Incident Narrative

An employee was approximately 3 feet off the ground on a step ladder doing demolition work to a shed. A pipe broke loose from above and hit the ladder, knocking the employee off the ladder. He landed on a concrete slab and sustained a head injury and broken ribs. The employee was hospitalized.
