Incident Narrative
An employee was removing the ejection plate from a mold while a crane was being used to "take a strain" on the plate. The employee was under the mold, which was sitting on saw horses, using a wrench to remove bolts. As he was removing the bolts, the mold shifted and broke his ankle.
