While traveling at the posted speed limit and approaching a signalized intersection, I attempted to brake for stopped traffic ahead. The vehicle did not decelerate as expected, and the automatic emergency braking/forward collision mitigation system did not activate or provide any braking assistance.
... Additionally, no forward collision warning or audible/visual alert was provided prior to impact. A red warning indicator illuminated immediately before the collision; however, there was insufficient time to take additional evasive action. As a result, my vehicle rear-ended the vehicle stopped in front of me. The apparent failure of the braking assistance and forward collision warning systems placed both myself and the occupants of the other vehicle at risk, resulting in a motor vehicle collision. The vehicle is available for inspection upon request. The cause of the malfunction has not yet been confirmed by a dealer or independent repair facility (UNKNOWN). The vehicle has been inspected by law enforcement and insurance representatives as part of the accident investigation. Prior to the incident, I had not observed any warning lamps, messages, or symptoms indicating a problem with the braking or forward collision systems. The service writer {Andy} said he hadn't heard of any issues with the update. I had him log in my issues to have a record that I complained. I returned several more times and talked to other service writers who told me that if they would check the vehicle and found nothing that I would have to pay service fees. the vehicle jerks and up/downshifts frequently and almost stalls sometimes when accelerating from a stoplight. It will lung periodically when navigating a parking lot at low speeds. It frequently searches for a different gear in traffic. To be clear, my car ran perfectly before this update. I asked them to put the previous tune back into the car but they said they can't do that. The car runs fine sometimes but the uncertainty of the jerking and shifting at improper times makes it dangerous to drive. Also almost stalling at an intersection is very dangerous too. I doubt that I am the only one to have this problem. ... haft position sensor circuit. The vehicle stalled while driving on a highway mountain overpass with multiple vehicles traveling behind me. When the stall occurred, the vehicle shifted into park and would not restart for several minutes. This created a hazardous situation due to loss of propulsion in active traffic at highway speed and limited shoulder space. Following diagnosis, the servicing dealer determined that the engine wiring harness requires full replacement due to internal wiring integrity issues affecting critical engine control signals. The crankshaft position sensor and related wiring are essential for engine timing and propulsion. Loss or corruption of this signal can result in reduced engine power, erratic transmission behavior, stalling, or no-start conditions. The earlier erratic shifting behavior appears consistent with conditions described in GM Technical Service Bulletin 21-NA-149, which identifies engine harness-related causes for drivetrain and shifting concerns. This issue appears to involve degradation or defect within the engine wiring harness affecting engine management and transmission operation. Given that this condition resulted in a highway stall and temporary inability to restart, I am reporting this for investigation as a potential safety-related defect involving loss of propulsion.Read more

While waiting to turn left at a light and driving in "L" (one pedal) mode, I started to release the break in anticipation of oncoming lanes clearing, as I lifted the pedal the car lurched into the intersection. I tried to accelerate to pull through, but the car did not respond. After several second of frantically pushing the accelerator, the car slowly and jerkily moved forward and cleared the intersection. My Chevy dealer says this is a known problem with service bulletin "Chevrolet TSB 19-NA-220". In my opinion this is incredibly dangerous and should have been recalled. However, there is no recall that I can find and outrageously Chevy will not cover this manufacturing defect under warranty as they specifically exclude wiring harnesses due to past issues with rodent damage, and they are hiding behind that rule.Read more

Then I called AAA again and they towed my car back to my house and checked the small 12 volt battery to see if that was the problem, they determined that the 12 volt battery still had 75% life in it. They were unable to get my vehicle started. The same message is still on the dashboard “Driver System Fault." The failure mileage is approximately 81,567. I want to know before I call the Dealer and get it towed again if this issue is the Hybrid High-Voltage Battery or the housing bonded seam could have lead to corrosion the service recall from a while ago. The third time it happened, first week of April 2025, I got home that night, cut my engine off, and noticed a light in my dash I hadn’t seen before. It was a fog lamp bulb, but the car was off, and it wasn’t like normal, it was an error light. When I walked around to the front if the car, I saw my fog lamps were still on, which was weird. All lights were off on toe column and dash. So, I restarted my Mini and cut her back off again, then the fog lamps went off. I had no park lights, or headlights then either. I didn’t check my windows. If they did go down they may not go back up! I didn’t check my turn signals either. I was just happy the fogs went off… It was after much research, that I ‘figured’ out what was going on with my Mini concerning all of the intermittent craziness, but it never occurred to me that there was a fire hazard. I actually looked into purchasing an FRM back in February, and had put it into my shopping cart, but it cost more than I could gather together at one time. I am pretty sure that is why my Mini caught fire & burned to a total loss on April 18, 2025. There is no other explanation. She ran too good. She was my dream car. She would boot, scoot & boogie! The service manager at the Harley-Davidson dealership says they cannot fix it as the bike is out of warranty. It is a defect. 2. The coolant hose from the radiator is routed approximately 1/4 inch from the front exhaust header causing the bike to run extremely hot. The service manager at the Harley-Davidson dealership said they cannot reroute the hose until there is visual damage to the hose or a failure. “This is a safety issue and concern as well as an environmental issue” it will eventually lead to the hose blistering and bursting spilling coolant on the ground. What if I am in heavy city traffic, in the desert with no cell phone service or some place at night with no help, any of these scenarios put me in great danger. At the very least this defect should be added to the 5 year environmental warranty list of items to be corrected by the motor company.
