This has occurred numerous times. Using the adaptive cruise control, the vehicle decelerates as I approach a slower moving vehicle. That is what it should do. When I move from the right lane to the open left lane, instead of accelerating back to the set cruise speed, the car decelerates as much as 10 mph before finally regaining its mind and accelerating to the set speed. I have had cars following

This car slams the breaks to a stop in the middle of an intersection while turning left with other cars across the intersection also turning left. It thinks we will hit but we are not going to. This has happened twice. Luckily, the driver behind us was paying attention both times and also slammed their brakes, avoiding a collision.

The vehicle's tonneau cover was defective from the day of delivery and remained unusable for approximately the first 12 months of ownership. Factory over-application of adhesive forced the rear window against the tonneau rollers, preventing operation. I purchased this vehicle specifically for its cargo bed functionality for business use. The cover was finally repaired at Tesla Lincoln, NE when a s

The vehicle has had an engine rattle at idle, oil consumption, and blow-by since early in ownership. The engine defect is progressive and predates the current mileage. The flashing check engine light and severe shaking while driving at highway speeds created an unsafe condition with other vehicles on the road. The problem has been confirmed by a Hyundai dealership. Hyundai Motor America was contacted and denied goodwill warranty assistance. The vehicle is available for inspection upon request.Read more

I am reporting a serious safety defect involving my 2021 Chevrolet vehicle. While driving, multiple warnings appeared including “Service Power Steering,” “Steering Assist is Reduced,” “Service ESC,” and “Service Battery Charging System.” The vehicle then lost steering assist, braking performance became unreliable, and it created a serious risk of crash. The vehicle later shut down
... completely and has remained inoperable. The failure appears electrical and affected multiple critical systems at once, suggesting a cascading system malfunction. Since the incident, the vehicle has not restarted or operated normally. I have replaced the battery, alternator, starter, ignition switch, brake vacuum pump, ECM, spark plugs, fuses, and relays, but the issue persists. The vehicle still shows charging system faults and severe battery drain, indicating an unresolved underlying electrical defect. I have found numerous reports from other 2021 Chevrolet owners describing nearly identical failures, including sudden loss of power steering assist, ESC warnings, charging system failures, battery drain, and complete vehicle shutdowns. This suggests a possible widespread defect affecting this model year. Given the loss of steering assist, braking irregularities, and full vehicle shutdown while driving, I request NHTSA investigate this issue for a potential safety-related defect and determine whether a recall or manufacturer action is necessary.Read more

My 2025 Cadillac Escalade ESV lost power and shut down while I was driving on the highway. The gas pedal stopped responding, and the vehicle would not restart afterward. This created a serious safety risk because the vehicle suddenly lost propulsion while driving in traffic. The vehicle is less than one year old and is already going to be on its third engine. The original engine was already repla

Pepe Cadillac confirmed that this is the second replacement engine, which means the third engine counting the original engine. The dealer also confirmed that the engine locked up due to internal bearing material failure and needs to be replaced again. The repair order number is 140134. I am concerned this is a serious safety-related engine defect. A brand-new GM replacement engine should not lock up after only about 22,000 miles, especially while driving on the highway. I am also concerned that simply replacing the engine again may not fully address the safety and reliability issue with this vehicle. Cadillac Customer Care case number: 100539041.Read more

My 2024 Hyundai Santa Fe (VIN: [XXX] ) was manufactured on March 14, 2024, in [XXX] . This creates a severe lag in acceleration, leaving the vehicle dangerously exposed to oncoming traffic. This compo

Because my manufacturing date falls just outside the official recall window, this vehicle has not received a formal recall remedy, yet it actively exhibits the identical, dangerous powertrain defects, suggesting the recall's production scope may be too narrow. Furthermore, in November 2024, the vehicle suffered a catastrophic safety failure where it completely lost all propulsion power for approximately 30 seconds while moving through the middle of a live intersection. Most recently, on XXX, right after being released from an extended 38-day service stay at an authorized dealership, the electronic infotainment displayed an active warning screen stating the car should be shifted to park while the vehicle shifter was physically and securely locked in Park. Time-stamped video evidence of this active transmission/braking sensor failure has been captured and preserved. The vehicle remains an active, uncorrected threat to my physical safety on the road. Approximately 8 months ago, I brought my car to the BMW dealership Keeler's in Latham, NY 12110 for a Recall 24V-608:474, the dealer held my car for a day. Two weeks later after taking my car in for the recall, my check engine light started going off and on, off and on, then it comes on and stayed on. On certain occasions I smelt burning wiring and antifreeze smell, yet I assumed that those partic
... ular smells come from outside through the car vents. The weather was in the fall season, so I never turned on the AC to make sure the cold air was working, the heat was working. It wasn't until the weather was getting warm that I try to use the Cold air in my car that I notice that there was nothing but hot air coming out and not the ice-cold air that I was getting before I took it to the BMW dealer in Latham. I connected a code reader to my car and the codes that come back were U019F and U0079 which stated that is was a lost communication with the Engin coolant pump. I call BMW and tell them about my complaints; they told me to bring in my car to get it look at. I brought the car to Keeler. The Service Advisor Daniel Purpura told me that my car has be reset and a shelter cap replace and the shop mechanic drove it, and now the check engine light is off. When I went to start the car the engine light was back on, and I told him that. He told me that the code they found was code 003B97. Hi [XXX] , Not sure if you are still working at NHTSA….. For what it’s worth, I finally diagnosed my ‘other’ 2015 Ducati with a failed spark plug wire (open circuit). This bike has about 5,000 miles, but has had the symptoms since about 1,000 miles (i.e., almost 10 years). So, Ducati is 2 for 2 in 2015 for bad spark plug wires. On ‘dual spark’ bikes (i.e., this one), having on
... e failed wire is not a catastrophe; but very hard to diagnose. For my other 2015 bike (emails below) with only one wire per cylinder, it can lead to erratic engine response and fire while riding the bike. Not much fun. I’m a bit surprised that NHTSA seems to dismiss this so readily. It is also pretty clear that Ducati knows of the issue, but has not bothered to provide owners with any information / warnings. Sad. If you needed them, I could send you some “crispy” parts that burned (actuator for the exhaust flapper valve) and the bad wire. Best, [XXX] From: [XXX] [XXX] > Sent: Friday, April 18, 2025 2:52 PM To: [XXX] (NHTSA)' [XXX] > Subject: RE: VOQ- 11647603 Hello again, Not sure if you are still interested in this motorcycle fire….. Turns out the culprit in this long ordeal was two spark plug wires slowly going bad. Unusual that a “Garage Queen” bike with 1,500 miles on it would have both plug wires go bad (open circuit) at almost the exact same time. Since the wires are a ‘dumb’ component, there were no ECU trouble codes or other indicators (except for the completely misleading ones) that identify this problem. With a partially ‘bad’ wire, a cylinder won’t spark, passing the gas directly into the hot exhaust….. where it burns, making the exhaust hot enough to ignite whatever might be nearby. “I authorize NHTSA to share my Vehicle Identification Number (VIN) and other personally identifiable information in my complaint record with the manufacturer,” INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Transmission went out while driving on highway, entering limp mode. Ended up needing a valve body replacement at only 20k miles which was covered, but now I am having more transmission issues still with delayed forward engagement and slipping gears and it is only at 65k miles. Seems to be a lot of customers having the same issue. Took months for the truck to be fixed as "many other trucks" were wa

2 unrepaired recalls. Frame rusted and unsafe to lift to repair the other recall which is an oil leak onto the alternator. Car won't run now. Corporate asked if I had a lawyer. They have declared my car not fixable or drivable. Neither corporate nor dealer will return my calls. No info from them at all. No offer of a buyback. No anything.
