On June 3, 2026, my 2021 Chevrolet Silverado 1500 LT Trail Boss, which has a 6.2L L87 V8 engine, stalled and shut off unexpectedly while I was stopped at a traffic light on a public highway. The engine died without warning, leaving me stranded in traffic and creating a serious safety risk for me and other drivers because of the chance of a collision. I had to call the police to come and sit behind
... me to keep myself safe from a collision while I waited for a tow truck. A dealer diagnosed the vehicle with DTC P0016, indicating a crankshaft/camshaft position correlation fault. This points to either an engine timing problem or an internal engine failure. The dealer confirmed the issue and gave me a repair estimate of about $11,000. Both the vehicle and repair records can be inspected if needed. Before the failure, the vehicle showed signs of an engine timing or internal issue and eventually triggered the check engine light with code P0016. After researching, I discovered many reports from other owners with the same 6.2L L87 engine who faced similar problems, including P0016 faults, engine stalling, internal engine damage, and complete engine replacement. While some vehicles with this engine have been part of recalls or special coverage programs, my vehicle is not included, even though it shows the same failure pattern. At the time of the failure, the vehicle has about 140,336 miles. I believe my vehicle has the same defect affecting other 6.2L L87 engines. I ask that NHTSA look into whether more vehicles should be added to any current or future recalls or defect investigations. Thank you for your attention to this matter.Read more

2016 Hyundai Veloster with 1.6L Gamma engine experienced repeat catastrophic engine failure consistent with NHTSA Recall 20V746000 / Hyundai Recall 198 for connecting rod bearing failure. Failure #1: At 124,036 miles, engine developed loud knocking noise increasing with RPM, check engine light, and DTCs P0300 (random misfire) + P0303 (cylinder 3 misfire). Dealer repair order documented "broken int
... ake valve on cylinder 3." Engine was replaced at my expense because Hyundai stated VIN was not covered under Recall 198, despite failure mode matching the recall. Failure #2: Hyundai replacement engine failed at 164,410 miles (40,374 miles on replacement engine). Identical symptoms returned: loud metallic knock that increases with RPM, same DTCs P0300/P0303, misfire cylinder 3. Vehicle is currently inoperable. Safety Concern: NHTSA Recall 20V746 states connecting rod bearings "may wear prematurely, which over time can result in engine damage" and "can increase the risk of a fire." Both of my engines suffered catastrophic internal damage at ∼40k-50k mile intervals with identical codes and cylinder 3 failure. A broken intake valve is engine damage directly caused by bearing wear allowing piston-to-valve contact. This is a pattern defect. My VIN was excluded from the recall, yet suffered the exact failure twice. The replacement engine supplied by Hyundai had the same defect. This presents a safety risk of sudden engine stall, loss of power, or fire while driving. Requested Action: Investigation into why VINs experiencing the exact failure mode are excluded from Recall 20V746, and assistance obtaining remedy from Hyundai.Read more

The night vision camera lens on my 2023 Cadillac Escalade ESV cracked from what appears to be road debris during normal highway driving. No collision occurred. The crack — a star pattern consistent with a rock strike — was discovered during a service visit on December 1, 2025 (RO 502394, 37,314 miles). I was not aware of the failure prior to that visit, meaning the safety system may have been
... inoperative for an unknown period. The lens is positioned very low on the driver side grille with no protection — directly in the path of road debris at highway speeds. This placement of a standard glass lens on a safety-critical system appears to be a design defect. Research confirms multiple Cadillac owners across the Escalade and XT6 have reported identical cracking from road debris, suggesting a systemic issue across GM night vision systems. Replacement cost is $1,701.23 and requires a proprietary tool available only at Cadillac dealers. GM has categorized this as a road hazard requiring an insurance claim rather than addressing it as a design defect.Read more

The panoramic sunroof shattered while the car was parked and there were no occupants. As the car was parked, there were no immediate safety concerns, but due to the risk of additional glass falling onto the driver and passenger seats, the car was not in a drivable condition. Had this happened while driving, the sound of the shattering and the glass falling may well have resulted in a reaction from me causing immediate danger to myself and others on the road. UNKNOWN if problem has been reproduced, but this is not an isolated incident. Vehicle component had not been inspected by manufacturer, police , insurance reps or others. I recieved this message after bringing my 2019 Chevy Tahoe to the dealership today for regular maintenanceand to check out a slight noise coming from under my hood: Rotational noise in your engine compartment had to remove the belts to be able to isolate where the sound was coming from. I have kept up all required/recommended maintenance on time and didnt have any reason to believe I needed anything more then belts replaced and fluids changed when I took my truck to the dealer. After recieving that message, I spoke with the manager at the dealer and he said its a common issue. I've loved on my vehicle and value it as it is my only vehicle that transports my 2 children and myself around. Therefore im confused how this could happen outside of a manufacturer defect.
