Shortly after purchase of the vehicle on November 26, 2021, the vehicle began experiencing repeated DEF (Diesel Exhaust Fluid), SCR, and DPF system failures, accompanied by dashboard warning indicators, drivability instability, reduced power warnings, and eventual shutdown conditions. The most serious safety concern is that when the DEF/emissions warning activates, the vehicle may enter reduced-p
... ower mode and ultimately become non-operable, creating a significant roadway safety hazard. The vehicle has repeatedly experienced conditions where it could lose power, become unreliable in traffic, or shut down unexpectedly after emissions-related warnings appeared. The vehicle required repeated repair attempts beginning almost immediately after delivery, including diagnostics and repairs involving: DEF system malfunction SCR/DPF restriction codes Reduced power warnings Fuel pressure-related faults Emissions-system failures Drivability instability Suspension-related concerns Despite multiple repair attempts by authorized facilities and warranty-related interventions, the same categories of failures continued to recur. This condition creates a serious safety concern because the vehicle can become unreliable or effectively inoperable while being driven, particularly after activation of emissions-related warning systems. The repeated loss-of-power and shutdown-related behavior creates risk during normal roadway operation. I am requesting review of the recurring emissions-system failure pattern, drivability instability, and operational shutdown behavior associated with this vehicle.Read more

While accelerating from a stop, I heard a loud bang from the drivetrain area and initially believed another vehicle had struck the rear of my car. After checking my surroundings, I realized there had been no collision. Immediately after the incident, the vehicle lost power delivery to the rear wheels. The vehicle is designed as an all-wheel-drive performance sedan, and the sudden loss of rear-whe
... el power significantly changed the way the vehicle handled. The vehicle became effectively front-wheel drive without warning, causing all engine power to be directed through the front wheels. This resulted in reduced traction, altered handling characteristics, and increased front tire wear. No warning lights, messages, or indicators appeared on the dashboard before or at the time of the failure. There were no symptoms that alerted me that a drivetrain failure was about to occur. The vehicle was inspected by an Acura dealership, and Acura confirmed failure of the transfer case and transmission components associated with the all-wheel-drive system. The transfer case and transmission are currently being replaced under the manufacturer's drivetrain warranty. I believe this failure created a safety risk because the vehicle unexpectedly lost its intended all-wheel-drive capability while in operation. Drivers rely on the handling, traction, and stability characteristics of the SH-AWD system. The sudden and unannounced loss of rear-wheel power changed the vehicle's behavior and could increase the risk of loss of control, particularly during acceleration, cornering, emergency maneuvers, or operation in poor weather conditions. The failed components are available for inspection through Acura as part of the warranty repair process.Read more

The HV battery cooling fan on the 2009 Toyota Camry Hybrid has no protective filter and no cleaning or inspection interval in Toyota’s published Scheduled Maintenance Guide. Toyota acknowledged this exact problem in TSB T-SB-0087-13 (July 2013) for 2012–2016 Camry HV vehicles, stating the cooling fan ‘may exhibit an excessive buildup of dust, lint, or other debris that can reduce the efficie
... ncy of the HV Battery Cooling System.’ However, Toyota excluded the 2007–2011 generation — which has the same unfiltered cooling fan design — from this TSB. My 2009 Camry Hybrid (VIN: [XXX] , 208,000 miles) has now failed with code P0A80, requiring full battery replacement. The failure is consistent with the cooling fan clogging defect Toyota acknowledged for later model years but never addressed for this generation. I am requesting NHTSA investigate whether Toyota should extend TSB T-SB-0087-13 or issue a recall covering the 2007–2011 Camry Hybrid. Component/System: 48V Mild Hybrid Electric Vehicle (MHEV) system including the DC/DC Converter (subject of active recall D126), Battery Energy Control Module (BECM), Telematics Control Unit (TCU), Battery Charge Control Module (BCCM), and Vehicle Immobilizer Module (VIM). All components are available for inspection upon request. What happened: My 2021 Range Rover Sport HSE MHEV (VIN: [XXX] ) exper
... ienced a complete no-start condition after sitting for approximately one week. Prior to towing the vehicle to Hendrick Jaguar Land Rover Charlotte, I performed a live data diagnostic using a Land Rover specific iCarsoft LR V3 diagnostic tool which confirmed the 48V hybrid battery at 17.86% state of charge critically below the minimum threshold required to authorize a start sequence. The TCU, BCCM, and VIM all showed not fitted simultaneously a known cascade effect of a critically depleted 48V MHEV system. Recall D126 for DC/DC Converter failure was confirmed on my vehicle. The DC/DC converter manages power transfer between the 48V and 12V systems. Its failure directly caused the 48V battery depletion and subsequent cascade module failures. Safety risk: The vehicle became completely immobile with no warning. The immobilizer went offline preventing the vehicle from starting, stranding the occupant in a parking lot. A vehicle that shuts down without warning and cannot be restarted poses a serious safety risk to the driver and other road users. Additionally the DC/DC converter failure affecting the MHEV system represents a risk of sudden loss of hybrid assist during driving. The vehicle began rolling backwards due to the grade of the garage and she had to try to physically stop the vehicle or it would crush her between the door and garage. After attempting to physically stop it she realized she couldn't and jumped back in the car and applied the brakes. Her back was injured in the process of trying to hold the vehicle from rolling. She called me and had to sit in the vehicle for 20 minutes until I could get home to chock the wheels and get her out of the car. Zeigler Land Rover out of Schaumburg is where the vehicle was purchased new in November. When I reached out to them they claimed they never had this issue before. Vehicle: 2015 Audi Q3 | Engine: 2.0L TFSI 4-Cylinder Turbo Component: Engine Complaint Description: While driving on the interstate at highway speed, the red oil pressure warning light illuminated suddenly and without prior warning. The standard low oil level warning light did NOT illuminate prior to the failure. Oil level was verified as correct and within specification at the time of failure.
... The vehicle suffered a complete and catastrophic engine failure due to oil starvation and rod knock, despite a verified proper oil level and oil pressure. The engine requires full replacement. This failure occurred at only 3,432 miles since the most recent oil change. The vehicle has a documented history dating back to 2021 of recurring engine oil consumption concerns, fuel contamination of engine oil, and high-pressure fuel pump failure — all serviced at authorized dealerships. A multipoint inspection performed approximately four months prior to this failure returned a fully satisfactory result with no engine concerns flagged. This is believed to be related to the known piston ring defect in Audi/Volkswagen 2.0L TFSI engines documented in federal class action litigation (Rieger v. Volkswagen Group of America, Case No. 1:21-cv-10546-NLH-EAP), which explicitly named the 2012–2017 Audi Q3 as an affected vehicle. The vehicle is experiencing a dangerous hesitation during acceleration at low speeds, immediately followed by a sudden, violent forward lunging motion. When attempting to accelerate from a complete stop, or when rolling at low speeds (such as yielding at a roundabout, turning through an intersection, or merging into slow-moving traffic), the vehicle suffers from a distinct **lag or complete fail
... ure to accelerate** for several critical seconds. In an attempt to get the vehicle to move out of harm's way, the driver naturally applies more pressure to the gas pedal. The vehicle's transmission then randomly responds after a few seconds with a violent lunge forward, spinning the tires and causing the vehicle to surge unpredictably. This unpredictable behavior creates an immediate and severe safety risk to the occupants of my vehicle and others on the road: 1. Intersection Collisions: The sudden lag leaves the vehicle stranded in oncoming traffic during turns, while the subsequent lunge threatens to overshoot the intended lane. 2. Pedestrian Hazards: The sudden, uncontrollable lunging makes low-speed maneuvers in school zones, crosswalks, and parking lots incredibly hazardous. 3. Drivers behind behind me do not expect a vehicle to suddenly stop accelerating after it has begun to move forward. I have taken the vehicle to the dealership we're we were able to reproduce the issue and Honda would not offer a fix. I thought I was going to get my entire family killed the other day when I pulled out at a stop the van hesitated in the middle of the highway for a few seconds before lunging forward. This is an intermittent issueand the severity varies each time, but is commonly cited online.Read more

While driving down a two-lane road at approximately 60 miles an hour I passed a semi truck coming in the other direction and the momentary change in air pressure caused my hood to unlatch. Thankfully the secondary latch kept the hood from fully opening but had that latch not worked the hood would have flown open into my windshield.

On May 6, 2026, while operating the vehicle on a neighborhood street, the vehicle experienced a sudden, violent, and dangerous failure. After accelerating away from a stop sign, the vehicle was traveling at approximately 20-25 mph when, simultaneous with a warning message, it came to an immediate and abrupt stop. A number of error messages appeared in rapid succession on the vehicle’s MMI. Th
... ose messages referenced, among other things, drive battery errors, a major electrical system failure, and drivetrain issues. After the abrupt stop, the driver, attempted to restart the vehicle. Following one failed attempt, the vehicle restarted; however, after traveling approximately 30 feet, the vehicle again came to an abrupt, violent, and sudden stop. After several additional failed attempts to restart the vehicle, the driver allowed the vehicle to sit idle for approximately five minutes. The driver was then able to restart the vehicle and drive it, very cautiously and slowly, approximately two miles to a safe location. Once in a safe location, driver contacted the dealer who confirmed that the vehicle was unsafe to operate. The vehicle was towed to the dealership. On May 7, 2026, the dealer informed us that they had found “several different faults stored in the vehicle but those faults all basically contradict each other.” The dealer further informed us that they would need to contact Audi in Germany for guidance on how to proceed with diagnosis and repair. On May 22, 2026, the dealer informed us that "(w)e do need to drop the battery. There is a module in one of the battery cells that we are going to replace and also the module that controls the battery as well".Read more
