AIR BAGS:FRONTAL:PASSENGER SIDE:INFLATOR MODULE
On January 9, 2018, Toyota Motor Engineering & Manufacturing (Toyota) notified NHTSA that they are recalling 14,585 model year 2010-2012 Yaris vehicles ever registered in the states of Alaska, Arizona, Arkansas, Colorado, Connecticut, Delaware, District of Columbia, Idaho, Illinois, Indiana, Iowa, Kansas, Kentucky, Maine, Maryland, Massachusetts, Michigan, Minnesota, Missouri, Montana, Nebraska, Nevada, New Hampshire, New Jersey, New Mexico, New York, North Carolina, North Dakota, Ohio, Oklahoma, Oregon, Pennsylvania, Rhode Island, South Dakota, Tennessee, Utah, Vermont, Virginia, Washington, West Virginia, Wisconsin, and Wyoming. As of January 30, 2018, Toyota is additionally recalling 3,347 model year 2010 Pontiac Vibe vehicles located in states listed above. Also included in this recall filing is 98,194 model year 2003-2008 Toyota Corolla Matrix vehicles, located throughout the U.S., that previously received a “like-for-like” interim air bag inflator replacement. These vehicles are equipped with certain air bag inflators assembled as part of the passenger frontal air bag modules used as original equipment or replacement equipment. In the event of a crash necessitating deployment of the passenger frontal air bag, these inflators may explode due to propellant degradation occurring after long-term exposure to absolute humidity and temperature cycling.
Remedy: Toyota will notify owners, and dealers will replace the passenger frontal air bag assemblies with an alternate inflator, free of charge. The recall began on January 29, 2018. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's numbers for this recall are G0R (Zone B), and H0A (Zone C), J0A (Matrix Zone A), J0A (Matrix Zone A), J0A (Matrix Zone A).

Toyota Motor Engineering & Manufacturing (Toyota) notified the agency on May 13, 2015 that they are recalling certain model year 2004 Sequoia vehicles manufactured August 18, 2003, to August 19, 2004, and 2003-2004 Tundra vehicles manufactured March 18, 2003, to August 25, 2004. On June 16, 2015, Toyota expanded the recall to include an additional 1,270,468 vehicles, including model year 2002-2003 and 2005-2007 Sequoia, 2005-2006 Tundra, 2003-2007 Corolla, 2003-2007 Corolla Matrix, 2003-2007 Pontiac Vibe and 2002-2007 Lexus SC vehicles. This is to address a safety defect in the passenger side frontal air bag inflator which may produce excessive internal pressure causing the inflator to rupture upon deployment of the air bag. This recall addresses both the passenger side frontal air bags that were originally installed in the vehicles, as well as replacement air bags that may have been installed as replacement service parts. A replacement air bag may have been installed, as one example, if a vehicle had been in a crash necessitating the replacement of the passenger side frontal air bag.
Remedy: 2003-2004 Corolla and Corolla Matrix vehicles that are recalled under both 15V-043 and 15V-285 are under a Do Not Drive advisory. Owners of these vehicles are advised not to drive their vehicles until the recall remedy has been completed. Toyota will notify their owners. Pontiac Vibe owners will be notified by GM. Dealers will replace the front passenger air bag. The recall began on July 13, 2015. Toyota owners may contact Toyota customer service at 1-800-331-4331. Pontiac owners may call 1-800-762-2737. Toyota Motor Engineering & Manufacturing (Toyota) notified the agency on May 13, 2015, that they are recalling 2005-2007 Lexus SC, Toyota Corolla, Corolla Matrix, Sequoia, and Pontiac Vibe, and 2005-2006 Toyota Tundra vehicles that were originally sold, or ever registered, in Florida, along the Gulf Coast, Puerto Rico, Hawaii, the U.S. Virgin Islands, Guam, Saipan and American Samoa. On June 16, 2015 Toyota expanded the recall to cover an additional 769,055 vehicles, including certain model year 2002-2004 Lexus SC, Toyota Sequoia, 2003-2004 Toyota Corolla Matrix, Toyota Corolla, Pontiac Vibe, and Toyota Tundra vehicles, as well the vehicles covered by the May 13, 2015 notification in the portions of Georgia, Alabama, Louisiana, Mississippi and Texas that were not previously included. The inflators in the front passenger air bags are susceptible to rupture in the event of a crash necessitating the deployment of the front passenger air bag.
Remedy: 2003-2004 Corolla and Corolla Matrix vehicles that are recalled under both 15V-043 and 15V-286 are under a Do Not Drive advisory. Owners of these vehicles are advised not to drive their vehicles until the recall remedy has been completed. Toyota will notify their owners. Pontiac Vibe owners will be notified by GM. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2003-2004 Toyota Corolla vehicles manufactured December 28, 2001, to May 2, 2004, 2003-2004 Toyota Corolla Matrix vehicles manufactured January 6, 2002, to April 29, 2004, 2003-2004 Avalon vehicles manufactured June 5, 2002, to December 20, 2004, and 2003-2004 Pontiac Vibe vehicles manufactured January 18, 2002, to April 27, 2004. Due to electrical noise, a component in the air bag control module may fail, causing the front air bags, side air bags, and/or seatbelt pretensioners to deploy inadvertently while the vehicle is being operated.
Remedy: Toyota will notify their owners. Pontiac Vibe owners will be notified by GM. Dealers will replace the air bag control module, free of charge. The recall began on August 24, 2015. Toyota owners may contact Toyota customer service at 1-800-331-4331. GM issued an interim notification to owners on March 27, 2015. The recall began on August 24, 2015. Pontiac owners may call 1-800-620-7668. Note: This recall supersedes recalls 13V-029 and 14V-147. Vehicles that had a noise filter installed as the remedy for one of those campaigns need to have the air bag control module replaced under this campaign.

Toyota is recalling certain model year 2003-2004 Corolla and Corolla Matrix vehicles and 2003-2004 Pontiac Vibe vehicles. The supplemental restraint system (SRS) circuits are susceptible to internal shorting. The electrical short may create an abnormal current flow and increased heat which can damage the circuits. This could result in an inadvertent deployment of the front airbags and/or seat belt pretensioners.
Remedy: Toyota will re-notify their owners who have already had their vehicles repaired with a noise filter, but did not receive a new airbag control module, and General Motors will re-notify theirs. Dealers previously installed a supplemental harness containing an electrical filter between the airbag control module and its wire harness, free of charge. Dealers will replace the airbag control module, free of charge. The recall began on March 13, 2013. The manufacturer has not yet provided a schedule for second notices. I was horrified to learn that the crash ratings are inaccurate for most women and all short people, because they were developed only for average sized men. It should be no surprise that when collisions occur, more women than men are seriously injured. More women than men are injured when air bags deploy. More women than men are injured by their shoulder belts. I am afraid to purchase a new car, because the crash ratings that I have relied on in the past are unreliable for me as a short woman. I am very disappointed that my own government, to which I pay taxes, has been so misleading to the public on important safety issues. Please develop and implement collision ratings for various height groups. In the meantime, please include notices with crash ratings that explain which height ranges the ratings are most accurate for.

The contact owns a 2004 Toyota Corolla Matrix. The contact stated that while driving at various speeds, the air bag warning light illuminated. The contact called the dealer to inquire about a recall for the air bag and was informed there were 2 previous air bag recalls that were repaired in the past. The manufacturer was informed of the failure and advised the contact to take the vehicle to the dealer for a diagnostic test. The approximate failure mileage was 159,086.

I have been unable to have my dealer repair the recalls on this car despite repeated times calling up Toyota and dealing with Larry H Miller Toyota dealership. They refuse to give me a replacement car while they fix it and won't say how long it will take. I also discovered that there are no child seat anchors in the back. I was told this is unlawful given the date of this car's manufacture. I hope NHTSA will help force Toyota to fix these recalls and install car seat anchors and give em a replacement vehicle to drive to work and for children while they fix it.
TIRES Aug 21, 2022
A 205/55/R16 tire exploded when I was driving on the freeway at 9:15 am. I had driven less than 3 miles. The tire that exploded had less than 16,000 miles. I didn't run over an object or I would have also punctured my front tire too. At the time of the explosion I didn't know what happened, but heard a loud noise that I thought was from a car next to me. Once both cars moved ahead of me, I realized the loud noise was coming from under my car. I then was able to pull to the side of the freeway. Once I pulled over I saw the tire had exploded. I took photos of the tire and took both parts to the tire shop where my tires are under warranty. I had inspected my tires the prior Monday, two days prior and didn't see an issue with any of the tires. I've never had a tire blowout before. The tire shop threw the tire away even though I asked for a report of the incident to appropriate entities. The tire shop told me the tire was under inflated or that I hit a road hazard but couldn't prove that's what caused the explosion. Also the tire shop did not show me where a road hazard penetrated the tire. In the past if a tire had run over an object this tire shop would show me the object. I have 3 of the same tires on my car which I don't trust because my tire exploded without warning.

The vehicle was destroyed several years ago. The contact stated that they have not owned the vehicle involved for many years.

My daughter was driving as usual when she went to apply the brakes about 5 car lengths back and the vehicle accelerated uncontrollably despite braking. Her foot was not on the accelerator, she does not have floor mats that would cause anything to hit the accelerator. This has happened on 5-6 occasions which almost resulted in an accident. She barely got stopped mere inches away from the car in front of her despite braking earlier than normal because she encountered this once already during her trip. No check engine codes, the brakes are good, the car idles high. That is really it. This is a serious issue that seemingly cannot be diagnosed. After doing research to see what the problem may be, I have seen countless complaints of a similar nature. My daughters life is invaluable. This needs to be addressed.

HE SHOWED ME THE PROBLEM AND SAID IT WOULD BE DANGEROUS TO DRIVE THE CAR. THE CAR HAS LOW MILEAGE FOR BEING 16 YEARS OLD (ABOUT 8K A YEAR). THIS CAR WAS BUILT IN CANADA AND SOLD IN NEW YORK STATE SO IT SHOULD HAVE BEEN DESIGNED TO TAKE MANY YEARS OF WET/WINTER ROAD CONDITIONS. TAKATA RECALL I TOOK MY CAR IN FOR THE RECALL AND WAS TOLD A FEW WEEKS LATER THAT IT WAS COMPLETE. WHEN I WENT TO PICK UP MY CAR, HE TOLD ME THAT IT WAS ONLY PARTIALLY DONE, BUT WAS "COMPLETELY SAFE". HE WENT ON TO EXPLAIN THAT THEY REPLACED THE ONE PART THAT CAUSED THE AIRBAGS TO DEPLOY, SO NOT TO WORRY ABOUT THE SAFETY ANYMORE. I JUST RECEIVED THE RECALL AGAIN SAYING THAT THE PARTS WERE AVAILABLE, AND THAT IT WAS IMPORTANT TO GET IT FIXED BECAUSE THE AIRBAGS ARE STILL NOT SAFE. THE CONTACT RECEIVED A RECALL NOTIFICATION FOR NHTSA CAMPAIGN NUMBER: 15V043000 (AIR BAGS) AND STATED THAT THE PART NEEDED WAS UNAVAILABLE TO PERFORM THE RECALL REPAIR. THE CONTACT MENTIONED THAT THE AIR BAG WARNING LIGHT REMAINED ILLUMINATED AFTER THE RECALL REPAIR. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE. THE FAILURE MILEAGE WAS 170,000.

THE DRIVER'S SIDE WINDOW HAS MADE NOISE WHEN LOWERED AND RAISED SINCE SHORTLY AFTER THE TIME OF PURCHASE IN 2004. IN 2008, WE HEARD ABOUT THE RECALL ON 2003-04 MATRIX VEHICLES FOR DEFECTS WITH THE FRONT DOOR GLASS BOLT (WE ALSO HAVE A 2003 COROLLA, AND RECOGNIZED THAT THE PROBLEM DESCRIBED IN THE RECALL NOTICE RECEIVED FOR THE COROLLA MATCHED THE PROBLEM WE WERE HAVING WITH THE MATRIX). WE TOOK OUR CAR IN 2008 TO THE LOCAL TOYOTA DISTRIBUTOR, WHERE CAR WAS PURCHASED NEW, BUT WERE TOLD THAT THE RECALL DID NOT APPLY TO OUR MATRIX, BECAUSE THE MATRIX VIN WASN'T INCLUDED IN THE RECALL. THE NOISE WHEN USING THE DRIVER'S SIDE WINDOW CONTINUED TO BECOME MORE PRONOUNCED SINCE 2008, AND NOW THE NOISE HAS BECOME SO LOUD THAT WE ARE CONCERNED THE WINDOW WILL BREAK IF LOWERED. I'VE CONTACTED THE TOYOTA CUSTOMER SERVICE CENTER, AND THEY SAY THAT THERE IS NOTHING THEY CAN/WILL DO, BECAUSE THE CAR'S VIN INDICATES THAT IT WAS NOT PART OF THE RECALL. IF WE WANT TOYOTA TO TAKE A CLOSER LOOK AT THE PROBLEM TO DETERMINE WHETHER IT MIGHT BE DUE TO A DEFECTIVE PART, THE CUSTOMER SERVICE CENTER SAID THAT WE WOULD HAVE TO PAY TOYOTA FOR A DIAGNOSIS. THE WINDOW MAKES THE LOUD CLUNKING SOUND AT ALL TIMES WHEN BEING LOWERED, WHETHER THE CAR IS IN MOTION OR STATIONARY. THE WINDOW NOISE HAS BEEN PRESENT FOR AT LEAST 7 YEARS, AND HAS WORSENED OVER TIME. SINCE THE SYMPTOMS MATCH THOSE DESCRIBED IN THE 2008 RECALL NOTICE, IT SEEMS VERY LIKELY THAT THE WINDOW BOLTS ON OUR 2004 MATRIX ARE SUFFERING FROM THE SAME MANUFACTURER'S DEFECT AS DESCRIBED IN THAT RECALL.

I OWN A 2004 TOYOTA MATRIX WITH DEFECTIVE AIRBAGS. I THINK IT IS APPALLING THE TOYOTA WITH THE BLESSING OF NHSTA IS ALLOWED TO CHOOSE WHICH VEHICLES TO RECALL ON THIS ISSUE BASED ON LOCAL HUMIDITY. I WAS TRAVELING AT APPROXIMATELY 35 MPH WHEN I FIRST STARTED TO APPLY PRESSURE TO MY BRAKES. THERE WAS NO TENSION WHAT SO EVER. I WAS STANDING ON THEM AND THE BRAKE PETAL WAS HITTING THE FLOOR. AS A RESULT, I RAN THOUGH A STOP SIGN AND HIT A CONCRETE BARRIER. MY STEERING WHEEL ALSO LOCKED UP AND MY AIRBAG NEVER DEPLOYED. I LOST MY ENTIRE FRONT BUMPER AND THE TANK THAT HOLDS THE WINDSHIELD FLUID WAS CRACKED. THE CAR WAS TOWED. I CONTACTED TOYOTA'S MANUFACTURER AND WAS TOLD " IT HAPPENS". HE SAID WE HAD TO FILE A CLAIM WITH TOYOTA AND GAVE US A NUMBER. WE FOUND OUT LATER THAT DAY THERE WAS A RECALL ON THE AIRBAG SYSTEM AND WE HAD NOT RECEIVED A NOTICE IN THE MAIL. HE DID NOT INFORM US THERE WAS A RECALL ON THE VEHICLE EVEN THOUGH TOYOTA'S WEBSITE SHOWED THE CAR WAS PART OF THE RECALL. WE HAD THE CAR TOWED TO ANOTHER DEALER, [XXX] OF OKLAHOMA CITY. THEY DID KNOWLEDGE THE VEHICLE WAS INDEED PART OF THE RECALL. SAFETY RECALL NOTICE (TO FIX SRS AND ECM) WAS RECEIVED FROM TOYOTA IN THE SECOND WEEK OF FEBRUARY, 2013. THE VEHICLE WAS TAKEN TO THE LOCAL TOYOTA DEALER (LEE'S TOYOTA, 139-65, QUEENS BLVD, JAMAICA, NY, 11435) ON 04/03/2013, TO HAVE FIXED THE PROBLEM. SINCE THE CORRECT PARTS WERE NOT AVAILABLE AT THE SERVICING CENTER, THE REPRESENTATIVE HAS INFORMED ME THAT, THEY WILL PLACE AN ORDER FOR THE PARTS AND WILL INFORM ME ONCE THE PARTS ARE AVAILABLE AT THE SERVICE CENTER (PER THE SERVICE CENTER THE WAIT PERIOD FOR THE PARTS IS ABOUT COUPLE WEEKS). TODAY, 04-30-2013, I CALLED THE SERVICE CENTER TO CHECK THE STATUS OF THE PARTS. THE SERVICE CENTER HAS INFORMED THAT, THEY DID NOT RECEIVE THE PARTS AND NOT SURE AND HAVE NO IDEA WHEN THEY WILL RECEIVE THEM AS TOYOTA IS TAKING TOO MUCH TIME TO PRODUCE THEM. I REQUEST THAT THIS MATTER MAY PLEASE BE EXAMINED AND SEE HOW SOON THE PARTS WILL BE AVAILABLE (I HAVE A FEAR THAT MY CAR MAY DEVELOP NEW OR OTHER RELATED PROBLEMS AS I AM USING IT WITH ECM AND SRS PROBLEMS AND WITH THE AIR BAG MALFUNCTION LIGHT IS ON THE DASH BOARD).
