford to see if there were any recalls and Ford told us that there were not. We had the sensor replaced, which cost us over $900, back in December of 2024. Well, the transmission continued to slip from 3rd to 4th gears constantly and would only occasionally drop gears, but is continuing to do so on a regular basis. We took the truck back to the transmission garage and he stated it would cost $5000 to rebuild the transmission. We have tried to call the Ford recall line, but just hangs up after giving us the same information the recall letter told us. So we have no one to report that we are having the issues we are having. So are we to continue to drive a truck that possibly could cause an accident on the highway or is there another solution to this problem other than waiting on Ford to come up with a fix possibly in July? On April 13, 20026 while traveling on a divided highway at about 70 mph, the car suddenly experienced rapid de-acceleration, similar to if someone slammed on the brakes. At the same time all dashboard warning lights began flashing. The car had failed and couldn't be driven. The Honda dealer in Roche
... ster Minnesota said the turbocharger had failed. This was a very dangerous situation as it posed a real risk of being hit from behind by others who were still travelling at high speed while the failed car was dramatically slowing and trying to get off the road. This an unreasonable risk of injury and death not only to the driver and occupants but also to the general public and other nearby drivers and occupants. The Honda dealer in Rochester inspected and repaired the failed turbocharger and flushed the system and oil as required. The charge was $5,042.41 to replace the turbocharger and $736.48 to flush and change with fresh oil. This is without tax. Full invoice is available upon request. The Honda dealer inspected the car. No police report was filed. The car was towed back into Rochester by AAA. There was no warning lamps prior to the incident. The cars maintenance was up-to-date and has only about 60,00 miles. For a [XXX] old car, an engine failure that creates a serious risk of injury or death to the driver and public would suggest a recall is necessary to maintain public safety and make owner's whole. Dear NHTSA Team, My car was in a minor accident Last. After that, the repair shop replaced my bumper and front camera. Don Franklin Somerset Hyundai performed a calibration on my new radar. They calibrated it six times, but there are still many issues with the HDA driving system. 1. False Dual-Si
... de Warning When a vehicle is present only on the right side, the vehicle camera system incorrectly detects vehicles on both the left and right sides. In reality, there is no vehicle on the left side. In this scenario, both the radar warning and the camera detection are incorrect. 2. Wrong-Side Warning In some cases, when a vehicle approaches from the left rear, no warning is issued on the left side, while a warning is incorrectly displayed on the right side. In this scenario, both the radar warning and the camera detection are incorrect. 3. False Warning During Smart Driving While driving on the highway using the smart driving function, when the vehicle is required to turn left or right, the system may generate a false dual-side warning. As a result, the vehicle aborts the intended left or right turn and continues driving in the center of the lane. In this scenario, both the radar warning and the camera detection are incorrect. 4. while driving manually (without HDA engaged) or parking, the vehicle shows left-side or right-side collision warnings without clear cause. I reported these issues to Hyundai USA approximately three weeks ago. I was instructed to bring the vehicle to the dealership for inspection. However, during my recent visit, the dealership representative only reviewed the videos I provided and concluded that no issues were present, without conducting a physical inspection or test drive of the vehicle. Given the ongoing safety concerns and lack of proper diagnosis or resolution, I am submitting this complaint for your review and assistance. Thank you for your attention to this matter. My 2024 Honda Prologue produces a rhythmic clicking and rattling noise from the front axle area whenever I make low‑speed left or right turns. The sound is tied to wheel rotation and is most noticeable during sharper turns such as entering or exiting a parking space. The noise is reproducible on e
... very drive and began early in the vehicle’s life. It appears consistent with the widely reported front CV joint/axle defect affecting other Prologue owners, many of whom describe the same clicking or ratcheting noise during turns. Honda has issued internal guidance acknowledging this symptom and advising dealers that the issue is under investigation. Because this involves the front drive axle/CV joint assembly, there is concern about premature wear or potential failure of a critical steering and drivetrain component, which could pose a safety risk if the condition worsens. 2013 BMW 328i, N20 engine. Timing chain guide assembly and oil pump drive chain failed catastrophically. Vehicle is available for inspection, however the authorized BMW dealership has already made clear they will accept no responsibility, and I would bear the expense of towing it two hours to their
... facility. The failure is a known issue with a class action case, but BMW took no responsibility, and falsified information stating the vehicle showed no signs of an issue when I brought my vehicle in for inspection. Eighteen months before failure, steam came from the engine compartment on a humid day, later connected to the water pump electrical connector recall. Following this, the vehicle ran consistently hotter than normal and lost noticeable power. Twelve months before failure, an intermittent oil pressure warning began triggering under load at highway speeds, particularly on inclines above 70mph. I brought the vehicle to Sandia BMW in Albuquerque, NM and reported all symptoms when I was contacted by them for the recall. The service advisor falsely claimed elevated operating temperature was optimal, suspected a faulty sensor, and returned the vehicle without offering further diagnosis or making me aware of the impending issue, all classic signs of the engine faults. Eight weeks later, the oil pressure light activated on a highway, followed immediately by a drivetrain malfunction warning, a violent grinding sound, and complete loss of power. The vehicle was towed and has not restarted since. I contacted BMW of North America's recall line. A case was opened and an advocate stated BMW would accept no responsibility because the class action was closed and I'mpast 70K miles. I raised the dealership's direct culpability. The advocate stated a dealership representative would contact me. No call was ever received. This failure is consistent with BMW Service Bulletin SI B11 03 17. A engine safety recall is warranted regardless of age to protect the public.Read more

This happens many times, at random intervals, and sometimes it does not happen at all, but I would say about 33% of the time, it happens. When you put the vehicle into reverse, the side mirrors will lower slightly, so that its easy to see the ground, or parking lines, etc. When you then go back

I am submitting this complaint regarding a serious transmission defect on my 2018 Acura RDX that materially worsened after an Acura-authorized warranty repair addressing a known torque converter defect. Acura issued a warranty extension / service campaign for the torque converter on certain 2017–2
... 018 RDX vehicles. Prior to this repair, my vehicle did not exhibit any severe jerking, slipping, or unsafe shifting behavior. After the Acura dealer replaced the torque converter under this program, the vehicle immediately began experiencing violent and unpredictable jerking during gear changes, worsening over time. These behaviors create an unsafe driving condition, particularly in stop-and-go traffic, intersections, and merging situations. I returned the vehicle to the dealer multiple times after the repair. As the problem worsened, I provided video documentation clearly showing the abnormal transmission behavior. After further evaluation, the dealer acknowledged the issue and determined that the transmission requires replacement. Acura Corporate declined coverage, stating the vehicle was no longer under standard warranty, despite: -The issue beginning immediately after Acura’s warranty repair -The torque converter being an integral component of the transmission system -The vehicle being in a worse and unsafe condition after Acura’s work I am requesting that NHTSA: -Review whether Acura’s torque converter warranty extension repair fails to adequately address associated transmission damage -Determine whether Acura vehicles repaired under this program are being returned to consumers in an unsafe operating condition -Assess whether Acura is improperly denying responsibility for post-repair transmission failures directly related to related to the covered defectRead more

... d: My 8 month old 2025 Chevrolet Equinox EV FRONT WHEEL DRIVE w/ACC(no Super Cruise) has had 2 instances where a very large "thunk" sound and vibration emanating from the rear of the car while stopped at an intersection, was immediately followed with sudden brake failure and sudden forward acceleration. The ACC was on in both instances. The first time this happened was alarming but since the was nothing in front of me there was no damage and the problem seemed to resolve itself thereafter. The second time was while waiting at a red light about 10 feet behind another vehicle. My car shot forward and into the other car, which was then pushed into the car ahead of it. Both my car and the car ahead of me had very expensive damages, but fortunately, no injuries. What I experienced sounds exactly like the recall problem described in NHTSA Safety Recall Report 25V-12 which has thus far only been applied to the AWD version of the 2025 Equinox EV, not the FWD version that I have. I believe this needs rapid consideration due to the danger of of sudden acceleration resulting in serious property damage, injury, or death. It appears to me that other instances of this problem have been described in various online forums and YouTubes.Read more

Two issues that I want to make known. First is a classic jeep issue with the death wobble. I currently have the jeep back in the shop for the current recall and having them fix the wobbling again. This would be the second time that it has happened. worried because the warranty is about to expire.
... Next is a problem that I almost forgot about until recently because I started driving a rental jeep wrangler. With my jeep, when I have the adaptive cruse control on, it some times thinks there is a car in front of me when it is in the next lane, and right in the middle of the highway, my jeep tries to stop because there is a car next to me in its own lane. This has been going on for a while now but i have never gotten it looked at because of the cost. im going to have both issues looked at during the recall because my extended warranty is ending and im worried that if i dont get it fixed now, im going to have to pay more in the future. Side notes about my jeep, it was a buy back before I purchased it. Was supposedly fixed. At around 10k miles I had a radiator issue and then needed to have a complete engine replacement. With both of these battery recalls, I have had to get a new battery. Im about to get by 3rd. There has also been hvac issues fixed in the past. I tried to contact jeep about another buy back, but they denied the claim without even talking to me. Currently working on trying to actually talk to someone.Read more

I am writing regarding my 2025 Toyota Sienna, purchased in June 2025, which is currently subject to Safety Recall Interim Notice 25TB12 concerning unsafe second-row seat welds that could result in serious injury or death. I appreciate the professionalism and concern shown by Sparks Toyota in acknow
... ledging the seriousness of this recall and providing a loaner vehicle while my Sienna is deemed unsafe to drive. However, the current loaner—a Toyota Highlander—does not meet my family’s needs and is not a reasonable substitute for the vehicle I purchased. We intentionally purchased a minivan due to our family size and lifestyle requirements. My daughter is a competitive gymnast, and we frequently travel with athletic equipment and multiple passengers. The Highlander is significantly smaller than a Sienna and cannot safely or practically accommodate our needs. Additionally, I am concerned that there is currently no repair solution or estimated completion timeline for this safety issue, leaving us indefinitely without access to a comparable vehicle. At this point, I am respectfully requesting the following: 1.Placement in a replacement vehicle comparable in size and function to a minivan (such as a Toyota Sienna or similarly sized vehicle). • If Sparks Toyota does not have an appropriate vehicle available, I request authorization for a comparable vehicle through an outside rental agency at Toyota’s expense. 2.Compensation for the ongoing inconvenience caused by the safety defect, lack of a repair timeline, and the inability to use the vehicle I purchased. 3.A clear estimate or update regarding when my vehicle is expected to be repaired and returned in a condition that is safe for my family. This recall involves a serious safety risk, and I believe it is reasonable to expect transportation that meets the same capacity and purpose as the vehicle I purchased—especially while awaiting a manufacturer-level remedy.Read more

The dealership refuses to inspect the system or try to recreate the problem despite the fact that there was a class-action suit against Honda for this exact CMBS malfunction in several other models. They just tell me to manually turn it off every time I get in the car. I am human and sometimes I forget, so this is not a solution. I'm afraid that this buggy "safety" system is going to get me and my child killed one of these days. I am happy to have the system inspected, but it doesn't keep a log of every time it activates so there's no real way to verify the false positives, and it happens randomly so it's very hard to recreate.Read more

The vehicle is affected by two unresolved safety recalls involving a high-voltage battery fire risk and potential engine contamination that may cause sudden loss of propulsion or catastrophic engine failure. The manufacturer has instructed owners not to charge the vehicle and to park it away from st
... ructures or other vehicles due to the fire hazard. No repair or remedy is currently available for either recall, and the vehicle remains in this unsafe condition. In addition to the recall issues, the vehicle has an ongoing electrical malfunction involving the infotainment display and backup camera. During normal driving, the screen frequently turns bright green, emits a loud screeching noise, and cannot be muted or controlled. The screen and backup camera also freeze intermittently while reversing, creating visibility and distraction hazards. This issue has occurred repeatedly over the past year. The problems place the safety of the occupants and the public at risk. The inability to charge the vehicle, the possibility of sudden engine failure, and the risk of fire make the vehicle unsafe to operate or park near homes, workplaces, or other vehicles. The infotainment and camera failures also impair navigation and rear visibility. The problems have been reported to the dealer, who previously acknowledged the infotainment issue and indicated the system would be replaced, but no repair was completed. Recent attempts to obtain service or a loaner vehicle have been unsuccessful. The dealer has confirmed that no remedy is available for the safety recalls. Warning symptoms include sudden screen discoloration, loud audio distortion, frozen rear-camera image, and intermittent loss of display functions. The recall-related safety hazards occur without warning. The issues have resulted in significant loss of use of the vehicle, and the manufacturer has not provided a timeline for repairs. The vehicle is available for inspection at any time.Read more

The contact owns a 2015 Ford Explorer. The contact stated that the exterior A-pillar trim had detached, allowing water into the vehicle after rainfall, and the infotainment screen was foggy. The contact later received notification of NHTSA Campaign Number: 24V031000 (Structure); however, the part to
... do the recall repair was not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The dealer was made aware of the failure but confirmed that parts were not yet available. The vehicle was not repaired. The manufacturer was made aware of the failure. The main water pump on my 2018 Porsche Panamera 4S failed prematurely. This is not a simple external coolant leak; the internal seal failed, allowing pressurized engine coolant to migrate into the vehicle's vacuum system. The coolant traveled through the vacuum lines and contaminated the vacuum sol
... enoids and change-over valves. This failure mode presents a significant safety hazard because the vacuum system controls critical engine components, including turbocharger wastegates and exhaust flaps. Contamination of these components can lead to sudden loss of power, "limp mode" activation while driving at highway speeds, or unexpected engine behavior, increasing the risk of a crash. Cross-Reference to Known Defects: This vehicle utilizes the EA839 V6 engine platform, which is shared with Volkswagen and Audi vehicles (specifically the Audi S4, S5, SQ5, and Q7). Audi and Volkswagen have already acknowledged this specific manufacturing defect in the class action settlement Fiscina v. Volkswagen Group of America, Inc. (Case No. 2:21-cv-11251). Under that settlement, Audi extended the warranty on this water pump to 10 years/80,000+ miles for the exact same failure mode (coolant migration into the vacuum system). Despite my vehicle using the same engine and suffering the exact same mechanical failure, Porsche refuses to offer coverage or acknowledge the defect. I am requesting an investigation into why Porsche models equipped with the defective EA839 engine are excluded from the safety remedies and warranty extensions provided to Audi owners.Read more

My explorer is only 4 years old and it suddenly suffered catastrophic engine failure due to low engine oil. The engine started knokcing and clattering BEFORE any warning lights came on the dash. This could also be due to premature bearing failure which is common in the 3.0 ecboost engines.

My VIN number is not included in the backup camera recall. It says the recall includes the 2017 Transit. My back up camera stopped working about a year ago. Also when the camera stopped working my cruise control stopped working also. I believe this is a larger electrical issue.

Power Window Master Switch Fire
On October 10, 2012, Toyota initiated a safety recall of approximately 2.5 million U.S. vehicles involving the following models and model years:2007-2009 Camry and Camry Hybrid, 2009 Corolla and Corolla Matrix, 2008 Highlander and Highlander Hybrid, 2007-2009 RAV4, 2008-2009 Scion xB and xD, 2008-20
... 09 Sequoia, 2007-2008 Tundra, 2007-2008 Yaris, and 2009 Pontiac Vibe.See NHTSA recall number 12V-491 for more details, including the affected vehicle manufacture dates for each vehicle model.Note that only early-production 2009 model year vehicles are included in the recall.Toyota and GM dealers will inspect the power window master switch (PWMS) assemblies and apply a specialized grease that inhibits heat build-up.The circuit board in the PWMS may be replaced with a new one if notchy or sticking feeling is observed during the inspection. The failure report summary above shows counts for the vehicles identified at the start of this Engineering Analysis, specifically the approximately 1.4 million vehicles that share an identical PWMS assembly design including, model year 2007-2009 Camry, Camry Hybrid, RAV4, and Yaris built from September 2006 through August 2008, as well as all 2008 Highlander Hybrid.Toyota subsequently identified approximately one million additional vehicles that have a slightly different PWMS design but contain the same defect condition.Note that the totals for complaints and fires shown above may include duplicative reports (between ODI and manufacturer reports). This Engineering Analysis is closed.See attached report for additional information. The ODI reports cited above can be viewed at www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification numbers: 10323848, 10360414, 10392622, 10394453, 10411328, 10429983, 10436663, 10446699, 10447416, 10447417, 10447419, 10447444, 10447515, 10447845, 10447947, 10449270, 10450133, 10450568, 10450606, 10450641, 10451821, 10455540, 10455614, 10456353, 10456438, 10457854, 10457955, 10459730, 10460166, 10460691, 10461220, 10462140, 10462200, 10462247, 10462404, 10462705, 10462761, 10464451, 10464769, 10465421, 10466279, 10466667, 10468126, 10468265, 10475230, 10470313, 10470684, 10471551, 10471824, 10472336, 10472910, 10473753, 10473967, 10473969, 10476921, 10477469, 10478563 and 10478657.Read more

Power Window Master Switch Fire
ODI has identified certain Toyota vehicles experiencing a higher rate of fire and thermal event incidents among all Toyota vehicles that use the same power window master switch design. They include a total of approximately 1.4 million model year (MY) 2007 through 2009 Camry, Camry Hybrid, RAV4 and Ya
... ris vehicles built from approximately September 2006 through August 2008, as well as all MY 2008 Highlander Hybrid vehicles.This Preliminary Evaluation has been upgraded to an Engineering Analysis (EA12-006) to further study the alleged defect and its consequences.The ODI reports cited above can be viewed at www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification numbers: 10461220, 10460691, 10460166, 10459730, 10457955, 10457854, 10456438, 10456353, 10455614, 10455540, 10451821, 10450641, 10450606, 10450568, 10450133, 10449270, 10447947, 10447845, 10447515, 10447444, 10447419, 10447417, 10447416, 10446699, 10436663, 10429983, 10411328, 10407290, 10394453, 10392622, 10360414, 10323848.Read more

ENGINE SURGING
THE PETITIONER OWNS A MY 2006 TOYOTA CAMRY WITH A 4-CYLINDER ENGINE THAT WAS PURCHASED NEW IN JANUARY 2006. THE PETITIONER ALSO PREVIOUSLY OWNED A MY 2005 CAMRY. HE ALLEGES THAT BOTH VEHICLES EXHIBITED VEHICLE ENGINE SURGING, WHICH HE DESCRIBED AS A SHORT DURATION (1 TO 2 SECOND) INCREASE IN ENGINE
... SPEED OCCURRING WHILE THE ACCELERATOR PEDAL IS NOT DEPRESSED. IN AN INITIAL INTERVIEW, THE PETITIONER ESTIMATED THAT 6 TO 8 SURGE INCIDENTS, OF VARYING SEVERITY, OCCURRED IN THE MY 2006 VEHICLE OVER THE COURSE OF 10,000 MILES AND 7 MONTHS OF OWNERSHIP. THE PETITIONER REPORTS THAT THE BRAKE SYSTEM IS EFFECTIVE AT OVERCOMING THE SURGE. HOWEVER, HE IS CONCERNED ABOUT REPORTS FILED WITH NHTSA ALLEGING UNCONTROLLEDSURGING IN MY 2002 TO 2006 CAMRY VEHICLES BRINGING THOSE VEHICLES TO A HIGH RATE OF SPEED (IN SOME CASES, PURPORTEDLY, WITH THE BRAKES APPLIED).AFTER REVIEW AND ANALYSIS OF THE AVAILABLE INFORMATION, ODI HAS NOT IDENTIFIED A VEHICLE-BASED DEFECT THAT WOULD HAVE PRODUCED THE ALLEGED ENGINE SURGE IN THE PETITIONER?S VEHICLE, NOR WAS IT ABLE TO WITNESS SUCH AN EVENT WHEN ROAD TESTING THE PETITIONER?S VEHICLE.EVALUATION OF A SUSPECT THROTTLE ACTUATOR REMOVED FROM THE PETITIONER?S VEHICLE DID NOT REVEAL A COMPONENT PROBLEM. WARRANTY AND PARTS SALES OF THE ACTUATOR ARE UNREMARKABLE.THESE DATA DO NOT SUPPORT THE EXISTENCE OF A WIDE-SPREAD DEFECT OR ONGOING CONCERN. THE FAULT DETECTION AND REACTION STRATEGY DESCRIBED IN TOYOTA?S TECHNICAL DOCUMENTS INDICATES THAT A LOSS OF THROTTLE CONTROL DUE TO A COMPONENT OR SYSTEM FAILURE WOULD BE DETECTED WITHIN A ONE SECOND PERIODAFTER WHICH ENGINE POWER WOULD BE LIMITED. THE PETITIONER?S MY 2006 VEHICLE BRAKE SYSTEM OVERCOMES FULL ENGINE POWER AT EASILY ACHIEVABLE BRAKE PEDAL FORCES. BASED ON THE ANALYSIS CONDUCTED, IT IS UNLIKELY THAT NHTSA WOULD ISSUE AN ORDER FOR THE NOTIFICATION AND REMEDY OF A SAFETY RELATED DEFECT AT THE CONCLUSION OF THE INVESTIGATION REQUESTED IN THE PETITION.CONSEQUENTLY, IN VIEW OF THE NEED TO ALLOCATE AND PRIORITIZE NHTSA?S LIMITED RESOURCES TO BEST ACCOMPLISH THE AGENCY?S SAFETY MISSION, THE PETITION IS DENIED.PLEASE SEE THE MARCH 9, 2007 FEDERAL REGISTER NOTICE FOR FURTHER DETAILS.Read more
