Transmission began failing. Whining coming from transmission when accelerating and shifting becoming more rough. Dealership said transmission is failing and needs to be replaced. Mileage is at 89,000 miles. Rough shifter made it dangerous to drive on the highways. Was inspected by warranty adjuster. No warning lamps or lights ever appeared.

History of Failed Manufacturer Remedies: This vehicle has been brought to an authorized Ford dealership seven (7) separate times for this exact rearview camera failure. The manufacturer has repeatedly failed to provide a permanent remedy. The dealership attempted repairs under Ford’s active safety campaigns (including Recall 23S48 / 25S38 regarding the Image Processing Module and rear wiring). Despite these recall repair attempts, software flashes, and component replacements, the system fails again immediately after every visit. The camera is currently non-functional. The manufacturer’s authorized dealer network cannot fix this federally mandated safety feature (FMVSS 111), leaving the vehicle inherently unsafe to operate in reverse. I am filing this complaint to document a failed recall remedy and a persistent, unresolvable safety defect.Read more

On Friday, December 5, 2025, I was coming out of the driveway at the Jack In The Box on the corner of El Camino & Nobili Avenue in Santa Clara. My foot was on the break as I was going to stop at the end of the driveway to check to see if there was any traffic coming down Nobili before I exited the Jack In The Box driveway. ... ot on the brake, the car started to speed up (unintentional acceleration)and I continued to press my foot down harder on the brake, but nothing could stop it. A lot of black smoke came up out of the steering wheel area where it attached to the dashboard & all of the air bags around the car deployed. Then my car completely turned itself around and the front of my car was now facing the driveway that I had just come out of. This torque of acceleration and braking is why my car spun around 180 degrees. The vehicle had sustained damage to the front end, the hood, front bumper cover, right front head lamp was cracked, and the Toyota emblem was missing. (See attached photos taken at the time of the accident) Since all of the air bags in the whole car had completely deployed, I found that the only way I could get out of the car was to slide myself under the air bag that surrounded the driver side door. As I got out of the car, the rib that is directly under my right breast began to hurt very badly and it was hard for me to take a breath with the pain from that rib. I also had a bad headache, too. Plus, the top of my left hand was all red and felt like something had scratched the whole back of my hand. It hurt to touch it. I was also light-headed.The car, I was told, looked like I had hit a wall. The whole left front side of the car was severely mangled - the hood, the fender, and the tire was flat. Installing a car seat and the belt buckle failed. The plastic part split and no longer holds the belt together. This was under regular pressure with standard leverage that the car seat design calls for. Nothing was ever on top of the buckle and it is not damaged in any other way at all other than the failure. They refused to replace it via warranty.

Toyota appears aware of this transmission-related issue, yet many owners outside the warranty period are being denied coverage despite similar failures and complaints. I am concerned this may represent a broader transmission defect that could lead to premature transmission failure, unsafe driving conditions, hesitation, loss of power, or expensive repairs for consumers. None prior to failure Detailed description: On 5/21/2026, during normal driving, the engine in my truck lost propulsion due to a major internal failure. This is a 2023 General Motors 6.2L (L87) engine that was inspected at the dealership where it was originally purchased for the previously issued NHTSA "loss of propulsion" recall and passed without any noted issues. This engine only has ~20,000 miles on it. The engine failed in relatively heavy traffic and could have easily resulted in a serious accident due to inability to route my vehicle to a safe area while under power. This all happened without any prior warning, no check engine lights, noises or lamps or symptoms. The dash simply illuminated with a "Press Start" message.The truck was towed to the dealer for evaluation. The dealer called me today (5/26/2026) stating the engine was seized and a new engine would have to be ordered from GM for installation. Driving at 30mph and all the dashboard lights turned on and all the warnings kept popping on the dashboard and 30 seconds later the brakes were not working when I was approaching a busy intersection. On May 9, 2026 I was driving to work when my car experienced acceleration failure. It had 80,600 miles on it.
