I was operating my Tesla using Full Self-Driving (Supervised) from my home to a coffee shop. The vehicle completed the entire drive under FSD without any manual intervention. After arriving at the destination, FSD automatically entered the parking lot, selected an available parking space, and initi
... ated the parking maneuver on its own. I did not manually select the parking space or start the parking maneuver. During the automatic parking maneuver, the vehicle failed to detect a fixed square concrete bollard located within the parking space and continued moving until the front bumper made contact with the bollard, causing damage to the vehicle. No collision warning, parking collision warning, or automatic emergency braking occurred before impact. The vehicle remained under FSD control until the collision. The incident occurred in daylight with good visibility. The obstacle was a permanent concrete bollard, not a temporary object or a low parking curb. Dashcam footage of the incident has been preserved and has been provided to Tesla Insurance. I believe Tesla should review the vehicle telemetry and FSD logs to determine why the system failed to recognize the fixed obstacle during the automated parking maneuver.Read more

Paint bubbling and aluminum body panel corrosion on hood and front doors of my 2021 Jeep Wrangler Unlimited (VIN [XXX] ), currently at 63,000 miles. This is a well-documented manufacturing defect affecting 2018-2021 JL Wrangler and 2020-2021 JT Gladiator vehicles with aluminum body panels. Stellanti
... s has issued multiple Technical Service Bulletins addressing this exact condition: TSB 31-001-18 (March 2018), TSB 31-001-19 (July 2019), TSB 31-002-20 (October 2020), TSB 31-002-22 (December 2022), and most recently TSB 31-001-26 Revision A (February 2026), all titled "Aluminum Body Panel Corrosion Repair." The corrosion presents as paint bubbling along the leading edge of the hood and on the door panels. Filiform corrosion develops under the paint over many months before becoming visible. My corrosion warranty expired in January 2026, just one month before the latest TSB was issued. I am reporting this defect for federal documentation. The manufacturer has been aware of this defect for over 8 years (since 2018) and continues to issue revised TSBs rather than implementing a recall. A class action lawsuit ([XXX] et al. v. FCA US LLC) has been filed regarding this same defect. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I am submitting a safety complaint regarding my 2016 BMW 435i. My vehicle experienced a sudden water pump failure while I was driving on the freeway. Without warning, the vehicle immediately entered “limp mode,” causing a sudden and dangerous loss of power in active traffic. This created a serio
... us safety hazard because surrounding vehicles had to quickly avoid my car to prevent a collision. I am aware that there is an existing recall and/or known issue involving electric water pump failures on certain BMW vehicles, including similar 2016 BMW models, but my VIN does not qualify for recall coverage despite experiencing the exact same failure and safety-related symptoms. I believe this defect should be investigated further because it creates a substantial risk to driver and public safety when the vehicle suddenly loses power at highway speeds. Additionally, my vehicle has an ongoing issue with the parking collision warning/driver assistance system. The system will randomly trigger collision warnings and emergency alerts even when I am driving safely with no obstacles or vehicles in front of me. These false warnings are distracting and could potentially cause unsafe driver reactions or unnecessary braking events. I request that NHTSA investigate both issues: 1. Sudden electric water pump failure causing immediate limp mode and loss of power while driving. 2. Malfunctioning collision warning system producing false collision alerts without hazards present. These issues present significant safety concerns and appear consistent with known defects affecting similar BMW vehicles. Received a recall letter that stated the engine in my 2023 escalade with roughly 49,000 miles on it at the time. Has problems with the engine breaking down and shutting the entire vehicle off. Took the car to the dealer to fix per the recall letter. They stated they only needed to replace an oil cap
... and put better oil in the car, and not replace the engine as stated in the recall letter. About two weeks later the transmission started to slip and the car would not go over 50 MPH, in addition it would switch automatically into 4wd and get stuck in 4wd. Took the car back to the dealership they did not replace transmission they stated it may have something to do with the recall or an electrical issue. About 8,000 miles later I was driving on the highway taking kids to school and the entire vehicle just shut down completely, I could not brake, steer, control the car in anyway possible. I was riding what felt like a death trap. I was so scared for my family and myself, the car came to a stop on its own pretty drastically. I thought I was going to get slammed into or hit the car in front of me. Luckily I was not hit. The car prompted to push the start button, I tried to start the car, nothing. There was still music playing but the screens were all black, also the engine did not turn over at all. I could not put on my hazards either. I called a tow truck while sitting there and got the car towed to the dealership. The dealership took 3 days to get back to me and stated it was the engine and they are going to have to replace it with a new engine. In addition they would not provide me with a time frame at all, or a temporary vehicle to take my kids to school in. I feel this should be illegal with what has happened to me, I got regular oil changes and maintenance done. If they are still using this engine someone will end up dying from this poorly engineered engine and car. Cadillac once had a great reputation, I regret putting my kids and my life into this automobile.Read more

My car is very hesitant while driving, it pulsates as well. I can be driving at 65mph and next thing I’m doing 45-50 and the engine is sputtering and the dashboard lights up with check engine lights.I know there’s a recall on the fuel injectors for the model year I own but mine is not included i

I am reporting a safety defect on my 2013 Kia Sorento (VIN: [XXX] ). Before the failure, the vehicle began experiencing sluggish starting, sputtering during acceleration, stalling when stopped at traffic lights or stop signs, a noticeable fuel odor, and eventually a check engine light. These sympto
... ms increased over several days, and once the check engine light illuminated, we had the vehicle towed to a Kia dealership for diagnosis. At the dealership, the problem was confirmed and reproduced by their technicians. They diagnosed: •Fuel leakage from the high-pressure fuel pressure regulator •Fuel contamination in the engine oil These components and diagnostic findings are available for inspection upon request. This issue poses a significant safety risk. Fuel leakage and fuel-in-oil contamination increase the potential for an engine fire, and the hesitation, sputtering, and stalling at intersections present a risk of unexpected loss of power in traffic, which could result in a collision. The vehicle previously had its engine replaced under a Kia recall in 2020, and the current symptoms are consistent with known Kia engine and fuel system defects addressed in past recalls and technical service bulletins. The vehicle has been inspected only by the dealership so far; it has not yet been inspected by the manufacturer, police, or insurance representatives. Kia Consumer Affairs has been contacted and is reviewing the case. I am requesting that NHTSA review this issue as a potential defect trend involving the high-pressure fuel system on this model, especially in vehicles with prior recall-related engine replacements. Documentation from the dealership, including the diagnostic report and repair estimate, is available upon request. ... hen I went to turn a corner suddenly, my car started sputtering, knocking sound and failed to accelerate having traffic behind me not to cause an accident I put my hazard flashers on, and I lean over to the right on grass. Puzzled and scared saying to myself “ What the Hecks” what’s going on. The car parked on the side was having hard time staying on and shut down. I then went and opened the hood to check to make sure nothing was in my engine causing it, everything looked normal except, the smell of oil has a burnt smell to it which means in technical terms it has thrown a rod and bent valves and smell of burned electric. Finally, we called AAA, they advised their busy and it will be about 2 hrs. So, I sat there to wait for AAA. As I sat in my car waiting, I started researching online. I currently do not have any recalls or ever got any notice. I came across there is an active class action lawsuit going on for the rod bearing going out in these models and years. After some time, I called AAA back to get an ETA, they told us they were extremely busy, it would be about another 45 mins. At that time another tow company pulled up and asked if we needed a tow, we didn’t want to wait longer for AAA to get home so decided to have it towed home … totally cost $175. Today I am trying to contact Hyundai to see what we going to do regarding my car and praying it can get fixed on this recall that I had no idea and thanks God, I turned it off my car could start a fire & burned up. I never got notice about recalls for my engine and It’s sad Hyundai does contact owners ahead to let them know about this problem, think about it we are talking about lives. It appears that either the rear differential is locked up, or the traction control system has decided to lock all tires for the same rotation. When making any turns at slow speed, the tires grind and pop on the road. If turning in grass it will rip up the grass where the turn was made. In addition
... , the RPMs and vehicle lurch at normal cruising speed and when accelerating. I understand this as an issue with the torque converter. When the tires locked up, it put excessive force e on this component. It was stated in a similar TSB with the exact issue. There was a lack of fluid in the rear differential when it left the factory. Determination on repair was based on an inspection of the differential fluid and its levels. The cascade of failures happened after a road trip from FL to NY, I believe the miles were around 45,000. Upon arrival, NY roads were covered in snow for the next 5 months of travel, masking the severity of a locked drive train. No recalls were out, we thought maybe we needed tires. Now at 66k miles, there is a strain on the entire drive train and tires, whenever turning, accepting, I would further speculate, that this has put excessive wear on the CV/half shafts, wheel bearings, and related components. Although there’s a system failure, No codes have been thrown. Vehicle Speed Sensors and system, and or the ABS system should have noted some kind of error. While numerous complaints have arose accross varying ford models, no recalls issued. Ford may honor some repair costs. The location and nature of the initial failing unit, the rear differential, as its not part of routine maintenance, Would lead to an increase accross all related components and systems, which would lead to vehicle failure. Possibly causing loss of life or serous physical harm.Read more

Several safety-critical systems on my vehicle are failing, and the symptoms match existing recalls shown on Kelley Blue Book for my VIN. However, the manufacturer (Dodge/Stellantis) shows only 4 recalls, all marked completed — despite ongoing issues. 1. ABS / Brake System: The ABS warning light i
... s on continuously, indicating a malfunction in the ABS control module. This disables anti-lock braking, increasing the risk of skidding or loss of control during emergency stops. KBB shows a recall for this issue, but Dodge says it’s either not open for my VIN or already completed. The ABS light has remained on since 2. Front Wheel Bearing: There is a grinding noise and vibration from the front wheel area, suggesting a worn or failing wheel bearing. This poses a risk of steering loss or wheel detachment. No warning lights have appeared for this issue. 3. Transfer Case / 4WD: The 4WD system intermittently fails to engage or disengage, sometimes with clunking noises or jerking. This can cause unpredictable handling or loss of traction. Issue is still ongoing. 4. Electrical System: Intermittent electrical issues include random warning messages, dimming lights, and occasional no-starts. These affect essential safety systems and match known electrical recalls. These issues have not been confirmed by a dealer but are ongoing and present serious SAFETY risks. I believe my vehicle may have been incorrectly excluded from applicable recalls or that prior recall work was not properly completed. These components are available for inspection.Read more

I am a [XXX] physician who has been driving for about 30 years. Purchased this vehicle as a CPO with VIN as identified above. It is a 2024 BMW 530i X-drive from Faulkner BMW in Lancaster, PA. It was delivered on July 25, 2024. I have only put approximately 200 miles on in it since delivery. On Th
... ursday, [XXX] there was catastrophic failure of the vehicle. At around [XXX] I was driving North on [XXX] about to make a right turn onto [XXX] for work at the Hospital System where I am employed. The traffic light was red and I was going about 5 miles an hour and attempted to brake the car. There was an 18 wheeler truck about 10 to 15 feet ahead of me. My car instead of braking underwent unintended acceleration. Despite my efforts to slam on the brake, the car was accelerating. None of the safety features including automatic emergency breaking were activated. Unfortunately I collided with the 18 wheeler. This was fortunate in some regards as I could have potentially struck a pedestrian or a motorcycle and have had a potentially fatal event. Please note that additionally my airbag did not deploy. The BMW dealership was contacted. They dropped off a loaner vehicle and picked up my vehicle in a flat bed for investigation. I was informed to contact BMW of North America which I did. Interestingly enough, there were no safety recalls on this vehicle on [XXX]. The car is at the BMW dealership in Lancaster PA for their internal inspection. I am being informed that the engineers are looking at the vehicle to find out what transpired. Interestingly enough, I did receive a phone call from the service manager advising me on August 9 that there was a safety recall as a relates to the brakes. As I review the NHTSA site, there is now a recall on my actual vehicle's braking system. I did a review of unintended acceleration and found multiple complaints. BMW is looking into this although into this, but external agency would be ideal. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C.Read more

I slowed down to turn into my daughter's driveway and park in front of their boat (maybe 5 mph if that). Suddenly, the vehicle accelerated at a high rate of speed on its own. I hit the brakes with no response. I had to veer to the left of the driveway onto the grass to avoid hitting the boat and gar
... age. The car continued to accelerate (approximately 60-70+ mph) as I tried pumping the break pedal several times with no response (went to floor board with no resistance). I had to maneuver a narrow area at a high rate of speed to avoid hitting the house to my right and trees to my left. As I came down the hill, I approached a large embankment of a knocked over tree, dirt and brush pile. I knew I had to do something or it would be a serious impact. I quickly swerved to my right. I didn't know there was a ditch to the right of this embankment, but thankfully it brought my vehicle to a stop as my front left tire became flat and stuck in the dirt This was very shocking and traumatizing. We verified there was nothing on the floor that would have caused the gas to be accelerated. My car is currently on 3 wheels where it stopped (1 wheel suspended in the air). I received medical care at the ER for back pain. A CT scan showed I had a T12 Vertebrae compression fracture. I was prescribed a back brace, pain medication and an anti-inflammatory. I have also been alternating with a heat pad and ice pack, plus using a cane to help me get around. I will be contacting Toyota as well for an investigation into the problem, but I am scared to drive this vehicle again if if they do check it over. A police report was started on 14 Jul with the Lafayette County Sheriff's Department, Oxford, MS, but it hasn't been completed. The report number is 24-003363. It wouldn't let me proceed selecting a report was filed and no report attached, so I chose no and provided report number.Read more
