Incident Narrative

On 12/2/2015, at approximately 12 p.m., an employee was climbing a ladder to the roof to install fall protection anchor points. The decking under the ladder was frosted over. As the employee climbed the ladder, the ladder slipped, causing the employee to fall approximately 10 feet to the decking. The employee was hospitalized with contusions to the left shoulder, left leg, and ribs.
