Incident Narrative
An employee was on a stepladder installing a sprinkler pipe in a stairwell. He was on the fourth rung of an 8 foot step ladder tightening a fitting when the wrench slipped causing him to fall over the railing to the lower level, then roll down the stairs, ending up two levels below his starting point. He was hospitalized with a concussion, five broken ribs and a punctured lung.
