Consequence
A fuel leak in the presence of an ignition source can increase the risk of a fire.
Remedy

Consequence
If the accelerator pedal binds, the engine may not quickly return to idle after the pedal is no longer pressed, increasing the risk of a crash.
Remedy

... body shop said that the car's paint seems to have a defect such as clear coat is gone on car that is why it is peeling badly which is resulting to rust. I do not know if the paint had been improperly applied or issues with chemical composition of the paint that lead to premature chipping and wear that is occurring with the car. I had a 2007 honda pilot older than the 2014 Honda Ridgeline and it is not even peeling and rusty like the ridgeline. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? By Martin--email [email protected] who is the body shop manager at Russ Darrow Honda. I brought it to Honda Corporation's attention at [email protected] Case #: 12815491 and they refuse to assist claiming that there is no recall and it is considered "normal wear and tear" which I do not believe that in my case is the truth. There have been several complaints about Honda ridgeline's customers who are unhappy about the peeling paint. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Started toward at the end of 2021 and then had it estimated in Mar 2022, cost over $10,519 for the paint alone. For a paint job that cost over $10519 that means that there must be something wrong with the car to being with during manufacturing. It should NOT be blame on normal wear and tear. Solution: I would appreciate for Honda to do a courtesy repair on the car such as a new paint job at their own expense. Perhaps NHTSA should contact and research about folks who are in the same position as I am with the peeling paint.Read more

This alarm is loud enough and persistent enough to cause driver distraction and would become intermittent as carpet dried. insulation had to be removed after it was noted still wet after three days of no rain and with what appeared visibly as black mold near steering column at floor board. This appears similar to water infiltration addressed in the Honda service bulletin 07-082 for the 2007 and 2008 year models of the Ridgeline truck.Read more

IN DECEMBER 2016 I BEGAN TO NOTICE BILLOWING CLOUDS OF BLUE SMOKE FROM MY EXHAUST PIPE ON STARTUP. ON WATCHING THE PROBLEM FOR THE NEXT COUPLE OF WEEKS I NOTICED THAT IT HAPPENED INTERMITTENTLY, USUALLY (BUT NOT ALWAYS) WHEN THE ENGINE WAS COLD. FOLLOWING THE INITIAL BLUE CLOUD (DEFINITELY NOT BLA
... CK OR WHITE SMOKE), ANY EXHAUST GASES BECAME INVISIBLE (BUT I AM UNABLE TO DETERMINE WHETHER MY TRUCK WAS SMOKING FROM THE EXHAUST WHEN I WAS DRIVING). I ALSO NOTICED THAT WHEN I STARTED IT UP WHILE IN THE DRIVEWAY (WITH THE INTENTION OF PULLING THE TRUCK INTO MY GARAGE), EVEN IF THERE WAS NO SMOKE VISIBLE THE GARAGE WOULD FILL WITH FUMES ? IT WAS SO BAD THAT I WOULD HAVE TO HOLD MY BREATH, EXIT THE TRUCK, AND SPRINT INTO MY HOUSE TO AVOID INHALING THOSE FUMES. IT QUICKLY BECAME APPARENT TO ME (AS AN AMATEUR MECHANIC) THAT SOMEHOW OIL WAS FINDING ITS WAY INTO THE COMBUSTION CHAMBER AND WAS BEING BURNED, RESULTING IN THE SMOKE & FUMES. I THEREFORE TOOK THE TRUCK TO THE HONDA DEALER IN ORDER TO HAVE THEM ISOLATE AND REPAIR THE PROBLEM. LATER IN THE DAY [XXX] PHONED TO SAY THAT THEIR SERVICE TECHNICIANS HAD NOT BEEN ABLE TO REPLICATE THE PROBLEM, THEREFORE HAD DONE NO SERVICE TO THE TRUCK, AND THAT I SHOULD COME PICK IT UP. WHEN I ASKED WHAT DIAGNOSTIC SERVICE HAD BEEN PERFORMED, [XXX] TOLD ME THAT THEY HAD STARTED THE TRUCK SEVERAL TIMES AND LET IT RUN FOR SEVERAL MINUTES, ALL THE WHILE VISUALIZING THE EXHAUST PIPE TO SEE IF THERE WAS ANY SMOKE: THERE WAS NONE. THEREFORE THEY SEEMINGLY DECIDED THAT THERE MUST NOT BE ANY PROBLEM AND THAT MY COMPLAINT WAS NOT VALID. No Restart After Auto Start/Stop Engages
On June 3, 2022, the Oï¬ce of Defects Investigation (ODI) opened Preliminary Evaluation PE22005 after the agency received VOQs and several ï¬eld reports concerning the Auto Idle Stop (AIS) feature on 2016-2019MY Honda Pilot vehicles. The complaints allege that the engine fails to restart on
... its own from a complete stop at a traï¬c light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (âPGM-FIâ) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. This Engineering Analysis is being opened to further consider the potential safety defect, including gathering additional data regarding Hondaâs service campaign efficacy. ODI is additionally expanding the scope to encompass all vehicles addressed in Hondaâs service bulletins 23-008 and 23-009 as well as newer model year vehicles, which are substantially similar to the vehicles covered by the service campaign. Recall 22V-867 states that the RVC wire harness was manufactured with a protective corrugated tubing which was insufficient to protect against bending wear and zip ties which were insufficiently tightened and would not keep the protective tubing in place. The consequence is that the RVC wire harness may fatigue and break after repeated opening and closing of the tailgate. This ultimately results in the failure of the RVC image to display due to a lack of connectivity. Hondaâs remedy identified in the recall filing was a replacement harness with longer protective corrugated tubing and sufficiently tightened zip ties. This remedy part was also installed as a production part in certain MY 2019 Ridgeline vehicles. On May 3, 2024, Honda submitted recall 24V-321 to ODI, addressing concerns of a loss of the RVC function in MY 2020-2024 Honda Ridgeline vehicles. Recall 24V-321 states that the RVC wire harness was manufactured with a material which was susceptible to breakage upon repeated opening and closing of the tailgate, which ultimately leads to a complete loss of RVC function. Hondaâs remedy identified in the recall filing was a replacement harness manufactured by a new supplier with improved material properties to withstand wear from bending. Since the remedy parts for recall 22V-867 and the production parts recalled under 24V-321 use the same supplier and materials for critical components in the RVC wire harness, ODI opened RQ24011. To date, ODI has received 1 allegation of a 22V-867 remedy RVC wire harness failure. Honda indicated that it is aware of a total of 14 reports regarding remedy RVC wire harness failures associated with Recall 22V-867. Additional remedy parts may not have yet seen sufficient wear to induce the failure. ODI also believes further testing is necessary to assess the potential failures in these parts. This Engineering Analysis is being opened to further assess the scope, frequency, and potential safety related consequences of the rear view camera harness failures in 2017-2019 Honda Ridgeline vehicles. Honda recalled these vehicles due to a crankshaft manufacturing defect that caused premature connecting rod bearing wear and engine seizure. While the out-of-scope failures involved in these complaints appear to have a very similar failure mechanism as the recalled engines, a thorough analysis of all available data from Honda has not indicated that the same manufacturing defect exists in the out-of-scope engines as the engines recalled by 23V-751. Based on this analysis ODI has determined that Honda has properly defined the scope of recall 23V-751. Consequently, ODI will close this RQ investigation. NHTSA will continue to investigate these complaints of engine failure as part of a new Preliminary Evaluation Investigation (PE25008). The RVC wire harness is routed near the hinge in the tailgate, in a location where frequent opening and closing of the tailgate causes bending stress on the harness. Honda determined the wire harness was manufactured with a material which is permeable to water and salt, which when it freezes and thaws, combined with tailgate usage, ultimately leads to breakage of the wiring in the tailgate harness. This results in a failure to display the RVC image. On May 9, 2024, Honda filed a Part 573 Safety Recall Report recalling all 2020-2024 MY Honda Ridgeline vehicles. The dealer will replace the RVC tailgate harness with a wire harness that is manufactured by a new supplier and has improved material properties to withstand wear from bending. In view of Honda filing recall 24V-321 addressing the subject of this investigation, ODI is closing this Preliminary Evaluation. Honda determined the length of corrugated tubing inserted into the harness guide was insufficient to provide optimal protection for the wire harness. This lack of protection for RVC wire harness led to severe wire harness damage that ultimately led to the failure of the RVC image to display due to a lack of connectivity.On November 23, 2022, Honda filed a defect information report recalling all 2017-2019MY Honda Ridgeline vehicles. The dealer will replace the RVC tailgate harness with an improved wire harness that has longer corrugated tubing to protect the wire harness.In view of the recall action being taken by Honda, 22V-867, ODI is closing this Preliminary Evaluation. THE OFFICE OF DEFECTS INVESTIGATION (ODI) OPENED THIS INVESTIGATION BASED ON THREE VEHICLE OWNER QUESTIONNAIRE (VOQ) REPORTS ALLEGING A FIRE OCCURRED IN THE INSTRUMENT PANEL.IN ADDITION, THERE WERE THREE VOQ REPORTS ALLEGING A BURNED, MELTED, OR SHORTED HVAC FAN MOTOR WIRING HARNESS.HONDA REPORTS TH

AT DURING AUGUST AND SEPTEMBER, 2006, IT CONDUCTED A STUDY REGARDING MELTING OF THE HVAC FAN MOTOR HARNESS CONNECTOR.HONDA CONCLUDED THAT THE DAMAGE TO THE HARNESS CONNECTOR COULD RESULT FROM CONTACT BY A PASSENGER'S FOOT OR HANDLING OF THE HVAC UNIT DURING VEHICLE ASSEMBLY.HONDA INSTITUTED CHANGES TO ADDRESS THESE ISSUES.ALL COMPLAINT VEHICLES WERE MANUFACTURED BEFORE HONDA IMPLEMENTED THESE CHANGES.ODI'S ANALYSIS OF THE DATA (BOTH ODI AND HONDA) SHOW THERE ARE SIX REPORTS (DUPLICATES REMOVED) ALLEGING A FIRE IN OR NEAR THE INSTRUMENT PANEL.FOUR OF THOSE RESULTED FROM AN UNKNOWN CAUSE, ONE RESULTED FROM AN HVAC FAN MOTOR FAILURE, AND ONE IS REPORTED TO HAVE BEEN ARSON. IN ADDITION, THERE WERE NINE REPORTS OF HVAC FAN MOTOR HARNESS FAILURE THAT RESULTED IN SMOKE IN THE PASSENGER COMPARTMENT BUT NO VEHICLE FIRE.THE REMAINING FIVE REPORTS RESULTED IN SMOKE IN THE PASSENGER COMPARTMENT AND INCLUDED FAILURES OF THE AIR CONDITIONING COMPRESSOR, HVAC FAN MOTOR, AND A POWER TRANSISTOR.THIS INVESTIGATION DID NOT FIND THAT FAILURE OF THE HVAC FAN MOTOR CONNECTOR, WHICH CAN RESULT IN SMOKE, WOULD ALSO LEAD TO A FIRE.SIMILARILY NO DEFECT TREND WAS IEDNTIFIED FROM EXAMINATION OF THE SIX REPORTED FIRES.A SAFETY RELATED DEFECT HAS NOT BEEN IDENTIFIED AT THIS TIME AND FURTHER USE OF AGENCY RESOURCES DOES NOT APPEAR TO BE WARRANTED.ACCORDINGLY THIS INVESTIGATION IS CLOSED.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT A SAFETY-RELATED DEFECT DOES NOT EXIST.THE AGENCY WILL TAKE FURTHER ACTION IF WARRANTED BY THE CIRCUMSTANCES.Read more
