I am filing a safety complaint regarding my 2014 Ford F-150 XLT equipped with the 6R80 transmission. My vehicle previously received Recall 24S37 in 2024 for the known transmission downshift safety defect involving Output Shaft Speed (OSS) sensor signal loss. Despite completion of the recall repair, my vehicle is now exhibiting the same dangerous symptoms associated with the recall condition, incl
... uding: • Sudden erratic transmission behavior• Speedometer intermittently dropping to zero while driving• Wrench warning light activation• Check engine warning light• Airbag warning light• Unsafe drivability conditions and intermittent limp mode operation These symptoms create a serious safety concern because the vehicle unexpectedly changes driving behavior and loses speed signal information while operating on public roads. I presented the vehicle to Dick Smith Ford in Columbia, South Carolina for evaluation and was informed the truck requires a complete transmission replacement estimated at approximately $11,000. After extensive research, I discovered many Ford truck owners nationwide reporting nearly identical symptoms involving the 6R80 transmission, molded lead frame failures, OSS sensor failures, and unintended downshifting conditions. Ford previously acknowledged similar issues in Recall 24S37 and Customer Satisfaction Program 19N01. I am concerned the original recall remedy may not fully address the underlying hardware failure associated with this transmission system. My vehicle is demonstrating the same safety-related symptoms after the recall repair was already completed. Vehicle Information:2014 Ford F-150 XLTVIN: [XXX] I respectfully request NHTSA review this issue for continued safety concerns involving the Ford 6R80 transmission system and post-recall failures associated with Recall 24S37. On [XXX], I purchased a 2025 Toyota Sienna XLE Hybrid subject to NHTSA Recall 25V668 (Toyota Recall 25TA12 — second-row seat rail suspect welds), publicly announced on 10/07/2025. The selling dealer marketed the vehicle as "Certified Used" on Cars.com for at least 64 days after the recall was announced. The dealer's sales representative also represented the vehicle in writing via SMS as "Gold C
... ertified" with the full Toyota Certified Used Vehicle warranty package on [XXX] — one day before sale. The dealer disclosed the recall only at the closing table on [XXX], after I had relied on the prior representations to negotiate the price, submit a credit application, drive to the dealership, and tender a down payment. Toyota's interim recall guidance prohibits occupation of the second-row seats until remedy is performed. The remedy is currently not available. My [XXX] daughter has [XXX] and uses a wheelchair; second-row sliding-door access is essential for her safe transport. The vehicle has been in dealer storage since 01/08/2026 because it cannot safely transport my family. As of 05/24/2026 — 229 days after the recall was announced — no remedy date is available per the dealer's Service Manager. The vehicle is available for inspection upon request. Component failure: second-row seat rails (potentially improperly welded per Toyota's recall description). The defect has not been independently inspected; the recall is documented by Toyota and NHTSA. No warning lamps or symptoms appear because the defect manifests only in high-speed collisions. Component that failed: Front lower lateral link and compliance link suspension bushings. Failed components were removed by Tesla Service and are no longer available for inspection. Safety risk: Failed suspension bushings directly compromise steering precision, vehicle stability, and handling — particularly at highway speeds. A driver unaware of this failure could experience loss of steering cont
... rol or unpredictable handling without warning. Confirmed by dealer: Yes. Tesla Florida (Tampa, 11945 North Florida Avenue) confirmed the failure in writing on May 22, 2026. Technician repair notes state: “inspection failed due to worn suspension bushings in the front.” Tesla confirmed this was the first time these original components had ever been serviced. Invoice #[XXX]. Manufacturer inspection: Yes, repaired by Tesla Service Center. Cost: $1,669.58. Warning lamps or prior symptoms: None. No warning lights, no alerts through the Tesla app, and no prior symptoms communicated to the owner. Failure was discovered only because vehicle was brought in for an extended service agreement inspection. Assessment: Original factory suspension bushings completely disintegrated at 56,258 miles on a 2021 Tesla Model Y (VIN: [XXX] ). This is not normal wear — photographic evidence shows full structural disintegration of rubber bushing material. Other Model Y owners may be driving with identical failures without knowledge. On 5/232026 .. there was a severe thunderstorm . No hail . No signs of cracked window prior or as I approached my Honda 2023 HRV . I got in car and noticed windshield were foggy so I turned on both defroster. 10 mins later I heard a loud “boom “ in the back of my car.. I was starteled and immediately turned around and noticed the rear windshield was shattered and had a circular shaped broken
... glass hole. I never rolled anything on the street and no none was in sight .. I approached my home and parked the car and immediately checked the back to see if a rock or some sort was in the trunk however there was no evidence of that . Researching I notice many others have experienced this random spontaneous shatttering related to manufacturing error of window/defroster glue on defroster coiling . This was a huge danger for anyone in the back including myself the driver .. I’m very upset that Honda has not set a full recall on this other customers . I could have or another person sustained a serious injury. Now I’m waiting to get this repair which is a huge inconvience because all weekend it has stormed and having a garbage bag as a cover is so embarrassing ., I’m very displeased with Honda right now .Read more

While accelerating onto the highway in my 2016 Kia Optima 2.4L, the vehicle suddenly lost all engine power without warning. The vehicle would no longer accelerate and I was forced to coast and brake to safety. Immediately after the engine shut off, the check engine light, oil light, and battery light illuminated on the dashboard. After the incident, the vehicle would not restart or crank at all.
... The electrical systems, dashboard lights, and chimes still function normally, and the battery is new. The vehicle had proper oil level at the time of failure. No warning signs were present prior to the sudden loss of power. This failure created a serious safety hazard because it occurred during highway acceleration and left the vehicle unable to maintain speed or move out of traffic safely. Based on the symptoms and sudden engine shutdown, I am concerned this may be related to the known Theta II engine issues associated with Kia Optima vehicles.Read more

The vehicle suddenly began vibrating noticeably when idling and at low speeds. About 30 minutes after this started happening a warning lamp lit up. We drove the vehicle about 2 miles to a local auto parts store to get an initial computer diagnosis of the issue. The test resulted in a code of "P0304" which detected a cylinder #4 misfire and recommended a fuel injector replacement. We brought
... the vehicle to a local mechanic to get the issue further evaluated. After this evaluation, it was determined that the cylinder #4 misfire was related to a lifter failure. The mechanic explained that this situation could possibly have caused more damage, specifically to the camshaft, but that job was bigger than he was willing to take on. He recommended we get the vehicle towed to the dealership for further evaluation. He also informed us that this situation was common with the engine in this vehicle and GMC is aware of it. He directed us to some articles online that elaborated on the potential known issues related to the automatic fuel management (AFM) system and the related engine damage. The vehicle was towed to a local GMC dealership for evaluation. They found the failed cylinder #4 with associated lifter failure and damaged camshaft. We went forward with the repair at the dealership. We asked the dealership about the known engine issues with this vehicle. They acknowledged the widespread issues but informed us that our particular VIN has not been included in any recalls or safety notifications, so we were responsible for the repairs. The vehicle spent nearly 3 weeks at the dealership. One of the parts was on backorder due to a national shortage, which delayed the repair by about one week. That seems to imply that the parts involved in this repair are in high demand, which raises the question of this being a manufacturing/parts issue that may need to be investigated further.Read more

To Whom It May Concern: I am submitting this complaint regarding my 2020 Mercedes-Benz GLE 350 and respectfully requesting review of a serious infotainment and electronic system failure that occurred at approximately 49,000 miles. Prior to the failure of the MBUX infotainment system, the vehicle began exhibiting abnormal electrical behavior. On multiple occasions, the right headlight remained illu
... minated even after the vehicle had been completely turned off and locked. The only temporary way to resolve this was to restart the vehicle and shut it down again. Shortly afterward, the vehicle displayed warnings indicating that the 12V battery was very low and required charging. Soon after these electrical issues appeared, the MBUX infotainment system became stuck on the Mercedes-Benz startup logo screen and has remained nonfunctional since. Although the backup camera still partially functions, the infotainment system itself will not boot properly or operate normally. The authorized Mercedes-Benz dealership documented the following diagnostic fault codes: • B14D913 – Internal hard disk malfunction/open circuit • B14F900 – Internal hard disk full According to the dealership service report, guided tests, MOST ring reset procedures, and hard resets of the control unit were all performed without resolving the issue. The dealership ultimately recommended complete replacement of the MBUX head unit at a very significant cost. This situation is especially concerning because the vehicle has relatively low mileage, has been properly maintained, and recently underwent Service B and 12V battery replacement through an authorized Mercedes-Benz dealership immediately after the battery warning appeared. A major infotainment head unit/internal storage failure at this mileage is not something we reasonably expected from a luxury vehicle. While researching this issue further, I discovered numerous online reports from other Mercedes-Benz owners — particularly owners of 2020–2023 GLE models —Read more

They tell me it doesn’t do it when they have it .It’s been aligned 4 times replaced tires and it’s getting worse. Something needs to be done I know several people with this issue. And I k

I am reporting a serious safety concern involving a 2024 Chevrolet Tahoe with approximately 23,000 miles.I have a video recording of it . While the vehicle was being driven, water suddenly began actively dripping from the rear cabin light/headliner area into the interior of the vehicle. There were children inside the vehicle at the time, including a child seated directly underneath the area wher
... e the water was dripping near the child seat. A significant amount of water is entering the vehicle and continues dripping throughout the interior cabin area. The water appears to be coming from the upper rear cabin/light area and may have entered the electrical system or interior electronic components of the vehicle. I am extremely concerned about potential electrical hazards, safety risks, corrosion, and malfunction of electronic systems caused by water intrusion inside the vehicle. This vehicle has relatively low mileage for its age and has not been modified. The condition appears to involve a possible manufacturing, sealing, or drainage defect. Because children are transported in this vehicle daily, I believe this issue presents a serious potential safety concern and should be formally investigated. The sunroof exploded. Nothing hit it, there was nothing wrong with it previously and it was not being used at the time it exploded. We were driving and it exploded with a loud boom.

Component/system: Rear tires — traction and wet-weather grip performance. Tires are available for inspection upon request. Tire size: 265/70R16 Four BFGoodrich Trail-Terrain T/A 265/70R16 112T all-season tires were purchased new and professionally installed at a retail warehouse tire center in August 2025. The vehicle is a 2022 Toyota Tacoma. Approximately nine months after installation, both re
... ar tires began exhibiting severely degraded traction during heavy rain conditions on paved roads. On at least one occasion, the rear of the vehicle broke loose during a normal turn at low to moderate speed in heavy rain, resulting in a near-loss-of-vehicle-control incident. The front tires, from the same set and installation, did not exhibit the same level of traction loss during the same conditions, suggesting the failure is concentrated in the rear axle tires. Safety risk: The rear tire traction failure created a sudden and unexpected vehicle handling instability on a public road. The near-loss-of-vehicle-control incident posed a direct risk of collision to the driver and other road users. Dealer/service confirmation: The problem has not yet been confirmed by a dealer or service center — a tire center inspection appointment is scheduled. Results will be available following that appointment. Manufacturer/police/insurance inspection: The vehicle and tires have not yet been inspected by the manufacturer, police, or insurance representatives. Warning symptoms prior to failure: No warning lamps, messages, or audible warnings appeared prior to or during the traction loss event. The failure occurred without any prior indication of a problem. Tread wear appeared visually normal at the time of the incident. Assessment: The BFGoodrich Trail-Terrain T/A is marketed and sold as an all-season tire suitable for wet road conditions. The tire's performance in heavy rain is inconsistent with that representation and appears to constitute a safety defect. Multiple independent tire testing souRead more
