Incident Narrative
At approximately 2:00 p.m., on 3/30/16, an employee was climbing down from a one-story roof using a metal extension ladder. The ladder collapsed, and the employee fell approximately 10 feet to the ground. He was hospitalized with two cracked right-side ribs and four cracked vertebrae.
