Incident Narrative
Employees were on-site delivering a bundle of pipes. An employee was unloading the pipes by tying the pipes with a chain that was hooked to the back of a pickup truck. When the pipes came off the flat bed, one of the pipes struck the injured employee, who was standing on the ground near the flat bed, on the head resulting in a laceration.
