Incident date: Sep 4, 2026
On September 4, 2026, I experienced an extremely serious and frightening braking failure while driving my 2020 Chevrolet Colorado 2.5L. I was driving my granddaughter to school and was pulling into the school's carpool/drop-off area at approximately 20 mph. This was an especially dangerous location
... because children, parents, vehicles, and school personnel were in the immediate area. As I attempted to slow and stop the vehicle, the brake pedal suddenly became extremely hard and the vehicle would not stop normally. The dashboard displayed “Service Brake Assist,” and the check-engine light illuminated. I pressed the brake pedal, but the vehicle continued moving. Realizing that the normal brakes were not stopping the truck, I engaged the emergency/parking brake. The emergency brake slowed the vehicle somewhat, but even then the vehicle did not come to a complete stop. I ultimately had to place the vehicle in PARK to get it stopped. Fortunately, I was traveling at a low speed and was able to prevent the vehicle from striking a child, pedestrian, another vehicle, or other property. My granddaughter was also inside the vehicle at the time. This was an extremely frightening situation that could very easily have resulted in catastrophic injuries or death. The fact that this occurred in a school carpool/drop-off area surrounded by children makes the seriousness of this failure especially concerning. There was no reasonable warning that I was about to lose normal braking assistance. I am reporting this incident because I believe this vehicle may have experienced a serious safety-related defect involving the braking/brake-assist system. I am requesting that this incident be documented and that NHTSA investigate whether other owners of 2020 Chevrolet Colorado vehicles, particularly those equipped with the 2.5L four-cylinder engine, have experienced similar sudden losses of brake assist or braking ability.Read more

While driving my 2020 Chevrolet Colorado through [XXX], the truck began having difficulty accelerating. As I exited the highway and turned into a gas station, the truck suddenly lost power. The steering became extremely difficult, and I lost normal brake assist. The brake pedal became extremely hard
... , and I had to apply extreme force to stop the truck and avoid crashing into a wall at the gas station. Fortunately, no collision occurred. The truck was towed to Ed Corley Ford in [XXX] because I was stranded in a remote area with very limited options. They diagnosed a failed/broken vacuum pump and told me replacing it should make the truck operable so I could drive it home to [XXX]. I authorized the repair. After replacing the vacuum pump, they told me they started the truck and heard an abnormal sound. They shut it off, and when they tried to restart it, the truck would not start. They wanted to perform additional diagnosis, but I declined and had the truck transported back to [XXX] for evaluation by a Chevrolet dealership. Huffines Chevrolet Lewisville inspected the truck. Their technician removed the camshaft position sensor and used a borescope to inspect the camshaft. They documented that the camshaft is damaged at the end where it connects to the vacuum pump and stated the actuator pin was most likely bent due to the vacuum-pump failure, damaging the camshaft and causing it to be slightly out of time. I have also been informed of engine damage. The truck remains inoperable and is currently available for inspection. I am concerned this is a safety defect because the vacuum-pump failure resulted in a sudden loss of normal brake assist while I was driving and was followed by camshaft/engine damage. I am asking NHTSA to investigate this failure and similar Chevrolet Colorado complaints. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I own a 2018 Honda Odyssey 3.5L V6 and while my wife and 9 year old daughter were driving down the highway at 70mph that car decided to throw a rod throw the bottom of the motor leaving my family stranded on the roadside with no motor oil left in the car. We have always replaced the oil at 3,500 miles with Mobile 1 syntenic 0w-20 and always check the oil level to make sure none is missing. For the car to just throw a rod at 157k miles is pretty concerning as this will require a entire engine replacement out of pocket cost as Honda will not take ownership of this even though this year and model is known to have issues. I would like to see more VIN numbers added to their recall because this isn't fair to the people who buy a car for reliability and are left stranded on the roadside for a known issue they are not being forced to stand behind and make right. I have the video BUT I AM UNSURE how to upload or share Incident Description: The vehicle is experiencing intermittent, complete engine stalls while in motion at highway speeds. The failure has occurred four times. Initial Symptom: The issue first manifested as "stuttering" and erratic behavior fro
... m the Advanced Driver Assistance Systems (ADAS) which I thought it was. Escalation: The vehicle has since progressed to a total loss of power. During these incidents, I was forced to pull over on high-speed corridors (Hwy 46 and Hwy 5) to restart the vehicle. Recurring Pattern: Most recently, after a stall and restart, the vehicle stalled again within a few blocks. I have captured video evidence of the stall occurring immediately after the vehicle was "reset" and put back into motion. Safety Concern: This is a life-threatening defect. The vehicle loses propulsion without warning in high-speed traffic. The manufacturer’s service department (Hyundai) has been unable to identify a diagnostic fault code, citing "No Problem Found," and has only attempted a software/values reset, which has failed to correct the physical stalling.Read more

Upon engine start up at the home of my S/O, [XX], the warning "Service Automatic Throttle Control" appeared on the dashboard. Placing the vehicle into gear first in reverse, then in drive resulted in sudden acceleration and erratic lurching. I had no brakes whatsoever. At one point my S/O nearly hit
... on the dash per signed affidavit. I had to place my 2024 Jeep Compass in Park in order to stop and then slowly maneuver it into a safe parking spot before having it towed to Golling Jeep Ram Dodge in Bloomfield Hills, MI. Tech diagnosis two days later showed Error Code 2299. But the vehicle started and operated as normal. Nothing wrong. After a test drive the next day to be sure, they had me get my vehicle. Although my door sticker gives a manufacturing date of 6/2024, I call your attention to NHTSA Recall 24V-240 over throttle control system software. Strangely enough, this exact same malfunction occurred at the same address on [XXX], almost a year earlier. It was towed at that time to Suburban JDR in Farmington, MI where, again, my vehicle showed the same 2299 Code but otherwise, performing well, advising me to come get my Compass. So, both times my 2024 Jeep Compass took control away from me, this time with no brakes, almost injuring my S/O. Fortunately, both incidents happened in her gated community with no pedestrians or traffic. In addition to this intermittent, dangerous malfunction, I owned both a 2018 and a 2021 Compass one of which, like the Dodge Ram recall, had a horribly malfunctioning turn signal. I am writing to express my profound concern and disappointment regarding a recent, terrifying incident involving the brake system of my 2014 Toyota Prius. On Saturday, June 21, 2025, while driving with other passengers, my vehicle experienced a complete and sudden brake failure. Multiple warning lig
... hts illuminated on the dashboard, and despite my efforts, the brakes became unresponsive, making it impossible to stop the car. The sheer terror and helplessness I felt in that moment, with the very real potential for a catastrophic accident involving myself, my passengers, and other road users, is indescribable. Fortunately, through sheer luck and quick thinking, I was able to make an emergency stop on the side of the road, preventing any collision or injury. However, the emotional toll of that experience, the lingering stress, and the fear of a recurrence remain significant. I was subsequently informed that the cause of the failure was a leak in the brake accumulator. I am attaching the diagnosis and repair invoice. The necessary repairs were not completed until Wednesday, June 25th. This unforeseen and critical malfunction also necessitated the expense of a rental car for four days, adding to the financial burden and inconvenience. My frustration is compounded by the fact that this brake accumulator issue is a well-documented and known defect in Toyota Prius models, specifically affecting 2010-2015 vehicles. Toyota itself has previously issued a Technical Service Bulletin (TSB T-SB-0043-13) acknowledging this problem and offering warranty extensions for affected vehicles, typically up to 10 years or 150,000 miles. Despite this being a recognized safety concern, I was informed that my vehicle falls approximately eight months outside the 10-year warranty period, thus disqualifying me.Read more

On January 22, 2025 on a clear sunny day (no wind) I was putting groceries in the trunk of my car when the trunk lid came down on my head injuring me. I was leaning from the right side of my car across to the left side. My key was in my pocketbook on my right shoulder. Volvos should be "Mom" cars
... where one should be safe putting groceries in the trunk. Instead of popping back open with an obstruction, it continued to press down on my head before eventually popping back open. According to Volvo chat boards, this has happened to others. On January 6 my car had its two year servicing at the dealer on my leased vehicle. The service department's initial reaction to this malfunction was to BLAME me for having the key too close to the kick open sensor and to leave the key in the car while loading groceries. When I got the car, we were told (and it was demonstrated) that the kick open feature was on the left side of the trunk under the first "V". It appears that there was an update which electrified the entire back of the trunk without any notice to owners. Even so, I am 5 feet 2 1/2 inches tall. It is extremely difficult for me to kick open the trunk which is why I always open it from the button in the middle of the trunk and close it with the controls on the top right of the trunk lid. Buried in the owners' manual is a statement that you can kick the trunk lid closed, but no one ever told us that or that it can be kicked closed from the right side of the trunk. I did not kick the sensor to activate the trunk lid closing on my head. Only after a second call to the service department did the dealer agree to "look at" my car, but that they were booked up for the next two weeks.Read more

The gauge cluster/infotainment screen dims automatically when the light sensor on the dash senses the environment has gotten darker. The problem is that this happens extremely early at dusk/dark hours when it's still fairly bright outside, and the brightness of the screen is too dim to see things c
... learly. Also, once this automatic dimming happens, there is no way to turn the brightness back up. This feature also CANNOT be disabled in the settings anywhere. This results in a safety issue where the screen is not sufficiently visible at times when the auto dimming (which is also tied to the automatic headlights) kicks in for the screen.Read more

ABS control unit failure. Part number is 68295368AA. Dealer cannot fix problem due to no part being available. I've researched this issue online and found that they've issued 2 additional part numbers to replace the unit with, both are on backorder with the manufacturer. I managed to search onli
... ne for all 3 part numbers and was able to locate one of them, the latest version actually and have ordered it. Here's the main issue. This is a well known and wide spread problem with the 2017-2018 Ram 1500 trucks, also why parts are not available at the dealer. When the ABS control unit fails, the ABS, traction control, trailer brake assist, and cruise control are INOP. This is a major safety issue while driving the vehicle and problematic when you can't easily get the problem repaired. I am out of warranty coverage as are most who have the same problem. I feel FCA should step up and do a Recall of these trucks and fix with a quality part before someone gets killed.Read more

The infotainment system on my 2019 Subaru Outback has failed. The problem arose suddenly. The system is randomly cycling through radio stations, the touch screen has stopped responding to being touched, the gps apps stopped working properly and across the board the unit no longer works. The touchs
... creen also appears to have bubbling under the surface of the screen, which may be contributing to the problem. This failure has deprived me of access to important apps -- gps, vehicle maintenance records, bluetooth phone calls. The constant beeping and cycling is an unwanted distraction. For now, the rear back up camera still seems to be working, but if I'm forced to turn off the touch screen, it obviously won't be of any help. I am bringing the car to the dealership tomorrow.Read more
