This issue involves a documented and recurring safety-related defect that was reported multiple times while my vehicle was still covered under Nissan’s factory warranty related to the airbag system. My 2020 Nissan Altima currently has approximately 53,900 miles and has experienced repeated airbag
... /SRS failures associated with diagnostic code B1436-13, indicating an open circuit within the passenger airbag system. This issue has occurred on multiple occasions, including in 2022, 2024, 2025, and 2026. During several warranty-period service visits, the diagnostic codes were reportedly cleared, however, the underlying wiring harness issue does not appear to have been permanently corrected. I have the full history service record for this car with notes from 3 Nissan Dealers. I have documented communications with Nissan Consumer Affairs dating back to May 13, 2024 regarding this ongoing concern. In addition, I have photographic evidence showing the same airbag fault code on February 22, 2025, along with additional correspondence with Nissan dated March 18, 2025. Both of these events occurred while the vehicle remained within the applicable warranty coverage period. The airbag warning light has now returned again, and I have been presented with a repair estimate exceeding $5,200 for a condition that was previously reported and addressed unsuccessfully while the vehicle was under warranty coverage. According to Nissan’s 2020 Warranty Information Booklet, the Restraint System, including airbags and related electronic control systems, is covered under Nissan’s 5-year/60,000-mile warranty coverage. Because this safety concern was documented during the warranty period and prior repair attempts did not resolve the issue. Given that this matter involves a critical vehicle safety system that could affect passenger airbag deployment, I am hopeful this can be resolved.Read more

On 5/11/26 at approximately 7:40pm, I was driving a leased 2024 Jeep Grand Cherokee in [XXX] , turning left at the busy intersection of [XXX] and [XXX] . I was in the outside lane of a double left-turn lane with a green left-turn arrow, while opposing traffic was also moving through the intersectio
... n in its own turn movement. In the center of the intersection, the vehicle suddenly activated Forward Collision Warning and then Auto Emergency Braking without warning, even though I was lawfully proceeding through the turn. The vehicle stopped abruptly and forcefully in the middle of the intersection, leaving my car stranded in active traffic. I braced for a rear-end collision, which surprisingly did not occur as there must not have been another car behind me After the sudden stop, I was unable to shift the vehicle back into Drive for several seconds. By the time I was able to move the car again, the light had turned red and other lanes of traffic were beginning to proceed. This created an immediate risk of a serious collision from cross traffic or vehicles approaching from behind. The force of the unexpected stop was strong enough to cause neck pain. By chance, no other vehicle struck my car, but this was a near-miss event with clear potential for severe injury or death. I no longer feel safe driving this vehicle. In addition, this vehicle has had multiple recalls and other issues, which heightens my concern that this may be part of a broader safety defect involving the Forward Collision Warning, Auto Emergency Braking, or related electronic control systems. I am requesting that NHTSA investigate whether the 2024 Jeep Grand Cherokee can falsely detect a collision risk during a lawful left turn and improperly engage automatic braking or otherwise prevent the driver from promptly returning the vehicle to Drive. Relevant documentation can be provided, including VIN, mileage, service records, recall history, and any dealer inspection results INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.Read more

While driving under normal conditions, the Lane Keep Assist system on my 2025 BMW Alpina XB7 unexpectedly and violently jerked the steering wheel without driver input. The sudden steering intervention caused the vehicle to abruptly change direction and strike a curb, resulting in a loss-of-control e
... vent. This occurred without warning and could have resulted in a serious crash involving other vehicles, pedestrians, or roadside obstacles. Additionally, the adaptive cruise control system has repeatedly applied sudden and aggressive braking without an apparent reason or obstacle present. These abrupt braking events create a significant risk of a rear-end collision, as following vehicles are not anticipating such sudden deceleration. These issues involve advanced driver-assistance safety systems that are intended to enhance control and safety. Instead, they are operating in a manner that overrides driver input and introduces unsafe driving conditions. The servicing dealership acknowledged the issues and advised me not to use the Lane Keep Assist and cruise control features. Being instructed to disable safety systems due to unsafe behavior is unacceptable and does not resolve the underlying defect. Given the loss-of-control incident and the ongoing unpredictable braking behavior, I believe these defects present a serious safety risk to vehicle occupants and the public.Read more

In August 2025, my 2017 Audi A3 (VIN [XXX] ) experienced repeated stalling, strong gasoline odors inside the cabin, and a check engine light immediately after Audi completed recall-related service. The safety issue was later confirmed by the dealership to be caused by a failed suction jet pump (NHTS
... A Recall 24V110), which allowed fuel to enter the EVAP system and leak into the rear seat area and cabin. The leak was so severe that the rear seat and flooring had to be replaced due to fuel saturation. Before the problem was properly identified, I was repeatedly told the issue was “just the gas cap,” despite reporting fuel smell, rapid fuel loss after filling the tank, and the check engine light. During this time, I unknowingly drove a vehicle with a significant fire hazard. The cabin fumes caused physical symptoms and unsafe driving conditions. I was not notified of this recall at the time I purchased the vehicle in March 2022, nor was I notified of a second recall affecting the passenger occupant detection system (69BY), despite having purchased a one-year maintenance warranty. I only became aware of the recalls after experiencing the hazardous symptoms described above. The problem was confirmed and reproduced by the Audi dealership. Fuel odor, fuel loss, and warning lights first appeared within 24 hours after the initial service visit, and the safety defect was later linked to the recall-related component failure. This defect presented a serious risk of fire and exposure to gasoline fumes to both myself and other road users. I am writing to formally raise a safety concern regarding my vehicle’s brake and rotor system, which has experienced premature failure on two separate occasions. First Occurrence: The issue was first identified prior to reaching 11,000 miles and was addressed by the dealership after surpassing 11,
... 000 miles. The repair involved replacement of the rotors and bolts, but notably, the brake components themselves were not replaced. Second Occurrence: Around 23,000 miles, I began to notice progressively worsening shaking and a disturbing sensation of the brakes skipping or slipping—particularly during highway downgrades. By 24,500 miles, the issue had become significantly more pronounced. I returned the vehicle to Kia of Stockton at 25,248 miles. During my visit, Ricardo from the Kia of Stockton Service Department attributed the issue to “driver error,” citing hot spots on the rotors. However, he also acknowledged that during the initial repair, only the rotors and bolts were replaced—not the brakes. Ricardo further stated that Kia could not offer any additional support beyond a 10% discount on repairs, per his manager’s directive. I explained that I live and work locally, and my driving habits do not involve long commutes or frequent stop-and-go traffic. Given this, I strongly believe the repeated brake system failure points to a potential manufacturing defect rather than user error. Ricardo agreed that if this were his own vehicle, he would be concerned as well. This issue poses a serious safety risk—not only to myself and my family, but to other drivers on the road. In addition my vehicle has also experienced powertrain issues, where it feels as if the vehicle is stuck in 1st gear. I am therefore requesting a full investigation into this matter. If necessary, I would like to initiate the process of reporting this vehicle as a potential lemon under applicable consumer protection laws. Thank you for your attention to this urgent safety concern.Read more

While driving in traffic, my vehicle suddenly shut down without warning. The engine stalled, and I lost power to steering and braking, creating a serious safety risk for myself and other drivers around me. The problem has occurred more than three time. At the time of the failure, there were no warn
... ing lamps a message display showing to continue to drive depress brake pedal and push switch there was no check engine light showing. I had to shut the whole car off open the door and re start the car. The issue has been inspected on 08-13-25 by a acura dealer / mechanic. I was told it was the battery and it was covered under warranty. They found no other issues. The car shut down on me again on 08-19-25 and on 08-30-25 with my family in the car. This unexpected shutdown placed me in danger of a potential collision, and I am concerned that this may indicate a larger defect affecting this vehicle. I can get a receipt from the acura service department if needed.Read more

On [XXX], the GV80 experienced a complete electrical system failure (dashboard lights began flashing and eventually went black, display screen went black, steering wheel locked and brakes were hard to engage), all while my wife was driving on a major highway in rush hour traffic. The dealership clai
... ms this electrical system failure was caused by improper battery cable hookup, but this is incorrect. I have time stamped photographs showing a wiring harness that was already burned before any battery cables were connected. I also have documentation proving when the tow truck was dispatched to the vehicle’s location. Dealer Findings *Service advisor stated that there was improper procedure from someone hooking up cables to wrong posts and burning the wire(The issue occurred prior to any battery cables being connected ) -*Wiring harnesses are burned and damaged, creating excessive resistance *The condition is causing a dangerous buildup of heat *Multiple fuses have already blown and required replacement *The dealer explicitly advised that the vehicle is not safe to drive and could cause a fire at any moment Safety Risk This level of electrical damage is not consistent with ordinary wear or misuse. It represents a serious safety defect: • The failure rendered the vehicle completely inoperable. • The failure occurred while driving and resulted in a loss of steering and difficult braking, creating a severe risk of crash and injury. Mazda issued a safety recall (Mazda safety recall 4320A, NHTSA campaign number 20V-063) to address daytime running light flickering/failure. At the time, only the passenger side daytime running light was replaced under the recall by Mazda, with a gasket/gas absorption sheet being installed on the dr
... iver's side. The driver's side daytime running light is now flickering/dim. Mazda did not repair the safety defect properly on the driver's side for the daytime running lights under Mazda safety recall 4320A, NHTSA campaign number 20V-063, because Mazda only installed the gasket/gas absorption sheets that have since failed, causing the daytime running lights to flicker, increasing the risk of crash. Mazda has refused to cover the cost for replacement of the daytime running lights per safety recall notice 4320A / NHTSA 20V-063, even though replacement of these lights is covered by the recall. Both a Mazda dealer and Mazda corporate customer service have been contacted regarding this issue and both have refused to provide the remedy as outlined in the safety recall. Mazda has provided an estimate of $1,466 to replace the daytime running lights. Attached Documents: -Correspondence with Mazda where Mazda's refuses to cover work -Estimate from Mazda to perform work -Original invoice from recall work performed (gasket kit on driver side only, not full light replacement)Read more

My Tesla Model 3 disengaged cruise control while going 68 mph and braked hard for no apparent reason. I was driving northbound on I-405 at 3:45 a.m. going to LAX airport in the number 3 or 4 of six or seven lanes. Cruise control had been activated for at least 10 minutes before the incident at a s
... peed of 68 mph. There was no traffic within half a mile before or after me due to the time of day. Neither my left or right foot were near either the brake or acceleration pedal, nor were they in motion. Both hands were on the steering wheel and no control was being activated. The only noticeable environmental condition is that the road surface had just changed from a dark gray asphalt to a bright white new concrete roadway. No emergency braking alert was seen or heard, so I do not think it was a false collision detection, and there was no car within a half mile. However, I do believe the severity of braking could have caused an accident if there had been someone behind me. Tesla Model 3 Software Version was v12 (2025.20.6 046c4575d120).Read more

Carvana sold me a 2016 Audi S7 (VIN [XXX] ) with what was later discovered to be an odometer discrepancy. I submitted a formal complaint to the Ohio Attorney General, and Carvana subsequently issued a refund. However, after refunding me, I learned that Carvana resold the vehicle at auction without
... disclosing the odometer issue or branding the title as required under federal law. Play also did disclose the damage that was done to the engine, turbo chargers, or from the car catching on fire. This raises serious concerns about odometer fraud and title washing, and I believe it warrants federal investigation under 49 U.S. Code § 32705. I request NHTSA open a case on this matter and audit Carvana’s practices relating to odometer disclosures and resale.
