Fuel leak(s) involving filler neck, fuel pump, fuel tank or associated parts in the fuel delivery system at rear of car. Gasoline smells is very strong, when the tank is filled (not overfilling the tank) gasoline puddles under the rear near the driver’s side rear tire. The fumes are so bad that the car cannot be stored in the garage (which is attached to my home). Smell is pervasive even sitting
... outside. I cannot drive the car for fear of gas landing on the exhaust pipe and starting a fire. I’m [XXX] and probably would not escape the vehicle. I just purchased the car and such a repair is not warranted. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

My 2021 Volvo XC90 Recharge has experienced multiple hybrid system failures under Volvo's factory 8-year/100,000-mile hybrid warranty, with fewer than 45,000 miles on the odometer. The high-voltage battery modules have failed multiple times, directly related to recall R10312 (NHTSA 25V-179), which identifies a supplier process deviation in LG battery modules creating a risk of internal short circu
... it and thermal runaway — an acknowledged fire risk. Critically, my vehicle has suffered two actual battery module failures, not preemptive recall replacements. This places my vehicle in a substantially higher risk category than others in the recall population with no prior symptoms. A vehicle that has already experienced the exact failure mode identified in a safety recall presents a materially greater ongoing safety risk. Additionally, my vehicle suffered a complete ERAD (Electric Rear Axle Drive) failure — a known defect that Volvo acknowledged by releasing a redesigned ERAD2 unit, confirming awareness of this systemic issue. These repeated failures left my vehicle un-drivable and parked at the dealership for more than 6 months, with replacement parts repeatedly placed on backorder, causing severe and prolonged loss of use. I filed a formal complaint with Volvo Customer Care requesting a vehicle buyback. After months of inadequate communication, Volvo declined to buy back the vehicle or offer reasonable trade-in accommodation. Instead, Volvo offered a nominal monetary concession contingent on signing an NDA, which I declined as insufficient given the documented safety risk, repeated failures of a recalled component, extended loss of use, nor out of pocket car rental expenses. I am filing this complaint to ensure my vehicle's actual failures are part of the NHTSA record for R10312/25V-179, and to request NHTSA evaluate whether Volvo's response to owners who have experienced actual failures — rather than preemptive recall repairs — warrants further regulatory scrutiny.Read more

My husband, [XXX] son, my [XXX] mother-in-law, and I were traveling on the expressway at approximately 70 miles per hour in our 2019 Chevrolet Silverado 1500 with the 5.3L engine when the dashboard suddenly lit up with warning lights and alarms. My husband looked in the rear-view mirror and saw a large amount of smoke coming from the truck. We immediately had to cross two lanes of heavy traffic t
... o reach the shoulder and get off the roadway safely. When I opened my door, I could see that engine oil was pouring from the truck and running down the expressway. We were stranded on the side of the expressway for over an hour before my brother was able to come pick us up. The tow truck company advised that it would take several hours before they could arrive. During that time, vehicles continued traveling at highway speeds and did not slow down despite our disabled truck being on the shoulder. The truck was shaking from the wind and force of cars and trucks passing by at high speeds. This created an extremely dangerous situation, particularly with our [XXX] child and my elderly mother-in-law in the vehicle. The truck was eventually towed to a local repair shop, where the mechanic determined that the engine oil cooler line had broken at the crimp, causing a significant oil leak. The repair required replacement of the oil cooler lines and related components at a cost of $2,109.92. Had we continued driving even briefly, the engine likely could have been severely damaged or destroyed due to the rapid loss of oil. This appears to match the known issue described in GM Customer Satisfaction Program N212326940 regarding oil cooler lines detaching from the crimp on 2019 Chevrolet Silverado trucks. This created a safety risk when attempting to turn or maneuver the vehicle in traffic or parking areas. Additionally, the inability to use reverse gear could create a hazardous situation if the driver cannot back out of traffic, parking spaces, or other areas. 3) The problem has not yet been inspected or confirmed by a dealership or independent service center. 4) No, the vehicle has not been inspected by the manufacturer, police, insurance representatives, or other parties. 5) The dashboard displayed the message “Service Power Steering Now,” and the steering assist stopped working. The problem appeared suddenly when the vehicle was started. The steering immediately became extremely stiff, and the warning message appeared at that time. This failure created a serious safety hazard because the steering assist stopped working without warning, making the vehicle extremely difficult to control. A sudden loss of steering assist can increase the risk of a crash, especially when turning, parking, or driving in traffic. I am concerned that other drivers could experience the same sudden steering failure. The vehicle displayed the message “Service Power Steering Now,” and the steering immediately became extremely stiff. The vehicle is currently undriveable in reverse and difficult to steer safely.Read more

The engine on my 2010 Kia Optima suddenly seized without warning around 150,000 miles. The vehicle had no knocking sounds or warning signs prior to failure. A mechanic diagnosed the vehicle with a seized engine. This created a sudden loss of vehicle operation. I am concerned this may be related to the known engine failure issues reported in Kia vehicles.

Recall number N192211320 ( Left Front Lower Control Arm Ball Joint Not Seated ) Chevrolet is refusing to fix this issue, saying that it's not a safety recall. This is a clear attempt to circumvent the law, by not classifying this as a safety issue.

Date: 3/7/2026 I am submitting this complaint regarding a potentially serious safety issue with my 2025 Ford Escape Plug-in Hybrid manufactured by Ford Motor Company. My vehicle began displaying multiple warning messages including “High Voltage Battery Warning,” “Powertrain Malfunction,” and “Service Engine Soon.” These warnings appeared while the vehicle was charging using a Level 2
... charger, which raises significant concerns about the safety and reliability of the vehicle’s high-voltage electrical system. After these warnings appeared, I brought the vehicle to an authorized Ford dealership for diagnosis and repair. The vehicle has now been in the repair shop for approximately 30 days, reportedly waiting on parts related to the high-voltage battery or powertrain system. This situation raises several concerns: The possibility of a defect in the high-voltage battery system. The potential for sudden loss of power or drivetrain failure. The risk associated with charging a vehicle that displays high-voltage battery warnings. Extended vehicle downtime due to the lack of available replacement parts. Given the critical safety implications of high-voltage battery systems in plug-in hybrid vehicles, I respectfully request that the National Highway Traffic Safety Administration investigate whether this issue represents a broader defect affecting other vehicles of the same model.Read more

Audi A6s have had massive issues with the door actuators. There are times when you go open the passenger side doors (both front and rear) and the doors will not open as there is either an electrical, software, or device specific issue. It requires 2-3 pulls of the door latch to open the front and rear passenger doors. The issues keep appearing a month or two after the dealership supposedly fixes t

I have asked them to push this up to Audi corporate and have received no response. Audi will also not offer a lemon buyout for these door issues. I believe this needs to be heavily reviewed as if you search the Audi forum websites online, this is a constant problem for the 2018-2023 A6s.Read more

The Audi software/MMI system has issues showing the 360 cameras in addition to the rear facing camera. Every few starts when I back out of a parking space/garage, the side mirror cameras show a black or camera with a cancel sign on the screen. The issue will not stop until the car is parked/off. for at least an hour. This issue has been happing since I purchased the car back in early 2021 and Audi
... did not "lemon-out" the car. I believe this is just as dangerous as the recent backup camera recall. I have asked Freehold, NJ Audi and Cherry Hill, NJ Audi to review, but they said if it doesn't happen when there, they can't action. I believe this is the same software issue the backup camera has and requires a similar recall as drivers rely on the side cameras for parking and there is a risk we c an hit another object or person.Read more

1. Electrical System Failure: While traveling on highway, alternator failed suddenly within 2 minutes of “service soon” and “check electrical system” warnings. Caused complete loss of motive power, electronic parking brake locked up while driving, loss of power steering, all gauges failed, transmission shifted into park while in motion. Battery was damaged (“fried”). Engine continued
... running for several minutes after stop without ability to shut off ignition. Incident documented by Ohio State Highway Patrol. 2. Seat Belt Issue: Driver-side seat belt interlock sticks in unlocked position, creating risk of failure to lock during a crash. 3. Electronic Parking Brake: Intermittently engages while vehicle is moving downhill to a stop (mostly at or below 5 mph). Parking brake light flashes three times then turns solid while in motion. Ford TSB indicates vehicle not eligible for related recall. 4. Infotainment System: Intermittently freezes, requiring forced reboot. Preceded by audio glitches, laggy dash computerized systems. 5. Transmission: Severe clunk and physical jolt when decelerating from ~30 mph to 8 mph. Also occurs during acceleration, particularly upshifts from 2nd to 3rd and 3rd to 4th gears, more pronounced when turning. 6. 4WD System: Intermittently fails to disengage from 4WD HI to 2WD HI. “Failure to shift” message appears several minutes after command to return to 2WD HI. 7. Front Differential/Axle: Previously, faulty front differential housing fractured due to system malfunction, causing intermittent loss of front driveline engagement in 4WD LO (250-500ms delay then re-engagement under throttle). Ford replaced front differential and axle under warranty after vehicle out of service for 2.5 months. Abbreviations: 4WD HI = 4-Wheel Drive High Range; 2WD HI = 2-Wheel Drive High Range; TSB = Technical Service Bulletin.Read more

My backup camera continues to not work sometimes it shows upside down or black screen and says I need to contact the dealership. When I contact the place that I got it from it says it's not covered under their warranty in the contact Ford. When I contact Ford they said there was some rear backup camera thing going on and that the recall had not been issued yet?

Vehicle went into limp mode while driving on the highway. Onstar said it was the transmission. Checked the oil and smoke was billowing out of the dip stick and there was no oil. None of the sensors indicated that there was low oil. After sitting and waiting for a tow truck rechecked the oil with the dipstick and there was oil in it. Now requires a new engine.

Component Involved: Visibility/Backup Device, Electrical System, Forward Collision Avoidance Description of Incident: The Android 12-based infotainment system in this vehicle is suffering from a progressive hardware/software failure that creates a severe, multi-layered safety hazard. CRITICAL DRIVER DISTRACTION: The system has become largely non-responsive, requiring me to repeatedly press o
... n-screen buttons to perform basic functions. This forces me to divert my eyes and manual attention away from the road for periods far exceeding 2 seconds. This "visual-manual distraction" is a direct result of the system's inability to process commands, making safe operation impossible while attempting to use vehicle controls. VISIBILITY FAILURE (FMVSS 111): The failure impacts the digital rearview mirror and the safety camera feeds. These cameras frequently glitch, lag, or freeze, depriving me of necessary rear and surround visibility while moving. This is a violation of federal safety standards for rearview visibility. SYSTEMIC ELECTRICAL FAILURE: This "slowly dying" system is also interfering with the vehicle's communication bus, causing the power door locks to malfunction. This creates an entrapment or security risk. I have reported these issues to General Motors/OnStar. Despite their awareness of these "electrical gremlins" and the existence of limited service updates (like Program #N242435631), no permanent fix has been provided. This is a systemic safety defect that requires a mandatory recall to prevent accidents caused by driver distraction and visibility loss. The system needs updating to Android 14. It is Android 12 now. This is the fix. Gm want me to pay $250 for a hour for a macanic to update it. Gm was updating over the air, but it was causing the battery on the vehicle's to prematurely die under warranty. It was affecting their bottom line.Read more
