Vantage Mobility International, LLC (Vantage) is recalling certain 2012-2018 AMS Genesis wheelchair vans, built on a Toyota Sienna vehicles. The rubber brake hoses that attach to the rear wheel assembly may be too short, causing them to rub against the rear axle trailing arm.
Remedy: Vantage will notify owners, and dealers will install a set of rear brake hoses that will provide the proper clearance, free of charge. The recall began December 16, 2019. Owners may contact Vantage customer service at 1-800-488-9082.

If the power sliding door is unable to be opened when commanded, such as if the door is frozen shut, the door may subsequently open unexpectedly, possibly while the vehicle is moving.
Remedy: Toyota will notify owners, and dealers will replace the junction block, rear sliding door wire harnesses, and install two sub wire harnesses, free of charge. The recall began on July 28, 2017. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is G04.

In August 2025, AutoNation Toyota on Sahara, checked the VIN of my Toyota Sienna and contacted Toyota while I was present. Toyota confirmed a broken fuel-door hinge was covered under a Customer Support Program. Instead, AutoNation attempted to repair, rather than replace, the hinge. This caused the metal fuel cap door to end up bent, paint to be gouged out from the bottom of the fuel door area, misalignment of the fuel door, and therefore issues with the door’s sensor reading correctly. I took the van back until they received shipment of a new fuel cap door. After the attempted repair, since the fuel door no longer aligned, the safety sensors repeatedly activated which strained the sliding-door ECU. The passenger sliding door began freezing intermittently, forcing my children to exit through the trunk several times. Finally, the door opened on its own as I was driving down the street. These issues did not exist prior to dealership service and presented a clear safety concern. Between October and December, I received conflicting explanations from Gerald, James, Ivan, and Rudy at AutoNation Toyota regarding whether the issue involved the fuel door, ECU, motor, or general wear and tear. AutoNation repeatedly failed to return my calls or respond to the emails I sent them. Toyota Corporate also informed me the dealership was also not responding to their outreach attempts after I contacted them due to not being able to get ahold of AutoNation Toyota. I emailed four managers on October 15 with detailed documentation and received no response. My vehicle remains unsafe, unrepaired, and in the possession of AutoNation Toyota. I approve allowing it to be inspected. While at AutoNation, the vehicle driver could not open the passenger sliding door after AutoNation's attempted repair. They have had my van for 4 months now so no one else has looked at or inspected it. There were no warning lamps, messages or other symptoms of the problem prior to the failure.

Odometer Fraud. The contact purchased a 2015 Toyota Sienna. The vehicle was purchased from a private seller. The contact stated that while registering the vehicle at the local Department of Motor Vehicles, the contact was informed that the odometer mileage was last recorded at 141,000. The contact later purchased a Carfax report, which indicated the past mileage recorded at 287,000. The contact stated that when the vehicle was purchased on November 8, 2025, the odometer was recorded at 98,043. In addition, the mileage recorded on the title was 97,043, and in parentheses, the words “Not Actual” were handwritten.

My husband parked our minivan and came in to pick up the kids. While he was inside the library, the van went out of park and hit a fire truck in the parking lot. It seems this has been an issue with Toyota siennas in the past but I do not see it reported yet for its year 2015. Our van had damage to the front fender, hood, and surrounding area and needed repair. We obviously didn’t have any idea this was possible nor noticed anything since purchasing this van recently. I have read online that a Toyota sienna cannot be turned off without being in park. Since my husband was not in the vehicle when it crashed in the parking lot, it is clearly a defect of the vehicle.

Fuel filler door upper hinge failed so that door would not seat properly when closed. Because the door was not seated properly, the left side power sliding door interlock did not release, preventing the sliding door from opening fully. I had visited a fast food restaurant in Feb 2025 and ordered some French fries for my[XXX], strapped into his child seat directly behind the driver. He was choking on a fry, starting to turn blue. I quickly stopped and attempted to open the sliding power door, but it would not open fully to allow access to my grandson because the failing fuel door wouldn't release the power door interlock. By the time I ran around to the passenger side to reach him he had nearly passed out, but I reached him in time to extract him from the seat and performed a Heimlich to expel the fry and restore his breathing. Toyota should be forced to extend the extended warranty for the full duration of the court agreement of 10yr, Dec 2025, regardless of purchase date. My request for fuel door replacement in Jul 2025 was refused because Toyota claimed the warranty was only good until 10yr after date of purchase, not to the end of 2025. The warranty period applies to vehicles purchased Dec 2025, so the warranty program still exists, but Toyota National refuses to honor it for my vehicle. The inoperable power door can cause passengers to be trapped from quick exit in case of other emergencies, such as vehicle fire or accident rollover. The contact owns a 2015 Toyota Sienna. While the vehicle was parked, the contact discovered that the fuel lid door hinge had fractured, causing the fuel lid to intermittently fail to open or close as needed. The contact stated that with the fuel lid intermittently inoperative, the rear driver’s side door failed to open when needed. No warning lights were illuminated. The vehicle was taken to an independent mechanic where it was determined that the fuel lid door and hinge needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was approximately 117,000.

My engine caught on fire. We had seconds to get out prior to it engulfing in flames. Fire department had to extinguish. Not sure what caused it. The entire vehicle caught on fire. There was an engine light warning I had the vehicle repaired but the warning light was not cleared

The slide door latch stopped working. The sliding door doesn't latch properly and I have to manually close the door and lock it to know it's secure. If it's not locked a slight push will cause it to pop open. My mechanic said there is a recall associated with the replacement part. I can't find the recall. However, I did find a related TSB (TSB-0244-17).

I have had this car for 7 years. Started having issues a year ago and despite bringing it to Toyota 2x, they cannot tell me what the issue is. I will be driving on a regular flat paved road when the car will suddenly brake and pull me towards the side of the road. I get the ABS code sometimes and also the slip indicator light. I've explained this each x I've brought it in and they have told me it's due to me "driving too fast" or "accidentally having my foot on the brake." LOL I am a typical soccer mom and drive conservatively and don't brake when I don't have to. It will do it on curving roads as well as straight roads. When this happens, it is terrifying! I have gone to different mechanics as well and no one can determine this issue. I've replaced the ABS sensors on this and am still having this issue. I don't know what else to do, as this is unsafe and unsafe to the extent that I no longer allow my 17 year old to drive it. Please help!!!!!!!!

Water intrusion from front sunroof when vehicle is parked at a 5% grade with the nose downward. Multiple vehicles exhibit this issue. Toyota is not creating any TSB and will mot even remedy it even if we have an extended warranty. Here is a link to the discussion thread: [XXX] I have not posted on this thread but I have the exact same issue for years. Toyota dealerships want me to first pay $1000 in labor to pull down the headliner and test. Then ask the warranty to cover the assembly, if they don't cover it... then I am on the hook for the work. The missing seam sealer is a common issue that will not manifest inside the 3yr / 36k mile warranty. My issue started around 2020 that I noticed and every dealership would not help me other than place a large burden on me. This can impact the electrical and the airbag curtains. The issue needs to have a seal sealer added to the sunroof assembly as the fix. As a family vehicle, this is a dangerous issue for multiple occupants in a single 7 or 8 person hauler minivan. Please ask Toyota remedy this issue that has been going on for over a decade. Please ask them to stop forcing customers to have sunroofs and panoroofs... when they can't build it correctly. I turned in my Toyota on 7/10/24 for a recall on the sliding doors and was told the vehicle needed tobe taken to the mobility dealer; SacVans Mobility, that modified it, for the repair. SacVans said they wanted a diagnostic fee first before they would decide to fix it or not.

The contact owns a 2015 Toyota Sienna. The contact stated that while his wife was driving on two occasions at various speeds, the rear passenger’s side sliding door opened inadvertently. The vehicle was steered to the side of the road and closed after several attempts. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure. The failure mileage was approximately 30,000.

There is an EVAP emission leak and the P0456 error states the gas cap is faulty. I had it replaced and replaced again. So I am waiting for another brand new gas cap-3rd brand new one. So 4th overall for this van. A smoke test was done and there is no leaks per the test. So if this continues after the brand new gas cap, then there should be a recall as the gas cap is an issue. Started to happen after 95,000 miles. The AWD is shut off and goes to 2 wheel drive, traction control is turned off, Check Engine light comes on and Message Check AWD system comes up on the screen. Any recommendations

This vehicle is a tire eater. It does not matter what the brand of tire it is- Michelin, Bridgestone, or Yokohama, tires last, at best, 18 months of local soccer mom driving. After five sets of 50K mile, or better rated tires, at $800 to $1200 hundred per set, I think it is time that Toyota owns up to the fact that something is wrong with this model (4x2) vehicle. In my mind thjis is similar to the Toyota airbag situation only on a smaller scale. The manufacturer always has an out, such as, did the customer have the vehicle aligned, have the tire pressure checked, rotate tire on schedule, was it driven off road? My concern is that just about any driver would expect to get 30 to 40 thousand miles of normal driving from a set of premium tires. If the tires wear abnormally quickly while expecting "normal" tire mileage, this puts the driver in danger due to unexpected tire failure and possible tire related accident. This needs some data analysis.

The intake valve became burnt/stuck, causing my car to lose all compression on cylinder 3. The CEL came on, flashing, and the car immediately started driving rough and not accelerating. I had the vehicle brought to 2 different auto shops that confirmed the issue. My and my childrens' safety were at risk as soon as we tried to turn right onto a high speed road and my car started the flashing CEL and would not accelerate. There was no warning before this incident.

I bought this carseat in March and began using it August 2nd for my infant who was born July 31st. The straps have slowly become an issue. We try so hard to tighten the straps but to no avail. This puts my child at risk if we were to get in accident as he is not secure and the straps are still loose. I did reach out to graco who informed me I had to do a warranty claim. I am not sure yet if thet plan to do anything in regaurds to this issue.

Cable that controls sliding door broke. Door now pops open about 1cm, not enough that you notice that it is open and when you drive it slides open more. It also can't be closed with the button and it's not easy to close.

All interior dome/lights (door side lights, visor mirror lights, all overhead passenger lights, etc) not lighting up at all. Fuse in engine compartment is good. Vehicle has been at auto dealership where originally purchased. Safety concerns are that at night, unable to determine if there has been access by person or if person is actually in vehicle. Vehicle diagnoses was begun on 6/12/2023 and is still in process of issues. Received a call from the service representative on 6/19/2023 that water had accumulated under the fuse box in the engine compartment causing corrosion at the connection of the wire harness to the underside of the fuse box. Initially this caused the the internal lights to fail. There is much more corrosion occurring which will eventually cause a complete electrical failure and possible engine failure/fire. Current recommendation from service manager is to completely replace the wire harness at a cost of $7,000+. Insurance may or may not cover the cost of repair. The insurance adjuster is meeting today with the service manager. In my opinion, this is a serious defect in Toyota Sienna manufacturing, design and materials used.

Door to gas tank fill breaking off. Very common problem noted online by several owners of this vehicle. If it does not close properly the engine will not start. As it begins to break at the hinge the door sticks out. I fear it will fall off one day. Dealer acknowledges this is a problem and knows there is no recall for it. I have seen other exact vans that the door has been removed completely.

Due to poor design, air system intakes (eg under front seats) and outlets have not been protected by grilles, allowing intrusion of rodents into air system blower motor and heat box. I had rodent feces in my air filter system and blower motor and dead rodents in my heat box in Oct 2022 and has recurred in Feb 2023. Rodent intrusion in air system presents serious risk of respiratory illness to humans. Rodent infestation in vehicle can also cause startling to driver which presents an additional safety risk. Air system access should be protected by grilles at all inlet and outlet points.

The contact's mother sought medical assistance and was informed that she had sustained soreness in the neck muscle. The contact was unsure whether the other driver sustained any injuries. The vehicle was towed to a tow lot. The vehicle was then towed to an autobody shop but was not repaired. The vehicle was then towed to the residence. A police report was filed. The dealer and the manufacturer were not notified of the failure. The vehicle was not diagnosed nor repaired. The failure mileage was approximately 93,955.

My vehicle had been repaired for the recall involving the power sliding doors. The entire cable assembly was replaced. That repair has now failed and the sliding door is inoperable. The cable snapped and the driver side door will not open. I am afraid that it may open while driving, which caused the recall in the first place. I don't want to drive the vehicle for fear of the other sliding door failing as well. These repairs should last much longer than 5 years, especially something as important as the sliding door, where all the children sit. What a huge safety issue for what is supposed to be a vehicle for hauling children around. Luckily the incident happened in a parking lot.

On a warm day after driving the van about 15 minutes then stop for 15 minutes or more. It is taking longer than usual to crank the van. I have search around online. It seems like a common problem Sienna's fuel pump. People on "SiennaChat - https://www.siennachat.com/threads/engine-starts-great-cold-or-hot-long-crank-when-warm.59549/page-2 " website is talking about it. My concern is that one day it could stall while driving on the highway. Since, it is fuel related issues. Without fuel supply engine will halt.

We got into an accident today, and none of the sliding doors would not slide open. It would not even allow us to pull the doors opens. The vehicles electrical was damaged, but that should not prevent the other doors from opening. This is a very bad situation if someone is needing to evacuate the vehicle. The rear passengers would be trapped. Please address this issue with Toyota asap. People who usually have Vans have big families and small children. This would be a very bad situation if vehicle needed to be evacuated asap. The rear passengers would be stuck.

I was at a stop light when I slowly started to lift my foot from the brake pedal. The engine started revving up and accelerate. I pressed the brake pedal normal , then harder when it would not stop revving. I looked down to see if I was hitting the gas pedal by accident. I was not, about after 5 seconds it stop revving. I went about a foot and nobody was in front of me. I going to have it checked out by my dealer. This happened before and I thought it was driver error .When I thought I was hitting the gas pedal and brake pedal. I searched for incidents like mine, but could not find something exactly just similar. I did not look at the dashboard so i do not know if there was a message . So I'm reporting this incident to get answers and alert others of this issue.

When vehicle is in reverse the rear breaks on passenger side start squealing and making a loud noise when applied specially when vehicle has been stopped for while however on occasion it does it every time you back up when applying break.

We recently had two Dunlop P 235 / 50 R 19 tires fail on our car. On 11/5/21, the front passenger-side tire tread and belt began to unravel while driving at freeway speed. This happened suddenly, with no warning lights, and we became aware of the problem when we heard a loud thumping noise from the unraveled tire striking the underside of the vehicle. We immediately exited the freeway, found a tire shop, and had the tire replaced. The following week, I inspected the other tires and found that the front drive-side tire was failing in exactly the same way. We immediately had that one replaced too. Neither tire shop that replaced the tires could identify a cause of the tire failures other than possible defects in the tires.

The contact stated that she was quadriplegic and was unable to exit the vehicle because the door lock switch was disengaged but failed to respond. There were no warning lights illuminated. The contact stated that she was able to jumpstart the vehicle with assistance. The contact then engaged and disengaged the door lock switch and she was able to exit the vehicle. The contact stated that she notified the modifier of the vehicle, but they said they could not assist with the failure. The dealer and the manufacturer were not notified of the failure. The vehicle was not diagnosed nor repaired. The approximate failure mileage was 48,000. The VIN was not available.

The vehicle rear sliding door had a recall. I had the recall preformed at Smithtown Toyota. After the correction was done. I continue to have same issues with the drivers side rear sliding door. Where it will not open when pushing the open button on my controls in the front. Or when my child 12 years old attempts to manually open the door. As this does not happen all the times. The dealer says they can not replicate the problem and it is working to manufacture standards. I spoke with Toyota corporate and they are aware of continued problems with the sliding rear doors. the have a customer support group, but I would have to pay for the dealer to inspect the vehicle. I have extended warranty so they said I wouldn't then have to pay. However every time you take it in with a problem they can never replicate the problem. They are not the only dealer with this response. It must be a set line throughout all brands as its always that answer until your warranty expires. When it expires then all of a sudden they can find all problems and fix everything.

The contact owns a 2015 Toyota Sienna. The contact stated while starting the vehicle he noticed that the vehicle was shuddering and shaking uncontrollably. The check engine warning light was illuminated. The contact attempted to restart the vehicle however, the failure persisted. The contact took the vehicle to an independent mechanic to be diagnosed. The contact was informed that the failure was due to low cylinder pressure on cylinder #6. The mechanic was not able to do the repair on the vehicle, so the contact took the vehicle to the local dealer who diagnosed and confirmed the failure as a misfire and low-pressure cylinder #6 and that the spark plug was damaged. The vehicle was repaired however, the failure persisted. The dealer recommended that the engine be replaced. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 87,000.

When I drive this van downhill on a curve not fast slower than 40mph, the antilock brakes engage violently and hard and jerky I do not have my foot on the brake when this happens. I'm not going fast. But the ABS is so violent it causes me to lose control of the steering. It happened 3 times in 24 hours and always in same situation. Only lasts a few seconds. Other Sienna owners have had same issues. 119k miles on the van. Bought it used. Had no idea. Of course dealership can't replicate the problem and won't help us. I'm scared to drive the car.
