I purchased the car on XXX. The car has been returned to the service department within the first 3 days. Sold to me without getting fully inspected. Service department did put new brakes and rotors on and I was told (no paperwork given to me) an oil change was done. On XXX in traffic on a dangerou
... s highway, the car cut off. The car was not able to be jumped with AAA, was towed to [XXX]/GMC Service department. I was told by the Asst Svc Manager, on XXX the Tech told him My car was the starter. The part would have to be ordered and would not be ready to the following week. On July 7th I received a call from the SVC Mgr that the engine blew. I had to pull teeth to get the Mgr to pay for the engine of $8500 and yet he wants me to pay $1500.00. With having a car less than 21 days regardless if I went over the 3k miles and as -is, there was a 90 day warranty when everything should be covered. He has now rescinded because I reported this to BBB, Atty Gen office, NHTSA. This car was never inspected. I requested the On-star reports, Inspection reports for the last year and the Tech's findings and nothing has been provided except one. I requested several times the service report and the General Manager/owner is so upset, he submitted nothing.I have felt intimidation throughout this entire process. On Thursday, XXX, at approximately 5:10 AM, I was driving my Ford F-150 XLT (VIN: [XXX]) eastbound on [XXX]. While exiting the elevated bridge at the intersection of [XXX] and [XXX] at 55 mph (6th gear), the vehicle experienced a sudden, uncommanded transmission downshift directly from 6th to 1st ge
... ar. This caused immediate extreme deceleration, engine over-speed, and locked the rear tires, causing tire slippage and an absolute loss of traction control on a high-speed highway. This severe event put my life and my wife's life in imminent danger of catastrophic injury. After safely pulling over, the Malfunction Indicator Light (Check Engine) and the Powertrain Fault Warning Light (wrench icon) illuminated. The electronic cluster froze and failed to display the gear position. Drive (D) mode was completely unresponsive. I managed to vacate the active roadway using Manual (M) mode. While driving in 4th gear in manual mode under extreme caution, a second violent, unintended downshift to 1st gear unexpectedly occurred, confirming the persistent, dangerous nature of this failure. Consequential mechanical damage includes severe transmission clunking noises during gear engagement and a constant internal metallic knocking sound from the engine block caused by the forced over-speed. This failure perfectly matches the technical failure mode described in NHTSA Safety Recall Campaign 26V237 (OSS to PCM signal loss). On XXX, at 11:10 AM, I personally visited Arceneaux Ford Inc. (Authorized Dealership in [XXX]) to report this safety defect, open a service case, and request a safe vehicle inspection and loaner. The dealership explicitly refused to open a case or take action, stating that "Ford Motor Company does not currently have an engineered solution for this problem," leaving me stranded with an unroadworthy, highly hazardous vehicle. While driving a 2021 Honda CR-V Hybrid, the vehicle has intermittently and without warning lost the ability to accelerate. When this occurs, depressing the accelerator pedal — even fully to the floor — produces no response, and the vehicle simply coasts and loses speed. This has happened on thre
... e occasions — January 13, 2025; December 16, 2025; and June 10, 2026 — including at highway speed. Each occurrence is documented by a Honda dealer service invoice. In each instance, the check engine light began flashing immediately before the loss of power. The only way to restore normal operation was to bring the vehicle to a stop, turn the ignition off, wait, and restart. Restarting restores power but also clears the check engine light and any stored diagnostic trouble codes. Safety risk: Losing motive power without warning, particularly at highway speed or while merging in traffic, creates a serious risk of a rear-end or loss-of-control collision, because the vehicle cannot accelerate out of danger. The affected systems appear to be the power train and hybrid propulsion system, possibly originating from the engine given the flashing check engine light, which typically indicates an active engine misfire. The exact failed component has not been confirmed (UNKNOWN). The problem has NOT been reproduced or confirmed by the dealer during inspection. The vehicle was taken to an authorized Honda dealer for each occurrence, but because restarting the vehicle clears the fault codes, the dealer found no stored codes and could not duplicate the failure during a test drive. The vehicle has not been inspected by the manufacturer, police, or insurance. The vehicle is available for inspection upon request. This matches NHTSA Preliminary Evaluation PE25016 regarding intermittent loss of motive power in MY 2020–2022 Honda CR-V Hybrid vehicles.Read more

Based on the sudden failure and the conditions at the time, this appears consistent with that issue. The Auto Rear Defog feature in the vehicle settings was turned off, however the truck had been started using the remote start feature when the glass shattered. Photos of the shattered window are available. I am submitting this report because the rear window defroster system may cause the glass to overheat and shatter unexpectedly.Read more

We purchased our 2024 Pacifica Pinnacle AWD new in July of 2024. The vehicle currently has 9,600 miles on it, and the issue has been present since it was new. We have experienced continuous issues while driving the vehicle at highway speeds, with the steering exhibiting a gap in responsiveness. The
... steering acts like there is a dead zone between 11 and 1 o'clock when turning the wheel, where nothing happens. Once the vehicle finally reacts to the driver's inputs, it causes the car to jerk from side to side. It is bad enough that it causes my entire family to become carsick during even short trips. Still, my primary concern is that the steering is not responsive, and if the issue worsens or changes, it could lead to a loss of control over the vehicle. In addition, the jerking caused by this may cause loss of control when traveling in inclement weather or icy conditions. We took the vehicle in for diagnosis, assuming an alignment issue, but the dealership verified the alignment was within the manufacturer's requirements. In addition, the dealership told us, "This is just how the Pacifica drives. All of them have this issue with the steering". Today, I came across an investigation into a similar issue with older model Pacificas, and I believe this is not only the same issue but also warrants investigation of all model year Pacificas. There were multiple comments within the forum post of owners experiencing this issue in newer vehicles than the current investigation. Please consider expanding your investigation to all model years. I have a 2024 Honda CR-V Hybrid AWD Sport model. On two occasions within the past 3 months, I have attempted to start the car, but when I did so, the dashboard lights, including all the error indicators lit up and flashed, the alarm went off, and I couldn't drive the car. In both cases, I called Ro

But when we then tried to start the car normally (pushing start button with foot on brake) the electrical light show resumed and alarm started up all over again. Both times this happened, by the time the tow truck arrived (1-2.5 hours) the car had "rebooted" so we just drove home. I tried three different times to restart the vehicle without success. Stranded on a busy roadway, two different vehicles behind us had to take evasive action to avoid hitting us from the rear. The vehicle was towed to the dealer, who has called back to say the engine starts fine now. They will conduct further diagnosis.Read more
