My 2025 Hyundai Ioniq 5 experienced a complete and sudden loss of drive power at high speed, generating open fault code DTC P1A9096 (Internal ICCU Failure) [24V-204]. This failure completely matches the component and safety hazard profile under active Federal Safety Recalls 24V-204 and 24V-868 [24V-
... 204].The vehicle was repaired at an authorized retail facility, Hyundai of El Cajon. The facility's un-finalized pre-invoice explicitly verified the ICCU failure code but deliberately omitted the federal safety recall context, routing the mechanical intervention under an internal manufacturer 'TSB 24-EV-004H' to hide the defect from consumer safety databases [24V-204].This represents a deceptive administrative effort to under-report active federal safety recall failures on newer model inventories across the E-GMP platform architecture [24V-204]. I am requesting a formal safety defect enforcement review into the manufacturer's regional reporting data.Read more

The vehicle displays a critical 'Front Passenger Safety Restraint System Fault' warning on the dashboard, indicating that the airbag system may not function as designed in a crash. After googling the error code, it appears to be isolated to either frayed wiring harness under the seat or the faulty
... Occupant Classification System sensor. The issue occurred on June 11 and again on Aug 11 while driving the car. The warning appeared intermittently and I tried to "reset" the car computer. Tesla Service has diagnosed the issue as wiring harness failure/sensor failure and quoted $3000 for repairs, claiming it is out of warranty. This is a safety-critical system failure that poses a significant risk of injury or death to front-seat passengers due to potential airbag non-deployment. Given the high frequency of this specific failure across the Model 3 fleet, this appears to be a design or manufacturing defect rather than normal wear and tear. I am requesting an investigation into this safety defect.Read more

I bought a VW 2019 Jetta used in 2023 from a Volkswagen dealer in Bayside, Queens with about 20k miles on it. There was a recall on the car for a faulty ignition switch and prior to getting my car inspected recently, it needed to be taken care of to pass inspection as well as an oil sensor contribut
... ing to a check engine light. Both were taken care of by VW Platinum Service in Hicksville and the car passed inspection with a mileage now with about 62k miles. Since then, my car on two extremely hot days has shut off on me completely while driving. The first time it happened, they inspected the car again and assured me after several tests there were no issues with the car. I had the car back for about 2 weeks and then this past Friday 7/17, the car shut off on me again and I towed it to the same service center. They keep claiming it has nothing to do with the recall and provide me with service reports that say "no codes found" after their inspection. They want to charge me for additional tests and "mechanical diagnostics" as per the owner of the center. There is ample evidence online that suggests the symptoms I am experiencing with my car are exactly attributed to the recall they claim to have fixed. There was never an issue prior to getting the recall taken care of and the inspection at this service center. They do not care about my safety concerns and I am legitimately afraid to drive the car around. The only reason why I am not injured yet is because it thankfully has died while driving at lower speeds leaving work in a parking lot.Read more

I just experienced a 2nd engine stall event. I was driving thru a neighborhood at 25 mph in 5th gear, lifted off the gas pedal as I approached a 90 degree uphill turn, and in the middle of the turn (approx 20 mph) the engine stalled and I lost all propulsion. I retired from Stellantis after 16 yea
... rs working as a contract transmission calibrator, so I am very familiar with what happened. When I lifted off the gas pedal the engine attempted to enter decel fuel shut off (dfso). I was watching the real time gas mileage and right before the corner I saw the mpg approaching 99 mpg (a clear indication the engine is entering fuel shut off). A second later the real time mpg started decreasing (indicating it was attempted to exit dfso) when the stall occurred. This is a MAJOR SAFETY ISSUE in need of a recall.Read more

The car was driving on the highway and out of nowhere the car just stopped in the middle of the highway. I brought it to my mechanic who said the car was rapidly consuming oil and my cars engine was seizing. and he safely brought me and the car to Atlantic Hyundai where I had bought the car to have
... them diagnose and fix the issue. After several days they diagnosed the issue as a simple oil pan issue and replaced the oil pan and sent me on my way. The same issue with the engine seizing up in the middle of driving on the highway endangering my life and others. I am still terrified thinking of this. We brought car back to Hyundai by tow truck because I was scared to drive it and after 2 weeks of waiting they then told me the car needed new engine however they never mentioned what I later discovered was a serious recall for this specific engine . Upon further research my car was listed under the recall however before I bought the car it says it passed the test to see if it needed engine replacement under recall 203 . They knowingly said my vehicle was safe they sold it to me knowing it was unsafe and then they put a bandaid ( oil pan) and sent me on my way knowing I was in danger. ... The vehicle failed to turn off, and the parking brake failed to engage. There were no warning lights illuminated. The air bags deployed as needed. A Police report was filed. The EMS workers were unable to depress the brake pedal. The undercarriage of the dashboard ripped the skin off both shins. The contact was transported to the trauma hospital, where medical attention was provided for four weeks to close the wounds. A skin graft was performed on both legs. The vehicle was towed to a tow lot. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. I was driving eastbound in my 2012 Kia Sportage on I-82 just past Benton City MP97 at about 11AM on Monday June 22, 2026, when my engine knocked twice, the red check engine light came on, and I lost power. I pulled over off the road next to a concrete barrier that was beside a rock roadcut. My flash
... ers would not operate, I put it into park, turned off the ignition and immediately noticed the smell of burning oil or rubber. I had 2 half gallon containers of water on my front seat, so I popped the switch, lifted the hood, used the prop rod to hold the hood up as I started dowsing the flames that were popping up on the passenger side of my engine compartment near the firewall. After using both half gallons, I ran to the rear of the vehicle, opened the hatchback, and grabbed a gallon spray bottle of water and brought that back around to the front of the vehicle. By this point the prop rod holding the hood was bending and the flames were popping back up, I pumped the sprayer and directed a stream of water with my left hand and used my right to hold up the hood as the prop rod was not working. After the fire was completely out, I returned to the rear of the vehicle, donned my reflective vest, put a portable flasher on my vehicle, and at this point a Benton County fireman in a pickup arrived - he was called by a motorist that saw me pulling over with smoke trailing my vehicle. The fireman was followed by a large firetruck and crew. They propped up the hood with a bar and inspected the motor to verify the fire was completely extinguished. They took my information and left. A Washington State Patrol arrived, assessed the scene and offered assistance, but I already had called my wife so he left me to wait for her. He told me I had 24 hours to get the car off the interstate. I had the car towed to my driveway by A&E towing at about 2:30PM. I just realized this morning that I ought to report it to you. Dear NHTSA, I am writing to formally report a dangerous design oversight and safety hazard regarding the trunk lid support mechanism on the Nissan Altima. On [Date of Incident], while the vehicle was parked, a gust of wind caused the trunk lid to slam shut unexpectedly. The metal door latch struck
... me directly in the frontal region of the skull, resulting in an injury that required medical monitoring for potential brain trauma. The current design relies exclusively on torsion bars or gas struts to maintain an open position. These components are prone to wear and environmental fatigue, and they lack a mechanical "positive lock" or secondary safety catch. Consequently, the trunk lid acts as a sail in windy conditions, easily overcoming the lift support and turning the heavy lid and metal latch assembly into a falling hazard for any consumer loading or unloading the vehicle. While Federal Motor Vehicle Safety Standards (FMVSS) focus heavily on internal trunk releases (Standard No. 401), there is a significant gap regarding the prevention of sudden, gravity-induced or wind-induced lid collapse. I am requesting that the NHTSA: Investigate the frequency of "unintended closing" injuries related to Nissan Altima trunk lift systems. Evaluate whether the lift-support specifications for this model are sufficient to withstand common wind gusts. Mandate a mechanical locking mechanism or more robust secondary safety catches to prevent these lids from becoming dangerous projectiles. The vehicle intermittently experiences delayed gear engagement and hesitation during acceleration, most noticeably when the transmission is cold. During several incidents, when pulling out from a stop onto a main road and attempting to accelerate into traffic, the vehicle failed to shift or engage p
... ower normally. The engine remained running, but the vehicle did not accelerate as expected, creating a temporary loss of forward momentum. After a delay, the transmission would suddenly engage and the vehicle would accelerate normally. This condition creates a safety risk because when entering traffic from a stop sign or side road, other drivers expect normal acceleration. The hesitation or delayed engagement creates a risk of being struck by oncoming traffic. The issue occurs most frequently when the vehicle is first driven after sitting for several hours, especially in colder temperatures. After the vehicle is fully warmed up, the condition is less noticeable or may not occur. The problem has been experienced multiple times over this winter season. The behavior is intermittent but repeatable under similar cold start conditions. The condition appears to be gradually becoming more noticeable and occurring more frequently over time. Occasionally, once the transmission begins operating and partially warms up, the transmission may shift harshly or abruptly during upshifts or downshifts. The vehicle has been discussed with independent service shops familiar with this transmission model. The issue has not yet been repaired, but inspection and repair are being scheduled. No manufacturer inspection has occurred at this time. At the time of incidents, there were no consistent warning lights or messages indicating a transmission failure. No persistent diagnostic warnings have been observed. Symptoms were limited to delayed or harsh shifting and delayed acceleration response. The vehicle is available for inspection upon request.Read more

The contact owns a 2019 Ford Expedition. The contact stated while driving 50 MPH, there was a rattling sound coming from the engine. The contact pulled over to side of the road, turned off and restarted the vehicle and the vehicle started to operate as intended. The check engine warning light was il
... luminated. An independent mechanic was contacted. The vehicle was taken to an independent mechanic where it was diagnosed and determined that the cam phaser needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure but offered no assistance. The contact was referred to the NHTSA Hotline for assistance. The failure mileage was 150,000.Read more

Vehicle must have thought a shadow line in the road from a bridge indicated a wall I was about to hit, as it slammed on the breaks in the middle of the freeway. Car behind had to swerve to avoid. On the same trip, several nags of 'keep eyes on the road' were incorrect, as my eyes were directly on th
... e road, but phone was in hand functioning as a microphone taking voice memos. Car proceeded to shut itself down while in the middle of the freeway, causing me to go slower than traffic and put me in danger as cars behind me had to swerve or lane change to avoid me as I tried to pull off to the shoulder. These forced shutdowns from incorrect 'nag fails' are causing safety issues. Nag failures should not forcibly shut the car down. Not sure if this is being enforced by NHTSA, or why Tesla is choosing to disable their auto steer or FSD when drivers are detected to be failing nags - nags should be there to help drivers, and potentially the nag data/cab video can be used after the fact should a user failing nags be involved in an accident for insurance dispute resolution, but shutting the cars down themselves is causing issues.Read more

The contact stated that the rad
... io became silent and then thirty seconds later the infotainment system operated as intended. A dealer was contacted. The vehicle was taken to the dealer where it was determined that the infotainment system needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure and opened a case. The approximate failure mileage was 44,540.Read more

The horn does not work properly when it gets wet. It will either not make noise at all, or it is extremely muffled. This is a major safety risk. Also, horn is required to work for Massachusetts state inspections. If I bring the car to be inspected on a rainy day, or after a car wash, it will fail b
... ecause it does not work. I brought the vehicle to the dealer twice. The first time 5/28/25, the technician was able to replicate the issue and replaced the horn and claimed everything was working properly. The next time it rained, I tried to use it and it would not make any noise once again. I brought it to the dealer a second time 7/7/25, and they claim they could not replicate it. They went ahead anyways and replaced it. Also they claim “this is normal, expected behavior due to the location of the horn and eventually it will drain out”. This is an unacceptable answer. The horn should work all the time. There has been no warning lamps, messages or other symptoms prior to the failure. First symptoms were approximately 5/6/25Read more
