Incident Narrative

An employee was applying fiberglass to a hanging part while standing on a 2-foot step ladder. The part became unstable and fell, knocking the employee off the step ladder. The employee struck their head on the concrete floor. The employee was hospitalized with a brain bleed and a contusion to the head.
