AIR BAGS:FRONTAL:PASSENGER SIDE:INFLATOR MODULE
Mazda North American Operations (Mazda) is recalling certain 2003-2008 Mazda6, 2004-2005 MPV, 2004 RX-8, and 2006-2007 Mazdaspeed6 vehicles. These vehicles had their passenger frontal air bag inflators previously replaced under a prior recall using inflators of the same design. The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.
Remedy: Owners are advised not to drive their vehicles until the remedy is performed. Mazda will notify owners, and dealers will replace the front passenger air bag inflator with an alternate one, free of charge. The recall began December 3, 2019. Owners may contact Mazda customer service at 1-800-222-5500, option 4. Mazda's number for this recall is 2618F. Note: This recall supersedes recalls 13V130, 14V362, 14V773 and 15V869.

Mazda is also recalling 2005-2006 MPV vehicles in Alabama, Arizona, Arkansas, California, Delaware, the District of Columbia, Florida, Georgia, Hawaii, Illinois, Indiana, Kansas, Kentucky, Louisiana, Maryland, Mississippi, Missouri, Nebraska, Nevada, New Jersey, New Mexico, North Carolina, Ohio, Oklahoma, Pennsylvania, South Carolina, Tennessee, Texas, Virginia, West Virginia, Puerto Rico, American Samoa, Guam, the Northern Mariana Islands (Saipan), and the U.S. Virgin Islands. On July 9, 2019 this recall was expanded to include certain 2004-2011 RX-8 vehicles that were previously recalled under 16V-354 but have not yet been repaired. These vehicles are equipped with certain air bag inflators assembled as part of the passenger frontal air bag modules, and used as original equipment or replacement equipment. In the event of a crash necessitating deployment of the passenger frontal air bag, these passenger air bag inflators may explode due to propellant degradation occurring after long-term exposure to absolute humidity and temperature cycling.
Remedy: Owners are advised not to drive their vehicles until the remedy is performed. Mazda will notify owners, and dealers will replace the front passenger air bag inflator with an alternate inflator, free of charge. The recall began July 6, 2018. Mailing to the affected RX-8 owners began July 6, 2019. Owners may contact Mazda customer service at 1-800-222-5500, option 4. Mazda's number for this recall is 2618F. The affected vehicles are equipped with a dual-stage driver frontal air bag that may be susceptible to moisture intrusion and other factors, including manufacturing variability that, over time, could cause the inflator to rupture.
Remedy: Owners are advised not to drive their vehicles until the remedy is performed. Mazda will notify owners, and dealers will replace the driver's frontal air bag inflator with an alternate inflator, free of charge. The recall began on September 12, 2017. Owners may contact Mazda customer service at 1-800-222-5500. Mazda's number for this recall is 1417G. Note: This recall supersedes recall 15V-382.

The contact replaced the bulbs, but the failure persisted. The vehicle was taken to the local mechanic who diagnosed that the electrical wires had shorted out due defective wire coating. The mechanic attempted to fix the failure by covering the electrical wires, but the failure persisted. The local dealer was notified of the failure. The manufacturer was not yet contacted. The failure mileage was 165,000.

I TOOK MY VEHICLE WHEN I RECEIVED THE FIRST NOTICE AND WENT TO JIM FALK MAZDA IN KAHULUI,MAUI,HAWAII AND THEY TOLD ME SORRY WE DON'T HAVE THE PART TO REPLACE YET BUT WILL CALL ME WHEN IT COMES IN AND THAT WAS ABOUT A YEAR AGO.NOW I RECEIVED THE 2ND AND FINAL NOTICE AND I CALLED TWO DAYS AGO TO THE SAME PLACE AND AGAIN THEY TOLD ME THEY WERE GOING TO LOOK INTO IT AND CALL ME BACK. WELL NO CALL BACK YET AND I AM GETTING VERY UPSET THAT THEY DON'T FEEL THAT THIS IS IMPORTANT TO CALL PEOPLE BACK TO FIX THIS IMPORTANT PROBLEM. EACH TIME, I HAVE SENT BACK THE ENCLOSED INFORMATION CHANGE CARD, TELLING MAZDA THAT I SOLD THE CAR, AND GIVING THEM THE NAME AND ADDRESS OF THE BUYER. YET MAZDA CONTINUES TO SEND ME THE RECALL NOTICES. I WANT TO COMPLAIN ABOUT MY RECALL NOT BEING REPLACED IN A REASONABLE AMOUNT OF TIME. GOING ON 11 MONTHS, STILL NO SOLUTION, AND I WAS TOLD NOT TO DRIVE WITH A PASSENGER UNTIL IT GETS FIXED. RIDICULOUS. AIRBAGS WERE RECALLED IN DECEMBER 2015. IN JANUARY 2016 I CALLED MAZDA TO GET THIS SCHEDULED AND WENT IN LATE FEBRUARY ONCE THE PARTS CAME IN. I WAS TOLD BOTH MY DRIVER AND PASSENGER AIRBAG WOULD BE REPLACED THAT DAY. WHEN I PICKED UP MY CAR I WAS TOLD ONLY THE DRIVERS SIDE HAD BEEN DONE AND I'D BE CONTACTED ABOUT THE PASSENGER SIDE SOON. ONE MONTH LATER I GET A CALL SAYING MY OTHER AIRBAG PART WAS IN. I CALLED TO MAKE AN APPOINTMENT AND THEN WAS TOLD, NO WAIT, IT WASN'T. A FEW MONTHS LATER I RECEIVED A CALL CONFIRMING MY APPOINTMENT FOR MY AIRBAG. GREAT BUT I DIDN'T HAVE AN APPOINTMENT. I CALLED AND THEY DIDN'T KNOW WHAT I WAS TALKING ABOUT BUT WOULD LOOK IN TO IT. 3 DAYS LATER I WAS CALLED AGAIN AND SAID MY PART WAS IN TO MAKE AN APPOINTMENT. SO I DID. I SKIPPED WORK TO BRING MY CAR IN, SHOW UP, AND WAS TOLD THAT THERE WAS NO PART AND THERE NEVER WAS. I WAS TOLD THE PART WASN'T EVEN AVAILABLE AND NOT TO DRIVE MY CAR WITH A PASSENGER UNTIL ITS FIXED. I THEN RECEIVED CALLS FROM MAZDA'S CHICAGO OFFICE AND LETTER IN THE MAIL STATING INDEED, PARTS WEREN'T AVAILABLE YET AND REITERATED NOT TO DRIVE WITH A PASSENGER UNTIL ITS FIXED. THAT WAS OVER 4 MONTHS AGO AT THIS POINT. IT'S ABSURD THAT A RECALL THAT CAN SEVERELY INJURE MYSELF OR MY PASSENGERS HAS TAKEN 11 MONTHS WITH NO SOLUTION. AND TO BE TOLD "DON'T DRIVE WITH A PASSENGER" IS EVEN MORE ABSURD. I USE MY CAR FOR DAY TO DAY LIFE, THAT IS NOT AN OPTION. I JUST WANT MY RECALL FIXED. I PURCHASED NEW TIRES AND HAD INSTALLED AT COSTCO ON AUGUST 1ST 2016. I WENT TO HAVE THE TIRES RETORQUED AT COSTCO ON AUGUST 12TH. ON SEPT 1ST I WAS DRIVING NORTHBOUND ON A CITY PARKWAY STREET SO MOSTLY STRAIGHT ROADWAY WITH SOME GENTLE CURVES AT APPROX. 50-55 MILES PER HOUR, WHEN SUDDENLY MY STEERING WHEEL STARTED TO SHAKE, THEN WEAVING LEFT TO RIGHT, I TRIED TO REGAIN CONTROL BY STEERING STRAIGHT AND APPLY BRAKES TO SLOW DOWN BUT THAT JUST MADE IT WORSE AND THE CAR BEGAN SPINNING OUT OF CONTROL ACROSS THE ROADWAY UNTIL IT FINALLY LANDED TO A STOP ON THE BANK OFF THE SIDE OF THE ROAD. SEVERAL PEOPLE STOPPED AT THE SCENE TO MAKE SURE I WAS OK. A COUPLE THAT LIVED NEAR BY SAID THEY HEARD A LOUD BOOM...A POLICE OFFICER ALSO CAME AND STAYED WITH ME WHILE I WAITED FOR THE TOW TRUCK. THE OIL LEAKS OUT OF THE FAULTY SEALS ON THE TIMING CHAIN COVER IN FRONT OF THE PASSENGER, AND THE OIL DROPS ONTO THE CATALYTIC CONVERTER. THIS OIL VAPORIZES AND THAT VAPOR ENTERS THE CABIN IN SLOW SPEED DRIVING AND WHEN STOPPED AT INTERSECTIONS. I COMPLAINED ABOUT A BURNING OIL SMELL MANY TIMES WHILE THE CAR WAS STILL UNDER WARRANTY, AND THE MAZDA SERVICE TECHS ALWAYS PRETENDED LIKE THEY DIDN'T FIND ANY PROBLEM. YEARS DOWN THE ROAD, I AM FINDING THIS IS A RAMPANT PROBLEM WITH ALL FORD AND MAZDA CARS AND SUVS WITH THIS "DURATEC" 3.0 V6 WHICH WAS MADE BY FORD. IT IS INSTALLED IN MANY FORD, MERCURY AND MAZDA VEHICLES. I CARRY A FIRE EXTINGUISHER WITH ME IN THE EVENT I HAVE AN UNDER HOOD FIRE, AND I'M VERY WORRIED ABOUT MY HEALTH AFTER ALL THESE YEARS OF BREATHING THIS VAPOR. OVER TIME, THE CLUTCH IN MY MANUAL TRANSMISSION 2004 MAZDA 6 V6 HATCHBACK HAS BEEN GETTING NOISY, DIFFICULT TO MODULATE WITH SURPRISING SLOPPY MOVEMENT IN A SIDEWAYS MOTION WHEN BEING DEPRESSED, AND HAS BEEN GREATLY WORRYING ME. UPON INVESTIGATION, I DISCOVERED CRACKS IN THE FORMED AND WELDED STEEL FRAME THAT HOLDS THE CLUTCH PEDAL WHERE THE WHOLE ASSEMBLY BOLTS TO THE FIREWALL. I'VE ADJUSTED MY CLUTCH LINKAGE, BLED THE HYDRAULIC LINE AND ATTEMPTED TO BEND THE FRAME BACK TO SQUARE, BUT EVENTUALLY THIS FRAME IS GOING TO FAIL AND I WILL BE UNABLE TO PUSH THE CLUTCH AND CHANGE GEARS. IF THIS HAPPENS WHILE DRIVING, I COULD CRASH AND BE INJURED OR KILLED. IN ADDITION, THE CLUTCH PEDAL FRAME, AS IT BENDS TO THE LEFT WITH EVERY SHIFT, MAKES CONTACT WITH AN ELECTRICAL/CPU BOX MOUNTING FRAME NEXT TO THE CLUTCH, WHICH IS ADDITIONALLY WORRISOME. THIS WELDED STEEL FRAME IS MADE FROM SURPRISINGLY LIGHT GAUGE METAL AND SHOULD HAVE BEEN DESIGNED TO LAST THE LIFE OF THE CAR.
