Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2021-2024 Highlander and Highlander Hybrid vehicles. The second-row seat backs may fail to lock into position during seat back adjustment.
Consequence
A seat back that fails to lock may not properly restrain an occupant during a crash, increasing the risk of injury.
Remedy
Dealers will replace the return springs in the seat back recliner assemblies, free of charge. Interim letters notifying owners of the safety risk were mailed April 27, 2026. This prolonged delay creates an ongoing safety risk. The manufacturer's inability to provide a timely fix leaves me with no choice but to transport my 3-year-old son in a temporary vehicle daily. I am also pregnant with my second child who is due to arrive in 3 weeks who will also be transported using the loaner vehicle if problem is not fixed yet. This situation exposes my family to unpredictable risks, as the safety history, structural integrity, and child seat compatibility of the long-term loaner are not equivalent to my personal vehicle. The manufacturer is failing to resolve a critical safety defect within a reasonable timeframe. Because of the delay in remedy , my health of my vehicle is being actively damaged on my paid off hybrid powertrain. As a Disabled Veteran, Toyota's inability to provide recall safety parts has caused a severe, ongoing physical and financial hardship. By grounding my vehicle for over 47 days, I have been entirely deprived of using my state-issued Disabled Veteran's license plates. This strips me of critical, legally mandated toll exemptions, parking accommodations, and physical accessibility benefits that I require on a daily basis. This 2023 Toyota Highlander XLE AWD has a recurring gasoline/fuel odor that began while the vehicle was still within the basic warranty period. The concern was reported to Toyota/dealer multiple times. At first, the odor was intermittent and difficult to duplicate, but the problem continued after re
... peated dealer visits. The fuel smell was later duplicated/confirmed while the dealer was open, before approximately 60,000 miles, but the vehicle was still not properly repaired. The odor has continued and recently occurred multiple days in a row. The smell has been noticed around/inside the vehicle and creates a safety concern because gasoline odor may indicate fuel vapor leakage or a fuel-system problem. Possible sources could include the fuel tank, fuel pump module or seal, filler neck, fuel lines, EVAP system, charcoal canister, purge valve, injectors, fuel rail, or another related fuel-system component. The vehicle has been inspected by a Toyota dealer multiple times, but the cause has not been identified or corrected. No warning light has clearly identified the source of the problem. The concern is especially serious because it involves fuel odor, possible vapor exposure, possible fire risk, and possible fuel-system failure. Occupants and others could be placed at risk if the odor is caused by a leak or defective fuel-system component. The dealer is now recommending removal/drop of the fuel tank for further diagnosis and quoted approximately $1,500 for that step. My concern is that full fuel-system diagnosis, including tank removal if needed, should have been performed earlier when the complaint was repeatedly made during warranty. The issue is not a new complaint; it is a continuing unresolved fuel odor concern that started during warranty and remained unresolved despite repeated dealer involvement. I contacted the FCA/Stellantis-recommended Jeep dealership in Lapeer, Michigan multiple times this week to schedule recall-related service. I was repeatedly transferred to service voicemail and did not receive return calls. When I called back and asked to speak with someone else, I reached the service manager. I explained that the vehicle has an illuminated brake warning light and that I suspect the brake booster may be failing. The service manager focused on where I lived, why I was not using a closer dealer, and mentioned that the vehicle is 14 years old, rather than clearly arranging recall service or addressing the brake safety concern. The problem has not yet been inspected or confirmed by a dealer because I have been unable to obtain timely service despite repeated attempts. The vehicle has not been inspected by the manufacturer, police, insurance representatives, or others. The warning lamp currently present is the brake warning light. While driving at a low speed (approx. 5–10 mph), the front passenger-side lower ball joint suffered a catastrophic structural shear without warning. This caused the wheel assembly to collapse outward and the CV axle to pull out of the transmission. The failure caused a complete loss of steering co
... ntrol, veering the vehicle into a curb. Physical evidence (outward axle pull and high-side rim scratches) indicates the mechanical failure preceded the impact. This occurred at only 38,155 miles on a 2022 Chevrolet Bolt EV. Outward Dislocation: Photos show the CV axle pulled OUT of the transmission and the wheel leaning OUT (positive camber). A curb impact would push these components IN. This proves the wheel fell away from the car first. Pre-Impact Grinding: The interior barrel of the rim and the brake dust shield show heavy rotational grinding and metal-on-metal scoring. This confirms the suspension collapsed while the wheel was still spinning at speed, prior to the car coming to a stop. Structural Shear: The lower ball joint stud suffered a complete transverse shear. High-resolution photos of the broken stud face show distinct "beach marks" (concentric rings), which are the physical fingerprint of a slow-growing metal fatigue crack rather than a sudden impact fracture. Rim Damage Profile: Deep gouges are located on the flat face of the wheel spokes rather than the outer lip. This confirms the wheel was already at an extreme tilt (collapsed) when it made contact with the curb. CONCLUSION: The vehicle was maintained and driven under normal conditions. This was a spontaneous, catastrophic structural failure of a safety-critical component. I am requesting that NHTSA investigate this as a potential manufacturing defect for the 2022 Chevrolet Bolt EV.Read more

After doing some research on Pilot owner online forums, as well as the NHTSA website, it seems a substantial number of similar complaints have been reported, and a recall had been issued for some vehicles in the same year and model under NHTSA campaign number 23V-751. The failures of my engine should be considered in the NHTSA recall number 23V-751, Honda recall number XG1, GG0. In addition, my claim should be considered as part of NHTSA investigation PE25008 which I understand is a current investigation into many reports of connecting rod bearing failures in the 3.5L V6 engine used in numerous Honda vehicles, including the 2016 Honda Pilot. If needed, I can provide an estimate of the work that needs to be completed on my vehicle from Hudson Automotive. This leaves me without transportation. The catalytic converter is fully covered under the Federal Emissions Warranty (8 years / 80,000 miles). I request that NHTSA investigate this as a potential safety and emissions defect and require Kia / Motorambar to: 1. These symptoms were first reported on February 4, 2026. Shortly afterward the vehicle was inspected by a Chrysler dealership, which documented a brake vacuum leak in the braking system. Despite that inspection and documentation, the braking condition has continued to worsen and the vehicle no longer feels safe to operate. The braking performance is inconsistent and stopping distance appears to be increasing. In addition to the braking failure, the vehicle experiences powertrain issues including transmission hesitation, delayed shifting, and loss of power during acceleration and when driving uphill. The vehicle also has severe vibration and instability originating from the front driver-side wheel or steering area while driving, which affects vehicle stability and control. These combined mechanical failures involving braking, drivetrain operation, and steering/wheel stability create a serious safety risk during normal operation, particularly while braking in traffic or attempting to maintain control of the vehicle. The vehicle's Freeze Frame Data recorded a catastrophic internal transmission slip, showing an engine speed of 1767 RPM while the vehicle was moving at only 4 MPH in 2nd gear. There are no previous warning signs or symptoms of this problem before the transmission failure. It was sudden and unexpected. The manufacturer’s technical service protocols (such as T-SB-0094-24/T-SB-0076-24) for these specific solenoid codes on this specific 8-speed hybrid transmission (L580F) – which is equipped on my vehicle – identify this as an internal mechanical actuator failure requiring full transmission, torque converter, and hybrid module replacement. However, the manufacturer has denied the repair because the vehicle’s specific VIN is not yet listed on a service bulletin and that they could not reproduce it in a short test drive. The manufacturer’s official recommendation is to continue operating the vehicle with a documented and non-repaired powertrain defect until a second failure occurs in traffic. ... driving in the left lane of the 2 lane southbound interstate. It happened right in front of me and I had no chance of completely avoiding it so I veered to my left onto the shoulder to attempt to avoid a direct hit on the large Nissan SUV that was now straddling both lanes. I hit the front of the Nissan and the guardrail almost simultaneously at somewhere around 50 to 60mph which caused considerable damage to the front right quadrant and the entire drivers side of my vehicle, but my airbags never deployed and I sustained moderate to significant injury to my arms, shoulders, chest and neck, most likely due to how I was gripping the steering wheel in anticipation of the impact and from the seat belt. I did not require medical treatment at the scene but have since received treatment at home. I believe the airbags should have deployed given my speed at impact and where my vehicle came in contact with both the Nissan SUV as well as the guardrail. I believe this was a clear failure of the airbag system in my vehicle, which has since been deemed to be totaled by my insurance company.Read more
