While driving on a high-speed interstate with two children in the vehicle, and without any prior warning signs or issues, the dashboard began displaying sequential warnings from multiple Advanced Driver Assistance Systems (ADAS) sensors. ... rnings, the power steering failed, making the vehicle difficult to control. Additional electronic systems also began malfunctioning, including the display screen, cameras, and safety sensors, while the driver attempted to safely maneuver the vehicle to the shoulder of the road. After reaching the shoulder, the driver powered the vehicle off in an attempt to reset the electrical systems. However, once powered down, the engine would no longer start. When the tow truck arrived, the technician was required to jump-start the battery in order to shift the vehicle into neutral for towing. The technician also showed the driver a low-voltage reading on the jump-start device, indicating a possible alternator or charging-system failure. Hyundai dealership representatives later stated that diagnostic testing of the alternator and electrical system showed no faults and informed the customers that the battery was simply dead. After the dealership replaced the battery, the customers drove the vehicle approximately 15 miles to their residence. During that drive, the vehicle again began displaying the same sequence of sensor and system failures. The power steering failed a second time, and the same electrical malfunctions experienced during the initial incident were repeated.Read more

I am filing a safety complaint regarding an Engine Control Module (ECM) failure in my 2017 Subaru Impreza and the handling of a prior recall repair. Dealership records show that a recall repair (WRE21 – ignition coil recall) was completed in June 2021. At that time, the dealership documented that

Diagnostic records from February 2026 indicate: •Camshaft slow response codes •Persistent sensor issues •A technician recommendation to replace multiple components, including the ECM, stating that codes were “burned into the ECM” The dealership has since attempted to attribute the ECM failure to “engine sludge” and lack of maintenance. Concern with Dealership Explanation: This explanation is inconsistent and raises serious concerns: •Engine sludge is a mechanical lubrication issue and does not directly cause an ECM (electronic control module) to fail or become “burned.” •The ECM is an electronic unit, and failure typically results from electrical defects, internal faults, or manufacturing issues—not oil sludge. •The dealership has not provided technical evidence showing how sludge could damage or “burn” the ECM. •This appears to be an attempt to shift responsibility away from a potential defect or improperly completed recall repair. Safety Concern: The ECM is critical to safe vehicle operation. ... rd Escape was subject to Safety Recall 14S05 (NHTSA Campaign 14V-284000), which addresses sudden loss of electric power steering assist due to a torque sensor fault. This condition can occur while driving and results in significantly increased steering effort, creating a safety risk. Ford records show the recall was marked “completed” on November 22, 2014. In a written response dated December 22, 2025, Ford Motor Company confirmed that the recall remedy consisted only of software reprogramming of the Power Steering Control Module (PSCM) and Instrument Panel Cluster (IPC). No mechanical components (including the torque sensor or steering column) were replaced. In 2025, the same steering assist failure recurred, resulting in heavy steering effort and renewed safety concerns. This indicates that the recall remedy did not correct the underlying defect. Ford Motor Company has acknowledged in writing that: The recall was software-only The recall is closed No current programs provide coverage for the recurring steering failure Ford denied further assistance strictly on eligibility grounds and did not dispute the recurrence of the defect or its safety implications. Because this is a recurrence of the same safety-related defect addressed by the recall, I am concerned that the recall remedy was inadequate, and that other vehicles repaired under this recall may also experience recurrence after software-only updates. The power system failure warning came on as I was driving up on an intersection. WhenI released the accelerator because I was in i-pedal mode (one pedal driving), the cars braking system did not activate. Consequently, I ran the red light. The car power was reduced, and I was able to pull into the
... parking lot of a strip mall as a police vehicle pulled behind me. After explaining our situation and receiving my traffic ticket, the officer asked me to move the car out of the driveway, but I was unable to shift the car into drive given the power warnings that were flashing on the display. The officer asked if I needed a tow and I let him know I would use the brand’s Roadside Assistance for my vehicle. I turned the car off and the officer left. I waited about 10 minutes and I tried turning on the car again and was able to shift into drive and get out of the driveway and into a parking space. However, the car operated in low power mode and could only travel about 3 miles an hour. The tow truck driver had to jump the 12 V battery which was completely depleted in order to turn on the vehicle to move it. At Hyundai Service center, the car turned off abruptly as he was trying to move it into a parking spot. Despite supposedly having been addressed in a recall, the failure of the vehicle’s power system due to the ICCU put me in my family at risk while actively driving on a road with traffic.Read more

The contact performed an inspection of the vehicle and noticed a thick, black sticky substance underneath the EGR valve. The vehicle was towed to the dealer, and the dealer became aware of burn marks on the left side of the engine. In addition, the contact was informed that coolant was leaking from several spots. The dealer declined to continue diagnosing the vehicle. The contact was advised to retrieve the vehicle. The manufacturer was made aware of the failure, and a case was filed. In addition, the contact stated that the EGR valve and the engine oil cooler were replaced under unknown recalls in 2021. The contact stated that the recall parts only lasted for three and a half years, and the original parts lasted seven years. The contact was informed that the vehicle was out of warranty. ... icle has now failed the Pennsylvania State Inspection due to a malfunctioning ABS—a defect directly covered by the factory recall. After the inspection failure, I took the vehicle to an authorized Chrysler/Dodge/Jeep/Ram dealership in Pennsylvania, where I was informed that the necessary parts and repair instructions are still unavailable from the manufacturer, and therefore, the recall repair cannot be completed. I have made two prior attempts to resolve this directly with Chrysler’s Customer Care department: Case #91837321 – Initial recall complaint Case #91655604 – Follow-up complaint regarding delay in remedy availability Despite these efforts, there has been no progress and no communication from Chrysler outlining when or how the repair will be available. If it would have shifted to 1st going 55 MPH it could have caused injury to myself and others. 3) No accident happened so I did not report it to any authority. I capped the shifting to 7th gear max. 4)I had the service center inspect the vehicle and they determined the CDF drum was bad and the transmission needed to be repaired or replaced. 5)a warning light wrench icon lit up on the display after it indicated a shift to 1st gear instead of 9th gear going approximately 55 MPH. ... calade at approximately 70 mph on the highway, the vehicle suddenly lost all power and stability. This was due to an injector breaking off inside the engine, sending metal fragments into the camshaft, which subsequently locked the motor. This created an extremely dangerous situation as the vehicle became unresponsive and unstable at high speed. Just two days prior to this incident, the vehicle had been serviced by Quantrell Cadillac in Lexington, KY, where a recall repair involving the engine was performed. Despite that service, a catastrophic failure occurred almost immediately afterward. After the breakdown, Quantrell Cadillac attempted repairs over the course of three weeks. These repairs included replacing the injectors, camshaft, and other components, before eventually replacing the entire long block. However, upon picking up the vehicle, it is exhibiting the exact same failure symptoms again—making a popping noise, stalling the engine, and becoming unsafe to operate. This vehicle is clearly not safe to drive and poses a serious risk to myself, my passengers, and others on the road. There have already been multiple recalls related to this issue, indicating a broader systemic problem. We are requesting the NHTSA’s immediate assistance to investigate this safety issue and help us obtain a replacement vehicle that meets all applicable federal safety standards. This level of repeated engine failure & instability is completely unacceptable for any vehicle—especially one with such recent manufacturer service and recall repairs. Rear Suspension Control Arm Failure
On July 29, 2019, the Office of Defects Investigation (ODI) opened EA19002 to investigate instances of alleged rear lower control arm failures in 2013-2018 Nissan Altima and 2016-2018 Nissan Maxima vehicles. At the time the investigation was opened ODI had 91 consumer complaints alleging rear lower
... control arm failures in the subject vehicles.ODI was concerned about these complaints because many of them alleged an increased steering effort or loss of vehicle control.At the time these complaints were received, ODI was also aware of a proactive service campaign addressing 2013 Nissan Altima in salt-belt states for a similar concern. Since opening the investigation, ODI has confirmed, based on consumer complaints, warranty data, and testing conducted by Nissan and ODI, that owners of the subject vehicles are experiencing a corroded bushing at one of three mounting points of the control arm to the rear subframe.ODI has also confirmed that drivers become aware of the problem because of sound and abnormal vibrations. In addition, the failure mileage is typically over 113,000 miles at the time of failure.In conjunction with Nissan and VRTC, ODI participated in testing to assess the potential safety consequence of a bushing failure.It was found the Vehicle Dynamic Control system allows the driver to maintain vehicle control in the event of a bushing failure.With between 7 to 13 years of field exposure, ODI is aware of only one low speed crash allegation describing minor contact with another vehicle’s trailer hitch. Nissan acknowledges that a crack can develop in affected control arms due to stress loading from normal use and that salts commonly used for roadway snow and ice treatment may result in corrosion that exacerbates the progression of the crack.Nissan implemented a design change in January of 2018 to improve the durability of the lower control arm.Affected vehicles have either been repaired with the countermeasure part under a customer satisfaction campaign or been provided a warranty extension from 3 years/36,000 miles to 10 year/unlimited mileage. Through testing and analysis of the information obtained to date, ODI has not identified a defect representing an unreasonable risk to motor vehicle safety involving corrosion of the subject rear lower control arm at this time.With a declining trend of reports and Nissan’s actions to implement a countermeasure and extended warranty coverage, further investigation of the issue does not appear to be warranted at this time.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The Agency reserves the right to take additional action if warranted by future circumstances. ... 2013-2018 shared a common rear lower control arm design. Nissan provided complaint and warranty data regarding 48 incidents of the subject failure. The majority of the complaints are from vehicles registered in salt belt states. There have been no crashes, injuries, or fatalities confirmed as of this date. Nissan advised that it has conducted testing to assess safety risk. Nissan states that due to low incident rate, high detectability, and low risk of adverse vehicle dynamics, it does not believe the subject lower control arm failure poses an unreasonable risk to motor vehicle safety.This preliminary evaluation has been upgraded to Engineering Analysis (EA) 19-002. During the EA, ODI will continue to collect and analyze complaint and field data in its efforts to fully assess the scope and frequency of the alleged defect. Brake Master Cylinder Internal Leak

... nty claims reported that brake malfunction indicator lamp (MIL) illumination as the only symptom of a problem.Only 12 complaints alleged any effect on brake performance and in each of these the drivers were able to stop the vehicles without incident.No crash claims could be attributed to the alleged defect. Returned part analysis data provided by Nissan showed that most of the master cylinders replaced under warranty did not have a leak condition.These replacements may have resulted from brake MIL illumination due to pad wear.For most of the warranty claims, in a few cases where a master cylinder leak was found, Nissan's testing showed that the leaks were extremely slow and would require thousands of miles or months of driving before brake performance could be affected.After an extensive study of replaced master cylinders for a presumed leak, Nissan was unable to identify any concern with the parts or design of the subject components that could affect brake performance. Nevertheless, Nissan does recognize a concern with premature brake MIL illumination and has decided to address this problem by conducting a voluntary service campaign covering approximately 1.17 million model year 2007 through 2012 Altima vehicles (up to June 25, 2011 production, the date of Nissan's running change for the longer life brake pad).Nissan's owner letter for the service campaign states, As your brake pads wear over time, the brake fluid level in the master cylinder reservoir will drop and may cause the brake warning lamp to illuminate.?Although this is a normal condition, there has been concern that the brake warning light illumination may be caused by a slow fluid leak over time.Nissan's service campaign will inspect the brake pads and test the master cylinder for leakage.If leakage is detected, Nissan will replace the master cylinder at no charge.If excessive pad wear is observed, customers will be notified and will be responsible for repairs.For more details concerning Nissan's service campaign, see the PE11-020 file on www.Safercar.gov.
