FORMAL COMPLAINT – 2024 CVO ROAD GLIDE ST 25TH ANNIVERSARY EDITION To Whom It May Concern, I am requesting immediate intervention from Harley-Davidson Corporate regarding ongoing and unresolved safety concerns with my 2024 CVO Road Glide ST 25th Anniversary Edition. The motorcycle was originally purchased through a Harley-Davidson dealership in Syracuse, New York. Since ownership, the motorcy
... cle has experienced repeated throttle-related failures that create a serious safety concern. The issue occurs during normal riding conditions. Upon slight acceleration, the throttle unexpectedly cuts out, resulting in a complete loss of throttle response. Simultaneously, multiple warning lights illuminate across the instrument display, and the check engine light activates before the motorcycle loses power. This issue has occurred numerous times and continues despite multiple repair attempts. The motorcycle has been serviced by multiple authorized Harley-Davidson dealerships, including: * Harley-Davidson of Greeley (John Elway Harley-Davidson), Greeley, Colorado * Harley-Davidson of Perrysburg, Ohio * Harley-Davidson of Napoleon, Ohio (current servicing dealership) The motorcycle has been taken in for service on multiple occasions. Due to scheduling limitations and dealership availability, there have been only two formal repair visits specifically addressing this issue, including one instance where the motorcycle remained in the dealership’s possession for nearly a month without the problem being successfully repaired. I am extremely concerned about the safety implications of a motorcycle that loses throttle response while in operation. This defect places both the rider and surrounding motorists at risk. I am requesting: 1. Immediate review and escalation of this case by Harley-Davidson Corporate. 2. Assignment of a corporate case manager. 3. A complete engineering review of the motorcycle and repair history. 4. Written documentation of all diagnostic findings and …Read more

This vehicle has experienced three separate powertrain failures stemming from the same transmission defect (DTC P0A43 — Drive Motor Position Sensor, eFlite transmission). Failed Repair — May 8, 2026: Chrysler corporate mandated a software flash ra
... ther than a hardware repair. Vehicle failed identically within 10 miles of pickup in live highway traffic. Towed back same day. Incident 3 — June 6, 2026: A replacement transmission was installed. Within one week of pickup, a progressive transmission whine developed followed by visible smoke from the engine compartment while driving. Vehicle was immediately pulled over and towed to avoid a third highway failure. Chrysler's own STAR engineering team (TSB 21-014-26) has documented this as a known defect requiring transmission rebuild or replacement. Chrysler blocked a full transmission replacement for over 5 weeks citing backorder, then located one within hours when a repurchase review was requested. This defect causes sudden loss of motive power at highway speeds with no warning, creating an immediate safety hazard. I am requesting NHTSA investigate this defect pattern on 2022 Chrysler Pacifica Hybrid vehicles and evaluate whether a safety recall is warranted.Read more

I experienced a sudden coolant leak on my 2017 Ford F‑150 equipped with the 3.5L EcoBoost engine. The Ford dealership diagnosed the issue as leaking turbocharger coolant lines. The coolant was leaking in the turbo area and dripping onto extremely hot exhaust components. This creates a risk of rapid coolant loss, overheating, loss of power while driving, and potential engine compartment fire. Th
... e Ford service advisor told me directly that he has “seen this many times,” which indicates this is a recurring failure on EcoBoost engines and not normal wear. This suggests the coolant line design or fittings may be defective and failing prematurely. The repair estimate was extremely high because the dealer included replacement of both exhaust manifolds. This failure occurred without warning. If the coolant loss had happened at highway speed, it could have caused sudden engine overheating or power loss. I am requesting NHTSA investigate turbo coolant line failures on Ford EcoBoost engines due to the safety risk of coolant leaking onto hot turbo and exhaust components.Read more

My 2019 Toyota Highlander XLE V6 developed a serious transmission-related safety problem that ultimately resulted in a sudden loss of propulsion while driving. For some time before the failure, I had noticed an unusual startup noise during cold starts and mentioned this concern during routine service visits at an authorized Toyota dealership. I was advised that the condition was normal. Prior to
... the major failure, the vehicle otherwise remained drivable. On December 2, 2025, the vehicle underwent scheduled maintenance at an authorized Toyota dealership, including a transmission fluid service. Within the following days, during cold weather conditions (approximately 35°F), the vehicle began exhibiting transmission shudder, RPM flare, harsh torque converter engagement, and illuminated both the Check Engine and Traction Control warning lights. I immediately returned the vehicle to the servicing dealership and reported that these symptoms had appeared following the maintenance visit. After inspection, I was informed that the torque converter clutch actuator had failed and that the transmission would require replacement or rebuilding. The vehicle was released to me, and I was not advised that it was unsafe to drive or that it should not be operated. On the morning of December 11, 2025, while driving home from work in approximately 31°F temperatures, the vehicle suddenly lost propulsion. The engine RPM increased, but the vehicle would not move under its own power. I narrowly avoided being rear-ended by a truck traveling behind me. I believe this created a significant risk of a serious collision. The vehicle was towed and remained out of service for approximately two months because replacement transmissions were difficult to obtain. The transmission was ultimately replaced with a used transmission assembly at a total repair cost of approximately $7,220. After the transmission replacement, the unusual startup noise that I had previously reported completely disappeared,Read more

I own a 2024 Volkswagen Taos with 20,600 miles. At my scheduled 20,000-mile service on June 6, 2026, the dealership measured my rear driver side brake pad at 3mm while my front pads measured 10mm. This extreme difference in wear is consistent with the known premature rear brake pad wear issue VW acknowledged on 2022–2023 Taos vehicles and addressed with a warranty extension bulletin. My 2024 Tao
... s was not included in that bulletin despite showing the identical problem. At only 20,600 miles, rear pads should not be at 3mm while front pads remain at 10mm — this is not normal wear and indicates a manufacturing defect in the brake pad material or an electronic parking brake calibration issue causing rear brake bias. Brakes are a critical safety system. I am requesting NHTSA investigate whether the 2024 Taos is affected by the same defect, and whether the warranty extension should be expanded to include 2024 model year vehicles.Read more

Tesla recently pushed a software update that introduces a major, unavoidable driver distraction right at the most critical moment of driving: the exact second you take manual control back from Full Self-Driving (FSD). Whenever you disengage FSD, a mandatory, multiple-choice survey pops up on the center touchscreen asking why you took over. Unlike previous versions where this would disappear a
... fter a few seconds, it now stays on the screen permanently. There is no close button, and it does not time out. This causes two clear safety hazards while driving: 1 It blocks the screen: The popup actively covers up part of the navigation map and basic screen controls. If you are trying to navigate a tricky intersection or see your route right after taking over, you can't see the map clearly because this giant survey is blocking it. 2 It forces you to look away from the road: Because the prompt never goes away on its own, it forces the driver to physically look down, read the small text options, and tap the screen to clear it. Forcing a driver to do a multiple-choice survey while actively managing a manual takeover in traffic is incredibly dangerous. Tesla should not be allowed to use a mandatory, permanent UI popup to collect data while a vehicle is actively moving. It is a severe visual and cognitive distraction at the exact moment a driver needs to be 100% focused on the road. Please investigate this software design and require Tesla to add an automatic timeout or a quick-dismiss button that doesn't require driver interaction.Read more

I am submitting this complaint regarding my 2022 Hyundai Tucson equipped with the 2.5L engine. The vehicle has experienced multiple incidents of severe engine misfiring while driving. During these events, the engine begins to shake violently, the entire vehicle vibrates, and engine power is significantly reduced. The vehicle becomes unable to accelerate normally, creating a serious safety hazard,
... especially when driving on highways or in traffic. The malfunction has occurred several times. Diagnostic trouble codes P0302 and P0303 have been recorded, indicating cylinder misfires. In some cases, turning the engine off and restarting it temporarily restores normal operation. On other occasions, adding fuel system cleaner appeared to temporarily improve the condition. However, the problem eventually returns. Because the condition is intermittent, the Hyundai dealership has been unable to reproduce the failure during inspection and therefore has not performed a repair under warranty. Despite multiple visits, the root cause remains unidentified. I am concerned that the sudden loss of power and severe engine vibration could result in an accident if the vehicle is unable to maintain speed or accelerate while traveling at highway speeds. This issue has significantly affected the safe operation of the vehicle. I request that this matter be investigated, as it appears to be a recurring safety-related defect.Read more
