Incident Narrative
An employee was attempting to build a wooden base for a manual sheet metal box pan brake. He lifted the brake onto the forks of a forklift. He then aligned the brake over bolts to secure the brake to the wood base. He exited the forklift and stood in front of it to make adjustments. He was leaning over to kneel on the floor when the brake slid off the forks and fell on him, trapping him underneath. He suffered a pelvic fracture, knee injury, and bruising and was hospitalized.
