The contact owns a 2025 Toyota Camry equipped with Bridgestone Puranza EL440tires (Tire Size: 235/45R/18). While driving approximately 75 MPH, the rear driver's side tire suddenly, the Tire Sensor illuminated. The contact was attempting to get to the nearest exit to pull over, but before being able to do so, the tire blew. When the failure occurred, it caused the rear bumper to detach from the veh
... icle. The contact moved to the emergency lane, where they noticed the tire had disintegrated. The tire was replaced with a spare tire and driven to their home. The contact stated their niece, who was a passenger in the vehicle at the time, had suffered a panic attack, at the time which caused their nose to bleed and necessitated medical attention. The next day, the vehicle was taken to the dealer, who confirmed it was a known issue with the tires and the tire was replaced. The bumper was reattached. The manufacturer was not informed. The failure mileage was approximately 29,000.Read more

Vehicle Information: 2024 Toyota Tundra 4x4, VIN: [XXX] livery: [XXX] Current Mileage: 24,147 Complaint Synopsis: My 2024 Toyota Tundra is subject to two active open safety recalls with no remedy available ("NO FIX YET" status per Toyota owner portal). 1. What component(s) or system failed or malfunctioned, and is it available for inspection upon request? Primary: Engine assembly (Recall 25TB14 /
... 25TA14) – machining debris left in the engine during manufacturing. This can cause engine knocking, rough running, no-start, and sudden loss of motive power. Secondary: Reverse lamp system (Recall 25TB06 / 25TA06) – moisture intrusion causing both reverse lamps to become inoperable. The vehicle remains available for immediate inspection at Clearwater Toyota or by Toyota representatives. 2. How was your safety or the safety of others put at risk? The engine defect creates a severe risk of sudden loss of motive power while driving at highway speeds, which can cause loss of vehicle control and increase the likelihood of a crash. The reverse lamp defect eliminates rear visibility and brake indication when backing up in low-light conditions, significantly increasing the risk of collisions with other vehicles or pedestrians. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? The defects are formally admitted by Toyota through active NHTSA safety recalls. Clearwater Toyota has refused multiple requests for inspection and service documentation. No independent inspection has been performed due to Toyota’s refusal to schedule one. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No. Toyota has not conducted a physical inspection of my vehicle despite repeated requests. Clearwater Toyota provided a temporary loaner vehicle but has refused to inspect or document the recalled conditions. No police or insurance inspections have occur INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Our '22 Audi Q5 experienced a spontaneous sunroof implosion while driving on the highway. Temperature would not have been a considering factor here, nor did anything hit the sunroof but it exploded and showered the occupants with glass - both pebble-sized pieces and small shards. The curtain was open at the time and glass went everywhere. Thankfully, the driver had the good sense to pull over i

This incident interrupted their trip, they had to find someone to tape a plastic cover over the gaping hole, then have the sunroof replaced, all at considerable expense and hassle. This was both a very dangerous AND frightening experience. Audi headquarters was contacted and denied any responsibility, saying that we need to handle this through our insurance company which I find outrageous as it has been recorded that quite a few Audis have experienced this same situation. It should be covered under their warranty!!Read more

My 2022 Hyundai Palisade recently blew the engine due to very little oil (6 quarts). My wife stated that she was driving when it started to act weird. Very shortly after that she heard the vehicle making a loud knocking / banging sound. She stopped the vehicle and turned it off in the nearest parking lot. We had the vehicle towed to our certified mechanic to have it looked at. 6 quarts of oil was
... put into the engine before it was full. Both him and my wife stated the vehicle never showed a check engine light, low oil light or oil pressure problems. He stated the vehicle had no leaks. He advised us that Hyundai/Kia was having problems burning oil. This is a high risk and safety concerns since there was no warning, happened suddenly and could cause problems for people to find a safe place to pull off the roadway and interstate. So many things can happen if someone didn't hear the noise or have lights indicating a problem, therefor if this happened on the interstate people could struggle getting to the shoulder when engine locks up. Also, a piston or connecting rod can be ejected from an engine risking the safety of pedestrians on foot or driving down the road.Read more

Ford's own recall notice stated parts were anticipated in the latter half of 2024 — it is now June 2026 and parts remain unavailable, representing an 18-month overrun on Ford's own published commitment. No warning was given prior to the trim becoming loose. No crash, fire or injury occurred. The passenger airbag sensor does not sense when I have a passenger in the vehicle. This was a recalled issue and was fixed. Vehicle: 2016 Subaru Outback Recall Number: WUJ95 – Front Exhaust Pipe Bellows Crack I incurred repair expenses to address a condition that was later covered by Subaru Recall WUJ95. At the time of the repair, I was unaware that the vehicle was subject to this recall because I never received any recall notification from Subaru. The affected component was the front exhaust pipe bellows assembly
... covered by Recall WUJ95. The repair was completed by an independent repair facility. The condition involved a cracked exhaust bellows assembly. The failure of this component allowed exhaust gases to escape from the exhaust system into the cabin and is the condition identified by Subaru in Recall WUJ95. The issue was diagnosed and repaired by an independent repair facility. The repair addressed the same component and condition that was subsequently identified by Subaru as part of Recall WUJ95. After learning of the recall, I contacted Subaru of America seeking reimbursement because the repair was completed after the recall had been issued. Subaru denied reimbursement and stated that repairs performed at third-party facilities are not eligible. During the reimbursement review process, I requested documentation showing when and how recall notification was sent to me, including the address used, mailing date, and method of notification. Subaru declined to provide this information. Subaru also declined to provide the specific guidelines relied upon in denying reimbursement. My concern is that I never received recall notification before obtaining the repair, which prevented me from having the recall performed through an authorized Subaru retailer. I am requesting that NHTSA review whether recall notification requirements were properly satisfied in this case and whether vehicle owners are being provided sufficient information to verify recall notification when reimbursement requests are denied.Read more

The vehicle has experienced repeated transmission and Electronic Oil Pump (EOP) failures resulting in loss of propulsion, loss of acceleration, and transmission-related malfunctions. The vehicle currently has approximately 69,000 miles. On September 3, 2022, the vehicle lost propulsion while traveling approximately 31 MPH in traffic and would not respond normally to accelerator input. This create
... d a dangerous situation and increased the risk of a collision. On the same date, while backing out of a parking space, the vehicle displayed a "Shifter System Malfunction" warning message and unexpectedly stopped while in reverse. The vehicle had to be powered off before it would operate again. Following these incidents, the vehicle was towed to a Hyundai dealership and the transmission was replaced in September 2022. Despite the transmission replacement, the vehicle continued experiencing issues involving the same system. The Electronic Oil Pump (EOP) was replaced in 2023 and again in 2024 due to recurring transmission-related concerns and diagnostic codes. In 2026, the check engine light returned and the vehicle was again diagnosed with a transmission-related failure. The dealership determined that the transmission requires replacement for a second time despite the previous transmission replacement and two separate EOP replacements. Internally, Hyundai has since revised its repair guidance for this condition, suggesting that prior repair procedures may not have fully addressed the underlying defect. The repeated failures of the transmission and EOP system despite multiple repairs suggest an underlying defect that has not been corrected. These failures create a significant safety risk because the vehicle can unexpectedly lose propulsion or fail to accelerate while operating in traffic. The vehicle is available for inspection upon request.Read more

The engine in my truck failed while driving down the interstate highway (I-10) on May 28, 2026. At the time of failure, a loud popping noise emitted from the engine. Power and speed were reduced as the result of the failure. The Check Engine Light and Service Engine lights illuminated as quickly as the failure occurred. As many vehicles were simultaneously traveling along the highway at speed,
... my vehicle suddenly slowing down without indication (brake lights) could have inadvertently caused an accident. As a result, I continued driving the vehicle under reduced power to my home which was only approximately 2 miles away. During the 2-mile drive to my home, the loud popping noise intermitted but reduced power was consistent. Diagnostic checks revealed a code P0305 indicating that cylinder #5 was misfiring. Checking the components of that cylinder revealed the intake valve of cylinder #5 was not actuating when the engine was running indicating a failed lifter & camshaft.Read more
