Incident Narrative
An employee was moving a 550-pound titanium slab to another location on the plant floor using a vacuum attached to a crane. She then went to mark the slab when the vacuum failed and the slab fell, pinning her right hand between the slab and the floor. This resulted in a fracture to the right middle finger and amputation of the right index fingertip.
