The car care center identified diagnostic codes related to low pressure fuel system pressure, engine stall history, etc. but cannot reproduce the issue. After checking multiple systems, he believes the issue is a failing fuel pump. Background: Over the past 6 months, we have had problems with the check engine light turning on. On multiple occasions, we have brought the car to get diagnosed and the reading has consistently been fuel pressure codes. I am reporting recurring safety-related defects on my 2018 Audi A4 involving Automatic Emergency Braking, Forward Collision Warning, Audi pre sense/front-collision functionality, and related electrical/driver-assistance reliability. The AEB/forward-collision system has shown two unsafe intermittent
... behaviors. At times it has failed to activate as expected when crash-mitigation assistance appeared reasonably available. At other times it has unexpectedly warned, jolted, or applied braking when there was no apparent vehicle, object, pedestrian, or hazard close to the car. Unexpected braking or false intervention can startle the driver, create rear-end collision risk, and reduce confidence in the system. Failure to activate can also increase forward-collision risk. The system was not intentionally disabled by me, and I am not aware of any driver-caused obstruction, modification, or normal explanation. The condition is intermittent and may not appear during a brief service visit or basic fault-code scan. This concern was reported through Audi/dealer service channels, including a documented repair request identifying the AEB system as malfunctioning and failing to activate as designed. A later dealer communication stated that there were no stored AEB faults and later clarified that no work was done on the AEB system, no inspection or verification of AEB operation was performed, and confirmation of operation would require an additional estimate. I am reporting unresolved safety status because intermittent active-safety failures or false interventions may not store permanent DTCs, and lack of stored faults is not complete safety verification. I have not received final written safety closure confirming inspection, calibration review, road-test verification, factory scan/freeze-frame review, module communication review, and technical verification. The vehicle as a result can stop up to six feet past the intended stopping point in some instances. This issue is unpredictable and has occurred multiple times. It creates a serious safety risk. Toyota performed T‑SB‑0047‑24 (Brake ECU Reprogramming) on my vehicle, whereafter the brakes seemed to function normally for several months, but the problem resurfaced and occurred threee time in one week in May of 2026, which prompted me to bring it backk to Toyota for another look. The dealer has been unable to reproduce the issue and therefore has not repaired it. The veicle has been returned to me, and continues to have the problem intermittently at a random frequency. Toyota corporate Brand Engagement Center has been notified, and a case was opened (Ref # 260528002416). This case was closed however after telling me that the dealership connot reproduce the problem, therefore they cannot repair it. This appears to be a brake control defect involving the hybrid brake‑by‑wire system. Because the failure is intermittent and leaves no diagnostic codes, it is extremely difficult for the dealer to diagnose, but the safety risk is severe. I'd like to advise that I have owned and driven earlier hybrid models (2012-14) that never exhibited this type of brake failure. I'd also like to point out... an internet search of this problem reveals that this problem seems to be prevelant in newer Toyota hybrid models. The dealer quoted repair including AC compressor replacement, battery chiller replacement, and inverter coolant flush. Kia Consumer Affairs denied warranty coverage under the federal emissions warranty despite the vehicle being within the 8 year/80,000 mile coverage window at approximately 58,309 miles. No written explanation for the denial was provided. This failure appears to be a known pattern on this platform based on owner reports. The battery chiller and inverter coolant system are integral to battery thermal management and should be covered under the federal emissions warranty. Auto Hold and the electronic parking brake's hold function were disabled, requiring manual brake application to prevent rollaway. Mileage at incident: approximately 21,344 miles. The simultaneous failure of Electric Power Steering, Dynamic Stability Control, and the Electronic Parking Brake while the vehicle was in motion in traffic created an immediate safety hazard. No crash or injury occurred, but a recurrence at highway speed, on a wet road, or during an evasive maneuver could foreseeably cause loss of vehicle control, crash, injury, or death. The vehicle was taken to an authorized BMW dealer. The technician stated this failure is a known issue following a recent vehicle software update, that BMW has no repair available, and that the only action taken was to power-cycle the vehicle and clear stored fault codes. No software remedy, parts replacement, or TSB was provided. I was given no assurance the failure will not recur and no timeline for a fix. To my knowledge, no recall has been issued for this defect on the i5 platform. I request that NHTSA evaluate the scope of this issue across the i5 fleet and determine whether a defect investigation or recall is warranted.Read more

In addition, there are ongoing system malfunctions including: •Volume controls becoming unresponsive (the slider does not adjust sound), requiring the system to be restarted to function again. •Intermittent failure of the phone connection/charging port, where the device does not connect properly to the system. A prior software update temporarily addressed one issue, but the core problems persist. I have been attempting to resolve this through Ardmore Toyota dealership in PA for approximately one year without a meaningful fix or access to a qualified technician. This appears to be a recurring software or system defect that affects driver attention and vehicle operation. I am concerned this creates a significant safety risk for myself and others on the road. There is a customer satisfaction program for 2019 Ford Escapes for Powertrain Control Module reprogramming (Campaign 19B37) that then makes the vehicle eligible for a one time repair under campaign 21N12. This campaign fixes exactly what is wrong with my vehicle. It applies to vehicles built in Louisville until April 2019. My vehicle was built in Louisville in May 2019. The dealership told me to call Ford Customer Relations/Recall Team to explain the situation and get a goodwill repair since my vehicle is having the exact same issues as the vehicles built at the same plant the month before. The recall team refused to provide me with a goodwill repair and also refused to give me information on when or if at all my case will be reviewed by the appropriate team. I asked to be able to contact the team handling my review by phone. They said I could only communicate with them by physical mail. This is unacceptable and I believe that Ford caused an unsafe situation for countless drivers for years by not including 2019 Ford Escapes built in Louisville in May 2019 in their customer satisfaction program and are still refusing to do so.Read more

Ford dealerships have reprogrammed the transmission PCM four times over several years with progressively worsening transmission functionality. Dealership service specialist has confirmed that the transmission failure is due to internal components with burnt fluid as a component. But, Ford will not allow them to open the transmission up, denied repeatedly by the manufacturer as the issue with this 10R80 transmission is widespread with thousands of complaints filed, sone with NHTSA and others online and with Ford Customer Service to no avail. Ford is denying this manufacturer's defective transmission to avoid financial liability likely in the multi million dollar range over a wide array of Ford products with this faulty transmission installed in them. During the four times that Ford dealerships have reprogrammed the transmission's computer at the behest and mandate of Ford Motor Company, this vehicle's transmission repair was covered by both the manufacturers warranty as well as a privately purchased Ford Extended Service Plan (Extra Care). Ford chose deny coverage under either warranty though it is warrantied under both plans explicitly. This failure can result in vehicular accidents because of transmission's erratic and flawed functionality. The braking is abrupt and forceful, and the system appears to misinterpret the brief angle change at the curbline as an obstacle. The vehicle is available for inspection upon request. This malfunction puts my safety and the safety of others at risk because the unexpected hard stop could cause a collision or loss of control. More importantly, if the system behaves this way on a simple residential driveway, it could be extremely hazardous on public roads that have larger hills, dips, or transitions. Sudden automatic braking on a main roadway or at higher speeds could lead to a serious crash. The problem has occurred repeatedly and consistently at the same driveway since I began driving the vehicle. It happens nearly every time. ... hen I came back at 12PM around 1 quart of coolant had leaked out. Luckily I was not on the freeway or driving the vehicle where it would ride on its torque curve increasing probability of catastrophic failures even after there are usually software induced engine protection derates in place. Official service interval for water pump and timing belt on this engine is 130000 miles but there have been numerous field incidents of pre-mature water pump failures which can be very expensive to fix (~2500 USD). Volkswagen/Audi already have a class action lawsuit against them on their 2.0 TFSI (EA888) water pump failures and I'm wondering why their MY2015 2.0 TDI (EA288) diesel engine has not been investigated for the same. This engine was only available in certain MY2015 VW/Audi vehicles but just because the cumulative number of vehicles with this engine are lower does not mean that it shouldn't be investigated for a very similar manufacturing/design defect of the water pump. I would strongly urge NHTSA to do a similar investigation on this 2.0 TDI engine and hold the manufacturer accountable.Read more
