I am writing to formally lodge a complaint regarding a recurring issue I have experienced with the head gasket in my vehicle. I have owned several vehicles of this make and model, and I have observed a troubling pattern of blown head gaskets that has not been adequately addressed. Specifically, my
... current vehicle has suffered from this problem, resulting in significant inconvenience and additional repair costs. This issue appears to be prevalent across this year model as well, suggesting a potential design flaw or manufacturing defect that warrants further investigation. I respectfully someone to look into this matter to determine if a broader safety concern exists that needs to be addressed. Owners of vehicles affected by this issue may be at risk of serious engine damage and costly repairs. Thank you for your attention to this important issue. I look forward to your response.Read more

As I was driving the Tesla Model X, I received a steering assist reduced error on the dash. There was no immediate noticeability, but after about 5 minutes, it felt like there was ZERO power steering, with no additional warnings in the car. With zero power steering, car required extreme forces req
... uired to get to the shoulder. If attempting any kind of turn, car would require forces in excess of 80lbs of pressure to execute. Once safely in a parking lot, I put the car into SERVICE mode to check the errors. There was an EPAS error, where it said Torque Sensor 1 not receiving, along with a Torque Sensor 2 not receiving. This has occurred to me 3 times, in the 3 years I've owned the car. Other than "Steering assist reduced", there is no other warning that the car's EPAS steering system reverts to just manual forces required to steer a car. I do not trust my wife to drive this car, as she would not have the strength necessary to get the car to a safe location. If this happened while turning, there's a strong possibility the car would not turn at all, a huge safety factor.Read more

NHTSA ODI Case Update – Supplement to Complaint #XXX Vehicle: 2019 Jeep Cherokee Latitude Plus (VIN: [XXX] ) Subject: Dealership Deflection Tactics and Denial of Recall Repair Verification This statement is submitted to update my ongoing federal safety complaint regarding the catastroph
... ic failure of the Mopar Two-Speed Power Transfer Unit (PTU) and the resulting loss of motive power/rollaway risks. On May 19, 2026, the vehicle was evaluated by an authorized FCA dealership, St. Helens Auto Center (Repair Order #XXX). The certified technician, Don Stockdale, explicitly confirmed the mechanical failure of the recall-associated components, stating in writing: “the propeller shaft at the rear of the PTU feels as though it is dry inside. Very loose and clanking around.” Despite verifying this severe manufacturing safety defect, Service Manager April McCall formally refused to submit a corporate Goodwill or Warranty Extension claim to FCA unless I paid out-of-pocket to replace three CV axles, which she claimed were "loose and dry." To verify this claim, the vehicle was immediately taken to a secondary licensed automotive facility (Dr. Auto LLC, Invoice #XXX) on May 27, 2026. The independent inspection confirmed that the dealership’s diagnosis was fraudulent. All CV axle boots are completely intact and sealed, meaning the internal grease has not escaped. The front axles have zero play, and the rear axles exhibit only normal, minor factory play. The dealership intentionally used a loaded 4-post lift to misdiagnose the vehicle, leveraging the high-frequency vibrations of the failing central PTU to fabricate secondary, non-covered user-pay repairs. This is a deliberate corporate tactic used to deflect financial liability for a known federal safety defect onto the consumer. I am requesting that the NHTSA review these attached service documents as evidence of predatory dealership behavior surrounding the Cherokee PTU failures. Severe transmission shuttering destroyed my transmission and I need a new one according to a local body shop and a GMC service center. This is a KNOWN issue with these 10 speed transmissions, but they will not recall the ones from this year, they only recalled newer models. Car has been inspected by
... a local autobody shop, a 3rd party warranty company, and a GMC dealership service center. Shuddering causes severe shaking while driving between 25-65mph. No lights or other symptoms, just started occurring randomly. Purchased car Feb 2025, brought it in to local auto shop November 2025 and they could not get a transmission for months. They stated one was set to be delivered by GMC Feb 6th 2026 and they day before arrival GMC called them saying it was back on back order. Called the dealership in April 2026 and they stated they were not backordered. Had vehicle transferred to dealership service center and they say it’s still on national back order with no ETA. I have been waiting for over 6 MONTHS for a new transmission. This car has been undrivable for almost longer than I have been actually able to drive it.Read more

I am filing this safety complaint regarding a documented engine wire harness chafing defect in my 2021 Chevrolet Silverado 1500 with the 2.7L L3B Turbo engine. While driving, my truck experienced erratic acceleration, engine throttling to 3,000 RPM without returning to idle after releasing the acce
... lerator, engine misfires, sudden loss of engine power, stalling, and multiple simultaneous warning lights. These conditions created immediate safety and collision risks on public roads. GM issued TSB 21-NA-149 in June 2021 and a second bulletin September 17, 2021, both before my November 2021 purchase, identifying wire harness chafing in the L3B engine caused by a manufacturing error. A wire harness clip was removed during production creating an uncontrolled harness branch that contacts sharp edges on the ECM/TCM bracket, upper control arm, and shock tower during normal engine vibration. This defect was built into the vehicle at the factory and was never disclosed to me at point of sale. The defect manifested after warranty expiration causing cascading failures; wire harness, catalytic converter, and transmission, all within five weeks. Vehicle has been inoperable since January 8, 2026. Defect confirmed by independent mechanic and by Ancira-Winton Chevrolet (authorized GM dealer) in their own repair documentation referencing GM Bulletin 21-NA-149. Dealer documented catalytic converter failure as consequential damage from wire harness repair. GM created Customer Satisfaction Program N192219370 for this exact defect. My VIN was excluded despite experiencing identical symptoms. No safety recall has been issued. Loss of throttle control and stalling at highway speeds represents an imminent collision risk. I request NHTSA investigate a formal safety recall for 2019-2023 Silverado 1500 and Sierra 1500 vehicles equipped with the L3B engine.Read more

My 2019 accord gets stiff at speeds over 25-30mph and resists driver input, so much that you tend to over-compensate and make constant steering corrections every second so you don't crash. The car does not track straight at any speed, and does the same when the Lane Keeping Assist (LKA) is off or on
... . When LKA is active the car will not go straight on a straight smooth road. The car wanders dangerously from the right to left line markers and often rides on the markers bringing you very close to other cars and then jerks the car back to the opposite side of the lane and continues to do the same back and forth like a drunk person is driving the car. Also the Driver Input Required amber light comes frequently on a straight road with highly visible lane markers. It is extremely tiresome to drive the car and try to keep it in its lane, even when I disable all the "driver-assistance" features. This is because the steering has a mind of its own and stiffens up. It resists the slightest driver input so you have to put more strength into it to make it go where you want to, but once the steering gives up fighting you and lets you do it, it results in overcompensating and constant steering corrections by the drive left-right, left-right, just going down in a straight smooth freshly paved road. It’s been over a year and the dealer has yet to detect anything wrong, they tell me my vehicle drives perfect, honda dealer after dealer.Read more

I am reporting a persistent safety defect involving loss of intended drivability in my 2017 Ram 1500 with an 8-speed automatic transmission. For the past 3 years, on almost every use of the vehicle, the transmission enters an abnormal operating state where it becomes stuck in a single gear (most oft
... en 3rd or 4th) and will not upshift or downshift. When this occurs, the vehicle experiences reduced ability to accelerate and loss of normal drivability. The condition does not resolve while driving and requires me to pull over, shut the vehicle off, and restart it to restore operation. This defect has occurred multiple times at freeway speeds, requiring me to pull over onto the highway shoulder due to loss of vehicle function. A vehicle that cannot reliably shift or accelerate at highway speed presents a clear safety hazard to the driver and surrounding traffic. Requiring a stop-and-restart is not a safe or reasonable mitigation strategy in real-world traffic conditions. The issue occurs more frequently when the vehicle is cold, indicating a predictable and repeatable failure mode, not driver error or misuse. I have had to alter normal driving behavior, including extended warm-up periods and avoiding highways, to reduce risk. I was informed by repair facilities that this is a known issue related to transmission control module (TCM) software, and that Chrysler has issued Technical Service Bulletin(s) describing TCM software updates to address abnormal shifting and drivability behavior. The existence of these TSBs demonstrates manufacturer knowledge of the failure mode and a defined corrective action, yet the issue has not been addressed as a safety recall. I am submitting this complaint so the issue can be evaluated based on objective safety criteria. A vehicle that routinely loses drivability and requires emergency stops on freeways warrants formal safety investigation.Read more

2021 ford f150 PowerBoost hybrid. There is a LOT of internet chatter and forum chatter about KNOWN issues with the following, and it recently almost led to a major and likely serious injury accident. Specific Parameters: Low speed (~5 mph) slow roll Under internal combustion power, not battery In
... 2nd gear only When gas pedal is pressed vehicle does nothing. Does not respond. Two choices are to hammer the gas, or hit the breaks. Does two things, slams forward eventually after 3-4 second delay if gas pedal depressed OR shifts down or into electric mode (internal combustion off) and then will once again go from first to third in normal acceleration progression and pace They must have an issue with the standard 1-3 skip programming with ICE power and 2nd gear being a hang-up I almost got T boned by a semi truck due to this with family in car. This could have been deadly. It happens as an annoyance at least once per day but this one was scary. Online, and based on what I can find - Ford is aware of this issue as they have the 1-3-5 programming in the transmission but when it finds itself (due to real world driving conditions) in 2nd gear under Internal combustion, it does not know what to do. I brought it to Ford last year in Anchorage and they are attempting to blame it on normal hybrid system operation. That is NOT the case. It would be something that could be duplicated but with effort. A basic, simple test drive is NOT going to find this. They are aware of this being an issue. If you have any situation where you are rolling through, merging into traffic, or other scenario where you end up in 2 under ICE you are in TROUBLE. Dangerous, scary, and totally a manufacturer issue that is known and needs to be taken VERY seriously.Read more

On [XXX], while operating my 2024 Tesla Model Y in Full Self-Driving (Supervised) mode, the vehicle experienced an Uncommanded Steering Input leading to a collision. The vehicle was traveling at a very low speed of approximately 5 mph in a clear, simple environment. Despite this walking pace, the FS
... D software failed to identify a visible concrete curb and initiated a sudden, sharp turn directly into the obstacle without any prior warning or driver input. The fact that the system executed such a violent and erroneous maneuver at 5 mph indicates a fundamental failure in the Neural Network's object detection and path planning logic. If this logic failure had occurred at higher speeds or near pedestrians, the results could have been catastrophic. The local Tesla Service Center (Watertown, MA) refused to conduct a physical inspection or a log review of the steering torque. They issued a Repair Estimate of $1,217.26 while claiming 'no hardware fault codes' were present, thus ignoring the documented software malfunction. As a leaseholder of this vehicle with only 15,489 miles, I am reporting this as a significant safety defect that the manufacturer refuses to investigate. Dear Bridgestone Customer Care Team, I am contacting you regarding factory-installed Bridgestone Turanza tires, size 235/45R18, on a 2025 Toyota Camry. Despite normal use and routine maintenance, the tires have experienced extreme premature tread wear: At 20,000 miles: three tires 4/32”, one tire

The vehicle’s infotainment/display screen (center touchscreen system) malfunctions. The screen flickers, freezes, becomes unresponsive, and frequently fails to connect. The malfunction creates a safety concern because the screen controls critical functions such as navigation, rear-view camera dis

When the screen freezes or flickers, I am unable to safely access navigation or hands-free phone features, causing distraction while driving and increasing the risk of an accident. The issue has occurred repeatedly during normal vehicle operation. There were no warning lamps prior to the malfunction. Symptoms include screen flickering, freezing, delayed response, and loss of connectivity. These symptoms began intermittently and have progressively become more frequent over time.Read more

Vehicle: 2021 GMC Denali Mileage: ~48,000 miles Purchase Price: $62,000 Issue: Airbags & crash sensors failed to deploy Complaint Text: I am filing a safety complaint regarding my 2021 GMC Denali. I was involved in a rear-end collision at approximately 45 mph, where I struck another vehicle straig
... ht on from behind. The impact was severe enough to total my vehicle, yet no airbags deployed, and no forward collision warning or crash sensor systems activated. This is especially concerning because this vehicle was previously involved in a front-end collision (not my fault) and underwent extensive repairs. I was assured all systems were repaired and functioning properly. Additionally, my vehicle had completed recall work related to safety systems prior to this crash. This is the second front-end crash involving this vehicle where airbags failed to deploy. Given the speed and severity of the impact, airbags should have deployed. The failure of these systems placed my life at serious risk. The vehicle has since been totaled and taken by insurance, but I am requesting that this failure be formally investigated as a potential safety defect in the airbag and sensor systems. I am extremely concerned that the vehicle’s safety systems were either defective or not properly restored following repairs and recalls.Read more

The check engine warning light was illuminated, with diagnostic trouble codes indicating that the Oxygen sensors had failed, and the timing was not being converted correctly. The vehicle was taken to a certified mechanic, who diagnosed that the catalytic converter needed to be replaced. The vehicle was not repaired by the mechanic. The contact replaced the catalytic converter and placed a seal around the windshield.
