On multiple occasions, my vehicle's 12v battery has completely discharged presumably from parasitic drain from connected services communications. The first time the 12v was completely discharged was on 6/10/26 following between 24-48 hours of sitting idle without being driven. It was recharged and t
... he following day had discharged almost completely, giving a warning that it needed to be charged back up. This was after ~24 hours without being driven. I ultimately brought it to the dealership on 6/17/26 where they were unable to replicate the issue initially and cleared the vehicle to be picked up on 6/19/26. Upon my arrival to pick up the car the following day (6/20/26), the 12v battery was dead on the lot. My vehicle then remained at the dealership for continued investigation before ultimately being returned to me after resetting the vehicle software and recharging the battery. At the point that I picked the car back up on 7/15/26, I installed a battery monitor device (Ancel BM200) and since then I have observed a cyclic pattern of voltage decline in a sawtooth pattern that begins 3 hours after turning the vehicle off and continues until the vehicle is driven again. I have also observed - via an outdoor security camera with IR capability at my home - a corresponding constantly repeating cycle during the sawtooth draining pattern where there is an IR light (presumed the driver monitoring camera) that turns on and off every 65 seconds. Although it has not fully discharged again, it is only because I have monitored the voltage and recharged or driven it every time I note the voltage getting too low (<12.0V). I am confident that if I were to allow the vehicle to sit for >24 hours without recharging it, the battery would completely discharge again. My safety and the safety of my family have been put at risk due to the risk of being stranded without the ability to operate the vehicle due to a fully discharged 12v battery in the vehicle.Read more

VEHICLE INFORMATION Year/Make/Model: 2020 Ford Escape Hybrid Titanium Component: Electrical / Instrument Cluster / Digital Display Unit (IPC) Hazard: Critical Visual Hazard & Operational Safety Defect STATEMENT OF SAFETY CONCERN The 12.3-inch digital instrument cluster in the 2020 For
... d Escape Titanium is experiencing severe pixelation and flickering, indicating imminent screen failure. Because this trim lacks mechanical gauges, screen failure blinds the driver to vital real-time operating metrics: Speedometer: Prevents real-time speed monitoring, increasing collision risk. Engine Temperature: Hides thermal warnings, risking sudden engine breakdown or overheating. Fuel Gauge: Obscures fuel levels, risking sudden loss of propulsion in active traffic. Warning Lights: Hides critical alerts for Brake System, Airbags, Tire Pressure, and High-Voltage Hybrid System faults. OPERATIONAL HAZARD ANALYSIS Driving with a malfunctioning instrument panel creates immediate hazards: Driver Distraction: Constant screen flickering directly in the driver's line of sight creates a dangerous visual distraction, especially at night. Power Loss Risk: Lack of fuel/hybrid monitoring increases the likelihood of stalling on highways or busy intersections. Loss of Safety Warnings: The vehicle cannot communicate critical restraint or braking system failures to the driver. REQUESTED ACTION / RESOLUTION This condition represents an active safety defect. I request: Immediate diagnostic and replacement of the digital instrument cluster unit (IPC) or control module under applicable technical/safety service guidelines. Formal documentation of this defect with Ford Motor Company and reporting to federal regulators (NHTSA).Read more

The vehicle was taken to the dealer, where the contact was informed that the sound was likely coming from the fuel injectors. The contact stated that while driving from a complete stop on several occasions, the vehicle hesitated to respond. The vehicle was taken to the dealer, where an unknown part was disconnected. Additionally, the contact stated that while driving approximately 25 MPH and making a turn on a bypass, the brake pedal travelled down to the floorboard while depressed, and the vehicle failed to respond, causing the contact to panic. The contact shifted to park(P) and drove over a grassy area, but the vehicle failed to respond as needed. The contact stated that the vehicle eventually rolled to a stop. The vehicle was towed to the dealer, where it was diagnosed and it was determined that the driver's side rear brake line was loose and leaking, and there was air in the brake line. While driving at highway speeds (approx. 65 MPH), the vehicle suffered a sudden and catastrophic loss of motive power. The "Hybrid System Malfunction" warning illuminated, followed immediately by a "Check Engine" light. The vehicle entered a fail-safe mode, then completely shut down. The steering wh
... eel locked, making it extremely difficult to maneuver the vehicle to the shoulder in a dark, remote area with freezing temperatures. This presented an immediate life-safety risk to all occupants. Following the incident, the vehicle was presented to two separate authorized Toyota dealerships in the US for emergency assistance. Both failed to provide the standard of care required for a high-voltage safety failure. The first dealer refused to perform even a basic safety inspection or intake the vehicle, stating they had no hybrid technicians on weekends. They turned the vehicle away without safety warnings, forcing the operation of a chemically failing vehicle on public roads. The second dealer attempted to charge a diagnostic fee. However, when questioned, they admitted they also did not have a hybrid-certified technician on site. They attempted to bill for a diagnostic service they knowingly could not perform. This fraudulent charge was only rescinded after it was challenged. They subsequently refused further assistance, leaving the occupants stranded. The vehicle was eventually towed to a third dealer (in Canada) where it was diagnosed with severe corrosion of the High-Voltage Floor Under Wire Harness (MGR Cable), a known defect. The dealer confirmed the rear motor cable was corroded and required replacement. The repair involved replacing the cable and installing a "splash guard" cap. No prior warning signs occurred before the sudden highway shutdown. This failure is a result of the manufacturer's failure to adequately seal high-voltage components against corrosion and the dealer network's gross negligence in refusing emergency safety support.Read more

At 2,000 miles car experiences an issue when going from drive to reverse the car issued warning that transmission needs to be serviced, wouldn’t go into any gear and wouldn’t turn off. Had to be dragged into flatbed truck and taken to dealer. At 3,000 miles had to stop in road due to medical h

Same errors as before (can’t get in gear, warning lights and won’t turn off) and car had to be dragged onto a flat bed tow truck. Fire department initiated the tow due to safety issues of being in dangerous location. After a week at dealer was told it was ready to pickup. Then told to see a mechanic there who would show me what to do next time this happened. Involved unhooking 12v battery and pulling some fuses. Informed issue wasn’t fixed just a work around was done. I talked to service manager and was informed this is a known issue and factory is working on a repair. The car isn’t safe breaking down twice while driving on public roads and not being able to turn off. Note: in both cases the car wouldn’t turn off for days even while parked at dealer lot.Read more

At one point i called the dodge home office again only this time i got a recording that said; "You are calling about your 2013 dodge truck. (I wondered how she had this information.) Please press a number to be connected to someone." I was connected to a lady in Taiwan. She spoke very broken English and I asked her to put someone else on the phone. She argued that she spoke English and I got irritated with her and hung up. I believe this is by design as dodge wants to do just that, dodge their responsibilities to their customers. No one has inspected my truck as I can't remove it from the garage. I was told that the tool that was plugged into my computer system would not reveal my problem if the steering system was working. •What component or system failed or malfunctioned, and is it available for inspection upon request? Driver and passenger side rear view mirrors will not hold memory and auto adjust while driving at times. Been to the dealership 5 times. They could not repair and just said that a software update sh
... ould fix it, but it did not fix it. I will program the mirrors to the preferred settings but when I re-enter the vehicle after shutting it down, it doesn't always retain the memory, and they go to random positions. This usually happens after 5 miles of driving the vehicle. The passenger side mirror will go down so I could see the curbs while in reverse but when I go into Drive, it does not go all the way back up. I will be flying to visit family but the vehicle will be at a different dealership from 3/26/2025 to 4/1/2025. •How was your safety or the safety of others put at risk? After adjusting the mirrors, I left my garage with my kids in the car and started driving to our destination. While entering the highway, I went to check my driver’s side rear view mirror and noticed that it was pointed to a random position. In that moment, I was trying to match the highway speed to enter, there was a vehicle in front of me, a vehicle behind me, and a vehicle on the highway exit/entrance lane that I was trying to be cognizant of location to adjust speed properly. I ended up doing multiple shoulder checks which caused me to lose visibility of the vehicle in front of me multiple times. •Has the problem been reproduced or confirmed by a dealer or independent service center? Installed trailer brake assist. When we shut the truck off, the brake lights came on and stayed on. Assured by B&B trailer of St Augustine, Fl that it was an issue with the truck and not the part. Other issues have come up- dash lights also come on when turned off, backup camera, remote start, rear
... defrost, cig lighter plug, rear speakers, and A/C stopped working. Took it to Daytona Dodge on 1/29/25 where they diagnosed the ABS module had failed. They did a system reset and everything was ok until we had to disconnect the battery again- had to install a switch on the battery so lights staying on doesn’t kill the battery. Was told they’d let us know when a new part came in- mid February. Called Ram HQ on 2/17- case # [XXX] and that was no help. Referenced recall 85B and she said it was for a 2d row seating issue and said no ETA on new parts being made. My truck was mfg 2/2020 so how the software update recall doesn’t apply makes no sense. Would it have prevented this, who knows. Then called the dealer up and spoke to Stephen, a svc mgr, who recommended buying a used one ( he said aftermarket but I don’t think there is such a thing) and they would put it in. Got one from eBay and went on [XXX]. After I sat there all morning, I was told they couldn’t override the Vin that’s already on a used part. I have tried calling and emailing the service management team for follow up since then with no response. This is an abomination of customer care and needs attention. I’m hoping these parts are coming soon and that people like me are reimbursed and compensated for time lost and diagnostic fees and parts and labor. Also hoping this recall is expanded/redone to include my truck as there’s no reason it shouldn’t. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Front Windshield Crack: On January 28, 2025, I noticed a curved crack approximately 7 to 8 inches long on the inside of the front windshield, directly in the driver's field of vision. This crack is clearly visible in sunlight, and when touching the interior side of the glass, it can be distinctly felt. At that time, the vehicle's mileage was 364 miles. After inspection, it was confirmed that the vehicle had not been involved in any collisions. I reported this issue to Nissan Sales on February 1, 2025. Nissan company conducted an inspection, confirmed the presence of the crack, and stated that the vehicle had not experienced any impact. However, nearly a month has passed, and Nissan has yet to provide a clear resolution. I am deeply concerned about whether this crack poses a safety risk while driving, particularly whether the windshield might shatter unexpectedly during operation. I do not know if this constitutes a serious safety hazard, nor can I determine whether the glass will suddenly break in the future. This uncertainty is causing me significant anxiety. Since Nissan has not yet provided a timely solution, I feel compelled to formally lodge this complaint with NHTSA. I sincerely hope that Nissan will promptly address this issue and provide an appropriate resolution. Thank you! Sincerely,Read more

The contact owns a 2016 Dodge Ram 2500. The contact stated that the vehicle was included in NHTSA Campaign Number: 19V021000 (Steering) and the vehicle was taken to the dealer for the recall repair. The contact stated that the vehicle was taken for an alignment and the contact was informed that an a
... lignment could not be completed because to the tie rods were welded together. The dealer who performed the recall repair was contacted and confirmed that the tie rods were to be welded as stated in the recall. The dealer informed the contact that the repair was approved by the NHTSA and that for the alignment to be performed, the entire part had to be replaced. The vehicle was not repaired. The recall repair stated that Chrysler would notify owners, and dealers would check the steering linkage jam nut tightness, replacing the drag link assembly or welding the nut to the adjuster sleeve, free of charge. The contact believed that the dealer failed to perform the remedy due to the tie rods not specifically being mentioned as what was expected to be welded together. The manufacturer was made aware of the issue and confirmed that the remedy was to weld the tie rods. The manufacturer opened a case regarding the failure. The failure mileage was 17,519.Read more

Last week I had the air dryer filter replaced at Colton Truck Terminal in Colton Ca. Subsequently, I did the complete 7 step air brake system check and discovered that during total loss of air pressure , the emergency brakes would not auto apply. I replaced the dash hand valve and the problem stil
... l persists, so I installed the original valve and took the coach back to Colton Truck Terminal to diagnose the problem. Attached is the paper work from Colton Truck Terminal diagnosing the issue and the correction of that issue. Also on that paperwork is the vehicle information. Apparently, two air lines were crossed going into the primary air tank during chassis assembly. Once corrected, the system worked properly. I did not take it to a FL dealer because the chassis is out of warranty and Colton Truck Terminal is a well respected local repair company that almost exclusively works on Class A diesel pusher chassis. My hopes are that FL will reimburse me for this repair that was required as a result of improper initial assembly. My other concern is that there are no other chassis that may have this issue that were built around the same period. Though the emergency brakes could still be manually applied during loss of air pressure, the incorrect assembly prohibited automatic operation as required. You are welcome to call the repair facility to gather further information if you require or contact me as well by email or phone at [XXX] . Same issue as others have stated with the power steering just stopping all of a sudden. Had it cleared by a mechanic who said it is a
