While driving, the engine began making a loud knocking/rattling noise and the check engine light began flashing. The vehicle experienced a loss of power and was unsafe to continue operating normally. The vehicle was taken to a Hyundai dealership for diagnosis. The engine requires replacement. Hyundai denied coverage, claiming excessive oil consumption and/or maintenance concerns. However, upon in
... spection, the dealership removed the valve cover and advised that the engine was internally clean and showed no significant sludge buildup consistent with severe neglect. I am concerned that this engine failure may be related to known Hyundai engine issues involving the 2.4L GDI engine. The failure occurred well before the expected service life of the engine and created a potential safety hazard due to the loss of power while driving. I am requesting that NHTSA review this failure and determine whether similar failures are occurring in 2020 Hyundai Tucson vehicles equipped with the 2.4L GDI engine.Read more

Passenger brake Led Circuit board prematurely failed resulting in the entire assembly needing replacement. The assembly is $500 and higher. This is a safety issue and though there was a class action lawsuit, the light fails after 72000 miles which appears to be when GMC wont replace the part. $500 for a tail light replacement is a financial burden to be able to keep the car on the road. There is n

Incident date: May 31, 2026
While driving, the vehicle experiences dangerous episodes of what is widely known as "sticky steering." Specifically, when initiating a sweeping left-hand turn, the steering wheel hits a point of sudden mechanical resistance, requiring an unexpected and sudden increase in steering effort to turn the wheel. When trying to maintain or exit the turn, this resistance causes the steering wheel to bind
... and pull to the right, requiring forceful manual over-correction to keep the vehicle in its lane and prevent a crash. This is an intermittent but recurring mechanical failure that completely disrupts smooth steering operation. It matches the exact profile of the ongoing NHTSA Recall Query into the failure of Mazda's original Recall 24V022 (Campaign 6524A) remedy.Read more

On [XXX], my 2015 BMW 328i xDrive (VIN: [XXX] , N20 engine, 124,250 miles) suffered a catastrophic engine failure while in operation. Component/System Failed: The timing chain guides and oil pump drive chain module failed. The plastic timing chain guide broke apart inside the engine. The vehicle was salvaged and is no longer available for inspection. Safety Risk: The oil pressure warning illumin
... ated suddenly while driving at highway speed with no prior warning. Sudden loss of oil pressure and engine power while operating a vehicle at speed creates a significant risk of accident, loss of vehicle control, and injury to the driver and others on the road. Confirmed by Service Center: BMW of Springfield, New Jersey performed a formal diagnostic confirming timing chain guide failure and oil pump drive chain failure. A BMW mechanic performed a scope inspection on April 3, 2026 visually confirming the broken plastic timing chain guide. Repair estimate: $7,191.42. Inspected by Others: The vehicle was inspected solely by BMW of Springfield. No police, insurance, or manufacturer representatives conducted a separate inspection. The failure mode is consistent with the defect acknowledged in BMW NA Technical Service Bulletin SI B11 03 17 and federal class action [XXX] v. BMW of North America, LLC (D.N.J. [XXX]). Warning Symptoms: The only warning was a sudden oil pressure warning message on the dashboard immediately before failure. Prior to that moment the vehicle operated normally with no unusual noises, no warning lights, and no fluid leaks. There were no gradual symptoms or prior indications of impending failure. Vehicle was declared a total economic loss and salvaged for $1,650.00. No injuries occurred. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

On march 5th 2021 @ 7946 miles on the car we took it in to the dealer because the passenger rear seatbelt had released on its own when braking hard with a child buckled in a booster seat. At that time they found the middle row left seat belt buckle not latching intermediately and said it was working again. Since then it has still continued to come unbuckled to this day. I have recently received from Hyundai a National safety recall about the issue and it's going back in for a third time now to resolve the issue. As of currently they will not be able to fix the issue because they do not have the parts and do not have an estimate of when they will be available. I also received a new National safety recall for the side curtain airbags in the third row not to the standards and are a safety concern. So the ultimate failure is from a catastrophically failed crankshaft journal thrust bearing. Which could be caused from one of two things: 1) The crankshaft thrust bearings they are putting in these motors are made with inferior materials, probably cheap Chinese crap or 2) The crankshaft end play tolerance was never set properly, giving the crankshaft the ability to move forward and backward in th
... e engine more than it was designed to. Which will over time beat up on the thrust bearing until it fails. In either case it would result in the same progressive issue showing itself. It will start as a squeak here and there. Which, as the soft thrust bearing material is unable to hold the proper oil film thickness for lubrication, will spiral into more and more heat being sent into the adjacent components, plus the buildup of metal particles being sent through the entire engine oiling system. Which will then start killing connecting rod and crankshaft bearings one by one. If the engine survived to this point, it’s now a ticking time bomb. It is either going to just seize up and not run anymore or the exciting route would be a connecting rod breaks and makes a window sized hole in the side of the engine block.Read more

The vehicle was purchased brand new from an authorized franchised dealership, sold and contracted as being equipped with a dealer-installed 2.5-inch suspension lift. At approximately 20,000 miles, a severe front-end squeaking symptom developed. The vehicle was brought to a separate authorized Toyota dealership service center for inspection. An authorized Toyota Master Technician formally inspec
... ted the vehicle and documented that the selling dealership did not install an engineered suspension lift. Instead, they installed unauthorized, budget aftermarket spacer pucks and non-certified upper control arms. The Master Technician documented in writing that the vehicle "isn't even lifted properly" and a factory front strut has failed prematurely. SAFETY RISK: The unauthorized spacer modification has severely altered and compromised the factory steering and suspension geometry. Because the front suspension, upper control arms, ball joints, struts, and steering rack are forced to operate continuously at unsafe, extreme, out-of-spec angles, the components are subjected to premature, destructive stress. The Master Technician confirmed that this improper configuration creates an imminent highway life-safety hazard, presenting an extreme risk of the front CV axles binding or snapping at highway speeds, which would result in an immediate, catastrophic loss of steering control. The vehicle is available for inspection upon request. The safety defect has been explicitly reproduced and confirmed in writing by an authorized Toyota Master Technician. The selling dealership management has been formally notified of this safety diagnosis but has actively refused to remediate the unsafe components, leaving a structurally compromised and highly dangerous vehicle operating on public highways.Read more

The vehicle’s rear/360 camera system, SYNC/APIM display system, and Reverse Brake Assist/back-over prevention system appear to have malfunctioned. The vehicle is available for inspection upon request. After a software update was performed by the Ford dealer, the vehicle began displaying the warning message: “Reverse Brake Assist not available — See manual.” At the same time, the rear/360
... camera image became distorted and unreliable. The camera display has shown abnormal behavior, including distortion, flickering, black/green screen behavior, and an inaccurate or unclear 360-degree view. This issue was not present before the dealer-performed software update. The safety risk is that the rear camera and Reverse Brake Assist are used when backing up to help detect objects, people, vehicles, or obstacles behind the vehicle. Because the camera image is distorted and Reverse Brake Assist is unavailable, the driver may not receive accurate visual information or automatic braking assistance while reversing. This creates a risk of backing into a pedestrian, another vehicle, property, or an unseen object. The problem has not yet been confirmed by a dealer or independent service center after the failure. UNKNOWN whether Ford has reproduced the issue. The vehicle/component has not yet been inspected by the manufacturer, police, insurance representatives, or others after the issue appeared. UNKNOWN whether the issue is caused by the dealer-performed software update, APIM/SYNC software, rear/360 camera module, camera calibration, camera wiring/connection, or another related system. Despite documenting these incidents, the manufacturer’s support team has refused to remediate the software conflict, instead instructing the user to power down the device during operation. The interface failure persists across multiple driving sessions, presenting a continuous risk of operator distraction and loss of critical navigation/safety data.Read more

I purchased a half shaft from Orielly on XXX and [XXX] in [XXX] on [XXX]. I checked the length of the half shaft and installed it. It fit as it should and the vehicle drove fine on the 3 mile test drive. on [XXX], i decided to drive the vehicle to work when 10 miles into the trip, the half shaft locked up on the freeway, put the crv on 3 wheels, and almost rolled. After caliming down and get
... ting the vehicle moved, I felt that the half shaft (not the wheel bearing) was extremely hot. I took a razor and cut a single slit in one of the boots to find aboslutely ZERO grease inside of it. The part was shipped without grease and is sealed. After having just a great morning, I called Orielly 3 separate times on a recorded line. I was dismissed 3 separate times when asking for them to check their supplier QC and make sure this does not happen to anyone else. After being dismissed, I went to linkden and started posting. A few hours later, someone higher up contacted me quite urgently claiming that they would "TRY" and help. THere were no warning lamps or any indication until it was too late. This put not only my saftey at risk, but we are lucky the car didn't roll or there would have been a substantial pile up. I am going to be returning the part in a few days for them to supposedly check it. I can keep it so you can inspect it yourselves if you like. The clear coat has completely failed on mainly the hood and roof of the entire vehicle. Each of the sides have lost a good amount of shine too but there hood and roof are a compete different color compared to the rest of the Tahoe. The paint is the pewter/grey so it is extremely noticeable and very much an eyesore.

I am filing a safety complaint regarding my 2025 Cadillac Escalade, which experienced a catastrophic engine failure while being driven on a public roadway with approximately 13,000 miles on the vehicle. While traveling from [XXX] to [XXX] , the vehicle suddenly lost power while in motion. A warning message appeared stating “Press to Start Again.” The engine shut off completely while driving,
... causing an immediate loss of propulsion. The vehicle went into neutral, and I was forced to maneuver to the side of the road to avoid a potentially dangerous situation. Once stopped, the vehicle would not restart. Multiple attempts were made using both the push-button ignition and key fob remote start, but the vehicle remained inoperable. The vehicle was towed and later diagnosed as requiring a complete engine replacement despite being less than one year old. I believe this is a serious safety defect because the vehicle lost power without warning while being driven. Had this occurred in heavier traffic, while crossing an intersection, merging, or traveling at higher speeds, it could have resulted in a serious accident, injury, or death. This was not the first major issue with this vehicle. Within weeks of taking delivery, multiple components required repair or replacement, including: • Complete replacement of the camera system due to defective cameras • Replacement of a defective lighting component • Transmission failure concerns • Cylinder #2 misfire issues The repeated failures of major vehicle systems, culminating in a complete engine failure and loss of propulsion while driving, raise serious concerns about the safety and reliability of this vehicle. Additionally, after the engine failure, the vehicle could not be placed into neutral and had to be dragged onto a tow truck, causing damage to the tires and raising concerns about emergency roadside recovery procedures.
