I am submitting this formal safety complaint regarding a structural defect in my 2023 Hyundai Santa Fe Hybrid that I believe poses a safety concern and warrants federal investigation under the National Traffic and Motor Vehicle Safety Act (49 U.S.C. § 30101 et seq.). The vehicle exhibits a persistent and recurring structural creak originating from the pinch weld area adjacent to the hybrid batter
... y storage compartment — specifically at a metal-on-metal contact point above the rear driver-side wheel well. The noise presents most prominently at low speeds over uneven surfaces, driveway transitions, and speed bumps. The root cause, as identified by service documentation and owner community reports, is a manufacturing defect in the pinch weld construction where two metal body components make contact due to insufficient clearance tolerances. This affects the structural integrity of the body-in-white in proximity to the hybrid high-voltage battery system. III. SAFETY SIGNIFICANCE I am reporting this as a safety concern for the following reasons: •The defect involves structural body components adjacent to the high-voltage hybrid battery storage compartment. Any degradation of body structure in this area presents a potential risk of battery exposure, puncture, or damage in collision scenarios; •The metal-on-metal contact at the pinch weld constitutes an ongoing structural stress point that may accelerate fatigue and cracking of load-bearing body components over time; •The defect is present across a statistically significant number of 2023 Santa Fe Hybrid units based on online owner community documentation, suggesting a manufacturing pattern rather than an isolated anomaly; •Multiple authorized Hyundai service centers have been unable to provide a permanent repair, indicating the defect is systemic and not addressable through routine service procedures; •The proximity of the defect to hybrid drivetrain components warrants evaluation for potential regulatory complianceRead more

While the van is at a complete stop at the intersection it shuts off and behaves as if it needs to be restarted. A message appears instructing me to press the brake and push the start button to start the van. I took it to Honda, but they were unable to find the issue. They replaced the battery to see if that would fix the problem, but about a month later, it happened again.

My 2023 Lucid Air Touring experienced a rearview camera/display failure while reversing, resulting in a backing collision and rear bumper damage. The main display/rearview image went blank or became unavailable during an active reverse maneuver, and the vehicle did not provide effective visual guidance or rear safety alerts before impact. This failure is consistent with NHTSA Recall 26V017 / Luci
... d SR-26-01-0, which concerns the rearview camera image not displaying when the vehicle is shifted into reverse, increasing the risk of a crash. Lucid denied responsibility but has not provided the underlying data, logs, timestamps, diagnostic records, software version history, OTA update records, or methodology supporting its conclusion. Lucid initially stated that the vehicle was not being operated during the reported time window, which I disputed because the vehicle was actively reversing when the display failure occurred. Lucid later acknowledged that a collision occurred but claimed no system errors were detected and that the recall update had allegedly been installed before the incident. If true, that creates an additional concern that the recall remedy may have been ineffective, incomplete, or insufficient. The absence of a recorded fault code does not prove that the rearview camera/display system functioned properly. I request that NHTSA investigate this as a potential rear visibility safety defect, FMVSS 111 issue, recall remedy failure, and inadequate manufacturer investigation. Lucid should be required to preserve and review all telemetry, event data, diagnostic logs, software records, OTA update records, camera/display/ACU logs, service history, and internal engineering analysis related to this incident.Read more

Component Failed & Inspection Status: The front-left upper and lower control arms, ball joints, wheel hub assembly, steering knuckle, and front axle shaft have failed. All original failed components are currently intact and are fully available for physical inspection upon request. Safety Risk
... : This severe structural corrosion causes the steering knuckle and axle to seize, creating an imminent risk of primary steering component separation while driving. A steering separation on a heavy electric vehicle (over 6,000 lbs curb weight) causes a sudden, complete loss of directional control, creating an immediate life-threatening hazard to the occupants and surrounding traffic. Confirmation & Testing:The failure and severe structural corrosion were officially diagnosed and confirmed by a certified technician at an authorized Ford dealership service center. Warning Signs & Symptoms: The symptom appeared as a very noisy, distinct creaking sound coming from the front-left suspension. This noise occurred specifically when turning the steering wheel and when operating the vehicle at speeds below 40 mph. The sounds may have been present but obscured by road noise at higher speeds. There were no dashboard warning lamps or error messages prior to the diagnostic inspection. Systemic Concerns: This 2022 model is experiencing a severe vulnerability to premature environmental corrosion that causes the steering knuckle, axle, and control arms to seize to the point of failure. This demonstrates a critical structural durability issue under heavy EV loads in road-salt regions that warrants federal safety tracking for the 2022 model year.Read more

I am submitting this complaint regarding a catastrophic CVT transmission failure in my 2019 Honda HR-V at approximately 97,500 miles. In May 2026, while driving in King of Prussia, Pennsylvania, my vehicle suddenly experienced a complete loss of propulsion. Multiple warning lights illuminated on the dashboard simultaneously. Although the engine would rev and the vehicle could be shifted into gear
... , it would not move properly and became unsafe to operate. The vehicle left me stranded away from home and required towing. I incurred significant expenses for towing, diagnostic fees, transportation, lodging, and related costs due to the breakdown. After diagnosis, I was informed that the CVT transmission had failed and required replacement at an estimated cost of approximately $10,000. I later learned that Honda had issued Service Bulletins 21-046 and 21-047 concerning CVT belt deterioration and transmission issues in 2016-2020 Honda HR-V vehicles. These bulletins acknowledge that an incorrect PCM value could cause excessive pressure on the CVT belt, potentially leading to partial or complete belt breakage and sudden loss of acceleration. Honda also extended warranty coverage on affected vehicles due to premature CVT belt deterioration. My vehicle falls within the affected model years and experienced symptoms consistent with those described in Honda’s service information. Despite this, Honda denied my request for goodwill assistance, even though the failure occurred at relatively low mileage for a modern transmission and only shortly after the warranty extension period expired. I am concerned that this transmission defect creates a safety risk because it can result in sudden loss of acceleration or vehicle propulsion while driving, potentially placing drivers and passengers in dangerous situations. I respectfully request that NHTSA investigate the CVT transmission failures affecting Honda HR-V vehicles and determine whether additional action is warranted.Read more

The front and rear evaporator core of my 2019 Volvo XC90 has failed and I no longer have air conditioning. This issue was diagnosed by a dealer. The evaporator core failure is a massive, widely documented issue specifically targeting the 2016–2020 Volvo SPA-platform models. According to my research, Volvo sold approx. 370,000 vehicles in the US during this period. At roughly $5,000 per vehicle,
... the owners are being asked to spend over a billion dollars on this known, product defect. Volvo is fully aware of this defect, yet they refuse to cover the full cost of the repair. They issued an official Technical Journal (TJ 35912.1.0) admitting that the system over-cools, causing the evaporator to freeze, expand, and crack. When technicians perform this repair, Volvo explicitly mandates installing a retroactive "jumper cable" resistor harness (Part No. 32241228) to prevent the replacement core from cracking again. The question for you is this: Is this a vehicle safety concern. We might consider why one would need air conditioning in the first place. Is it only for comfort or could it sufficiently cool air to help venerable people form having health issues (e.g., elderly, those with asthma). A recent Lancet study, for example, showed heat related deaths in Europe rising to 63,000 per year. This may be extreme, but if one lives in a southern state where temperatures climb to 100s or even a northern state where this routinely occurs in July and August, lack of A/C could be a serious health and safety concern.Read more

Taking my car in for now the 3rd time for the headlight error "Headlight System Malfunction. Have the vehicle inspected. The first 2 times it occurred around 3000 miles where they first tried a software and reset then it did it again so took it back and they swapped the left headlight which had failed. Then it was good for a few months until the issue came back recently around 7500 or so miles. No
... w it's doing it pretty consistently again and I suspect the right headlight has failed. Taking it in today to get checked out again. I'm surprised it's not a recall at this point as headlights are important and allow you to see at night. Seems to have the issue whenever the lights come on now so it's pretty consistent. I've checked forums and I am not the only 1. There are many ND3 owners with the same issues of failing headlights or in some cases other modules.Read more

The DQ250 6-speed DSG (dual clutch) automatic transmission in my 2017 Volkswagen CC Executive has catastrophically failed at 37,768 miles. COMPONENT FAILURE: The mechatronic unit, the electro-hydraulic controller that manages all gear selection and clutch engagement inside the DSG transmission has failed. The transmission is available for inspection upon request. WHAT HAPPENED: Symptoms began with
... intermittent failure to engage gear after stopping at intersections and stop signs. The transmission would slip out of gear or refuse to engage, leaving the vehicle momentarily unable to accelerate from a dead stop in traffic, a dangerous condition with cross-traffic approaching. The condition progressed to include an audible whining and slipping noise during downshifts, consistent with inadequate hydraulic clamping pressure on the clutch pack. The transmission then lost the ability to engage reverse gear entirely. The vehicle was driven home in forward gears only and is now inoperable and parked. SAFETY RISK: On multiple occasions, the vehicle was unable to accelerate from a complete stop at intersections with active cross-traffic. Loss of reverse gear eliminates the ability to maneuver out of dangerous situations in parking lots, driveways, and streets. This vehicle is unsafe to operate. WARNING SIGNS: No warning lamps or dashboard messages appeared prior to or during the failure. The first symptom was intermittent hesitation and slipping when attempting to engage from a stop. The condition worsened rapidly over a short period until reverse was lost completely. DEALER/MANUFACTURER CONTACT: The manufacturer was contacted. A callback was promised within two business days and was never received. The vehicle has not yet been inspected by a dealer because the servicing dealer, a Volkswagen-authorized dealership, has permanently closed. The vehicle is awaiting tow to a specialist for diagnostic confirmation. MAINTENANCE HISTORY: At SM VW until it closed. Once at Indy ShopRead more

Within a week of buying my vehicle in May of 2021 I went to Clearwater Toyota where I bought the vehicle from and complaining about the three seat belts in the back malfunctioning. When nobody is sitting in the backseat they sound off as if somebody is sitting there. I know that there is a recall on the front seat belt sensor doing this same issue for the Toyota Corollas but everyone I have spoken
... to that owns a 2021 Toyota Corolla complains about the back seats doing it as well the sensors misfire or sound off every hour as I'm driving at least once an hour. As an Uber Lyft driver I am concerned about my passengers in the backseat wearing the seat belts and the seat belt malfunctioning due to the sensors malfunctioning since I bought the vehicle in May of 2021. Any of the above questions I would have to stay unknown.Read more

The 2025 c300 rwd sedan was parked outside for about an hour. After getting back into the car and reversing, the rear camera would not display. The camera/parking interface was activated however there was an error symbol where the camera image is displayed, with a camera crossed through. Restarting the car, shifting back into drive, and stowing the camera did not restore the rear camera image.
... The camera deploys upon reversing and the lens/housing is clean. The image is not displayed in the driver display. This is unacceptable and another software issue that causes intermittent equipment failure, disabling vital safety systems and greatly increasing the risk of a collision.Read more

WHAT FAILED: Ford's recall-tracking and consumer-information system. Ford has internally inconsistent records re: Safety Recall 25V315 / 25S49 (Rear Camera Screen Software Update) on my VIN [XXX] . Vehicle and Ford-account portal available for inspection. STATUS SUBSYSTEM SAYS CLOSED: (1) Mailed "Successful OTA Update" notice received 6/1/2026 (verbatim): "the software to remedy this concern has
... been installed on your vehicle through a free Over-The-Air (OTA) software update, meaning the recall is now closed" (2) NHTSA database refreshed 6/2/2026: 25V315 no longer in unrepaired list (3) Ford VIN-specific recall dashboard 6/3/2026: 25S49 removed; only 25C69 remains DELIVERY SUBSYSTEM SAYS NOT DELIVERED: (4) ford.com/support/software-updates 6/2/2026: 9 of 10 expanded OTA detail cards captured, NONE references 25S49. Portal DOES show Compliance Recall 26C10 (ITRM-25.B04) with explicit completion language ("This completes Compliance Recall 26C10. No further action is required.") - proving the portal surfaces recall OTAs when they actually deploy. (5) In-vehicle SYNC Software Updates 6/2/2026: silent on 25S49. SYNC version 25221_PRODUCT Rev 2025 unchanged from 3/26/2026 despite 25S49's remedy being an APIM software update. (6) Ford MODEL-level recall page 6/3/2026 (verbatim): "Recall service not available right now. We have a remedy but parts are not available right now." SAFETY RISK: Ford simultaneously tells me the recall is closed via OTA delivered to my VIN AND publishes that the remedy parts are not yet available. Both cannot be true. Consumers cannot verify whether safety-critical recalls have been delivered. REPRODUCED/CONFIRMED: Contradiction is Ford-generated, reproducible by any reviewer. INSPECTED: No dealer or independent inspection of this discrepancy has occurred. WARNING LAMPS / FIRST APPEARANCE: Not lamp-driven. Confirmed [XXX]. WHAT FAILED: Ford-shipped OTA software (update ID OBCC-AS.AU, "Plug & Charge Improvements," delivered May 10, 2026) generates an admitted false-positive instrument-cluster wrench-light and Ford App service-needed notification. The OTA is installed; vehicle available for inspection on request. FORD'S OWN ADMISSION (verbatim, from Ford's published OTA release note): "In some instances after this upd
... ate, a 'Service vehicle soon' wrench icon may appear in your instrument cluster... You may also receive a Ford App notification indicating service is needed on the vehicle, which you can disregard. Please be assured this is a visual notification only..." SAFETY RISK: The same wrench-light is the documented symptom of NHTSA Safety Recall 25V863 / Ford 25C69 (Park Pawl engagement failure) - active and unrepaired on this VIN per NHTSA's June 2, 2026 refresh (see complaint 11741722). Ford's 25V863 NHTSA filing states a Park Pawl failure displays "A WRENCH LIGHT AND SHIFT SYSTEM FAULT MESSAGE... IN THE INSTRUMENT PANEL CLUSTER" with automatic EPB application and roll-in-park crash risk. Ford has instructed me in writing to disregard the same notification surface that would warn of a genuine 25C69 failure. The two are visually indistinguishable without dealer diagnosis. REPRODUCED/CONFIRMED: Ford itself confirms the defect in its written release note. INSPECTED: No dealer or independent inspection of this OTA-induced false-positive has occurred; Ford preemptively instructed disregard. WARNING LAMPS / FIRST APPEARANCE: The wrench-light icon and Ford App service-needed notification ARE the symptoms. They began with the May 10, 2026 OTA install per Ford's published note. Both are also the warning channels for the unrepaired 25C69 safety condition. I request ODI review of whether OBCC-AS.AU constitutes a defect under 49 U.S.C. 30102(a)(11) when it masks an active federal-recall warning channel on the same VIN.Read more
