I am filing a safety complaint for my 2024 Toyota Corolla because the infotainment screen intermittently goes black while driving. This is most dangerous when I am using GPS/navigation while driving. The screen has gone black while driving on busy highways at 60+ mph, leaving me without functional navigation. This creates a distraction and safety concern because I suddenly lose directions and may
... need to look at my phone or troubleshoot the screen while driving. The car has also had recurring phone connectivity issues. Sometimes my phone does not connect through Bluetooth or even when plugged in directly. However, the black screen while driving is the bigger safety issue. I contacted Toyota multiple times. Toyota reference number: #[XXX]. Toyota told me to take the car to the dealership, which I did many times while the vehicle was still under warranty. I took it to two different Toyota dealerships multiple times. They repeatedly said there was no issue because they could not duplicate it. They also blamed Fitbit devices connected to my phone, which did not explain the screen going black while driving. I sent Toyota and the dealerships videos of the issue. However, it is difficult and unsafe to record the screen failure while actively driving. I also contacted Toyota about lemon law concerns, case [XXX]. Toyota denied the claim. The vehicle is now out of warranty, but the screen still occasionally goes black while driving. This issue was reported multiple times during the warranty period and was never resolved. What component or system failed or malfunctioned, and is it available for inspection upon request? I was driving when I heard a loud boom, it almost sounded like a gunshot. How was your safety or the saf

Yes, I brought the car to the Toyota dealership. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? I did call my insurance company but I am not sure if they actually inspected the damage. They have been dealing directly with the Toyota dealership service department. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No, that's why I continued to drive until I heard the wind whistlingRead more

We bought the new 2026 Tesla Model 3 and picked it up on 5/3/2026. The next date 5/4/2026, I was trying to drive it to work. I got in the car and used the Tesla’s Full Self-Driving function and let it drive the car by itself. It started moving forward and tried to make a U turn. Since there was not enough space for it to make a U turn because there was a car parked on the sidewalk, so it stopped
... for a moment. But unfortunately, it continued to move forward and hit the car parked on the sidewalk. It supposedly backed up a little after the stop, but it didn’t detect the car parked on the sidewalk and still moved forward and hit the parked car. The Tesla's FSD is malfunctioning and caused my accident because there was such a big car in front and it didn't detect it and still hit it.Read more

There was a recall for the clutch. I took it to the dealer (Autonation Jeep in Santa Clarita, CA). Ever since then, there is an intermittent, non-reproducible grinding and clicking (sound and feeling through the pedal) in the clutch (like a loose piece/nut is bouncing around and clanking and grinding against the clutch). The clutch pedal is also more spongy, catching only about halfway down, and s
... ometimes feeling like at full depression it is not entirely engaged. I have taken it back to the dealer twice, and both times they will not work on it because they cannot hear or reproduce the feel/sound. The sponginess of the clutch is subjective, they say, and their opinion is that it feels fine.Read more

2019 Chrysler Pacifica. The vehicle has ongoing steering, electrical, and power steering/EPS failures. ... urn after fully restarting the vehicle. The exact root cause is UNKNOWN, but the affected systems appear to include the EPS/power steering system, steering/suspension components, battery/electrical system, and possibly related control modules. My safety and others’ safety were put at risk because the vehicle has lost steering assist and had electrical failure while being driven, including a prior failure in an intersection. When the steering assist drops or the wheel jerks, the vehicle becomes difficult to control. This is especially dangerous because I transport my family in this vehicle. The issue has been reported repeatedly since purchase. The vehicle was inspected/repaired through Carvana/SilverRock’s repair process at Brockman Automotive, including rack and pinion/steering-related work. After that repair, the vehicle continued to drift and handle improperly. A later independent inspection at Davidson Automotive identified a loose/not properly secured tie rod, alignment problems, and a failed auxiliary battery. The shop could not complete deeper diagnostics until those prerequisite issues were corrected. The vehicle has also suffered abnormal front tire wear after the steering/alignment problems. Warning/symptoms before or during failures include steering wheel jerking/pulling, clunking/clanking/grinding/groaning noises, vehicle drift, service start/stop warning, electrical malfunctions, intermittent steering wheel control/light issues, loss of backup camera guidelines, and loss of steering assist. The vehicle/component is available for inspection upon request.Read more

My 2013 Hyundai Tucson with 89,417 miles experienced complete engine failure due to severe rod knock from connecting rod bearing wear. This vehicle was brought to our local Hyundai dealership with confirmed diagnosis. This is a well-documented manufacturing defect in the 2.4L Theta II engine. This vehicle was bought 10/28/205 with 29,494 miles. The vehicle was properly maintained with regular o
... il changes from time of purchase- records attached. Hyundai denied warranty coverage under the TXXM extended program citing incomplete Campaign 966 (KSDS software update) and improper maintenance. However, I never received any recall notices or campaign notifications. When inquiring about notices, Hyundai cited an address that I have not lived at for over 10 years. This sudden engine failure has left me without a vehicle with Hyundai refusing to address this known problem with their engines. I request that NHTSA investigate this issue as well as Hyundai's inadequate notification practices.Read more

At that point I had call a towing service and arranged to have the vehicle taken to Autonation Jeep Chrysler the next morning where I had purchased the vehicle. This was a newly purchased vehicle and only had a total of 78 miles on it. Surprisingly the next morning I went into the garage to check on what was happening with the vehicle and to my surprise the vehicle had stopped running. I then disconnected The 12 Volt battery and left it disconnected for an hour or two. After this time I reconnected the battery. I again attempted to start the vehicle and surprisingly the vehicle started up and all functions in the car seem to be performing normally. At this point I drove the car about a mile to the Autonation Jeep Chrysler dealership and had conversation with service technician. Surprisingly he told me that Jeep has had this problem with other vehicles and the solution is to disconnect the 12 Volt battery for 20 or 30 minutes and then reconnect the battery. At that point the car seems to reset itself and it will run normally. End This car of mine only had 78 miles on it This is totally unacceptable. Furthermore this seems to be a problem with other Jeep vehicles. It is my opinion that Jeep Needs to correct this problem so I will not be stuck with a car that's running in my garage and I can't shut it off. All in all it seems to me this is a software problem that needs to be reworked and corrected.Read more

Model: Sierra Denali Ultimate Engine: 6.2L V8 VIN: [XXX] Mileage at Most Recent Incident: Approximately 52,000 miles Safety Complaint Summary I am submitting this complaint to report a recurring loss-of-propulsion and engine power event involving my 2023 GMC Sierra Denali Ultimate equipped with the 6.2L V8 engine. This vehicle has experienced two separate incidents in which it suddenly lost power
... while being operated at highway speeds. During both events, the vehicle unexpectedly lost propulsion without warning and without any apparent driver input or mechanical indication beforehand. No warning lights, check-engine lights, reduced-engine-power messages, service transmission messages, or other warning indicators were displayed prior to the failure. The vehicle simply lost power while traveling in traffic. During both occurrences, I was forced to safely maneuver the vehicle to the shoulder of the roadway while rapidly losing speed among surrounding traffic. Once stopped, the vehicle would not resume normal operation until it was restarted. After restarting, the vehicle appeared to operate normally; however, the fact that this condition has now occurred twice raises significant concerns regarding the reliability and safety of the vehicle. Safety Risk This condition presents a significant and immediate safety risk because sudden loss of propulsion during highway operation can leave the driver unable to maintain speed, safely merge, maneuver, or avoid surrounding traffic. The unexpected reduction in speed creates the potential for rear-end collisions, loss of vehicle control, or other traffic accidents, particularly when operating on high-speed roadways. Because the vehicle provided no warning before losing power, there is little opportunity for a driver to take preventive action. ... while being operated at highway speeds. Car completely turns off when I’m stopped and idle stop has engaged. Seems to happen when the AC is running hard. Sometimes, the car turns back on by itself in seconds. Other times, I have to turn the car back on myself by pressing the “engine on” button, and I struggle to get the car to turn back on. Very dangerous to be obstructing traffic at busy intersections or on a busy highway when
... stopped while another car ahead is making a turn. I’m scared to drive this car with my children in it. It is a minivan! I understand this is a known issue under investigation by NHTSA, but I’m struggling to get my local Honda dealer to acknowledge and address it. This issue happens at least 1x a day when I’m running the AC.Read more

I am writing regarding my BMW X5 xDrive40i, model year 2019. For this G05 model coded cars, repeated transfer case failures have been reported due to a known issue: on the factory they put a wrong oil in it and then published a Technical Bulletin about it as if they are not the ones responsible for that. The problem is, due to that mistake, transfer cases of cars manufactured between 2018-2020
... becomes dead in time. Cars shudders, jerking at low speeds, cannot deliver the power to the ground effectively due to intermittent and very disturbing, powerful shudders. This creates a huge safety concern cause: 1) if you are on an intersection the car might not join safely to the traffic in a perceived timing due to the shudders, intermittent power delivery. 2) If you are on a driveway and moving to the street the car might not move to the street for the perceived timing or once moved cannot move safely to join the traffic. 3) If there is a failure on the transfer case and if there is a sudden stop is needed. On the road with no shoulder access or if this is a bridge where there is no shoulder, it creates a huge risk to safely move the vehicle. An unavoidable sudden stop can create a huge accident risk for everybody. This is a very known and very common problem for BMW cars equipped with B58TU engine especially years 2018-2020. BMW is also definitely aware of that and this safety concern but instead of fixing it they moved away their responsibility as if they are not the ones made the mistake. But, when contacted with BMW NA, they refused to help.Read more

I bought this trailer from the original purchaser. It was build 13 months ago. They sold it as turnkey. I went to get the first ever annual fire suppression system check, and they said it was a dummy set up. Upon further inspection of the rest of the trailer, I believe I have found multiple things deliberately and knowingly installed and bypassed for a quick fraudulent sale. The pictures show
... how many completely unsafe things I have found. There are others I have found with the same issues from the same manufacturer and dealer. We all could have died!Read more

I got it to the dealership safely and they confirmed that the 12V battery was fully dead and car only turns in if jumped. They also confirmed that the ICCU had a failure as well and they are going to need to replace the entire battery pack/ICCU assembly.Read more

I am writing regarding my BMW X5 xDrive40i, model year 2019, engine code B58TU. The vehicle has been serviced at authorized BMW dealerships. For this engine type B58TU especially years 2018-2021, repeated oil pump failures have been reported due to a known design issue: an internal plastic component can develop cracks, potentially leading to a complete oil pump failure. BMW silently replaced the
... oil pump with the updated version by keeping the same part number but featuring metal internals. Once there is a fail on the oil pumps with plastic components, engine loses access to get the oil an immediate stop is needed. This creates a huge safety concern cause: 1) If you are on a highway at highway speeds it might not be possible to move to the shoulder safely with no engine power or even worse: with a locked engine. 2) If there is a maintenance on the road with no shoulder access or if this is a bridge where there is no shoulder, it creates a huge risk to safely move the vehicle. An unavoidable sudden stop can create a huge accident risk for everybody. Of course any car can get a engine problem on those occasions but this is a very known and very common problem for BMW cars equipped with B58TU engine especially years 2018-2021. BMW is also definitely aware of that and this safety concern cause they updated the design silently. But, when contacted with BMW NA, they refused to help, refused to admit there is a design problem. They also refused to change the oil pump with the updated oil pump even with my proposal of goodwill pricing.Read more
