The vehicle exhibits two distinct and dangerous behaviors: 1. Engine stalling upon deceleration: When slowing to a stop, the engine stalls abruptly before the vehicle comes to a complete stop, causing a sudden, unexpected deceleration that throws occupants forward with significant force. 2. Unintended surge upon brake release: When releasing the brake pedal from a complete stop, the vehicle surg
... es forward unexpectedly and with excessive force, causing occupants to be jolted harshly without warning. Both incidents occur without any prior indication to the driver, leaving no opportunity to prevent the danger to occupants or others nearby. SAFETY RISK: Passengers were subjected to sudden, unexpected forward jolting without warning, posing a risk of injury. The vehicle's behavior also creates a hazard for other drivers and pedestrians in the vicinity. DEALER / SERVICE CENTER: The problem has not yet been confirmed by a dealer or service center. INSPECTION BY MANUFACTURER, POLICE, OR INSURANCE: The vehicle has not been inspected by the manufacturer, police, or insurance representatives at this time. WARNING LAMPS OR PRIOR SYMPTOMS: No warning lamps, dashboard messages, or prior symptoms appeared before or during these incidents. The failures occur without any advance notice to the driver, leaving no opportunity to take corrective action.Read more

A 2015 Toyota Sienna wheelchair-accessible conversion van has two open, unremedied safety recalls related to its braking system: a Braun Corporation recall for incorrect tube nut fittings on the rear brake lines that can cause a pressure drop in the braking system, and a Vantage Mobility International recall for rear brake hoses that may be too short. There is no indication either recall has been
... completed on this vehicle. The vehicle has been operated as a paid passenger-transport service carrying elderly and disabled passengers, including persons with cognitive impairment, to and from medical and rehabilitation facilities. This commercial use places vulnerable passengers at safety risk given the unresolved brake-system recalls. The vehicle has not been inspected by a dealer, manufacturer, police, or insurance representative to the reporter's knowledge. No warning lamps or other symptoms are reported — this submission is based on the open-recall status visible in the NHTSA VIN lookup. Supporting documentation is available on request.Read more

📄 NHTSA Norstar trailer and tire complaint Tire Safety Complaint – no safety margin I am reporting that there is no safety margin involving a Norstar Iron Bull 22’ 14K bumper pull deckover tilt trailer with (2) 7,000 lb axles and E rated Taskmaster Rambler trailer tires rated @3520 lbs each @80 psi. I have seen that the tires cannot handle the maximum payload of 9700lbs. The tires bow extre
... mely with 9100lbs and show a safety-related defect. Tire brand: Taskmaster Tire model: [Rambler] Tire size: [st235/80R16] Load range: [E load range] DOT code: [07A B5HFT7 2225] On [05/15/2026], while loaded 9143lbs stationary the tire experienced a sudden sidewall collapse. The Taskmaster Rambler tire was properly inflated according to the manufacturer’s specifications, and I was working within the trailers rated load capacity. An obvious failure is inevitable with this setup and the trailer is unsafe to operate at any speed. Norstar is manufacturing trailers with under rated tires for their claimed payloads. Based on the nature of the failure and the fact that the tire was being used within its rated limits, I believe this may represent a potential defect. I request that NHTSA review this incident and determine whether similar failures have been reported involving this trailer manufacturer and tire line. I am submitting this report to help identify any broader safety issues that could pose a risk to other driverRead more

I am reporting a safety‑critical issue with the automatic windshield wiper system on my 2024 Tesla Model Y. The vehicle uses a camera‑based “Auto Wiper” system, and it consistently fails to activate fast enough during heavy rain, spray from passing trucks, or sudden visibility loss. During moderate to heavy rain, the Auto wipers often stay on a slow or intermittent setting even when the w
... indshield becomes fully covered with water. The system does not increase wiping speed quickly enough to maintain visibility. This results in several seconds where I cannot clearly see the road ahead. This problem is made worse because the Model Y makes it difficult to quickly switch to manual wiper control while driving on the freeway. The driver must look down at the touchscreen and navigate menus to adjust wiper speed, which is unsafe at highway speeds. The delay between losing visibility and being able to manually increase the wiper speed creates a dangerous situation. This behavior has occurred multiple times at highway speeds. When passing large trucks or encountering sudden water spray, the wipers do not react, and the windshield becomes momentarily opaque. This creates a serious risk of losing lane position or failing to see vehicles in front of me. This is a safety hazard because: 1.The Auto wipers do not clear the windshield when visibility is urgently needed. 2.Manual override requires taking eyes off the road to use the touchscreen. 3.The delay in response can lead to loss of visibility for several seconds. I am requesting that NHTSA investigate this issue, as it affects visibility and could lead to collisions, especially at highway speeds or during sudden heavy rain.Read more

We took delivery of the vehicle in March 2025. The vehicle has never been involved in any accident or collision during our possession. • In April 2026, we discovered that the passenger-side rear brake light was no longer functioning. • Upon inspection, dealership personnel informed me that a small crack—approximately one inch in length—in the plastic housing of the driver-side rear turn si
... gnal allegedly allowed moisture intrusion that ultimately caused the passenger-side brake light failure. • I was further advised that the repair would not be covered under warranty and was presented with an estimate totaling $1,377.54 (copy enclosed). • The dealership speculated that the crack may have resulted from a minor parking-lot incident. However, no evidence was provided to establish either the cause of the crack or when it occurred. My concern is not merely the cost of the repair, although the quoted amount appears disproportionate to the nature of the issue. More importantly, the explanation provided raises serious concerns regarding the integrity and durability of the rear lighting assembly itself.Read more

There is a known issue with the transmission in my 2017 Toyota Highlander that causes premature failure. This is noted in a Toyota Customer Support Program Bulletin. I have written several times before about my car's phantom braking in cruise control. When the Santa Fe Tesla dealer, where I bought the car, told me recently that the problem had been fixed, I was disappointed they didn't bother to tell me, but thrilled to hear the news. Not so soon: on May 19th I was driving 2 friends up to volunteer at the Wildlife refuge on I-25 in northern New Mexico. The phantom braking happened again. My friends were dismayed...one thought I had seen something in the road that she didn't see. The other, in the back seat, tried to understand what happened. Please, this is beyond the pale! Of course I will call and take the car in to the dealer...an hour's drive for me. I will be surprised if they fix it, and if they do it under warranty. I did just buy new tires (see previous report about original tires with multiple flats and not even making it to their 40,000 mile warranty) so I'm no longer worried about getting a flat in the rural areas where I live and drive.Read more

The vehicle has experienced repeated transmission-related failures beginning at approximately 5,000 miles. The dashboard warning “Transmission System Error – Check Transmission Oil Immediately” first appeared while driving and was accompanied by sudden loss of power. When this occurs, the vehicle loses acceleration and drivability while in motion, creating a safety risk in traffic. The only
... way to temporarily clear the warning is to pull over, shut the vehicle off, and restart it. Photographs of the warning message were documented at approximately 6,738 miles on [XXX] and again at approximately 12,664 miles on [XXX]. The vehicle was returned to the selling dealer in Massachusetts multiple times for the same issue. During the first visit, the dealer drove the vehicle approximately 19 miles but stated they could not duplicate the problem. During the second visit on or about 3/25/2025, the dealer kept the vehicle for approximately six days while providing a loaner vehicle, but again stated they could not duplicate the condition. The vehicle was later inspected by another Hyundai dealer in New Hampshire on 8/19/2025 for the same dashboard warning and loss of power condition while driving. That dealer found the transmission fluid level below specification but stated they could not duplicate the warning message during inspection. Following that visit, the transmission warning did not reappear until [XXX], when the same “Transmission System Error – Check Transmission Oil Immediately” warning returned again while driving. On [XXX], the situation escalated into a possible rollaway event. While at an auto detailing facility, the vehicle was reportedly placed in Park with the engine running after being repositioned. The operator stated the vehicle indicated it was in Park. Moments later, after the operator exited the vehicle, the vehicle moved from its parked position and rolled into a building. The event was documented in writing by the detailing facility. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ARead more

NTIAL INTERSECTION W/ NO STOP SIGNS IN PLACE, THE TIRE IS EITHER SLIICED BY SHARP OBJECT IIN THE STREET OR JUST BLOWN FROM MANUFACTURER DEFECT, HEAT, OR THE LIKES, ANYWAY IT BLOWS ON THE PASSENGER SIDE OF THE TRUCK DIRECTLY BESIDE A VEHICLE PARKED ALONG THE RIGHT SIDE HEADING EASTBOUND, WHEN IT BLOWS THE VEHICLE DROPS ON THE PASSENGER SIDE LOSES CENTER OF CONTROL AND DESPITE EFFORTS TO AVOID, IT CAUSES MY COLLISION WITH THE PARKED CAR ON THE RIGHT, BECAUSE IT IMMEDIATELY WENT FLAT TO RIM, AND I WAS COLLIDED INTO, THE REAR-END OF A UNOCCUPIED VEHICLE WHICH THEN COLLIDED SLIGHTLY WITH VEHICLE IN FRONT OF IT WHICH LIGHTLY REAR-ENDED A THIRD PARKED CAR IN FRONT OF IT ALSO UNOCCUPIED AND PARKED. AND THE AIRBAG THEN DEPLOYED WITHIN THE VEHICLE ON IMPACT. THIS COULD HAVE RESULTED IN WORST OUTCOMES, MY DEATH OR SERIOUS INJURIES, OR SOMEONE ELSES INJURIES. AND THIS HAPPENED WITH NO WARNING PRIOR TIRE ISUES OR PUNCTURES AND VERY LIMITED TREAD USE OR TIRE PROLONGED ROAD EXPOSURES AND OTHER WEAR AND TEAR RELATED ACTIONS/ACTIVITIES. SO IM ISSUING HIS COMPLAINT BECAUSE THAT IS A RATHER SERIOUS DEFECT IF INDEED THE TIRE AND ITS MODEL, BRAND AND MANUFACTURER HAVE A POSSIBLE RECALL AND PRODUCT DEFECT ON IT'S TIRES THAT COULD BLOW ANYTIME, ANYWHER ON IT'S OWN THATS VERY DANGEROUS FOR ANYONE TO BE USING THE TIRES FOR VEHICLE OPERATION. THEY SHOULD BE PULLED FROM THE PLACES BEING SOLD AT PROMPTLY DISPOSED OF AND NEW REPLACEMENTS FOR AFFECTED DRIVERSRead more

Purchased vehicle from Wildcat Used Cars in Somerset, KY on [XXX]. Took home and notified dealership of not having seatbelts in middle row. After having an inspection completed, found the SRS system was disabled, airbags removed, knee airbag just stuffed back in. All cables and components unplugged, fuses missing to cause the system to not load up and show the defect. had resistance from dealershi
... p in undoing the sale, until i made note of their mechanics checking off and noting in the carfax that it was "all good" and had be completely inspected. Dealership admitted that they don't check fuse boxes and cables if the lights on the dash aren't illuminated. This is a huge failure of safety. All is on a recorded line from their phonelines from the date range of 4/27/26-5/2/26. All is notated in documentation and photographic evidence. Finally settled and they bought the vehicle back on [XXX] and assured me it would be sent back with the correct salvage title, and it is currently out for resale on their lot as of today 5/24/26. They are fully aware of all the issues. The SRS,ABS, HVAC 40 Relays, and the Traction control system have all been disengaged/compromised to bypass the system of any issues ongoing. The middle row seat isn't secured down, and the airbags have been entirely removed. [XXX] Above is the website listing for this vehicle. The carfax is not updated to the correct report. This vehicle was involved in an accident where the SRS system has been compromised & I was driving this with my [XXX] in this vehicle for approximately 2 months like this. I’ve been dealing with some intermittent problems with my 2022 transit for a couple years now. Basically I’ll be driving on the highway and all the sudden I’ll lose gas control, and power braking and the dash displays a message that says “shift to neutral to restart vehicle”, battery icon flashes red and I have to coast to the side of the highway. The electronics in the car will still be
... working when I pull over but the battery is dead. If I shift to park my engine doesn’t turn over or make any clicks at all. The few times it’s happened it eventually will turn back on after 10 min or so after using a jumper pack. I’ve taken it to dealers and they all say the computer isn’t throwing any codes so they can’t help. I’ve nearly been in 2 accidents now on the highway trying to get to a shoulder/lack of shoulder and the fact that I lose my brakes makes this an undrivable vehicle. My vehicle is still under manufacturer warranty but the ford dealers have not been able to help and keep sending me on my way with a very dangerous vehicle. I need help, I’m concerned about being involved in a a dangerous accident that could hurt myself or others when I lose brakes and gas. This has happened 5times now over the last 25k miles. I have a long video of the last incident that I would love to share if a video upload link is made available.Read more

While driving and starting to go into the turn lane on the right, to make a right turn, the car would not slow down, while pressing the brake the car would not slow, I could not turn at the speed and was unable to get back into the flow of traffic either , so was left driving into a guard wire holding up a pole.

I am writing to formally report a critical safety defect regarding the OnStar Telematics Control Module in my 2022 Chevrolet Tahoe High Country VIN: [XXX] . My vehicle was recently diagnosed by SERRA CHEVROLET as having a failed module, which has rendered numerous essential features inoperative. This is not an isolated incident; there is clear evidence of a widespread failure affecting thousands o
... f GM and Chevrolet owners. The failure of this module creates significant safety risks, including: * Loss of Emergency Services: The OnStar SOS button and automatic crash response are disabled, leaving me without a lifeline in the event of an accident. * Systemic Vehicle Malfunctions: This module is deeply integrated into the vehicle’s architecture. Its failure has caused [list your specific issues, e.g., "Check Engine" lights, heads-up display failure, and battery drainage]. * Disabled Connectivity: Core features I paid a premium for, such as navigation, remote start, and Wi-Fi, are now "bricked". It is unacceptable that a flagship $80,000+ vehicle is operating in a degraded and potentially dangerous state due to a known hardware defect. I am requesting that GM take immediate responsibility for this repair at no cost to me, regardless of warranty status, as this is a fundamental safety issue. Please provide a timeline on how GM intends to rectify this widespread defect. I look forward to your prompt response. While driving a drivetrain warning ⚠️ was detected and code was displayed on the I drive, along with a CEL. The vehicle is in limp mode so I was unable to accelerate past 35-40 mph. Took the car into the dealership and was informed that my waste gate actuator failed @59,388 miles. The part is an electric controller and can fail without warning. In my opinion 59,000 miles is extremely early fo
... r a part that controls boost pressure at the turbocharger. Found numerous complaints about this component and I feel it should be recalled by the manufacturer. This component was an issue on older models and BMW extended the warranty to 15 years/150,00 miles.Read more
