Vehicle is experiencing a distinct, high-pitched mechanical whine or grind sound from the transmission during acceleration that increases in pitch with vehicle speed. The noise does not happen while cruising or coasting. This symptom aligns directly with Toyota Technical Service Bulletin T-SB-0008-2

This is related to the 23V-010 / XE1 recall and extended warranty for the 2018 Chrysler Pacifica Hybrid Limited. The manufacturer is refusing the recall remedy because the catastrophic nature of the failure (transmission rupture) makes it impossible for the dealer to pull diagnostic codes. This cre
... ates an impossible standard for the consumer and effectively nullifies a federal safety recall. On XXX, I brought my 2018 Chrysler Pacifica Hybrid Limited to D’Ambrosio Service in Downingtown, PA. I reported that the car was making a sound during acceleration. On April 16, I was advised that the noise was the rubbing of the heat shield which was reportedly adjusted and tested that the noise was gone. On April 19 I sent a message to D’Ambrosio that the noise remained however I never received a response. I believe that the noise was misdiagnosed and was actually warning signs of an imminent transmission failure. On XXX, while driving, my wife and I heard a pop, saw smoke pouring out the back and immediately pulled over to discover a large amount of fluid spilling out of the bottom of the vehicle. The vehicle was towed to D’Ambrosio service in [XXX] where it has remained unaddressed until June 10. On XXX I received a phone call from D’Ambrosio that the vehicle transmission had a hole in it (quote from service advisor "internal failure of the transmission, which has caused components to puncture the casing") and would need to be replaced at my cost. When I inquired about the XE1 warranty extension, I was told that the lack of diagnostic codes didn’t justify the warranty. I received a check engine light, code P0303, misfire Cylinder 3. There is evidence my engine was overheating as the spark plug associated with cylinder 3 was melted and unable to be removed. I took my vehicle to Ford to diagnose the issue and was advised there was coolant intrusion into my engine.
... Upon researching the issue there is a trove of information relating to my vehicle and other Ford vehicles with 1.5L Ecoboost engine. My Vehicle is a 2019 Ford Escape Vin [XXX]. My vehicle fits all the specifications of Ford Customer Satisfaction program 21N12, which acknowledges coolant leaking into the engine and what symptoms a vehicle exbits when coolant leaked is present. From my understanding there is a risk of engine fire related to this issue. I can no longer safely drive the vehicle. I was using Full self driving (FSD), but I notice the autopilot gets "impatient" when (correctly) stopped for a "long" period of time (~2 minutes), but will then try to proceed in dangerous conditions. 1. I had FSD engaged while waiting to make an uncontrolled left turn. The street I'm turning onto

I was honked from both sides (rightfully so) and put in a very dangerous position as my car was now stopped in the middle of the row of both lanes. 2. ... rt. The failure appears related to the electrical/charging system (12V battery, battery management system, alternator, or related modules). The vehicle is available for inspection upon request. This failure placed my family and others at serious risk. The loss of propulsion occurred in traffic, creating a collision hazard. After stopping, the vehicle had no heat and limited power while unable to start leaving us stranded on the shoulder of a heavily traveled highway in 10-degree weather. My two toddlers were in the vehicle, creating additional risk from traffic exposure and cold. This was a repeat incident. Approximately two weeks earlier, the vehicle experienced a similar power loss. A Cadillac dealership replaced the 12V battery and stated the issue was resolved. However, on January 22, 2026, I contacted the dealership and provided a video showing abnormal starting behavior, indicating the problem may not have been fixed. I followed up by phone the next day but received no response. Despite this prior repair and warning, the same failure occurred again. There were no clear warning lights or messages immediately prior to the power loss. After the failure, the vehicle was unable to restart. I also attempted to use OnStar, a paid emergency service, but was placed on hold for approximately 30 minutes. Due to unsafe conditions, my wife had to come pick us up to remove our children from danger. This incident involves a repeat loss of power while driving, prior warning signs, and failure of emergency support systems, raising serious safety concerns.Read more

I was driving home Friday August 29th 2025 around 10:30 am on I-70 W when I began to have trouble accelerating my vehicle. There were vehicles trailing closely behind me so I increased pressure on the gas pedal. I would feel my car forcibly trying to compensate and the RPMs on the vehicle increased
... significantly. The check engine light then illuminated and the car stedily beagan to decelerate although RPMS were increasing and the force on the gas pedal was at its maximum. I could no longer get past 30 MPH. The cars behind me had to quickly change lanes behind me to avoid rear end collision and I finally pulled over to the shoulder of the highway but it was a very narrow shoulder so my safety and the safety of those driving around were compromised because they could have easily resulted in a high speed rear-end collision that could have been fatal and the location of my car on the shoulder was not safe and I could have been side-swipped waiting for a friend to come get me and for me to move my car off hat narrow shoulder. I was frantic and very emotionally distressed by this incident because all the maintenance on the car was up to date and there was no check engine light on in the car prior to this incident. The unexpected and decelleration and inability to compensate on the highway was very unsafe. I have taken my car to the delearship and they have yet to address my engine issues or the safety concerns. The rep actually laughed while I was recounting the events. What component or system failed or malfunctioned, and is it available for inspection upon request? The Engine Control Module (ECM) is malfunctioning, triggering persistent P0101 codes related to the Mass Air Flow (MAF) sensor. This specific issue is addressed in Nissan Technical Service Bulletin DE9
... 7AA, which recommends an ECM reprogramming. The component is still installed in the vehicle and is available for inspection. ⸻ • How was your safety or the safety of others put at risk? When the vehicle is at a complete stop and I attempt to accelerate, it becomes jumpy and unresponsive, refusing to accelerate properly. This creates a serious safety hazard, especially when trying to merge into traffic or cross intersections, as I am unable to match the speed of other vehicles. This increases the risk of being rear-ended or causing an accident due to unexpectedly slow acceleration. ⸻ • Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. An independent mechanic diagnosed the issue and confirmed the code P0101. After reviewing Nissan’s official TSB DE97AA, they advised me to go to a Nissan dealership to have the ECM reprogrammed as outlined in the bulletin. ⸻ • Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle was inspected by an independent mechanic, but not yet by the manufacturer. The dealership is aware of the issue but has refused to perform the repair without payment. ⸻ • Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes. The Check Engine Light is consistently triggered by code P0101, and the traction control warning light also comes on when the issue is active. The symptoms appeared recently and continue to recur even after the code is reset.Read more

Despite having over 200,000 miles, a Firestone inspection from September 2024 confirmed the vehicle was in good condition prior to the failure. Additionally, Hyundai Theta II engines are widely known to suffer from excessive oil consumption, a defect that can lead to premature engine failure even in well-maintained vehicles. Hyundai has replaced engines in vehicles with similar mileage under the same extended warranty, yet in this case, no proper inspection was conducted before denial. Safety Concerns: The fuel pipe recall was not inspected, raising concerns about whether unresolved recall defects are leading to engine failures or fire risks. When driving on a cold rainy day, I turned on defrost heat to clear my windscreen. No heat came out. AC, fans and zone select worked fine, but no heat. So I had to stop repeatedly on a narrow road to wipe windshield. After reviewing user forums, some pointed to a known problem with these Sprinte

Also mentioned in forum, there is also a manufacturer software update that prevents this issue by keeping the actuator servo motor from moving to the very last position (cold or hot). This simple preventive fix was never offered to us during our warranty period. Some forum users went further with a modification to servo motor shaft - apparently this fixed the undesirable flexing on heater flap. Now that the servo motor has failed and I no longer have any heat, I do recognize that there were warning signs. I now recall there were occasional instances where my temperature select did not have any effect until I drove the temperature all the way up or down. I believe this was the servo motor getting stuck briefly but it became unstuck before it completely failed. I feel this can be a grave safety issue because of individual circumstance. Most times it may be an annoyance. But in certain conditions (as I experienced) it will be a significant impediment to driver visibility. It also exposed me to roadside risk when I stopped to wipe inside of windshield.Read more
