Filing regarding NHTSA Recalls 25V-636 and 26V-056. My 2021 BMW X3 M40i (VIN: [XXX] , built September 2020) contains the recall-affected starter part number ending in 997 — confirmed by a BMW Level 1 certified technician — not the non-recall part number ending in 505. BMW denied coverage based solely on manufacture date, with no consideration of the actual components installed. My vehicle dis
... plays an active fire risk. On April 30, 2026, my dashboard displayed Engine cannot be started, Starter system fault. Call Roadside Assistance, and Repeat start attempt — all photographed and timestamped. Fault codes 216119, 35AE00, and 8040A9 were confirmed by diagnostic scan. BMW of Catonsville (Work Order XXX) attributed the fault to the battery and charged me $280.50. I replaced the battery with a BMW OEM unit on May 13, 2026 at a cost of $590.31. On May 21, 2026, the identical starter fault returned — conclusively disproving the battery diagnosis and confirming the starter as the root cause. BMW denied recall coverage by phone through BMW of Catonsville. No written denial was provided. I am requesting NHTSA investigate whether the recall scope incorrectly excludes vehicles built with the recall-affected starter component, compel BMW to expand coverage to include my VIN, replace my starter at no cost, and reimburse my out-of-pocket costs of $870.81. I am filing a complaint regarding a recurring safety-related electrical and AWD defect in my 2020 Ford Explorer. My vehicle repeatedly displays the following warnings while driving: “4WD Fault Service Required” “Service AdvanceTrac” “Pre-Collision Assist Not Available” “Hill Start Assist Not Available” The issue began about a year ago occurring intermittently every few weeks and
... disappearing after restarting the vehicle. It has progressively worsened and now appears almost immediately after driving only a few feet. The defect affects multiple critical safety systems including AWD/4WD, traction/stability control, hill assist, and collision avoidance systems. I have contacted Ford multiple times. Ford only opens customer service cases but has not provided a permanent repair. Ford is aware this is a common issue with 2020 Explorers specifically, and later model years appear to have been corrected. Numerous owners report the exact same failures online. Ford has issued service information involving software reprogramming, module communication faults, and wiring harness issues, but only Ford dealerships can perform the required programming. This appears to be a widespread manufacturing or engineering defect and creates a serious safety concern while driving. Vehicle Information: 2020 Ford Explorer Mileage: Under 55,000 miles I respectfully request that NHTSA investigate this issue for a potential recall or mandatory corrective action.Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? The Lane Keep Assist (LKA) system on my 2023 Range Rover Velar P250 has lost its active steering intervention capability following one or more Software Over The Air (SOTA) updates. The system no longer applies corrective steering torque to keep the vehicle within lane markings. The vehicle is availa
... ble for inspection upon request. How was your safety or the safety of others put at risk? Lane Keep Assist was a functional active safety system at the time of purchase, applying steering corrections to prevent unintended lane departure. That active intervention has been removed or disabled through a software update without my knowledge or consent. The vehicle now drifts across lane lines at highway speeds with no corrective action from the system. This increases the risk of a lane departure collision, particularly during momentary driver inattention — the exact scenario this system is designed to protect against. I was not notified that this safety capability would be altered or removed. Has the problem been reproduced or confirmed by a dealer or independent service center? I brought the vehicle to my authorized Land Rover dealership under warranty. The dealer acknowledged the system behavior but told me it was "working as intended" under the current software version. They did not reproduce the original functionality or offer a remedy. The dealer did not dispute that the active steering correction is absent — they simply classified the current behavior as acceptable. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle was inspected by my authorized Land Rover dealership, which is a manufacturer-affiliated service center. No independent inspection has taken place. I have also submitted a formal written complaint directly to Jaguar Land Rover customer relations at [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5Read more

On XXX, I was driving my 2025 Cadillac Escalade (6.2L engine) on [XXX] , at 55 mph in a construction zone. The right lane had a cement barrier one foot from the yellow line, eliminating any shoulder, and the left side had only a narrow berm before the median. Without any prior warning, I heard a loud “pop” and felt a jolt. Immediately, the check engine light illuminated, the vehicle went silen
... t, and it began to coast. A message appeared instructing me to press “start” to start the vehicle. The battery remained operational, but the engine was unresponsive. I had not struck any object. I activated my hazard lights and, because I could not pull to the right due to the barrier, I coasted across lanes into the median. I could not exit the vehicle to safety due to location of the vehicle and the construction on both the north and southbound lanes of the highway as well as concrete barriers. The vehicle would not restart and could not be shifted into neutral, requiring specialized towing. Police responded and positioned a patrol car behind me due to the dangerous location. I could not safely exit the vehicle. The tow truck had to block the left lane to remove the vehicle. The vehicle was towed to Medina Auto Mall. The service technician advised this is “more likely than not” the same 6.2L engine failure subject to recall in 2021–2024 GM vehicles. However, the vehicle cannot be diagnosed for 3–4 weeks due to backlog. If confirmed, an engine replacement may take several months. The vehicle has approximately 11,500 miles, has run perfectly with no prior issues, and was properly maintained, including an oil change performed by the selling dealer, VanDevere Cadillac (Akron, Ohio). This sudden loss of propulsion on a highway in a construction zone created a serious safety risk, including the risk of a high-speed rear-end collision resulting in injuries to myself and other drivers. It was a frightening situation. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Problem 1, Check Engine Light: I purchased the used car on Jan 9 2025, around 23,400 miles from Kia Dealership, capitol Expy in San Jose. Today 05/27/2026, the odometer reads 26, 407 miles. I completed my 25K service after Thanksgiving in Nov 2025. I ha

Around Mar 15, 2026, the Check Engine Light displayed. I called the Kia Dealership on Capitol Expy in San Jose 3-4 times, but they Kia dealership don't call back. On 05/15/2026, for the first time I drove about 50 miles return trip. While driving North on I-280 from [XXX] to [XXX] , over rolling hills that had some incline, the Check Engine Light came on.I scheduled service. I experienced that the vehicle struggle to climb the incline of rolling hills, after the battery charge was exhausted and the engine was running only on gas. The gas was 3/4 full. This is a safety hazard for me to be interrupted by Check Engine Light twice while driving and that too when my car has not even completed 30K miles. My car is sin Service on 05/26/2026 and in the shop for 3 days diagnosis. The Kia Dealership has yet to determine the problem. I have had to rent my own rental car even though I purchased Extended Warranty on the car. Problem 2, Blind Spot Warning Light on right side mirror: On 05/13/2026 and on 05/22/2026, the Blind Spot warning light on right side mirror does not display oncoming traffic when my car is parallel to the lane markings. On 05/13/2026, I was driving at 30mph and on 05/22/2026 I was stationary in the center divider. In both instances, the passing driver had to honk at me really loud -- to avert an accident. While driving my 2025 Toyota 4Runner in rainy conditions, the vehicle’s stamped hood channels act as a reservoir, trapping and pooling large volumes of standing water. Upon braking or decelerating—even at low, around-town speeds—the pooled water surges forward off the front nose of the hood onto the windshield. The vehicle's forward aerodynamics catch this massive sheet of water and instantl
... y launch it backward across the entire windshield, causing total and sudden loss of forward visibility. This creates an unpredictable, immediate crash hazard that catches the driver completely off guard, as the windshield wipers cannot clear the sudden volume of water fast enough to maintain a safe view of the road. This occurs around town driving (25mph to 45mph) and highway speeds (60mph - 70mph). I understand you can turn the wipers on high, but it catches you off guard when it happens.Read more

Corporate Honda subsequently issued a verbal agreement to fully reimburse my out-of-pocket teardown fees. A professionally manufactured and dealer-installed engine is reasonably expected to last well beyond 40,000 miles. The manufacturer network cannot utilize arbitrary part-ownership rules or uncooperative, fragmented dealer communications. The original factory-installed engine and the dealer-installed replacement engine both suffered identical connecting rod bearing failures.Read more

Gunn Acura (San Antonio, TX) confirmed with my vin, that Engine #3 was a warranty/dealer-installed replacement unit at 22,000 miles. It failed after less than 40,000 miles of total operation.

I am formally requesting a neutral third-party arbitration review under the Magnuson-Moss Warranty Act to compel American Honda Motor Co. to provide a permanent mechanical remedy—specifically, the sourcing and installation of a brand-new, redesigned engine assembly (Engine #Four)— This vehicle frame is now on its third engine block. Car was bought 11/30/2024. At Grubbs Acura, fuel pump safety
... recall that involves flashing/re-calibrating the Engine Control Unit (FI-ECU) software was done. Just a few weeks after new fuel pump, the vehicle displayed sudden stalling symptoms. The engine suffered a terminal, catastrophic internal collapse. The vehicle was towed to Grubbs Acura where Corporate Honda directed a forced out-of-pocket internal engine teardown. The physical teardown explicitly confirmed a catastrophic connecting rod bearing failure (spun bearing) identical to the structural manufacturing flaws currently under active federal investigation by the NHTSA under Preliminary Evaluation PE25008 / RQ24-025. Corporate Honda subsequently issued a verbal agreement to fully reimburse my out-of-pocket teardown fees. Thankfully no one was next to me but if that had not been the case, it would have a caused a serious and life threatening acc

It was EXTREMELY scary, especially as I am a mother and my baby is frequently in the car. I have contacted two separate Toyota dealerships neither one of them giving any concern to my safety or well-being and are charging me $100+ to run a diagnostics. I no longer feel safe driving this vehicle, I no longer believe that Toyota cares about their clients and will no longer be recommending Toyota for their lack of concern of their drivers safety. There were no warning messages or notifications before, during, or after the incident.Read more

VEHICLE: 2019 Honda Accord LX SUMMARY OF SAFETY DEFECTS & FAILURE: My vehicle is experiencing severe, dangerous mechanical and electrical failures identical to existing national safety recalls, yet my specific VIN is being excluded by the manufacturer. 1. FUEL SYSTEM FAILURE & ENGINE STALLING: The vehicle is experiencing sudden engine stalling and failure to start due to a defective low-pressur
... e fuel pump. This matches the exact failure profile of NHTSA Recall Numbers 23V-858, 21V-215, and 20V-314 (Honda Campaigns KGC/KGD). This defect causes the vehicle to lose power unexpectedly, creating an immediate crash hazard on active roadways. 2. BODY CONTROL MODULE (BCM) DEFECT: The vehicle is suffering from critical electrical disruptions, resulting in erratic component failures including [mention any specific symptoms you have, e.g., malfunctioning windshield wipers, defrosoter failure, rear-view camera blacking out, or power window errors]. This perfectly mirrors the systemic programming defect outlined in NHTSA Recall Number 20V-771 (Honda Campaign X95). DEALER DIAGNOSIS & MANUFACTURER DENIAL: I have taken the vehicle to an authorized Honda dealership multiple times. - In June 2025, the BCM software issues were initially dismissed by the dealer as a general electrical problem. - A subsequent comprehensive diagnostic confirmed the fuel pump and BCM failure, resulting in a repair estimate of $4,688. The dealership refused to cover the repairs under the safety recalls, citing that my VIN is not currently flagging "open recalls" in their corporate database. When referred to American Honda Motor Co. Corporate Customer Support, corporate summarily dismissed my complaint and refused any cost assistance or repair remedy, despite their clear knowledge that these exact defects plague the 2019 Accord LX model year. Because the manufacturer is utilizing restrictive VIN range loopholes to evade financial accountability for known safety hazards, I am filing this complaintRead more

The vehicle experienced failure of the HVAC heater flap/blend door mechanism on the driver’s side. Specifically, the plastic gear assembly broke at its mounting point, causing the gear to spin freely and preventing the airflow door from opening and functioning properly. As a result, the driver’s side heating and airflow system no longer operated correctly. The failed component is available fo
... r inspection upon request. Because the manufacturer does not sell the small gear component separately, I was forced to obtain a custom 3D-printed replacement part in order to restore operation. This defect created a safety concern because the inability to properly control heat and airflow can affect windshield defrosting and cabin visibility during cold or wet weather conditions. Reduced defrost performance can impair driver visibility and increase the risk of an accident. The problem was confirmed during inspection and repair. Accessing the failed gear required extensive disassembly of the vehicle’s front dashboard area and HVAC system components. The repair process was extremely labor-intensive for what is essentially a small plastic gear failure. Prior to complete failure, the HVAC system began making abnormal clicking/grinding noises and airflow operation became inconsistent. Eventually, the gear mounting point completely failed, causing the actuator gear to spin freely without moving the heater flap. This appears to be a common problem affecting Nissan Rogue vehicles, particularly involving the weak plastic HVAC gear and mounting design. The manufacturer does not offer the individual failed gear as a replacement part, which unnecessarily increases repair complexity and cost.Read more
