Incident Narrative
An employee was operating a lift to move a pallet of plywood when the forks extended to another stack. When the lift started to lift the intended pallet, the subsequent stack behind it started to lift and tilt and then fell. An employee walking nearby was caught under the pallet of plywood and suffered a fractured skull, two broken ribs and lacerations.
