Subject: Supplemental Evidence – ODI Complaint [NHTSA complaint 1663939 and 11673063.] Dear NHTSA, I am submitting supplemental evidence regarding my complaint of loss of propulsion in my 2022 Jeep Grand Cherokee 4xe (VIN ending [XXX] ). On September 4, 2025, Chrysler (FCA US, LLC) filed Recal
... l 25V576 covering 91,787 Jeep Grand Cherokee PHEVs (2022–2026) for the exact defect I reported: a software error in the Hybrid Control Processor that may cause a sudden loss of drive power, increasing the risk of a crash. This defect was documented in my case as DTC B273C-00. My dealership confirmed the code on multiple service visits (February 25, 2025 and May 21, 2025), and the arbitrator in Case #[XXX] also confirmed the defect in his July 7, 2025 decision. Despite this, FCA repeatedly denied any safety concern, asserting my vehicle was “safe to operate.” The recall proves that: •The defect is real and poses a safety risk. •FCA was aware of the defect months before acknowledging it publicly. •FCA misled me, the Illinois Attorney General’s office, and arbitration proceedings by denying the existence of a safety issue. For the record, I experienced loss of propulsion while driving with my children, which created a dangerous situation. I ask that my complaint remain open and that NHTSA investigate FCA’s delay and misrepresentations in disclosing this safety defect. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Complaint Message – 2016 Kia Optima Engine Failure Without Oil Light Warning I am the owner of a 2016 Kia Optima (VIN: [XXX] ). My vehicle suffered complete engine failure due to oil starvation — without any prior oil warning light or indication. The engine died while driving, and it was only af
... terward that I discovered the engine oil was extremely low. The oil pressure light never illuminated at any point to warn me. I am aware that Kia previously recalled earlier model years (2011–2014) for similar engine issues under Recall SC147 and others, related to the Theta II engine and oil pressure-related failures. Many owners reported the exact same symptom — catastrophic engine failure without oil light warnings — that I am now experiencing in my 2016 Optima. Although my model year is not included in the existing recall, the failure pattern appears identical. I believe this presents a significant safety issue that should be investigated. A critical warning system (oil pressure light) failed to function properly, which directly led to engine damage. I am requesting that this issue be reviewed and investigated by NHTSA and Kia for potential expansion of the recall or warranty coverage to include the 2016 Optima. I have already contacted Kia and am pursuing further review through the dealership and customer support. Please note that other 2016 Optima owners have reported similar complaints online and in forums, and this issue may affect a broader number of vehicles than currently acknowledged. In 2023, I purchased my 2012 Chevrolet Equinox from a private seller. It was in excellent condition, inside and out with only 75,000 miles on it. She had purchased it brand new. I immediately pulled over and called triple A and got towed to an auto repair shop. The triple A dispatcher told me what the problem was as soon as I told him it was a 2012 Chevrolet Equinox. There was NO OIL in it. Sure enough, the mechanic added 5 quarts of oil and it thankfully, I had pulled over immediately before it blew the engine. I'm a [XXX] so I really don't know how mechanicals and motors work but the gist of it, as explained to me by Chevrolet dealers and mechanics have, the 2012 Chevrolet equinoxes are notoriously famous for excessive oil consumption due to faulty Pistons. At approximately 65,000 miles, a burning oil odor was detected, and we took our vehicle to the nearest Mazda dealership. Techs confirmed a failed master cylinder resulting in a leak, and that this master cylinder failure is common with 2018 CX-5s resulting from porosity in the cast metal wall creat
... ing a leak path over time. I was able to retain the failed casting and all parts which were replaced and am saving for records and analysis. Mazda customer service was contacted about the master cylinder casting failure and declined to cover it because the vehicle was beyond warranty period and because the failure was repaired before I was able to establish contact with Mazda customer service. Note that the failure happened in the approach to the holidays, and while efforts were made to establish contact with Mazda service via website chat, e-mail form submission, and phone, they were unsuccessful due to technical issues on the Mazda site and a non-responsive phone line. I can confirm that my local dealership POC provided me with Mazda corporate contact info per my request when the diagnosis was made (e-mail copies available.) The issue was not on my end, as efforts were made again immediately after Christmas, and phone and website chat were immediately successful when attempted. The website chat log indicating their denial and rationale for denial is available. This same vehicle has also had an issue with the front timing cover leaking as several others have noted, and this issue was also acknowledged by local dealership technicians as an endemic issue with this model. The front timing cover issue were identified and repaired within the 60,000-mile warranty. Copies of available correspondence with Mazda, including dealer invoices/receipts, service records for the vehicle (all done at Mazda dealerships) and chat history with Mazda customer service can be provided.Read more

This made accelerating to driving speeds take far longer than usual, which was an interesting situation as I was pulling into traffic when this occurred. There seems to be some oil burning off when I drive. But none leaking on the ground. My car is consuming oil. Also if I press the gas to speed up, my car will stall a bit and then some black smoke will exit my exhaust.

Purchased Dec 2021. Immediate Hydraulic leak, repaired at dealer. Jan 2022- hydraulic slides creeping out 6-8 inches. March 2022- serviced at Tiffin in Red Bay. No solution for creepy slide. July 2022- received recall for creepy slide. Nov. 2022- Feb 2023. Picked up RV on Feb 9 at service cent
... er, completed recall for creepy slide. Before leaving service center, hydraulic pump went out and had to be replaced. In April 2023, I notified service center creepy slide issue continued. From that point, made several phone calls and sent numerous emails to Lippert (slide manufacturer) and Tiffin and Connecticut Motor cars and coaches, the service center I use to search for solution. After months, Sept 26, 2023- Lippert Tech came to my house to work on problem. Following that visit on Oct 10- sent picture to Lippert, creepy slide continues. Nov 30, 2023, which is tomorrow is next visit for Lippert Tech. Cautiously optimistic since the solution is replace same cylinders from recall that this will finally fix the problem. I was told by an Tiffin rep on the phone that Lippert’s solution is to replace the cylinders that were replaced during the recall and the Tiffin rep indicated they are still seeing the same creepy slide issues after the second cylinder are replaced. Everyday I have to go out and retract the slides so there is less wear and tear on rubber seals and to protect the slide from weather, especially freezing rain or snow. And driving down road, have to worry that slides are not going to start to extend. I have had to pull over at a rest stop to retract the slides more than once. There is a webpage dedicated to creepy slide issues that other Tiffin RV owners have written about the same same problem. Tiffin needs to recall and switch, at their cost, from Lippert slides to a more functional, safe slide manufacturer so that RVer’s feel safe and do not cause harm or injury to themselves or others. Thank you.Read more
