I own a 2021 Honda Accord Hybrid. Shortly after purchasing the vehicle as a used vehicle in 2023, I began experiencing intermittent air conditioning performance. The A/C would gradually lose cooling capacity over time until it was no longer able to cool the cabin effectively. To temporarily restore
... operation, I added refrigerant approximately twice per year, believing the system had developed a minor leak. As the condition worsened, I brought the vehicle to an authorized Honda dealership for diagnosis. The dealer diagnosed the vehicle with a leaking A/C condenser and a leaking heat exchanger pipe and recommended replacement of both components. Honda declined goodwill assistance because the vehicle is outside the original warranty period. I later learned that Honda previously issued an extended warranty for A/C condenser failures on 2018–2020 Accord models due to a known condenser defect. My vehicle exhibits the same symptoms of gradual refrigerant loss and declining cooling performance, despite being a 2021 model year. In researching this issue, I found multiple reports from other 2021 Accord and 2021 Accord Hybrid owners describing similar A/C condenser failures and refrigerant leaks. I am submitting this complaint because I believe there may be a broader pattern affecting 2021 Accord vehicles that warrants review. I am requesting that NHTSA document this complaint and evaluate whether similar failures are occurring in 2021 Honda Accord and Accord Hybrid vehicles.Read more

Seat belt on driver's side is not coming out smoothly. It will remain locked or come out only a few inches. It takes many tries before you can put the seatbelt on. It clicks out incrementally or sometimes it goes on smoothly with no problems. Once seat belt is on the driver, it locks even when the d
... river isn't braking or pushing break pedal. The seatbelt may also sometimes be locked when braking and the belt will slip out a couple of clicks allowing forward movement of driver. Seatbelt malfuntion or lack of predictability is a safety issue. This problem has been happening for 3 weeks- at first I thought it was high temperature related and just quirky but it happens throught the day across cooler evenings. I have not had it inspected yet but will be taking to the dealership in the next week or so. No pervasive lights are on indicating an issue. Occasionally my airbag light will come on for a trip and go back off before next start up. It is available for inspection upon request.Read more

The Automatic Emergency Braking (AEB) system activates unexpectedly and dangerously. Incident 1 5/7/2026: While driving approximately 2 mph in a drive-thru and rounding a curve, the vehicle suddenly applied full braking without warning. The braking force was severe enough that I struck my face on th
... e steering wheel and my child was thrown forward in a secured car seat. Incident 2 5/19/2026: While merging onto a busy road behind a semi truck, the vehicle again applied sudden hard braking after I safely merged. This caused the vehicle behind me to nearly rear-end me, creating a high-risk traffic situation. These events occurred with no obstacle or reason that would justify emergency braking. In addition, the vehicle developed a coolant leak around 20,000 miles, raising concerns about overall reliability and potential related system issues. The unexpected braking presents a serious safety hazard and increases the risk of rear-end collisions and injury. The vehicle is being brought to a dealer for inspection, but the issue is intermittent and may not be easily reproduced.Read more

Incident date: Oct 7, 2025
Initial Oil Change Visit: Brought leased vehicle under full warranty to the dealership for its first oil change. No issues were mentioned at this time. First Return (After 5 Minutes Driving): After leaving the dealership, warning lights and unusual sounds began within 5 minutes of driving. Returned
... immediately. The service team claimed the issue was due to the 'oil gun not being recalibrated' and that they had topped the oil off using a jug. This was later revealed to be a lie. Second Return (After Another 5 Minutes Driving): The same warning lights and noises returned after driving only 5 minutes. Returned again to the dealership. This time, they admitted that no oil had ever been put into the vehicle after the oil was drained. The service manager stated that the engine was 'cooked' and would need to be replaced. Admission by Service Manager: The service manager personally drove me to the rental car location. During this time, he admitted that the technician had simply forgotten to refill the oil after draining it, stating they had 'just gotten busy.' He had no explanation for why the dealership had lied during the first return visit. Safety Concerns: This negligence could have had catastrophic consequences. If the engine had blown up while driving, I could have crashed, potentially injuring or killing myself, my pregnant fiancée who was with me, and our unborn child. This was not just an inconvenience but a severe, reckless breach of safety and trust. Dealer’s Resolution: The dealership later acknowledged fault and offered to replace the engine with a new one. However, given the severity of the negligence and the safety risk, we refused and ultimately walked away from the lease when they would not provide an equivalent vehicle under the same terms. They instead paid off the remaining lease balance ($42,286.94). -Sent certifed formal demand letters to dealer and manufacturer -sent follow up emails to both -have not received response to my complaints.Read more

I am writing to file a formal complaint regarding a safety-related recall (H514/H536) issued by Jaguar Land Rover North America, LLC for my 2019 Jaguar I-PACE. While Jaguar acknowledged the defect and offered a vehicle repurchase, I believe their resolution places an unfair financial burden on me, t

Jaguar has offered me a “Statutory Refund” of $31,987.25, while my vehicle loan payoff is $44,060.98—leaving me responsible for $12,073.73 out of pocket. I find this unacceptable given that the defect is not due to misuse or wear but rather a confirmed manufacturer issue under NHTSA Safety Recall H514/H536. This situation has created significant financial stress and emotional strain. I have not been negligent or abusive with the vehicle. I am simply a consumer seeking safe and fair resolution. It seems inappropriate that I should be penalized for Jaguar’s faulty product by being left in debt on a car I can no longer safely use. I respectfully request that NHTSA: •Review Jaguar’s handling of this recall and repurchase process •Investigate whether other owners are receiving similarly inadequate offers •Provide guidance on how consumers like me can be protected from incurring thousands in losses due to a confirmed manufacturer safety defect Thank you for your time and advocacy on behalf of consumer safety and fairness. ... the truck rolled forward a bit when I attempted to accelerate. I then waited for a very large opening and attempted to accelerate again. Pushed the pedal and again nothing. I then pressed it to the floor and held it there. The truck very slowly began to accelerate and then after about 20 seconds accelerated like normal. The truck seemed normal the rest of the way home.Read more

At [XXX] on Tuesday [XXX] while driving our 2023 Ford Mustang Mach E GT eastbound in the left lane on [XXX] near the [XXX] intersection the car suddenly stopped. There was a second or two of multiple warning notices then the car instrument panel went blank. The car “bricked” as is known in Ele
... ctric Vehicle jargon. There was no power to anything, and the stop was sudden and steering was extremely difficult. Washington State Patrol and Washington Department of Transportation responded to provide safety assistance while eastbound traffic on [XXX] backed up for several miles. The car is now at Frontier Ford in Anacortes Washington and they have confirmed its status as completely dead. In fact it took almost an hour to get the car off the tow truck. The dealership tried several battery jumper options to the 12v battery but when they worked it seemed to only work for a few seconds before the car “bricked” again. There is probably a WSP police and WSDOT report about responding to this incident. In the seconds before the complete electrical shutdown there was a cascade of warning notices then everything went blank and the car abruptly stopped in traffic. It took over an hour to get a tow truck and then the cars “bricked” status made the towing job very difficult and eventually the car was literally dragged onto the flatbed with a winch. This incident happened during rush hour in heavy traffic and both occupants safely was put at high risk of a high speed collision from traffic approaching from the rear of the car. We have made two reports to Ford Customer Service. Case # [XXX] And case # [XXX] The suddenness and inability to maneuver and get out of traffic is a real safety problem that can lead to a severe accident. After the update to the Polestar 2's software to version 3.6.4, which was released to fix the rear camera not working when in reverse, the rear camera still will not work on occasion. The vehicle has not been inspected, but can be if needed. There were no errors or warnings aside from the "Camera i
... s temporarily unavailable" message on the main screen. The camera will likely work if the car is taken out of and put back in to reverse. Previous recall notice: [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Component or system that failed: Electric Drive System / Charging System / Software – repeated failure displaying “EV System Off” message, rendering vehicle inoperable. Description of incident: This is the second time within 30 days that my 2025 Nissan Ariya (VIN: [XXX] ) has completely shut
... down and become undriveable. On [XXX], while attempting to use a DC fast charger at Electrify America (1150 E Flamingo Rd, Las Vegas, NV), the vehicle failed to begin charging. After a few attempts, the dash displayed “EV System Off” and the car could not be restarted or shifted into gear. It had to be towed again — same as the incident on [XXX], which occurred under nearly identical circumstances. Safety risk: This failure left me stranded twice, with no warning or ability to drive. Had this happened on a freeway or in a rural area, it could have been dangerous. The dealer has only reset the vehicle each time with no resolution. Reproduction and inspection: The issue was inspected by Findlay Nissan Henderson. They stated that other Ariya owners have reported similar failures, especially after using Electrify America stations, and implied it may be a compatibility issue — yet this is not publicly disclosed by Nissan. I have charged successfully at Electrify America many times before, and the same charger worked immediately after my failure. Warning signs: No warning lights appeared before the failure. The “EV System Off” message appeared suddenly after connecting to the charger. Summary: This is a critical systems failure that recurs, impacts safety, and limits reliable use of the vehicle. I request this be formally investigated for a potential design or software flaw. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The contact owns a 2017 Ford Fusion. The contact stated that the vehicle was stuck in gear while accelerating from a stop. The contact stated that the vehicle failed to move. The brake and ABS warning lights were illuminated. The vehicle was towed to an independent mechanic, who determined that the
... transmission bushing needed to be replaced. The vehicle was repaired, but the failure recurred. While reversing, the vehicle became stuck in the street. The contacts' son was able to maneuver the gears, and the vehicle started moving. The local dealer was contacted, but the vehicle was not diagnosed or repaired. The manufacturer was contacted, opened a case, and referred the contact to the NHTSA Hotline. The failure mileage was approximately 97,000.Read more
