While operating my 2024 Toyota bZ4X at very low speed on the road , I observed a stationary shipping container directly ahead and applied the brake pedal with maximum force approximately 4 seconds before contact. I held the brake pedal at full pressure continuously. I never touched the accelerator.
... The vehicle came to what appeared to be a complete stop then continued to advance and made physical contact with the container despite my maintained maximum brake input. The vehicle's own ECU-recorded Freeze Frame Data (retrieved by an authorized Toyota dealer using PCS Data Viewer) confirms the following at the time of the event: Driver brake pedal torque was at sensor saturation (4,048 Nm — the maximum value the sensor can record) for approximately 1.75 seconds prior to impact. Brake hydraulic line pressure peaked at 12.144 MPa and remained above 7.8 MPa throughout that period. Throttle position was 0.0% for the entire recorded interval. All four wheel-speed sensors recorded 0.00 km/h for approximately 2.95 seconds prior to impact. Steering input was effectively neutral; no skid or loss of directional control occurred. The longitudinal accelerometer recorded a peak deceleration spike of -8.97 m/s² at the moment of impact — a value that requires forward velocity and is physically inconsistent with the wheel-speed data showing the vehicle stopped. The Pre-Collision System (PCS) main switch was ON throughout, with no faults, no obstruction flags, and no warning, pre-brake, brake-assist, or auto-brake activation recorded. The Parking Support Brake (PKSB) did not activate. No diagnostic trouble codes were set. In plain terms: with maximum brake pressure applied and zero throttle input, my vehicle continued to move forward into a clearly visible stationary obstacle. This is a completely unacceptable design as the vehicle will move seats in auto-cycle despite the fact that there is a person sitting in the seat. I reported this problem to the dealer and to Hyundai Corporation as a safety issue. I insisted that the vehicle design should be immediately changed and that they treat this as a safety issue. Hyundai engaged with a third party inspector (EAA Bosch) to review the vehicle and the design. Hyundai's final disposition on this to me (sent on Feb. 27, 2026 stated: "...there is no evidence of a defect or malfunction contributing to the reported condition with the seat, at this time. Therefore, while we sympathize with you about the incident, in the absence of a finding of a defect, Hyundai will not be accepting any liability or offering any financial assistance." We asked Hyundai to fix this problem before someone got hurt. We drove approx. 1.5 miles to a parking lot where the engine seized or locked up. The truck was towed to the above referenced dealership. The dealership confirmed the engine was locked up the next day. During the Summer of 2025, the dealership inspected the engine and performed an oil consumption test after I reported the engine had burned two quarts of oil between oil changes. Their fix was to transition from 0W-20 to 0W-40 oil, which has been adhered to since. The truck only has 56,000 miles on it.Read more

This failure placed my family and others at serious risk. The loss of propulsion occurred in traffic, creating a collision hazard. After stopping, the vehicle had no heat and limited power while unable to start leaving us stranded on the shoulder of a heavily traveled highway in 10-degree weather. My two toddlers were in the vehicle, creating additional risk from traffic exposure and cold. This was a repeat incident. Approximately two weeks earlier, the vehicle experienced a similar power loss. A Cadillac dealership replaced the 12V battery and stated the issue was resolved. However, on January 22, 2026, I contacted the dealership and provided a video showing abnormal starting behavior, indicating the problem may not have been fixed. I followed up by phone the next day but received no response. Despite this prior repair and warning, the same failure occurred again. There were no clear warning lights or messages immediately prior to the power loss. After the failure, the vehicle was unable to restart. I also attempted to use OnStar, a paid emergency service, but was placed on hold for approximately 30 minutes. Due to unsafe conditions, my wife had to come pick us up to remove our children from danger. I was driving home Friday August 29th 2025 around 10:30 am on I-70 W when I began to have trouble accelerating my vehicle. There were vehicles trailing closely behind me so I increased pressure on the gas pedal. I would feel my car forcibly trying to compensate and the RPMs on the vehicle increased
... significantly. The check engine light then illuminated and the car stedily beagan to decelerate although RPMS were increasing and the force on the gas pedal was at its maximum. I could no longer get past 30 MPH. The cars behind me had to quickly change lanes behind me to avoid rear end collision and I finally pulled over to the shoulder of the highway but it was a very narrow shoulder so my safety and the safety of those driving around were compromised because they could have easily resulted in a high speed rear-end collision that could have been fatal and the location of my car on the shoulder was not safe and I could have been side-swipped waiting for a friend to come get me and for me to move my car off hat narrow shoulder. I was frantic and very emotionally distressed by this incident because all the maintenance on the car was up to date and there was no check engine light on in the car prior to this incident. The unexpected and decelleration and inability to compensate on the highway was very unsafe. I have taken my car to the delearship and they have yet to address my engine issues or the safety concerns. The rep actually laughed while I was recounting the events. I also called hagerstown police department to have a police officer come out to the breakdown site because I did not feel safe but my frined arrived before the police.Read more

... r vans. Apparently the actuator servo motor that controls the heater flap can get stuck at limit positions and fail. This explains why my heater is stuck on cold. Also mentioned in forum, there is also a manufacturer software update that prevents this issue by keeping the actuator servo motor from moving to the very last position (cold or hot). This simple preventive fix was never offered to us during our warranty period. Some forum users went further with a modification to servo motor shaft - apparently this fixed the undesirable flexing on heater flap. Now that the servo motor has failed and I no longer have any heat, I do recognize that there were warning signs. ... r vans. Apparently the actuator servo motor that controls the heater flap can get stuck at limit positions and fail. This explains why my heater is stuck on cold. Also mentioned in forum, there is also a manufacturer software update that prevents this issue by keeping the actuator servo motor from moving to the very last position (cold or hot). This simple preventive fix was never offered to us during our warranty period. Some forum users went further with a modification to servo motor shaft - apparently this fixed the undesirable flexing on heater flap. Now that the servo motor has failed and I no longer have any heat, I do recognize that there were warning signs.
