Consequence

I am submitting this report regarding a serious and potentially fatal unintended vehicle acceleration incident that occurred immediately after my vehicle was returned from repair. I originally brought the vehicle to the dealership’s body shop following a collision. The repairs included the replac
... ement and repair of cracked grille covers, sensors, rear damage, and the rear camera system. The vehicle remained at the repair facility for approximately one month during the repair process. After finally picking up the vehicle, I drove and tested it for many miles without noticing any obvious issues. I then parked in front of a shopping center. The vehicle was fully stopped, and I shifted the transmission into PARK. After engaging PARK, I removed my foot from the brake pedal. At that exact moment, the vehicle suddenly and violently lunged forward on its own without any warning or driver input. The vehicle accelerated into a concrete pillar located less than two feet in front of the parking space. The impact was severe enough to deploy two airbags and caused a very loud, explosive sound inside the vehicle. The force of the collision shocked the interior seats and coverings, and I was left disoriented and briefly unconscious. This incident could easily have resulted in a fatality if a pedestrian or another person had been standing in front of the vehicle at the time of the unintended acceleration. I am requesting a full investigation into the cause of this sudden forward movement after the vehicle had already been stopped and shifted into PARK, especially considering the recent repair work involving sensors and collision-related components. This was an extremely dangerous incident and raises serious concerns about vehicle safety and possible malfunction before or after repair.Read more

I just had gotten my 2025 Dayton R/T EV and i was on the highway in my first drive…. Enjoying the hell out of my experience. I had driven abt 12 miles. Even got a powershot in when i had a free right away in the highway and man… 😔😞☹️ im so saddend to share that a car got over on my car
... and completely swiped the right side, then got over in front of me and slammed on their brakes…. All at 65 mph. Man… my car’s sensors didnt help at all and my airbags didnt deploy. My sensors never detected the car on the right getting close or that it slammed on brakes…. All my sensors are still green in the car, but my abs and other lights are on. Airbags didnt deploy crashing st 65 mph snd I stopped breathing for a few minutes the other driver snd passengers said. All I saw was the csr slam on brakes and then I woke up with 5 cops and 2 ems bombarding me with questions I had no clue to answer as my head was dizzy. Once I got to my senses they asked why my air bags didnt deploy and if it was a 2025 ev how did i get brake checked with security sensors and regen brakes. And I cant answer that only dodge can… ☹️ man im devesated. My car is also still registered to dodge… so when I went to the impound and shiwed them my info they told me I couldnt get the car out even with me showing them my welcoming emails apps etc. so now the car is stuck there. Racking up a crap ton of daily fees. Starting today. And I csnt even get the csr out if I tried. Now my payments. The storage fees. And to fix the vehicle as my deslership didnt add the car they switched me in because the one i signed for had problems to my policy. They left the one I never left the dealership with and I gave me the new one without asking or inquiring abt insurance for the new vin… please help me guys. My lifes falling apart but im keeping my head up.. my right shouoder blade snd bsck is such a mess. Even after the hospital.Read more

My husband was able to get us safely out of harms way. After being diagnosed at a mechanic, it turns out this problem happened due to a cracked cylinder head and had no previous warnings of an issue and no warning lights on the vehicle alerting to a problem, I will have the cylinder head in my possession and the mechanic I took my vehicle stated it cannot be ruled out this could be a manufacturer’s or casting defect. I tried contacting Acura Corporation about this situation but was told that since I did not take it to a dealership and have a certified tech look at my vehicle, there will be no case and no type of remedy or reimbursement for this situation.Read more

The electric power steering has slowly become sluggish. At first I noticed the steering wheel was not returning to center after a turn and I would have to assist steering the truck back straight. It was difficult parking and even more sluggish returning to center from a turn requiring full hands on steering back to straight ahead. Looking the issue up on the Internet, I find a recall for this issue in 2016 on a small number of this model and now an new ongoing NHTSA investigation since August 2023. I am the third owner of this truck and it has had great care and is in generally fine condition for a 10 year old truck. I have already had to convert the truck back to coil spring suspension due to the air suspension system crapping out. I don't believe a steering system should give out at only 109,000 miles. I believe Stellantis should be held accountable for major system such as steering failing with such low mileage.Read more

... the vehicle will drive normally. This problem is infrequent which is what makes it so vexing. Each incident was more than 6 months apart so you never know when it will just stall out. Because our van was converted to a rear entry wheelchair van by Braunability we've had the conversion components/connections thoroughly inspected by the conversion company dealer. I can see online that this is a known issue with multiple vehicles equipped with the Pentastar V6 engine with stop-start and that there have been recalls but many people on the forum, the recall work didn't resolve the issue. We've had the vehicle to Chrysler dealers several times (four or five) to address the issue without permanent resolution. We've replaced the primary and secondary batteries several times. Additionally, because our Pacifica was converted for our disabled child, we will probably need to sell the vehicle at a significant loss because we can't risk having the vehicle stall on the interstate highway or freeway as we have no way to remove him from it safely. This issue seems so widespread that Chrysler/Stellantis should have continued responsibility to resolve it permanently.Read more

... the vehicle will drive normally. This problem is infrequent which is what makes it so vexing. Each incident was more than 6 months apart so you never know when it will just stall out. Because our van was converted to a rear entry wheelchair van by Braunability we've had the conversion components/connections thoroughly inspected by the conversion company dealer. I can see online that this is a known issue with multiple vehicles equipped with the Pentastar V6 engine with stop-start and that there have been recalls but many people on the forum, the recall work didn't resolve the issue. We've had the vehicle to Chrysler dealers several times (four or five) to address the issue without permanent resolution. We've replaced the primary and secondary batteries several times. The vehicle is making a loud clunking noise in the steering column when driving down the road. It is veering all over the road and when I hit a bump it’s very bad. It’s difficult to keep in the lane. I brought it to my mechanic who said it was most likely the electronic steering motor assembly.

The vehicle was repaired; however, the failure had reoccurred. The vehicle was taken back to the same local dealer and was left for further investigation. The contact received notification of NHTSA Campaign Number: 25V011000 (Power Train) however, the part to do the recall repair was not yet available. The local dealer was contacted. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the failure. The failure mileage was approximately 70,000. ... cal dealership. They looked at the vehicle. Initially they thought it was a faulty valve cover gasket. They ordered the parts. When they actually started taking the vehicle apart, the issue was not the valve gasket but the crack in the valve cover itself. They replaced the valve cover. It went to the dealer on date 03/03/2025 and they gave he vehicle back to me on 03/21/2025. They said this crack had to of happened on the production line but before it got delivered to the initial dealership.Read more

... c’s recall websites, it appears General Motors failed to include my car in the recall altogether. Evidently, on 10/15/2024, my car wasn’t braking properly, the dashboard lights for ABS, Brake, engine, and traction control were on, and the 6 months old car had to be towed as it wasn’t drivable due to failure to brake. The dealership informed me the eBCM was the issue and required repairs. It is very concerning my car wasn’t included in this safety recall. I drive my 3 young children daily and I would’ve never driven the car had I known a recall was issued a month prior. For separate reasons, I am currently in a legal dispute with General Motors and the dealership. It took me a lot of research to realize exactly what happened to my car on 10/15/2024. Only after avoiding near collision while driving on I5 freeway the failure illuminated various dashboard warning lights, such as the ABS light, VSA light, and brake system warning light. This failure has placed me, my passengers, and other road users at risk, especially with the stop and go traffic of rush hour. Luckily it was sunny clear weather because if there was even slight wet or slippery conditions I'm certain the harm would have been disastrous. -Known Technical Causes: Failure of internal solenoids or valves in the VSA modulator unit. Corrosion or damage to electronic components within the modulator. Malfunctioning sensors associated with the ABS and stability systems. -Repair Details: Replacement of the VSA modulator unit is often necessary. Cost of repair typically ranges between $2,000 and $3,000. Many owners report delays in repairs due to part availability. -Reports and Complaints: Numerous complaints can be found on forums, owner groups, and repair sites detailing identical failures in 2013–2017 Honda Accords. Commonly reported safety risks include increased stopping distances and difficulty maintaining control on slippery roads. Honda’s Response: Some owners report that Honda dealerships acknowledge the issue but do not offer repairs under warranty unless an extended warranty is in place. They brought it in and seem to have primarily done some software update but didn’t express much confidence in the repair. They just said there were no DCT’s but there was some software updates and that should fix it. They also claimed there were no recalls present on the vehicle which is even more scary. So the only way there fixing this if they even are fixing it is it already happened to you and put you in danger.Read more
