The Styrofoam is cracked in half on two sides and it's losing pieces when I contacted the manufacturer. They said they were sending me tape and I don't feel that's very safe. I just wanted to check on this because I don't feel it's very safe for me to tape my child car seat back together especially since there's pieces missing chunks and the Styrofoam is completely cracked in half. I could separate the two pieces and they're telling me to tape it back together.Read more

(NHTSA Tracking Number 11726105) I am updating my existing complaint regarding my 2024 Hyundai Tucson (NHTSA Tracking Number 11726105) for an ongoing safety concern involving an intermittent warning chime while driving. This issue began shortly after I purchased the vehicle in Nov 2025 and has been
... reported to the dealership multiple times over several months. The warning chime occurs randomly while driving without any visible warning message or dashboard alert. This creates uncertainty because I do not know what the vehicle is trying to warn me about, which is distracting and creates a safety concern while driving. The dealership previously replaced both front seatbelt buckle assemblies after identifying intermittent high resistance as a possible cause. However, after that repair was completed, the warning chime occurred again while I was driving to the dealership for a follow-up visit. This confirmed that the issue was not fully resolved. I immediately returned the vehicle to the dealership, and they are now performing further diagnosis. I was advised that if the issue reoccurred, the next possible areas for diagnosis could involve the instrument cluster or the control module, both of which are safety-related systems connected to warning alerts and seatbelt functions. In addition, during this same visit, the dealership confirmed there is an actual oil leak requiring repair. This is significant because when I first raised this concern after an outside inspection in December 2025, I was told it was likely leftover residue from a prior oil change. The dealership has now confirmed it is a real leak and kept the vehicle for repair while providing a loaner vehicle. Because this vehicle was sold as Certified Pre-Owned and these safety and mechanical concerns continue shortly after purchase, I remain concerned about the reliability and safety of the vehicle.Read more

The contact owns a 2018 Honda Accord. The contact stated that while driving at various speeds, the vehicle either hesitated or stalled with the "Emissions System Fault", "Engine Power Reduced", "See your Dealer" messages displayed. The contact then received notification of NHTSA Campaign Number: 23V

The vehicle was taken back to the dealer approximately six times after the recall repair. The spark plugs, ignition coils, and turbo charger were replaced after several visits; however, failure persisted. The vehicle was also taken to an independent mechanic, and the battery was replaced but the failure persisted. The vehicle was taken back to the dealer, and the contact was informed that the wiring harness and head gasket needed to be replaced. The manufacturer was notified of the failure and provided a case number, but offered no additional assistance. The vehicle remained with the dealer unrepaired. INCIDENT SUMMARY On April 16, 2026, at approximately 2:06 PM local time, my 2022 Tesla Model Y was traveling on a multi-lane divided roadway in the Austin/Lakeway, Texas area with Autopilot engaged. Traffic in my direction of travel was heavily congested and traveling with 32 mph. Without warning,
... alert, or any input from me, Autopilot commanded an uncommanded left steering maneuver. The vehicle veered left out of its lane and into a painted buffer separator area. The direction of travel pointed directly toward a building, an electrical transformer/utility box, and a landscaped area containing mature trees. I immediately recognized the unexpected maneuver and manually gripped the steering wheel to override Autopilot. I steered the vehicle to avoid collision with the building and the electrical box, both of which posed serious collision and electrocution risks. In the approximately 1.5 to 2 seconds available to me, I successfully redirected the vehicle away from the building and the transformer. The vehicle came to rest after striking a tree in the landscaped area adjacent to the road. The vehicle was totaled. Driver and curtain airbags deployed. I was wearing my seatbelt, was alert, and sustained no major injuries as a result of my intervention and the successful redirection of the vehicle.Read more

I had my front axle assemblies replace under T-SB-0082-24 around 20,000 to 25,000 due to rotational clicking noise per bulletin. The tsb uses the old part number which was the defective original part. The axles are failing agin at 50,000 and the dealer and toyota are refusing to fix it as they ca

On the left side of the steering wheel there is a group of Settings buttons with a pressure sensitive dial, called the OK button, that controls which menu you choose. The button no longer allows me to select a menu option because the depression sensor does not spring back. For instance, if I want to
... adjust the driver assistance options, I need use the OK button to scroll through the options to find the setting I want, then depress the OK button to select it. However, the OK button sticks with the slightest pressure instead of scrolling. This makes it impossible to change or adjust any of the safety features and there is no other option to manually adjust them. I am unable to turn on the blind spot warning and lane departure warning. I am reporting a defect with the driver-side sun visor on my 2019 Jeep Wrangler Unlimited. The sun visor mounting mechanism is faulty and has broken/detached during normal use. When this occurs, the visor drops down unexpectedly and obstructs the driver’s forward view. This creates a serious safet
... y hazard, as the visor can suddenly block visibility while the vehicle is in motion, increasing the risk of a crash. The failure happened without excessive force or misuse, only routine flipping and positioning of the visor. I am aware that many other owners of 2019 Jeep Wrangler Unlimited vehicles have experienced the same sun visor failure, suggesting a widespread defect in the design or materials used. This appears to be a known and recurring issue affecting multiple vehicles of the same model year. I believe this defect should be investigated as a safety-related issue and considered for a recall, as it directly impacts driver visibility and safe vehicle operation.Read more

The vehicle has been taken to the dealership at least 4 times. Each time for the same reason: when driving at 65 or higher, it sounds like it's on a flat. My screen also glitches when foot is taken off the break after a stop. The service advisor told me that there was a shortage of mechanics and ask
... ed me to bring it back next week (this is the response I have gotten each time). I have asked for a loaner twice and was told that there were none available. Therefore, I had to pay $40 to uber to work. Today, I said that I couldn't come back because it's the same issue and I have to work next week. He then let me speak to a service manager. The service manager drove it (with me in the passenger seat) and heard the sound. He noticed that it was not all of the time and that my steering wheel was a little shifty. When it was finally "fixed" (tires being rotated and some steering update), I was told to come back if I hear it again. I left at about 2pm and heard the sound again, as well as my screen began to glitch when I get off the break after a complete stop. I took a video of the glitching and took it back for both issues. I was told that some bolts were tightened and some update was done to the screen, and if I hear it again come back. On my way home at 4:35pm, the issues were not resolved.Read more

While driving my brand new 4-day old 2025 Audi Q3 at highway speeds, the vehicle suddenly lost engine power and shut off without warning. At the time of the incident the fuel gauge indicated approximately 1/4 tank remaining and there were no prior warnings. I was forced to pull over in traffic, crea
... ting a dangerous situation. The vehicle had to be towed to the dealership. I was initially told the vehicle had "run out of gas", despite the fuel gauge showing fuel remaining. This same failure occurred a second time under similar conditions, again while driving at highway speeds with fuel indicated on the gauge. The vehicle stalled and shut off unexpectedly, requiring another tow. During the second repair visit, Audi technicians confirmed that the fuel tank's internal level sensor was defective and that the vehicle was inaccurately reporting fuel levels. Audi's Technical Assistance Center instructed the dealer to replace the entire fuel tank assembly on this brand-new vehicle. This defect caused repeated loss of engine power while driving, creating a serious safety risk to myself and other motorists. I no longer feel the vehicle is safe to operate due to the risk of sudden stalling without warning.Read more

My 2019 BMW X7 is under Safety Recall 25V-644 for an overheating/fire-risk defect, but the recall remedy is unavailable. The vehicle has now experienced multiple safety-related failures that appear to be progressing. BMW diagnosed the following failures: • Emergency Call System Malfunction • GPS
... and maps not working • Failure of the TCB telematics module • Failure of the antenna and TCB battery due to fluid ingress • Battery voltage in the red zone • All four tire pressure sensors beginning to fail • Collapsed engine mounts attributed to heat • $11,296 in repair estimates related to electrical and heat damage Immediately after picking up the vehicle from BMW service, a new battery/charging system warning light appeared that was NOT present before the visit. This suggests further deterioration of the electrical system. The combination of: • an open fire-risk recall with no fix available, • fluid ingress damaging telematics components, • multiple electrical and safety-system failures, and • new warning lights appearing after service makes this vehicle unsafe to operate, especially for long-distance highway travel. I am requesting that NHTSA document this as a safety-defect case and assist in ensuring BMW provides a safe resolution.Read more
