My 2015 KIA optima experienced a catastrophic engine fire while I was driving. There was no warning, I just heard a boom in the engine bay the breaks stopped working and all electrical cut out. As I'm coasting, because I'm unable to stop, flames started to come in through under the dash. I couldn't
... turn the car off, nor put it in park so the doors stayed locked. The driver window was opened just enough for me to grab the top and break it, allowing me to crawl out. The car coasted itself to the shoulder then blew up (I had a full tank of gas.). All this happened in the manner of 30 to 35 seconds. It's important to note that KIA just replaced the engine 2 months prior to this incident.Read more

An authorized Dodge dealer replaced the original rearview backup camera under warranty in 2021 with Mopar replacement part 56054158AG. The replacement camera later became inoperative, displaying a black or distorted image and eventually no usable rearview image when the vehicle is shifted into rever
... se. I later received an official FCA US owner notification for Mopar replacement camera part 56054158AG under Safety Recall 60C (NHTSA Recall 25E-052). The owner notification states that the remedy is available. Attached are the dealer's 2021 warranty repair invoice documenting installation of part 56054158AG, the FCA recall notices, and the dealer's July 2026 service record documenting that there were no active recalls associated with the vehicle. When I presented the vehicle, the owner notification, and the repair documentation to the authorized dealer, I was told there is no active recall associated with the vehicle, and the dealer documented that there were no active recalls for the vehicle at that time. As a result, the dealer did not perform the recall repair. I am concerned that owners of vehicles with recalled replacement cameras may have no clear path to obtaining the recall remedy when the replacement part is documented but no active recall is associated with the vehicle. The backup camera remains inoperative, reducing rearward visibility while backing and increasing the risk of a crash.Read more

**Incident Report — Tesla Model Y Safety System Failure** The subject vehicle, a Tesla Model Y equipped with Full Self-Driving (FSD) technology and advanced safety features, was involved in a collision at an estimated impact speed of 40–45 MPH. The vehicle's onboard recording system captured th
... e incident and clearly documents the impact dynamics, vehicle speed at impact, and the contributing actions of the other party involved. **Incident Details:** - **Impact Location:** Left front side of vehicle - **Impact Speed:** 40–45 MPH - **Damage Assessment:** Vehicle declared total loss by insurance - **Recorded Evidence:** Onboard system footage available **Safety System Failures:** Despite the severity of the impact and speed involved, the following safety systems failed to activate: - Emergency braking system did not engage - Forward Collision Warning Failed - Seatbelt pre-tensioning/locking mechanism did not activate - Airbag deployment systems failed (front, side, curtain, and knee airbags) **Concern:** The vehicle's advanced safety features—which are primary design components marketed for collision mitigation—did not function as designed or expected. Given the impact speed and severity, these safety systems should have either mitigated the collision or deployed to protect occupants. The failure of multiple redundant safety systems raises questions regarding the reliability and function of these critical systems at the time of impact. **Available Evidence:** - Onboard system footage (speed, impact angle, timing) - Insurance total loss assessment - Medical records (if applicable) - Vehicle service historyRead more

I am reporting multiple factory defects on my 2024 Honda CR-V, led by a severe, confirmed steering safety defect commonly known as 'sticky steering' where the steering wheel sticks/notches while driving and requires excessive force to correct, causing dangerous lane swerving. The authorized Honda de
... alership confirmed the steering gearbox has failed and requires replacement but the necessary parts are backordered indefinitely, forcing me to leave the unsafe vehicle at the dealership. Furthermore, the vehicle suffers from two additional major unresolved warranty issues recorded under the same Repair Order: a defective door weather stripping seal that has peeled off and detached from the door frame, compromising cabin weather sealing, and a strong, musty mold odor emitted from the AC system on low fan speeds that poses an air quality and health concern for occupants. Due to this unaddressed steering safety hazard, additional factory defects, and backordered parts, the vehicle remains out of service at the dealership (visited on 11 July, noticed that it's unsafe driving and returned the vehicle to service on 16 Jul) and no replacement car has been provided.Read more

I am submitting supplemental evidence regarding my 2021 GMC Sierra (VIN [XXX]) for Recall Query RQ26001, Campaign 25V-274. Despite multiple repair attempts, my vehicle remains unsafe and non-compliant with GM Service Bulletin N252494002. The recall remedy was performed deficiently. [XXX] Northeast
... Buick GMC installed a remanufactured short block instead of the required complete engine, reused the original seized-engine oil pan (which continues to leak), and failed to address critical electrical damage. Documentation proves they deleted Labor Operation 14BUZ (Engine Electrical) after their technician noted a blown mega fuse. A second dealer, Penske Chevrolet, subsequently deleted Labor Operation 77CVZ01 from my repair order, demonstrating a systemic pattern of ignoring recall-required repairs. Electrical damage has cascaded, resulting in a burnt fuse block and active faults P0615, P0617, P26E4, P26E6, U0623, and U0284. More critically, within 24 miles of my last service, the vehicle developed a new, severe safety failure: the "Service Brake Pad Monitor" system failed, accompanied by a red "Service System" dashboard alert and a noticeably spongy brake pedal. These brakes were recently serviced, yet this failure introduces a new risk of crash not present prior to the recall "remedy." GM Executive Resolution Case XXX has provided no resolution, reimbursement, or safety assurance, ignoring all deadlines since XXX. I possess extensive documentation, including service orders showing deleted labor operations and diagnostic logs. I request this update be integrated into the investigation of RQ26001, as the current remedy is demonstrably ineffective and has exacerbated safety hazards. The manufacturer’s refusal to remediate these issues, despite my compliance, leaves me with an unsafe vehicle and significant financial loss. I am prepared to provide all supporting records to substantiate this complaint. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

COMPONENT: 2021 Ford Expedition Max 10R80 transmission. The CDF clutch cylinder (7H351) sleeve exhibits axial movement, breaking the clutch apply-pressure seal so commanded torque is not transmitted to the wheels. Engine revs while the vehicle fails to accelerate or move forward. Vehicle is availabl
... e for NHTSA inspection and testing. INCIDENTS (7/5/2026, same drive cycle): (1) Highway — after braking from a 60 mph cruise to a 15-20 mph roll, the transmission failed to engage any gear on re-acceleration. (2) City — at 35 mph while accelerating to merge lanes, the transmission again failed to engage a gear. Engine 205-220F, Trans 200-210F, Indicated Gears 4th/5th, Odometer 120,217. Second owner; purchased at 115,067 mi. SAFETY: Sudden, unannounced loss of forward propulsion with no dash warning directly endangers my wife, my children, and other motorists during ordinary driving. Merging: stuck at 40-50 mph in 65-80 mph traffic invites a high-differential rear-end impact. Intersections/left turns: a mid-turn stall exposes the passenger side (where my family sits) to a T-bone at posted speed. Evasive maneuvers: failed acceleration converts a near-miss into an unavoidable collision. Grades: the ~7,000-lb SUV can roll backward or stall in-lane on short-sight-line hills. Towing (rated capacity) magnifies every risk to trailer and following traffic. Shoulder stranding invites secondary collisions, a known highway-fatality cause. The defect is unpredictable — I cannot pre-empt merges, intersections, hills, or tows. DEALER/INDEPENDENT CONFIRMATION: None. I have not brought the vehicle in for service, to avoid compromising any NHTSA inspection or testing. MFR/POLICE/INSURANCE INSPECTION: None; no crash has occurred to this date. PRIOR WARNINGS: None. No lamps, messages, or symptoms preceded these failures, which first occurred 7/5/2026. Ford has documented awareness of the 10R80 CDF clutch cylinder design defect and has not issued a recall (TSB 23-2250).Read more

There are complexities as to wh this lawsuit is on behalf of the turbo engines, and not the non turbo engines, however the same problems are well documented and widely acknowledged throughout hundreds if not more instances of people online and by Mazda's acknowledgment of the problem through the TSB's. This put me at great safety risk. I was driving north bound right beside semi trucks and busy traffic at speed limits of 70 mph when my engine started smoking. I was able to pull over and get it to a nearby mechanic. They officially diagnosed the vehicle w the cracked cylinder head matching the aforementioned TSBs. Recurring electronic transmission shift control failure on a 2023 Chevrolet Bolt EV. The Electronic Transmission Shift Control Pushbutton Circuit Board (GM part designation K212) has failed twice within approximately 9,300 miles. First failure occurred at approximately 45,695 miles in July 2025. The
... dashboard displayed a malfunction indicator and a "Service Transmission" warning. Diagnostic trouble code U18D2 was set: Lost Communication with Transmission Range Selector Control Module on the Powertrain Sensor CAN Bus. The selling Chevrolet dealer replaced the K212 pushbutton circuit board and reprogrammed the HPCM2 module. Second failure occurred at approximately 54,969 miles in March 2026, only about 9,300 miles after the first repair. Same diagnostic code family, including the identical U18D2 code, plus U0076, U18C6, and U240E. The same dealer replaced the same K212 pushbutton circuit board a second time. The repair was processed under manufacturer warranty. Safety risk: when the failure occurs, communication with the transmission range selector is lost intermittently and without warning, leaving the vehicle unable to be reliably shifted into Drive, Reverse, or Park. This creates a clear hazard in traffic, parking lots, and on inclines. The failure mode is sudden and the warning lamps appear only after the malfunction has begun. The component is located beneath the center cup holder area. There is no visible waterproofing or moisture isolation around the circuit board, despite its location directly under a known liquid exposure zone. The failed component from the second repair was reportedly shipped back to GM by the dealer. The vehicle is available for inspection. The failure has been confirmed twice by an authorized Chevrolet dealer.Read more

We have had this truck since August 2025, the engine has stalled while driving numerous times since we have had it. This happens randomly and unexpectedly, the vehicle loses power and stalls, shifts itself back into park and engages the emergency break. It does not show any check engine lights for t
... he dealership to replicate the issue. The engine will start back up after it stalls. We have always bought Ram trucks and never had this issue before. When speaking to many different mechanics they are relating this issue to the 2021 E-torque malfunction. (37A/NHTSA3A-265). The reliability of this truck causes severe safety concerns, as our truck stalled in the middle of a highway with my children in the vehicle this past weekend. The ongoing investigation should be expedited to prevent severe or even fatal accidents.Read more
