I am reporting a recurring and unresolved electrical/safety issue with my new Tesla Model Y. The issue has occurred 5 times within the past 4 weeks and affects multiple systems simultaneously. When the issue occurs: * The air conditioning stops working completely. * The driver-side front and rear windows cannot be rolled down or up. * The driver seat adjustment may not function properly. The pr
... oblem appears intermittently. It may disappear temporarily and then return hours or days later. Tesla service has inspected the vehicle multiple times, but the issue continues to recur. This creates a significant safety concern because: * The driver window may not function during rain or emergencies. * The AC may fail during extremely hot weather. * The driver seat may not adjust properly, potentially affecting safe vehicle operation and driver visibility. * The intermittent nature of the failure makes the vehicle unpredictable and unreliable to operate safely. The issue has not been permanently resolved despite repeated service visits. I am concerned there may be a broader electrical or control system defect affecting vehicle safety.Read more

NHTSA Safety Complaint Draft Vehicle: 2025 Ram 1500 VIN: [XXX] Mileage at First Incident: 3920 Mileage at Most Recent Incident: 13,000 (approx.) Complaint Summary: My 2025 Ram 1500 has a recurring safety critical defect that causes the truck to suddenly and unpredictably decelerate from highway speeds (around 70 mph to 55 mph) down to approximately 5 mph, even though the engine RPMs r
... emain around 4,000. This creates an immediate risk of being rear ended or causing a multi vehicle crash. Full Description of the Problem: While driving at normal highway speeds, the vehicle suddenly loses propulsion without warning. The truck slows rapidly to about 5 mph, dealership verbally described as “limp mode” to protect itself, despite the engine continuing to rev at high RPMs. During these events, the dashboard and infotainment system display incorrect and erratic information, including: - Transmission indicator switching into 4WD, Park, or 1st gear while the vehicle is moving - Navigation activating on its own - Screen freezing, glitching, or displaying inaccurate data This defect has occurred multiple times and is unpredictable, making the vehicle unsafe to operate. Repair Attempts: I have taken the vehicle to an authorized Ram dealership on multiple occasions for the same issue: - Repair Attempt #1: 03/20/2025 - Repair Attempt #2: 10/02/2025 - Repair Attempt #3: 10/30/2025 - Repair Attempt #4: 11/17/2025 - Repair Attempt #5: 05/12 2026 The dealership has replaced PCM harness, transfer case control module, DTCM, performed software updates and claimed the issue was resolved, but the defect continues to occur. They have also stated they “cannot replicate” the problem, despite multiple documented incidents. Safety Risk: This defect poses a serious risk of collision, especially at highway speeds. The sudden loss of propulsion and incorrect gear display could easily result in a rear end crash INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The gps/map display, displays the wrong speed, on some traffic circles it directs me to go around in the wrong direction. Tomtom did commit to investing the issue after I gave them the specific locations, but they closed my record/complaint file. Auto stall at stop is not working safely on my vehicle there are two issues that can cause safety problems .A) when system restarts engine the vehicle has jumped forward several feet quickly causing very nearly hitting the car in front of me as a traffic light turned green . B) when I slow not quite to a stop making a left turn I have experience very low or no power and could be hit if a car is c
... oming towards me . Both these issues do stop if I remember to turn off the auto stall system but I do not always remember ! I did make a compliant at a Chrysler Dealer they found no issues and did not reproduce the problem no warning lights come up when the problem happens . I have not had damage to the vehicle but have had two close calls one nearly rearending a vehicle at a intersection as my Pacicifca lurched forward and the other as an oncoming car needed to brake when I made a left turn in an intersection and had a loss of power .Read more

The problem occurred immediately after picking my car up from a valet on 5/17/2026 around 9:00pm. The valet had pulled my car up to the front of the Seattle Park ‘N Fly Valet by Sea-Tac airport. I pulled out onto International Blvd, and within a few seconds, the accelerator pedal became completely unresponsive. I was able to coast the car into the Denny’s parking lot just north of the valet lo
... t (about 0.2 mi north of the valet). Everything else seemed to work just fine, except for the accelerator pedal (brakes worked just fine). I turned off the engine, and when I tried to restart the car, it was turning over but wouldn’t start. I called out someone to jump the battery, but the battery was fine. We then disconnected and reconnected the battery, and this resolved the issue and I was able to restart the car and drive away. It was dangerous that I was able to pull out onto a road, just to immediately lose any ability to accelerate. I’m lucky there was a nearby parking lot and I didn’t pull into busy traffic. Something seems to have happened at the valet, that was then triggered as soon as I pulled out onto the road. My guess is they tried to start the car and use the twist shifter in some combination that caused a fault that only triggered once I started to get up to speed. It’s strange because they were able to pull my car up to the front of the lot just fine, and I was able to pull out onto the roadway just fine, but the loss of acceleration only happened once I tried to get it up to speed.Read more

Very infrequently the airbag light would briefly come on (solid, never flashing). It would only happen when turning the wheel counter clockwise, typically the wheel did not need to be turned very far for this to occur. The light would always go back off and no codes were found. On the night of 12/26/2025, I was driving onto the interstate (turning the wheel counterclockwise) when suddenly the pass
... enger side curtain and passenger seat airbags deployed. My hearing went out, my windows fogged up, and I became disoriented. My vehicle briefly left the roadway and swerved into the narrow shoulder before I slowed down to a near stop. I resumed driving as there were vehicles approaching from behind. The vehicle has no impact damage and has never been in an accident. The airbags had stickers dating back to 2014 from when the vehicle was manufactured. The vehicle is rarely driven now as it is a safety concern.Read more

This is the second time the horn has stopped working. The first time was a year or two ago (records available on request), and both of the two horn units failed. The manufacturer Hyundai replaced them both and told me the issue they were having was the stamped housing of the horn has a seam that folds up and collects water, may freeze, and causes the unit to fail. Clearly they installed two replac

The way I found out was someone almost changed lanes into me, I tried to honk to alert them to not collide with me, and the horn didn't work. Thankfully I was able to stop fast enough to avoid the collision. I will be taking it to Hyundai for replacement again within the next few days. This needs to become a recall because clearly Hyundai hasn't accepted yet that they need to correct their design of this safety feature.Read more

My 2022 Kia Carnival, approximately 80,000 miles, had intermittent dashboard warning lights and grinding brake noise for a few weeks. I brought the vehicle to Tasca Kia White Plains first time on 4/14/26 then again on 5/8/26. The dealership initially could not reproduce the issue, then later suspected an ABS sensor issue and ordered the part. On 5/12/26, before the repair was completed, I was dri

At that time, the brakes would not respond normally, and I could not immediately shift the vehicle into Park. Police and towing services were involved. The dealership later replaced an ABS sensor. However, when I reviewed the codes with the service manager, I was told the ABS sensor code did not appear on the date of the brake incident. I was also told that an ABS sensor failure itself would not typically cause hydraulic brake failure. I am concerned that the sensor replacement does not explain the brake failure and that this may involve a brake/ABS/electronic stability control safety issue.Read more

I was driving at freeway speeds when the vehicle lost engine power and all the electronics in the vehicle stopped functioning. I was able to safely get the side of the road and stop the vehicle. Had vehicle towed to dealership. Tech inspected remote frequency hub (RFH) and observed corrosion on the circuit board. It looked as if water intrusion had occurred inside the vehicle, entered the RFH,
... causing corrosion and circuit failure. Believe intrusion occurred in the third brake light at the top of the cab. I am reporting this matter because a failure in the RFH caused the vehicle to stop functioning. I was lucky to be traveling on a straight, wide road and could safely get to a stop. I don't believe this would have been the case if I was on a curve or narrow road.Read more

Description of Safety Defect: The driver-side exterior rearview mirror vibrates excessively, escalating into a violent shake depending on vehicle speed, crosswinds, and road surfaces. The issue is highly pronounced at highway speeds (65+ mph) and deteriorates aggressively as speed increases, particularly on open, wind-swept highways with like MA-3. The resulting violent vibration causes severe
... visual distortion, creating a blurred, "low-resolution" image that makes it incredibly difficult to accurately judge the distance, speed, and presence of overtaking vehicles. When checking the driver-side blind spot, this rapid blurring induces sudden, brief physical dizziness and vertigo, posing an immediate safety hazard during high-speed lane changes. The passenger-side mirror remains perfectly stable and clear under identical driving and road conditions. Attempts to Correct / Dealer History: The vehicle has been taken to an authorized dealership (Tulley Mazda, Nashua, NH) twice for this specific issue: On 03/18/2026 (Mileage: 5,862), Repair Order #XXX/1 noted the complaint. The technician verified the vibration on a road test but dismissed it as a "normal characteristic of the vehicle". On 04/21/2026 (Mileage: 6,846), Repair Order #XXX was opened because the condition worsened and began occurring at lower speeds. A ride-along was conducted, and the Shop Foreman confirmed the defect. Upon mirror disassembly, the technician found "loose internal components causing the issue". Current Status: Under warranty, the dealership replaced the body door mirror assembly (Part KJL1-69-181C) and the mirror glass/holder (Part TK71-69-1G7). However, the violent shaking remains completely unresolved and continues to worsen at high speeds. The dealership is now refusing further diagnostic or repair work, claiming the vehicle is "within spec," despite the persistent visual distortion and subsequent driver vertigo. No crash yet, but the defect is serious safety concern.Read more

I have owned the car for years, and have always taken good care of vehicle with scheduled oil changes, tune ups, etc. and now I am being told I need a new head gasket because there is a leak. Being a 2017 with just over 100,000 miles and regular care this should not be happening. It caused issues with a fuse and now a new starter needs to be put in too. An awful smell was coming from the vehicl
... e making me sick and my child while having to drive. What appeared to be heavy smoke started coming out. A light came on staying engine was hot so had to pull over on the side of the road and be towed. I had the mechanic who towed the chrysler pacifica tell me I had a head gasket leak and needed a new one but he does not repair them. I searched over 10 other repair shops and they would not touch due to the ongoing issues with the Pacifica's. I finally found a mechanic who stated the same as the prior mechanic that my head gasket needed to be replaced, and it was a big job that would be awhile before he could even get to. I am asking for assistance with this repair as this seems to be an ongoing issue with them.Read more

... on risk. Dealer-confirmed: Porsche Woodland Hills documented (July 2025): "vehicle just shut off on him as he was driving — multiple occurrences." A second shutdown was confirmed August 2025: "vehicle shut off while driving — high voltage battery error." Four or more repair visits under my ownership for this same defect. Fault codes confirmed by authorized Porsche dealers: P0AA600 (HV Battery Voltage System Isolation Fault — highest severity), P0A4000 (Rotor Position Sensor Fault), U178800 (Front Axle Power Electronics Drive Fault), P061B00 (Torque Calculation Fault). November 2025: Porsche Downtown LA isolated all 33 battery modules, found modules 6, 19, and 32 defective — all physically replaced. Vehicle out of service 9+ weeks. Shutdown warning recurred during same visit. Porsche NA (Case #XXX) denied repurchase. Warning lamps: "Pull over and park safely" warning and HV battery error appeared suddenly before each shutdown. No gradual symptoms. Recurring since January 2025. Additional: Prior owner records show HV battery water intrusion (February 2024) requiring full battery removal and reinstallation. In July 2024, dealer cleared "numerous inactive fault codes" without disclosure prior to my purchase. Porsche NA has acknowledged the issue by offering goodwill compensation while refusing repurchase. Cumulative out-of-service days for this defect exceed 75 days across multiple repair visits. I request NHTSA investigate whether this defect exists across other Taycan vehicles and whether a recall is warranted. Dear NHTSA Safety Defects Investigation Team, I am filing a safety complaint regarding my 2017 Lincoln MKC involving a child entrapment incident. On 05/17/2026, my vehicle locked itself while the engine was running, the key fob was inside the vehicle, and my [XXX] child was still inside the car. I was unable to gain entry to the vehicle and had to call 911/emergency responders for assistance. It
... took around 30 minutes from the time the vehicle got locked and for me door to be opened by the towing company sent by the 911 operator. Thankfully, the AC was turned on and my child was unharmed. The vehicle should not have allowed itself to lock under these conditions with the key present inside the cabin. This created a serious safety hazard involving a trapped child. Compounding the issue, the LincolnWay remote access system no longer functions because the vehicle’s original 3G modem became obsolete after U.S. 3G cellular shutdowns. As a result, I no longer had access to remote unlock functionality that had previously been available with this vehicle. Vehicle Information: • Year/Make/Model: 2017 Lincoln MKC • VIN: [XXX] • Approximate Mileage: [INSERT MILEAGE] • Incident Date/Time: 05/17/2026 at 4.15pm • Location: [XXX] Additional Details: • Vehicle was running at the time • Key fob was left inside the vehicle • Doors became locked unexpectedly • Child was trapped inside • 911/emergency assistance was required • LincolnWay remote services were unavailable due to obsolete 3G modem support ending I believe this issue represents a significant vehicle safety concern and request that NHTSA investigate whether similar incidents have occurred in other Ford/Lincoln vehicles equipped with these systems. The 911 call was placed around 4.15pm and I have the receipt from the Towing Service company Please let me know if any additional information or documentation is needed. I am submitting this complaint regarding ongoing and potentially dangerous mechanical failures involving my vehicle that have repeatedly occurred despite numerous repair attempts by the dealership. Over the past year, I have taken my vehicle to the dealership more than 10 times for serious recurring issues. The problems have included: Drivetrain malfunction warnings Engine warning lights Oil leaks
... Smoke coming from the vehicle The vehicle unexpectedly shifting into neutral while driving Sudden loss of drivability requiring the vehicle to be towed The dealership has repeatedly stated that the issues were repaired, and at one point even provided a one-year warranty on the repair work. Despite this, the same problems have continued to occur. Most recently, the drivetrain malfunction warning and engine light returned again, smoke came from the vehicle, and the car shifted into neutral on its own, becoming completely undriveable and requiring towing. I have spent more than $10,000 on repairs related to these ongoing issues, yet the vehicle remains unreliable and unsafe. My primary concern is the safety risk posed by the drivetrain failure and the vehicle unexpectedly shifting into neutral while in operation. I am requesting that this matter be reviewed as a potential vehicle safety issue. I believe these repeated failures may present a serious risk not only to myself but potentially to other drivers as well. Thank you for your attention to this matter.Read more

While in a parking lot going about 10mph or less, a car pulled out from a parking space on the left. I did a full brake and despite being dry, warm pavement, the car activated abs and released some brake pressure and allowed the car to continue to move. This ment that my braking distance increased by about 5-10 feet more than you'd expect. I have had this happen 3 times, each time a car was pulli
