UPDATE to previous complaint – Reference Number: [11738920] VIN: [XXX] Case: Tesla [XXX] UPDATE to my previous NHTSA complaint regarding sudden unintended acceleration in my Tesla Model Y (VIN: [XXX] ) I previously reported a sudden acceleration incident on [XXX], at [XXX] 4 parking garage ramp. Tesla has now provided the Vehicle Data Report and EDR data (Case # [XXX]). The data shows accelera
... tor pedal position increasing to 100%, with brake input occurring late. However, I am 100% certain that I did not press the accelerator pedal at any time. I have a clean driving record of over 27 years. This discrepancy between the logged data and actual driver input strongly suggests a potential safety defect in the accelerator pedal sensor system (both redundant sensors), drive-by-wire system, or brake override / Automatic Emergency Braking functionality. Attachments included with this update: Tesla Vehicle Data Report (VDR) Tesla EDR Report High-resolution photos of vehicle damage and airbag deployment I request that NHTSA review this additional technical information as part of their evaluation for a possible safety defect investigation. BTW, it is really difficult to contact Tesla to report such incidents. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The engine was burning oil, so did a consumption test. The dealer took apart the engine and found the rings for the pistons were leaking and scaring was resulting. This caused me to buy part of a new engine because it was 6days past warranty with only 44,000mi on it. Also, at 18,000mi, a lifter bent and needed that replaced. There are numerous reports for this engine. It should be recalled! I shou

Hello, I am reporting a steering-related issue involving the inner tie rods on my 2021 Nissan Sentra. Nissan previously issued safety recall campaign PM985 for bent inner tie rods on certain Sentra vehicles from the same model year. Two separate repair facilities inspected my vehicle and confirmed problems involving the inner tie rods and steering system components. No warning lamps or dashboard m
... essages appeared prior to the issue being identified. Although my VIN may not currently be included in the recall population, my vehicle is experiencing a similar issue involving the same steering-related component identified in recall PM985. Because tie rods are directly related to steering control and vehicle stability, this issue raises concerns about driving safety. I am submitting this complaint so the issue can be documented and reviewed in case similar failures are occurring outside the current recall population.Read more

While traveling on the highway in traffic, brakes became very stiff when depressed after attempting to slow down to exit. Was able to put full bodyweight into the brake pedal to slow enough to exit and navigate to the gas station right off the exit. Had to roll stop signs and unsafely navigate traffic because brakes were not working. Service Brake System warning light came on after the incident.

Daytime running lights have both failed within the last 79 days. Passenger side failed at ~32660 miles o/a 05 Mar 26. It was replaced under warranty. Driver side failed around 36234 miles on 22 May 26 (5-days out of warranty). It is pending a shop appointment for replacement. This is not a “wear and tear” issue, it is a product quality control issue. It is not a simple plug and play re
... pair. It is a costly repair due to the design of the headlamp assembly. I have owned vehicles since 1988 and last changed a headlamp or taillight bulb in the early 2000’s and that was on a MY93 vehicle. I have also owned two other Land Rovers MY07 LR3 from 2008-2011 and MY14 LR4 from 2014-2022 and did not have to replace a single bulb in the vehicle. Point is that technology and quality have improved over the years and these headlamp/DRL assemblies should not fail with less than 36500 miles on the vehicle. This is a known issue that warrants further investigation. Jaguar Land Rover has issued a Special Service Message 75479 concerning the known issue with the Defender DRL. My DRLs failed in similar fashion described in the SSM. Inoperable DRL reduces safety due to lower visibility since they should operate during normal driving conditions. This issue is dangerous since there is no indication on the dashboard or other warning unless the headlights are manually inspected while the vehicle is operating. Additionally, inoperable DRL leads to a failed State vehicle safety inspections.Read more

My car was brand new with only 1442 miles. I was hit in the rear passenger side at a four way stop, this was not much force, however I was injured and my car was deemed a total loss. I fear this car ranks low in safety. Thankfully my daughter was not with me at the time.

I own a 2021 Kia Telluride with approximately 111,000 miles. I initially brought the vehicle to a Kia dealership for an oil consumption issue and engine noise. The dealership diagnosed and repaired a timing chain and tensioner issue, stating the vehicle was “working properly” upon return. After picking up the vehicle, I immediately began experiencing electrical warning lights, including the b
... attery warning light and system alerts (blind spot and general warning indicators). I returned to the dealership, where they performed diagnostic testing and indicated a potential alternator issue, but stated it was in a “borderline” range and that the vehicle was safe to drive. Shortly after leaving the dealership, while driving with my family in the vehicle, the electrical system began to fail. Multiple warning lights illuminated, and the vehicle lost electrical function while in motion. The vehicle ultimately became completely inoperable and shut down in a parking lot, creating a serious safety concern. Prior to the dealership visit and repairs, the vehicle had no history of electrical issues, warning lights, or charging system problems. The dealership later acknowledged that the alternator issue was not present when the vehicle was initially brought in. I am submitting this complaint due to the sudden and complete electrical system failure while driving, which posed a significant safety risk. The progression from no prior symptoms to total failure immediately following dealership service and diagnosis raises concerns about vehicle reliability and potential safety defects related to the charging system.Read more

To Whom It May Concern: I am submitting this complaint regarding a serious and ongoing safety defect involving the second-row driver-side seat belt in my Hyundai Palisade. I purchased this vehicle new on December 11, 2025, after being informed that the applicable seat belt recall repair had already been completed by the dealership on December 4, 2025. Despite the recall repair being performed prio
... r to sale, the second-row driver-side seat belt continues to malfunction. On multiple occasions while the vehicle has been in motion, the seat belt has unexpectedly unlatched on its own while occupied by a passenger. This creates a severe safety hazard and leaves the passenger completely unprotected during travel. The issue has occurred repeatedly and has made the second-row driver-side seat unsafe to use. This defect is especially concerning because the vehicle was sold as repaired and safe following completion of the recall campaign. The continued failure of the seat belt system raises concerns that the recall remedy may be ineffective or incomplete. I am requesting that Hyundai Motor America and the National Highway Traffic Safety Administration investigate this matter immediately. I also request that Hyundai provide a permanent repair solution and determine whether additional Hyundai Palisade vehicles may still be experiencing similar seat belt latch failures even after recall service has been completed. Vehicle Information: •Year/Model: Hyundai Palisade •VIN: [XXX] •Purchase Date: [XXX] •Recall Repair Completion Date: December 4, 2025 •Selling Dealer: Holler Hyundai 1150 N Orlando Ave Winter Park FL 32789 •Current Mileage: 8,169 I am extremely concerned about the safety of my passengers and believe this defect presents a significant risk of injury in the event of sudden braking or a collision. Because the loss-of-power issue is intermittent and occurs at highway speeds, I no longer consider the vehicle safe or reliable to operate. Family members won't drive or ride in the vehicle. The vehicle is being reported to Volvo and the dealership as a formal safety concern.Read more

The warnings on the car all failed and I was lead off the road while going on a roundabout. This has trigged my car to completely lose all control and even though I was attentive to my steering wheel I still didn’t stop the car from leading me to that curb and off road onto grass etc.

This complaint documents a recurring automatic transmission failure resulting in three complete vehicle shutdowns during active operation — two at highway speed and one in an active turning lane. The most recent failure occurred within 15 minutes of the vehicle being released from an authorized Hyundai dealership in College Park, Maryland, whose general manager admitted in a recorded call that t
... he vehicle was released knowing the transmission was unrepaired and unsafe. The first failure occurred in 2025 on a highway with no prior warning. The vehicle was towed to the dealership, where the transmission was confirmed defective and repaired under warranty. The second failure occurred in January 2026, again on a highway with no warning. The dealership held the vehicle from January through May 2026 — four months — with no updates despite multiple owner inquiries. In April 2026 the dealership confirmed the transmission had failed again and was seeking Hyundai corporate authorization for the warranty repair. On May 21, 2026 I was told the vehicle was ready and the transmission had been fully repaired under warranty. This was false. On [XXX], within 15 minutes of leaving the dealership, the transmission failed a third time, completely shutting the vehicle down in an active turning lane with no warning. The general manager at Hyundai College Park later admitted in a recorded call that the transmission was never repaired and the vehicle was released only to recover the dealership's rental car, with full knowledge it was unsafe. All three failures were confirmed by the authorized dealer. Component is available for inspection. NHTSA investigation and recall consideration requested.
